1.The comparative study of population characteristics and clinical performance differences in the liver cirrhosis with different causes of disease

Objective To investigate the differences of the characteristic and syndrome of patients with alcoholic cirrhosis(AC) and viral cirrhosis(VC).Methods Seventy patients with AC and 300 patients with VC in the Binzhou People's Hospital were selected as our subjects.The information including gender,age,disease history,chnical syndrome were collected.Meanwhile,the levels of Aspartate aminotransferase/alanine aminotransferase (AST/ALT),γ-glutamyl transferase (γ-GT),alkaline phosphatase (ALP) and total bilirubin (TBil) in serum were measured.Results The proportion of males in the AC group was 91.43% (64/70),significantly higher than that of VC group(64% (192/300),x2 =15.76,P =0.003)).The age,disease periods in AC group were (50.13 ± 12.35) years old and (2.09 ± 0.67) years,lower than the VC group ((58.66 ± 7.45) yearsold,t =3.97,P =0.042 ; (4.56 ± 1.14) years,t =5.22,P =0.034).There was no significant difference regarding of liver function index (P > 0.05).The rate of nasal carp (18.57%),gum bleeding (27.14%),liver palms(64.29%),spider(45.71%) in the AC group were significantly higher than the VC group (6.33%,15.00%,47.00%,29.67% respectively,P =0.017,0.036,0.025,0.016 respectively).The ratio of splenomegaly and esophageal varices were (81.43%) and (65.71%) in AC group,significantly lower than VC group (90.33%,86.00% respectively,P =0.037,0.011 respectively).The cirrhosis laboratory parameters results showed AST/ALT ratio (1.97 ± 0.45),gamma-GT ((152.33 ± 23.41) U/L),ALP indicators ((232.46 ±35.16) U/L in AC group patients,which were significantly higher in the VC group(1.00± 0.22,(45.89 ± 11.23) U/L and (102.23 ± 21.78) U/L,P =0.035,0.011,0.007 respectively).Conclusion There are difference in term of characteristic,manifestations and the testing laboratory indicators between alcoholic cirrhosis and viral cirrhosis.
2.Analysis of the efficacy and security of entecavir combined with interferon sequential therapy in the treatment of chronic hepatitis B

Objective To discuss the efficacy and security of entecavir and interferon sequential combination therapy on chronic hepatitis B.Methods 108 patients with chronic hepatitis B were randomly divided into the sequential combination therapy group,the entecavir group and the interferon group.Each group had 36 cases.The efficacy and security of different therapy were observed.Results The HBV DNA negative rate of the sequential combination therapy group was 77.78%,and the ALT normalization rate was 91.67%,which were both higher than those of the entecavir group and the interferon group(x2 =14.40,22.12,20.07,18.47,all P < 0.05).The total effective rate of the sequential combination therapy group was 91.67%,which was obviously higher than that of entecavir group and the interferon group(x2 =12.09,6.82,all P < 0.05).The entecavir and interferon sequential combination therapy had a good security.Conclusion Entecavir combined with interferon sequential therapy in the treatment of chronic hepatitis B had a significant clinical efficacy and deserved promotion.
3.Correlation Study Between Synchronic Heart Rate Variability, Blood Pressure Variability and Blood Vessel Damage in Patients With Essential Hypertension

Objective: To investigate the relationship between heart rate variability (HRV), blood pressure variability (BPV) and autonomic nerve function, blood vessel damage in patients with essential hypertension (EH) via synchronous monitoring. Methods: A total of 275 EH patients admitted to our hospital from 2011-04 to 2014-01 were enrolled. The vascular function was assessed by carotid-femoral pulse wave velocity (PWV). Based on PWV, the patients were divided into 2 groups: Normal PWV group (PWV<9m/s),n=185 and High PWV group (PWV≥9m/s),n=90. Synchronic 24h dynamic electrocardiogram (Holter) and 24h ambulatory blood pressure monitoring (ABPM) were performed in all patients. t-test, chi-square test, person liner correlation study and multi stepwise regression analysis were conducted to explore the relationship between HRV, PBV and PWV. Results: HRV and BPV in High PWV group had been changed unusually. Compared with Normal PWV group, High PWV group showed decreased standard deviation of the average of all normal-to-normal intervals in all 5-minute intervals (SDANN) (159.66±66.50) ms vs (194.36±119.29) ms and increased 24 h systolic blood pressure standard deviation (24h SSD) (14.40±3.65) mmHg vs (12.98±3.46) mmHg, all P<0.01; increased new index of night/day HR ratio (0.90±0.08) vs (0.87±0.06), P<0.01 and it had liner correlation to PWV (r=0.169, P=0.005). Multi stepwise liner regression analysis indicated that 24hSSD and HRV at low frequency (LF) portion had obvious and independent correlation to PWV (standard β value=0.352 and 0.212 respectively). Conclusion: ① EP patients were with decreased HRV (SDANN), increased BPV (24h SSD) and the higher incidence of arteriosclerosis; 24h SSD and HRV at LF portion were the most 2 important risk factors affecting PWV. ②Autonomic nerve dysfunction, vagus nerve over-excitatory were the independent risk factors for promoting the occurrence and development of arteriosclerosis in EH patients. ③Night/day HR ratio as a sensitive index for examining autonomic nerve function was independently related to hypertensive vessel damage. Synchronic monitoring of HRV and BPV is helpful to identify blood vessel damage in EH patients.
5.Progress of cell death induced by Neddylation modification pathway inhibitor MLN4924

Protein ubiquitin-like modification of Neddylation is an important post-translational modification mode of protein to widely regulate cell cycle, growth, development and other biological processes. Recent studies have found that MLN4924, small molecule inhibitor of Neddylation modification, has a significant anti-tumor effect, which can inhibit the tumor growth by inducing cell cycle arrest, senescence and apoptosis, and inhibiting tumor angiogenesis. This paper reviews the related mechanism research of cell programmed death induced by Neddylation modification pathway inhibitor MLN4924.
6.Causes for kyphosis after removal of internal fixators for thoracolumbar vertebrae burst fractures

Objective To retrospectively analyze whether the kyphosis exists after removal of the internal fixators for thoracolumabar vertebrae fractures. Methods A total of 18 patients (35-68 years old) with thoracolumabar vertebrae fractures (T11-L2 ) were fixed with short segment pedical screw. The fixators were removed one year postoperatively to observe the changes of the Cobb' s angle and trauma vertebra'height. Results All the patients were followed up for 6-24 months ( average 18.7 months),which showed no intraoperative or postoperative complication, breakage or loosening of the screws. Compared to the Cobb angle and the vertebra height before removal of the internal fixators, the average loss of the Cobb angle was 0.7° and that of the vertebra height was 0.8 mm six months after removal of the fixators, 1.9° and 1.1 mm respectively one year after removal of the fixators, and 2.4° and 1.3 mm respectively two years after removal of the fixators in 16 patients without osteoporosis (P >0. 05). Among two patients with osteoporosis, the average loss of the Cobb angle and the vertebra height was 6° and 8°respectively and 3 mm and 5 mm respectively six months after removal of the fixators; 13° and 17° respectively and 5 mm and 7 mm respectively one year after removal of the fixators; 15° and 19° respectively and 6 mm and 7.5 mm two years after removal of the fixators. Conclusions After the internal fixation for thoracolumbar vertebrae burst fractures, kyphosis develops mildly, with insignificant change of the vertebral height. While the kyphosis becomes worse after removal of the fixators for thoracolumbar vertebrae burst fractures in patients with osteoporosis.
7.Application of viral load for differentiating diagnosis of early HIV infection

Objective To study the application of viral load for differentiating diagnosis of early HIV infection. Methods Thirteen indeterminate specimens, which showed early HIV infection of antibody detection, were selected. Viral load of the specimens were detected. People with suspicious infection were followed up and certified infection status through EIA and Western blot. Results Twelve of 13 indeterminate specimens which indicated early HIV infection, had positive viral loads. One antibody-positive infant was confirmed to have been infected by HIV and 11 recent infected (window period) persons were certified during the follow-up. One antibody-positive infant had negative viral load and was certified noninfected per-son during the follow-up. Viral load testing results accorded with HIV infection status. Conclusion Viral load testing can be used to diagnose HIV early infection, including antibody-positive infants (within 18 months) and recent infected persons. Viral load testing could be diagnostic in determinate specimens during early HIV infection.
8.Combined Activity of Polymyxin B with Meropenem Against 110 Strains of Multidrug-resistant Acinetobacter baumannii

OBJECTIVE To evaluate the antibiotic effects of polymyxin B combined with meropenem against 110 strains of multidrug-resistant Acinetobacter baumannii.METHODS The protocol was designed by checkerboard method and the MICs of polymyxin B combined with meropenem against the 110 strains of A.baumannii were determined by broth dilution method,the FIC index was calculated according to MIC results.RESULTS The percentage of the FIC indexes less than 0.5,from 0.5 to 1,from 1 to 2 and more than 2 were 98%,2%,0% and 0%,respectively.CONCLUSIONS When polymyxin B combined with meropenem against 110 strains of A.baumannii the synergism and additivity are the main,there are no autonomy and antagonism.
9.Nursing study in abdominal breathing training ameliorating sleep status in elderly patients with chronic heart failure

Objective To investigate the effects of abdominal breathing training on sleep disorders in elderly patients with chronic heart failure. Methods Total of 100 patients with chronic heart failure complain of sleeping disorders and Pittsburgh Sleep Quality Index (PSQI)>7 points were assigned into two groups by random digits table method, 50 cases in each group. The observation group and the control group were nursed in the same way except that abdominal breathing was adapted to the observation group. Sleep status, heart rate, blood pressure, SpO2 and brain natriuretic peptide (BNP) were evaluated before training, one week and eight weeks after training respectively. Statistics was used to analyze the differences between two groups. Results After training one week, the sleep status of the observation group was ameliorated, but without significant difference compared to the control group (P>0.05). And after training eight weeks, the PSQI, BNP and heart rate were (9.21 ± 6.38) points, (193.78 ± 152.16) μg/L, (63.5 ± 10.8) times/min in the observation group, and (12.92 ± 0.33) points, (417.55 ± 262.47) μg/L, (70.7 ± 8.5) times/min in the control group, and there was significant differences between 2 groups (t=3.627, 2.041, 2.767, all P < 0.05), while the blood pressure, SpO2 did not change obviously(P>0.05). Conclusions Abdominal breathing training could ameliorate sleep status in elderly patients with chronic heart failure.
10.Effect of thymopentin combined with levofloxacin for retreatment smear positive pulmonary tuberculosis

Abstract: Objective To analyze the effect of adjuvant to levofloxacin in the treatment of retreatment smear positive pulmonary tuberculosis, as well as its effect on respiratory function, immune function and inflammatory factors. Methods One hundred cases of retreatment smear positive pulmonary tuberculosis patients admitted to Rudong County People's Hospital in Nantong city in Jiangsu province from 2017 to 2021 were randomly divided into a control group (n=50) and an observation group (n=50) according to random number table method. Both groups received conventional treatment (3 months of isoniazid, rifampicin, ethambutol, pyrazinamide / 6 months of isoniazid, rifampicin, ethambutol), with levofloxacin added to the control group, and thymopentin added to the observation group for the first three months in addition to routine treatment. The treatment effect of the two groups were compared. Results The sputum smear conversion rate of the observation group was significantly higher than that of the control group after 3 months and 5 months of treatment (χ2=7.142, P<0.05; χ2=6.250, P<0.05). The cavity absorption time and lesion absorption time in the observation group were significantly lower than those in the control group (t=4.006, P<0.05; t=5.165, P<0.05). The turning time of bacteriological culture in the observation group was significantly lower than that in the control group (t=4.220,P<0.05). After 3 months of treatment, CD4+, CD3+, CD4+/CD8+ of the observation group were higher than those of the control group, the differences were statistically significant (t=8.885, P<0.05; t=6.274, P<0.05; t=4.357, P<0.05). After 3 months of treatment, the IFN-γ (interferon-γ) of the observation group was higher than that of the control group (t=8.892, P<0.05), whereas the , IL-10 (interleukin-10) was significantly lower than that in the control group (t=5.986, P<0.05). After 3 months of treatment, forced vital capacity (FVC), forced expiratory volume in one second (forced expiratory volume in one second, FEV 1) and the one-second rate (forced expiratory volume in one second / forced vital capacity, FEV1/FVC) in the observation group were significantly higher than those in the control group (t=11.223, P<0.05; t=10.128, P<0.05; t=4.464, P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (χ2=0.378, P>0.05). Conclusions Thymopentin combined with levofloxacin had a significant application effect in the treatment of retreatment smear positive pulmonary tuberculosis, s, which led to improved inflammatory reaction, respiratory function and immune function. Additionally, it can increase sputum smear conversion rate and accelerate patient recovery, improving overall treatment efficacy, with a relatively high clinical application value.
