Objective To analyze and summarize the clinical diagnosis and therapy features of chronic liver disease patients with limbs infection.Methods A retrospective analysis was performed on 29 chronic liver disease patients with serious limbs infection in our hospital.Results Chronic liver disease patients with limbs infection specially vibrio infection,had a high ratio of MODS,so early diagnosis,early therapy with antibiotics and early operation to expose and cut lesion has a good effect.Conclusions The chronic liver disease patients with limbs infection should early diagnosed,early treated by antibiotics,and early exposed the swelling limbs or cut lesion by surgery.
2.Gene expression of matrix metalloprotease and their inhibitors in liver tissue of rats with Vibrio vulnificus sepsis

Objective To investigate gene expressions of matrix metalloprotease (MMP-2, MMP-9) and tissue inhibitor of metalloproteinase-2 (TIMP-2) and RECK ( reversion inducing cysteine-rich protein with Kazal motifs) in the liver tissue of rats with Vibrio vulnificus sepsis. Methods Vibrio vulnificus sepsis model was induced by injection of 1 mL/100 g Vibrio vulnificus (9 × 106 cfu/mL) at hind limbs in 50 Sprague-Dawley male rats of clean grade, each time 10 animals were sacrificed at 2, 6, 9, 12 and 16 h after injection and liver samples were taken. Ten rats served as control group. The ethological changes were observed. The total RNA was extracted from liver tissue and the gene expressions of MMP-2, MMP-9, TIMP-2 and RECK were evaluated by semi-quantitative RT-PCR. Results The rats manifested shortness of breath, fever and swelling in hind limbs at 4 h after injection of Vibrio vulnificus. These symptoms gradually deteriorated, and the rats presented cyanosis and convulsion at 12 h. The gene expressions of MMP-2 and MMP-9 were markedly up-regulated, while those of TIMP-2 and RECK were down-regulated. Conclusion The study suggests that the elevation of MMP-2 and MMP-9 and down-regulation of TIMP-2 and RECK is associated with the development of Vibrio vulnificus sepsis.
3.Septic patients caused by Vibrio vulnificus: epidemiology, clinical findings, diagnosis and treatment

Objective To study epidemiology, clinical findings, diagnosis and treatment of sepsis caused by Vibrio vulnificus. Method Patientss with Vibrio vulnificus sepsis were collected from 1995 to 2008. The medical records including epidemiological and clinical data were analyzed. Results The male-to-female ratio of 34cases was 4.7:1 and 76. 5% of these patients suffered from chronic liver disease. Most patients occurred from April to October with signs of abrupt fever, characteristic cutaneous lesions, hypotension and progressive multiple organ disfunction syndrome (MODS). The mortality was over 47.1% . The criteria proposed for early diagnosis of Vibrio vulnificus sepsis were abrupt onset with fever during the period from April to November, characteristic cutaneous lesions, such as the most commonly occurred haemorrhagic bullae on the extremities or even extensive necrosis of skin and muscular tissue, progressive hypotension or shock accompanied by MODS, pre-existing liver disease or chronic abuse of alcohol, and consumption of raw seafood or exposure to seawater within 12 week. Early administration of the third-generation cephalosporins with the quinolones in full dosage, aggressive wound debridement,appropriate dermoplasty and supportive care contribute to a better outcome. Conclusions Vibrio vulnificus sepsis progresses rapidly with high mortality. Early diagnosis, rapid treatment with prompt antibiotics and aggressive surgery treatment are very important to improve the outcome.
4.Effects of oral motor interventions on feeding performance of preterm infants with feeding and swallowing disorders

Objective To evaluate the effects of oral motor interventions on feeding performance of preterm infants with feeding and swallowing disorders. Methods According to the bed number, the premature infants were divided into two groups in equal number: the control group received non-nutritive sucking for 30 minutes before feeding , while the experiment group infants received oral motor interventions. The proficiency and volume transfer were observed and recorded. Result The proficiency and volume transfered in the experiment group were significantly better than those of the control group (P<0.05). Conclusion The oral motor interventions can effectively improve the feeding performance in preterm infants with feeding and swallowing disorders.
5.The effects of antibiotics on gene expression of CD14 and pro-inflammatory/anti-inflammatory cytokines in liver of Vibrio vulnificus septic rats

Objective To investigate the potential role and changes of CD14, TNF-ct and IL-10 in liver in Vibrio vulnificus septic rats, and detect the intervention effects of eefoperazone sodium combined with levofloxacin. Methods To make Vibrio vulnificus sepsis model (VV group) and drug intervention model (AA group) in rats, the expression of CD14, TNF-α, IL-10 in liver were detected by RT-PCR. Results Compared with normal control (NC) group,CD14 mRNA and TNF-α mRNA expression increased markedly at 2, 6, 9, 12, 16 h in VV group (P<0.05), IL-10 mRNA raised greatly at 9, 12, 16 in VV group (P<0.05). CD14 mRNA expression was also rised in AA group at 9 h(P<0.05). TNF-α mRNA at 9, 12 h and IL-10 mRNA at 9, 12, 16 h in the AA group increased (P<0.05). Compared with VV groups, CD14 mRNA expression diminished greatly at 9, 12, 16 h in AA group (P <0.05), TNF-α mRNA and IL-10 mRNA diminished in the AA group at 16 h(P<0.05). Conclusion The treatment with cefoperazone sodium and levofloxacin may reduce expression of CD14, TNF-α and IL-10 in liver of rats with VV sepsis, it may inhibit the level of pro-inflammatory/anti-inflammatory cytokines, thereby regulating the balance of the inflammatory response in VV sepsis.
6.Application of viral load for differentiating diagnosis of early HIV infection

Objective To study the application of viral load for differentiating diagnosis of early HIV infection. Methods Thirteen indeterminate specimens, which showed early HIV infection of antibody detection, were selected. Viral load of the specimens were detected. People with suspicious infection were followed up and certified infection status through EIA and Western blot. Results Twelve of 13 indeterminate specimens which indicated early HIV infection, had positive viral loads. One antibody-positive infant was confirmed to have been infected by HIV and 11 recent infected (window period) persons were certified during the follow-up. One antibody-positive infant had negative viral load and was certified noninfected per-son during the follow-up. Viral load testing results accorded with HIV infection status. Conclusion Viral load testing can be used to diagnose HIV early infection, including antibody-positive infants (within 18 months) and recent infected persons. Viral load testing could be diagnostic in determinate specimens during early HIV infection.
7.Causes for kyphosis after removal of internal fixators for thoracolumbar vertebrae burst fractures

Objective To retrospectively analyze whether the kyphosis exists after removal of the internal fixators for thoracolumabar vertebrae fractures. Methods A total of 18 patients (35-68 years old) with thoracolumabar vertebrae fractures (T11-L2 ) were fixed with short segment pedical screw. The fixators were removed one year postoperatively to observe the changes of the Cobb' s angle and trauma vertebra'height. Results All the patients were followed up for 6-24 months ( average 18.7 months),which showed no intraoperative or postoperative complication, breakage or loosening of the screws. Compared to the Cobb angle and the vertebra height before removal of the internal fixators, the average loss of the Cobb angle was 0.7° and that of the vertebra height was 0.8 mm six months after removal of the fixators, 1.9° and 1.1 mm respectively one year after removal of the fixators, and 2.4° and 1.3 mm respectively two years after removal of the fixators in 16 patients without osteoporosis (P >0. 05). Among two patients with osteoporosis, the average loss of the Cobb angle and the vertebra height was 6° and 8°respectively and 3 mm and 5 mm respectively six months after removal of the fixators; 13° and 17° respectively and 5 mm and 7 mm respectively one year after removal of the fixators; 15° and 19° respectively and 6 mm and 7.5 mm two years after removal of the fixators. Conclusions After the internal fixation for thoracolumbar vertebrae burst fractures, kyphosis develops mildly, with insignificant change of the vertebral height. While the kyphosis becomes worse after removal of the fixators for thoracolumbar vertebrae burst fractures in patients with osteoporosis.
8.Subtype and sequence analysis of gag genes in HIV-1 circulating in sexual infectors in Beijing

Objective To investigate the subtype distribution and sequence characteristics of HIV-1 strains prevalent among sexual infectors in Beijing. Methods We collected the blood samples from 100HIV sexual infectors in Beijing during 2008 and separated plasma specimens. RNA was extracted from the plasma and the gag gene was amplified by RT-PCR and nest-PCR. The PCR products were sequenced directly and phylogenetic analyses of gag gene was performed using the MEGA4 software. Results Among 100 HIV-1 plasma samples,84 gag gene fragments were amplified and analyzed. Eight HIV subtypes including B(22 strains), B'(8 strains),C( 1 strain) ,CRF01_AE (38 strains) ,CRF02_AG (2 strains) ,CRF07_BC(9 strains) ,CRF08_BC(3 strains) and C/CRF01_AE recombinant like strain( 1 strain) were identified circulating in Beijing. Conclusion CRF01 _AE and subtype B were predominant in Beijing account for 45.2% and 26.2% and the surveillance of HIV gene variation should be paid more attention.

Objective:
To explore the application effect of preoperative administration of immunomodulating enteral nutrition for gastric cancer.
Methods:
From January 2016 to December 2017, 90 cases with gastric cancer in the Second People′s Hospital of Lianyungang were selected and randomly divided into observation group, control group A and control group B, with 30 cases in each group.The observation group was given enteral nutrition emulsion(TPFT, Ruineng) before and after operation, the control group A was given Ruineng before operation and the enteral nutrition emulsion(TP, Ruisu) after operation, the control group B was given Ruisu before operation and Ruineng after operation.The recovery, serum markers of nutritional status and immune function were evaluated and compared, and the postoperative situations were observed.

To compare the effects of laparoscope and open complete mesocolic excision (CME) in the treatment of right colon cancer.
Methods:
The retrospective case-control study was adopted.The clinical data of 139 patients with right colon cancer who underwent CME at the Second People's Hospital of Lianyungang from January 2014 to November 2017 were collected.The patients were divided into laparoscopy group(55 cases) and open group(84 cases).
