The innovation in each aspect of medical and public health territory in our state gradually becomes the social focus. We analyze the key issues in the new type medical innovation in this paper, including clarifying government-dominant leadership of the medical innovation, identifying public welfare property of station-owned hospitals as the direction of medical innovation, impelling public health agency to perform total amount of money budget, and setting everyone's enjoying elementary medical service as the ultimate target of the medical innovation.Meanwhile, some beneficial opinions are also introduced in the paper.
2.The value of procalcitonin in the diagnosis and treatment of infective endocarditis patients
Chinese Journal of Emergency Medicine 2013;22(8):907-911
Objective To evaluate the role of procalcitonin (PCT) in the diagnosis and treatment of infective endocarditis (IE).Methods It was a retrospective study of 20 cases diagnosed as IE with positive culture between June 2010 and June 2012 of First Affiliated Hospital of China Medical University,Department of Infectious Diseases. Results The concentration of C-Reactive Protein (CRP) and PCT in the dead group recorded on the first day and the fifth day in the treatment was significant higher than the survival group. The decreased degree of CRP and PCT concentration after treatment in the dead group was much deeper than the survival group. Moreover, the concentration of CRP and PCT in Staphylococcus aureus IE and negative bacilli IE were significant higher than Streptococcus viridans IE or Enterococcus faecalis IE,which was commonly above 2 μg/ml level. Conclusions Serum procalcitonin, lined with CRP,has the similar significance in the assessment of infective endocarditis severity, however,PCT is slightly better than CRP in assessing prognosis. The concentration of procalcitonin has implications to the strains of IE pathogen,in the mean time,it' s beneficial to guide the early use of antibiotics.
3.Metabonomic analysis of the urine from rat model with abnormal sapra syndrome
Journal of Central South University(Medical Sciences) 2017;42(10):1143-1149
Objective:To investigate the correlation between the change in metabolic components of urine and the abnormal sapra syndrome by using a rat model of abnormal sapra syndrome. Methods:Multiple factors, such as dry environment, dry feed, and chronic electrical stimulation, were used to establish the abnormal sapra syndrome in Wistar rats by Uyghur medicine. The differences in metabolites were detected through the metabonomics method. Results:The urine of rats in abnormal sapra syndrome group showed significant high abundance metabolites as follows:Leucine, isoleucine, and glycoprotein. And that significant low abundance metabolites as follows:Glutamine, creatine, citric acid, and phenylalanine. Conclusion:The urine of rats with the abnormal sapra syndrome displays abnormal energy metabolism. It is likely that the dysfunctional metabolisms of three major nutrients might be the molecular basis for the abnormal sapra syndrome.

5.Depression And Its Predictors Among Breast Cancer Patients In Nepal
ASEAN Journal of Psychiatry 2015;16(1):106-115
Objectives: The main objectives of the study were to determine: (i) the prevalence
of depression among breast cancer patients in Nepal; (ii) the association between
depression and socio-demographic and disease related variables; and (iii) to
explore correlates and predictors of depression among these patients. Methods: A
cross-sectional descriptive design was used. Structured form for sociodemographic
and disease related information, and Hospital Anxiety and
Depression Scale (HADS) were used to collect information from 120 participants.
The study was conducted at Bhaktapur Cancer Hospital, Nepal. Data were
analyzed using Statistical Package for Social Sciences (SPSS) 16. Results: The
mean age of respondents was 51.92 (S.D=10.1783).The mean depression score
was 11.266 (S.D=2.782) and mean anxiety score was 11.81 (S.D=3.47). 4.2% had
severe depression, 60% had moderate depression, and 29.2% respondents had
mild levels of depression. There were no significant difference between age
group, marital status, family status, time since diagnosis, and treatment done
with depression level of respondents where-as educational level (p=0.014),
occupation (p=0.001), and anxiety level of respondents (p=0.000) had significant
impact on depression. Depression was found to be negatively correlated with
educational status of respondents (r=-0.226, P=0.013) and positively correlated
with anxiety level of respondents (r=0.450, P=0.000). Educational status
(P=0.008, B=-0.466 and Beta=-0.212) along with anxiety level of respondents
(P=0.000, B=0.369 and Beta=0.461) accounts for 30.4% of variance in depression
level of respondents (F=25.494 and R2=0.304). Conclusion: The prevalence of
depression among breast cancer patients was high. Psychological morbidities
compromise the treatment and quality of life of the patient. Thus management of
cancer should include screening for psychological morbidities, Psychological
rehabilitation should be provided along with anti-cancer treatment.
6.Alteration of p38 MAPK activity in lung tissue in diabetic rats
Chinese Journal of Pathophysiology 2000;0(11):-
AIM: To observe the pathologic changes in lung and the role of p38 MAPKinase signal pathways in pulmonary alteration in diabetic rats. METHODS: Diabetic rats were induced by intraperitoneally injected streptozotozin (STZ). After 4 weeks,we observed the pathologic changes in lungs, tested protein kinase C (PKC) activities by isotope in lungs of model rats, tested transforming growth factor (TGF-?_1) by Western blotting and immunohistochemical analysis, and determined the expression of p38 MAPKinase mRNA using in situ hybridization.RESULTS: After STZ administration for 4 weeks,we observed thickened pulmonary capillary basal lamina and increased number of fibre in Diabetes mellitus (DM) rats. TGF-?_1 levels,PKC and p38 MAPK activities were also found increased. CONCLUSION: The increased activities of TGF-?_1 and p38 MAPK suggeste that TGF-?_1 may play an important role in diabetic lung, and hyperglycemia-PKC-p38 MAPK signal pathways may be involved in the pathogenesis of diabetes.
