Objective To investigate clinical and pathological characteristics and the expression of interleukin (IL)-6, IL-17 and tumor necrosis factor (TNF)-α in elderly patients with primary nephrotic syndrome.Methods A total of 75 elderly cases with primary nephrotic syndrome were chosen as the observation group, and 40 healthy elderly persons served as the control group.Fasting serum IL-6, IL-17 and TNF-α levels were measured by enzyme-linked immunosorbent assay (ELISA) for both groups.Results 67 of the 75 patients received pathological diagnoses, of whom 46 had membranous glomerulonephritis, 5 had focal segmental glomerulosclerosis, 5 had mesangiocapillary glomerulonephritis, 3 had mesangial proliferative glomerulonephritis, 3 had IgA nephropathy, and the rest included one case for each of the following disorders: glomerular minor lesion, sclerosing glomerulonephritis, diffuse proliferative glomerulonephritis, segmental proliferative glomerulonephritis, and minor glomerular lesions.The expression of IL-6, IL-17 and TNF-α had statistically significant differences between the two groups.There were no statistically significant differences in the expression of IL-6 and IL-17 between patients with different pathological characteristics, and the expression of TNF-α was higher in membranous glomerulonephritis patients than in patients with other pathological characteristics.Conclusions Membranous glomerulonephritis is the main pathological type in elderly patients with primary nephrotic syndrome.Serum IL-6, IL-17 and most notably TNF-α levels are elevated in patients with membranous glomerulonephritis.

Objective To investigate the efficacy of Ginkgo‐Leaf Capsule combined with tripterygium glycoside in the treat‐ment of diabetic nephropathy(DN) and its influence on CysC ,Hcy and VEGF .Methods Eighty patients with DN treated in our hospital from January 2013 to April 2015 were selected as research subjects and equally divided into the control group and the ob‐servation group according to the random number table ,40 cases in each group .Besides the conventional Western medicine treat‐ment ,the control group was given tripterygium glycoside ,while the observation group was given Kinkgo‐Leaf Capsule combined with tripterygium glycoside .The treatment course was 3 months .Then ,the curative efficacy ,indices of renal function ,levels of Cy‐sC ,Hcy and vascular endothelial growth factor(VEGF) ,and occurrence situation of adverse reactions were compared between the two groups .Results The total therapeutic efficacy rate in the observation group and the control group was 92 .50% and 80 .00%respectively without statistically significant difference (P>0 .05) .In the comparison with the control group after treatment ,the re‐nal function indices of serum creatinine ,urea nitrogen ,24 h urinary protein ,level of serum CysC ,Hcy and VEGF in the observation group were statistically decreased (P< 0 .05) .During the treatment process ,no severe adverse reactions occurred in the two groups .The incidence rate of adverse reactions in the observation group and the control group was 17 .5% and 12 .5% respectively without statistical difference (P>0 .05) .Conclusion Ginkgo‐Leaf Capsule combined with tripterygium glycoside is effective and reliable for treating DN ,which can significantly improve the renal function ,reduces the level of serum CysC ,Hcy and VEGF .

Objective To explore the influencing factors of ALT abnormality among workers from food industry and related people from public places in Shenzhen and influencing factors.Methods 2,411 workers from food industry and people from public places who had physical examinations in our department from May to October in 2013 were involved in the investigation.ALT abnormality rate and the influencing factors were analyzed.Results The incidence of ALT abnormalities among workers from food industry and related people from public places in Shenzhen was 9.37%.The ALT abnormalities were correlated with censue register,gender,age,marital status,work duration in Shenzhen,education level and monthly income respectively(all P<0.05). Conclusion The ALT abnormality has a higher rate among people from cities,of male gender and old age,with longer work duration in Shenzhen and relatively high level of education and higher monthly income.Therefore,for the population,the health education should be strengthened and the healthy lifestyles should be advocated to effectively reduce the ALT abnormality rate.

Objectiye To investigate the expression of regulated on activation, normal T-cell expressed and secreted (RANTES) and occluding in brain, and investigate the role of expression of RANTES in the change of blood-brain barrier permeability of rats with acute pancreatitis. Methods 49 Sprague-Dawley rats were randomly divided into 7 groups according to random number table, including sham-operated group (SO) , acute edematous pancreatitis (AEP) 3, 6 h group and acute necrotizing pancreatitis (ANP) 3, 6, 12 and 24 h group. AEP and ANP were induced by retrograde injection of 0. 5% and 5% sodium taurocholate, respectively. RT-PCR, Western blot, and immunohistochemistry methods were performed to assess the expression of RANTES and occludin mRNA and protein in brain tissue. Results The RANTES mRNA expressions in SO, AEP 3, 6 h, ANP 3, 6, 12, 24 h group were 0, 0, 0, 0.36±0.05, 0.47±0.04, 0.65±0.05, 0.83±0.07, respectively; The RANTES protein expressions were 0, 0, 0, 0.42±0. 03, 0. 57±0.04,0.78±0.08, 1.05±0.08, respectively; the values in ANP group were significantly lower than those in SO group and AEP group (P<0.01). The occludin mRNA expressions were 1.21±0.07,1.17±0.07, 1.15±0.08,0.84±0.07,0.77±0.05,0.64±0.09,0.56±0.09, respectively, the occludin protein expressions were 1.18 ±0.08, 1. 16 ±0. 10, 1. 11 ±0. 10, 0. 90 ±0. 03, 0. 65 ±0."05, 0.57 ±0.05, 0.48 ±0.05, respectively, the values in ANP group were significantly lower than those in SO group and AEP group (P< 0.01). The expression of RANTES was positively related to the pancreatic pathologic score (r = 0. 936,P< 0. 001) ; the expression of occluding was negatively related to the pancreatic pathologic score (r = - 0. 943, P < 0.001). The expression of RANTES was negatively related to the expression of occluding (r = -0. 943, P <0. 001). Conclusions The expression of RANTES was progressively increased in the brain tissue in rats with ANP, while the expression of occluding was progressively decreased; RANTES may play an important role in the change of blood-brain barrier permeability via down-regulating the expression of occluding.

It has been shown that Spirulina platensis can regulate imminological functions.We report here that crude extract and purified components (phycocyanin and polysaccharide) from Spirulina platensis can induce secretion of IL 2 in splenocyte of BALB/C mice by means of MTT method.In the present study,we showed that all experimental components can't enhance proliferation of CTLL which was used in MTT method,but induce IL 2 secretion in splenocyte of BALB/C mice in three different concentration (0.01,0.1,1 g?L -1 ).Indeed the purified components especially phycocyanin part showed stronger IL 2 inducing activity than the crude one.IL 2 level was grow up when the incubation time of splenocyte and Spirulina platensis increased.In the concentration of 1 g?L -1 ,detected Spirulina platensis in our study assist IL 2 inducing of ConA (2mg?L -1 ) in splenocyte of BALB/C mice.

Objective Using the indicator system to objectively evaluate the content and effectiveness of the new culture construction in typical public hospitals,so as to identify problems and put forward suggestions for improve-ment.Methods Applying the new culture index system for the high-quality development of public hospitals con-structed by the research group,it quantitatively evaluated the practice of new culture construction in a tertiary hospi-tal in Shanghai.Results It found that the main body of the hospital's new culture construction scored 98.9 points.The 2 dimensions of hospital culture system construction and carrying out effectiveness(additional indicators)were all full scores.In addition,the dimension of caring for medical staff was 24.8 points,the dimension of strengthening pa-tient demand orientation was 24.6 points,and the dimension of distinctive hospital culture was 24.5 points.There are still deficiencies in the dimensions of the post-diagnosis experience of patients,the cultivation of health habits of staff,and the transformation of scientific research.Conclusion The hospital has taken advantage of the top-level de-sign of cultural construction under the leadership of the Party Committee and has improved the relevant systems.The hospital also focuses on several special cultural projects such as cultural heritage,employee care,optimization of ac-cess to medical care,and popularisation of scientific brands.It propose to promote the high-quality development of the hospital's new culture by carrying out Internet medical care,organizing special staff activities,strengthening the guidance of scientific research policies,and increasing the financial support for projects.

Objective It investigates medical staff's perception and experience sense of hospital culture,and analy-ses its influencing factors.This can further play the role of hospital culture construction on medical staff's motiva-tion,and improve medical staff's work motivation to provide reference basis.Methods The Medical Staff's Percep-tion and Experience of Hospital Culture scale developed by the research team was used to conduct a questionnaire survey with on medical staff in the sample hospital.Descriptive analysis,single factor analysis and multiple linear re-gression analysis were used to study the medical staff's perception and experience of hospital culture and its influ-encing factors.Results A total of 629 valid questionnaires were collected.The overall score rate of medical staff's per-ception and experience of hospital culture was over 90%.Among them,the highest scoring rate of the mission bearing subscale,and the dimension with higher scores was vision and goals.However,the experience of work environ-ment,promotion system and research environment is relatively low.The results of the one-way test showed that the differences in the scores of hospital culture perception and experience among medical staff with different educa-tion levels,titles and job positions were statistically significant(P＜0.05).Conclusion Medical staff generally support the construction of hospital culture.It is suggested that the hospital culture development strategy should be formulated from the perspective of the staff and according to their goals,problems and needs.It is also recommended to con-tinuously improve the medical staff's perception and experience of the hospital culture according to their needs,so as to play a positive role in the high-quality development of the hospital by culture construction.
