1.Observations on Pulmonary infrastructures in Experimental Respiratory Distress Syndrome Treated with Anisodamine in Dogs

Experimental respiratory distress syndrome was induced with oleic acid injection to 8 dogs. 4 dogs received no treatment and served as controls, and the other 4 were treated with anisodamine (654-2) after oleic acid injection.All the animals were killed at the 72nd hour after injection, and their lung tissues were studied under electron microscope.It was found that anisodamine was able to reduce the severity of pulmonary edema. There was accumulation of neutrophils in the pulmonary capillaries and hyaline membrane formation on the epithelial surface of the alveoli and alveolar duct walls. Neutrophil accumulation in the capillaries may indicate that the content of the blood was kept at a fairly high level at the 72nd hour after injection in spite of anisodamine treatment. The chief constituent of the hyaline membrane was fibrin. At the sites of hyaline membrane formation, focal cyto-plasmic necrosis of the type I epithelial cells was often observed.

3.The study on optimization of inner environment of a negative pressure isolator for transportation of infectious disease patient

Objective Study and determine the suitable negative pressure condition for a negative pressure isolator. Method The filter efficiency, pressure, capabilities of inner environment and physiological parameters of the user were tested in different negative pressure conditions. Results The different negative pressure conditions influence above mentioned capabilities of the isolator considerably except for the filter efficiency Conclusion Under the conditions of -30Pa~-50Pa,those are optimized the capabilities of inner environment and physiological parameters of the user.
4.Inhibitory effects of angiogenesis inhibitor TNP-470 on growth of a human malignant glioma cell line SHG-44 and its xenografted tumor

Objective To investigate the effects of TNP-470 on the growth of a human malignant glioma cell line SHG-44 in vivo and in vitro. Methods The colorimetric MTT assay, soft agar culture, flow cytometry,light and electron microscopy were used to determine the proliferation, the cloning efficiency, cell cycle and the morphological changes of SHG-44 cells as well as the growth of its xenografted tumor. Results TNP-470 (20~2 000 ng/ml) significantly inhibited the proliferation of SHG-44 cells in vitro (the 50% inhibitory concentration was 200 ng/ml). Cloning efficiency reduced obviously. The number of cells in G0/G1 phase increased, while that in S, G2/M phases decreased significantly. Weight and volume of xenografted tumors treated with TNP-470 (30 mg/kg, injected subcutaneously every other day) reduced notably. Furthermore, there were necrotic area and apoptosis in the tumor. No severe side effect of TNP-470 was found in this study. Conclusion The inhibitory effect of TNP-470 on the growth of SHG-44 cells correlates with its functions of regulating cell cycle and inducing apoptosis, which suggests that the angiogenesis inhibitor TNP-470 has strong inhibitory effect on human malignant gliomas.
5.Operations and Application Experience of Perpendicular Acupuncture at the Back-Shu Points of the Five Zang-organs

The Back-Shu points of the five Zang-organs are a group of points where qi and blood pour into the back and lumbar regions. Acupuncture at these points has a wide indication scope. However, it’s reported that internal organs were punctured in some cases because of improper control of needling depth, angle, and intensity during acupuncture. We used perpendicular needling instead of conventional oblique needling during the acupuncture of the Back-Shu points of the five Zang-organs. We found that when patients were completely relaxed, acupoints were correctly selected, the needling depth, the way of holding the needle, and inserting methods were well controlled, perpendicular needling of the Back-Shu points of the five Zang-organs can produce stronger needling sensation, and it’s easier to operate, safer, and more effective compared to oblique needling.
6.Effect of early initiation of continuous renal replacement therapy based on the KDIGO classification on the prognosis of critically ill patients with acute kidney injury

Objective To investigate the impact of early initiation of continuous renal replacement therapy (CRRT) based on Kidney Disease: Improving Global Outcomes (KDIGO) classification on the prognosis of critically ill patients with acute kidney injury (AKI). Methods A retrospective analysis of clinical data of patients diagnosed as AKI in Department of Critical Care Medicine of Zhejiang Provincial People's Hospital from January 2011 to January 2015 was conducted. All patients included should be 18 years old or older, having stayed in intensive care unit (ICU) for more than 48 hours, and received CRRT. All subjects were divided into three groups according to their renal function before CRRT according to the KDIGO-AKI guideline: AKI-stage 1 group, AKI-stage 2 group and AKI-stage 3 group. The general condition, original disease, severity of disease, duration of mechanical ventilation, the length of ICU or hospital stay, 28-day survival rate and in-hospital mortality rate were compared among these three groups. Additionally, risk factors for the 28-day survival rate and hospital mortality of critically ill patients with AKI were screened by logistic regression analysis. Results A total of 258 critically ill patients with AKI were enrolled, with 64 cases in AKI-stage 1 group, 62 cases in AKI-stage 2 group, and 132 cases in AKI-stage 3 group. 116 patients survived with 28-day survival rate of 44.96%. 154 patients died with hospital mortality 59.69%. The precipitating factors of AKI in all three groups (stage 1, stage 2, and stage 3) were similar, with sepsis, heart failure and poisoning (drugs or poison) being the main triggers for AKI, accounting for 35.66%, 19.38% and 13.18%, respectively. There were significant differences in the rate of vasoactive agent usage (31.25%, 41.94%, 50.00%, χ2 = 6.241, P = 0.044), acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score (20.87±7.37, 17.19±7.02, 22.58±7.95, F = 5.292, P = 0.006) and sequential organ failure assessment (SOFA) score (8.41±3.46, 6.22±2.43, 9.58±3.71, F = 10.328, P = 0.000), while there was no significant difference in gender, age, primary disease, time from ICU admission to the beginning of CRRT, mean arterial pressure (MAP), lactate level or 24-hour lactate clearance rate (LCR), mechanical ventilation time, the length of ICU or hospital stay, 28-day survival rate or hospital mortality among these three groups (all P > 0.05). According to the logistic regression analysis, time from ICU admission to start of CRRT and lactate level were the independent risk factors for 28-day survival rate or hospital mortality of critically ill patients with AKI [odds ratio (OR) for 28-day survival rate was 0.850 and 0.774, 95% confidence interval (95%CI) was 0.752-0.960 and 0.638-0.940, P value was 0.009 and 0.010, respectively; OR for hospital mortality was 0.884 and 0.756, 95%CI was 0.781-1.000 and 0.610-0.939, P value was 0.049 and 0.011, respectively]. Conclusion Early initiation of CRRT based on KDIGO-AKI classification could not improve the prognosis of critically ill patients with AKI, the optimal timing of RRT for such patients remains to be further explored.
7.Effect of nordihydroguaiaretic acid on the morphological changes of endothelial cells in vitro

Objective To investigate morphological changes of endothelial cells after nordihydroguaiaretic acid (NDGA) treatment in vitro. Methods The morphological changes of human umbilical vein endothelial cells (HUVEC) cell line ECV-304 and the cell apoptosis rate in sub-G0 phase were observed with invert, light and electron microscope and flow cytometry after NDGA treatment at different concentrations or with PBS (0.01 mol/L) as control. Results ①After the treatment of NDGA at 50~200 μmol/L for 1~3 d or up to 8 d at 100 μmol/L, ECV-304 cells tended to elongate into a shuttle-like sparse appearance and those in mitosis were decreased, indicating the suppression of cell proliferation. All these alteration was in a time-and dose-dependent manner. ②NDGA-treated ECV-304 cells displayed morphological features of apoptosis, especially at the 48th h after the treatment. With flow cytometry, the cells in sub-G0 phase were significantly increased, and reached its peak at hour 12 (20.42%) after NDGA treatment. In addition, the degeneration and necrosis of ECV-304 cells were related to the concentrations of NDGA. Conclusion NDGA can inhibit the proliferation and growth of endothhelial cells, and induce apopotosis, which might also inhibit angiogenesis.
8.Significance of bcl-2 and c-myc gene expression in nordihydroguaiaretic acid-induced apoptosis of a human malignant glioma cell line

Objective To investigate the changes and their significance of bcl-2 and c-myc in nordihydroguaiaretic acid (NDGA)-induced apoptosis of human malignant glioma cell line SHG-44. Methods The apoptosis of SHG-44 cells was observed with light and electron microscopy and TUNEL method. The expression of bcl-2 and c-myc gene was measured with immunohistochemistry, in situ hybridization and image analysis. Results ① The SHG-44 cell apoptosis was induced by NDGA at a concentration lower than 200 μmol/L in a time-dependent manner. ② The expressions of bcl-2 and c-myc gene in SHG-44 cells were decreased after the treatment of 100 μmol/L NDGA with the elapse of time, indicating a close association with cell apoptosis. ③ The expressions of bcl-2 and c-myc mRNA in SHG-44 cells were decreased after the treatment with 100μmol/L NDGA, which was apparently consistent with the immunohistochemical results. Conclusion The NDGA-induced apoptosis in human malignant glioma cells might be related with the down-regulated expressions of bcl-2 and c-myc gene. The exact mechanism needs further research.
9.Roles of cyclin-dependent kinase 4 (CDK4) in nordihydroguaiaretic acid-induced inhibitory effect on proliferation of human malignant glioma cells

Objective To investigate roles of cyclin-dependent kinase 4 (CDK4) in nordihydroguaiaretic acid (NDGA)-induced inhibitory effect on proliferation of human malignant glioma cells. Methods The techniques of cell culture, cell counts, flow cytometry, immunoprecipitation, immunohistochemistry and image analysis were employed in this study. Results ①A concentration-dependent inhibition of proliferation was demonstrated in the SHG-44 cells incubated for 24 hours in the presence of NDGA, and cell proliferation was blocked in the G1→S phase. ②The activity of CDK4 was decreased apparently in the SHG-44 cells treated for 24 hours with 10 to 200 μmol/L NDGA in a concentration-dependent way. ③The expression of CDK4 gene was downregulated in the cells after NDGA treatment. Conclusion CDK4 plays an important role in NDGA-induced inhibition of glioma cell proliferation.
10.Application of hybrid artificial kidney in the treatment of uremia

Objective To investigate the clinical efficacy and safety of hybrid artificial kidney in the treatment of patients with uremia.Methods 30 maintenance hemodialysis patients who were altered to be treated by hybrid artificial kidney(treatment group)were compared with 29 patients who were treated by hemodialysis only(control group).The amelioration of clinical symptom,remove of urea nitrogen(BUN),creatinine(Cr).Middle Molecular Substance(MMS)and variation of blood regular test were observed.Results BUN,Cr,haemoglobin level,leucocyte count and blood platelets count improved between after and before treatment(P <0.05),but there was no significant difference between the two groups(P > 0.05).Clearance rate ofMMS(25.6%)and improvement of clinical symptoms in treatment group were better than those of control group(5.9%)and difference was statistically significant(P <0.05,P<0.01).There was no serious adverse events in two groups.Conclusion Hybrid artificial kidney could remove molecular substances better and markedly improv clinical symptoms.It can make up for the shortcomings of conventional hemodialysis.
