1.Effect of different methods of intestinal preparation on intestinal cleaning for elderly patients undergoing gynecological surgery

Objective To investigate the effect of different methods of intestinal preparation on intestinal cleaning for elderly gynecological patients undergoing surgery. Methods Sixty-four elderly patients undergoing gynecological surgery were divided into two groups according to admission seguence. The control group was treated with enema using 800~1000 mL 0.2%soapy water, while the experiment group was treated with enema using sodium phosphate salt oral solution mixed with 800 ~ 1000 mL 0.9% sodium chloride solution. The intestinal cleanness and enema times of the two groups were observed. Results The effect of intestinal cleanliness in the experiment group was better than that in the control group. There was significant differences between the two groups. (P<0.01). Conclusions For the elderly patients undergoing gynecological surgery, sodium phosphate salt oral solution can not only reduce the frequency of enema but also achieve the purpose of cleaning intestinal tract, to ensure the operation effect. It is the ideal method for cleaning bowels of the elderly patients in preoperative prepartion of the surgery.
2.Diagnostic value of GRE T2 * WI sequences in low field strength MR with oblique coronal plane in acute anterior cruciate ligament injury

Objective To explore the diagnostic value of GRE T2 ? WI sequences in low field strength MR with oblique coronal plane in patients with acute anterior cruciate ligament (ACL)injury.Methods 68 cases of acute traumatic hemarthrosis of the knee joint proved surgically were reviewed.The cases were divided into two groups:conventional MR group (axial image,standard coro-nal image and sagittal oblique image),and conventional with oblique coronal MR group.The diagnostic results with surgical and path-ological results were compared.The diagnostic accuracy of two images were evaluated respectively.Results For conventional MR group and conventional with oblique coronal MR group,the sensitivities in diagnosing ACL were 72.9% and 97.9%,the specificities were 80% and 85%,the positive predictive values were 89.7% and 94%,the negative predictive values were 55.2% and 85%,the accuracies were 52.9% and 82.9%,the misdiagnosis rates were 20% and 1 5%,the missed diagnosis rates were 27.1% and 2.1%, and the coincidence rates was 75% and 94.1%,respectively.The difference in diagnosis rates between two groups were statistically significant(P <0.05).Conclusion GRE T2 ? WI sequences with oblique coronal plane can significantly improve the diagnostic rate of anterior cruciate ligament injury.
3.Determination of Particle Size of Gefitinib by Laser Scattering Method

Objective:To establish and validate a method for the determination of particle size and size distribution of gefitinib. Methods:A Malvern Mastersizer 3000 laser size analyzer and a Hydro LV wet autosampler were used,a light scattering method for the determination of particle size and size distribution was adopted to analyze the particle size of gefitinib,and then the methodological study was carried out. The pump speed was 2 000 r·min -1 ,the obscuration was 8% -20% ,the measurement time of background and sample was 10 s,the sample RI was 1. 500,and the sample absorbance was 0. 00. Results:The D10 of 3 batches of gefitinib was less than 7 μm,the D50 was less than 15 μm and the D90 was less than 25 μm,which all met the preparation requirements of final products. Conclusion:The method is accurate,simple and repeatable,and suitable for the particle size analysis of gefitinib.
4.Correlation between serum albumin level and intestinal mucosa change in rats with cerebral hemorrhage

Objective To explore the correlation between the serum albumin level and the jejunal mucosal morphology in rats with cerebral hemorrhage and to find out the mechanism of the resulting hypoproteinemia.Methods A rat model of cerebral hemorrhage was used to detect the serum albumin level and the morphologic parameters of jejunal mucosa on day 1,7 and 14 after the hemorrhage.The results were compared to the normal control group and the sham-operated group. The correlation between the serum albumin level and the parameters of the jejunal mucosal morphology was explored.Results The serum albumin level was positively correlated with the small intestinal villous height(r =0.869,P<0.01),villous area(r=0.659,P<0.01),mucosal thickness (r=0.915,P<0.01),depth of intestinal glands(r=0.545,P<0.05)and density of intestinal glands(r=0.475,P<0.05).Conclusion The serum albumin level is closely related to the morphologic changes of the jejunal mucosa.
5.Continuous hemofiltration for prevention of contrast induced nephropathy in patients with advanced chronic kidney disease after percutaneous coronary intervention

Objective To assess the value of hemofiltration in the prevention of contrast induced nephropathy(CIN) post percutaneous coronary intervention(PCI) in severe chronic kidney disease stages(CKD) patients. Methods We evaluated 30 CKD patients who underwent PCI followed by bedside hemofiltration. We measured serum creatinine levels before PCI, 24 hours, 72 hours, and one week after PCI, and calculated creatinine clearance(CrCl) according to Cockcroft and Gault equation.We observed the incidence of CIN, and the short term clinical efficacy of hemofiltralion. Results The average age of the 30 patients was (72.87 ± 8.71) years old, with 21 (70%) male patients. The stages of CKD among the patients included CKD 3 (3 patients, 10%), CKD 4 (20 patients, 66.7%) and CKD 5 (7 patients, 23.3%). The average duration of hemofiltration was (7.5±4.1) hours. Serum creatinine before PCI, 24 hours, 72 hours and 1 week after PCI was (498.7±143.7)μmol/L, (353.2±128.0)μmol/L, (450.0±132.2)μmol/L, (488.0±145.7)μmol/L respectively,and CrCl was (20.3±10.2) ml/min, (36.5±14.3) ml/min, (28.3±10.4) ml/min, (21.0±10.3) ml/min respectively. There was no CIN. Mean follow-up was (3.1 ± 2.6) months. There was no new cardiovascular events, and no new patients need to rely on long term hemodialysis. Conclusions For patients with severe renal insufficiency, hemofiltration may reduce the incidence of CIN, It is an alternative preventive measures to prevent CIN.
6.Risk of Tuberculosis Infection in Hospital Workers: A Discussion

OBJECTIVE To discuss the risk of tuberculosis infection in a general hospital workers.METHODS In a cross-sectional survey,we studied purified protein derivative(PPD) test in exposure group and control group.(RESULTS) The positive rate of PPD test in the hospital workers was 84.1%,significantly higher than that in(controls)(52.1%,P0.05).The positive PPD in workers of respiratory(department) was 2.95 times higher than in those of other departments.CONCLUSIONS The(environment) of(hospital) is special.Tuberculosis infection threatens the health of hospital workers.So we suggest that hospital workers(improve) self-protective consciousness and actively prevent tuberculosis infection,especially for(internes) and(physicians) with negative PPD.
7.Comparison of Effects of Different Drying Methods on Contents of Effective Components of Forsythiae Fructus Leaves Tea

Objective To compare the contents of total flavonoids, forsythoside A and phillyrin in Forsythiae Fructus leaves tea under different drying conditions; To determine the best drying method for Forsythiae Fructus leaves tea. Methods With the sodium nitrite-aluminum trichloride-sodium hydroxide solution as color reagent, total flavonoids in forsythiae Fructus leaves tea were determined by UV spectrophotometry at the wavelength of 500 nm. HPLC was used to determine the contents of forsythiaside A and phillyrin. The analysis was performed on a Diamosil C18 (2) column (4.6 mm × 250 mm, 5 μm); the mobile phase was composed of acetonitrile and 0.4% acetic acid with gradient elution; the detection wavelength was set at 277 nm; the flow rate was 1.0 mL/min at column temperature of 30 ℃. Results The content of total flavonoids of Forsythiae Fructus leaves tea under different drying conditions was basically the same; the sequence of the contents of forsythoside A and Forsythin of Forsythiae Fructus leaves tea under different drying conditions was: ventilated drying > vacuum drying > blast drying. Conclusion Different drying conditions have no effect on the contents of total flavonoids in Forsythiae Fructus leaves tea, but have obvious effects on the contents of forsythiaside A and phillyrin. Ventilation shade is better than blast drying and vacuum drying for preverence of forsythiaside A and forsythin.
8.Study of Psychosis Associated with Parkinson Disease by Diffusion Tensor Imaging

Objective Using 3.0T magnetic resonance imaging and diffusion tensor magnetic resonance imaging, quantitative analysis of nerve fibers in region of interest was conducted in patients with psychosis associated with parkinson disease (PDPsy), PD patients without psychotic symptoms and normal control group, then investigate the relationship between FA value change in the region of interest and PDPsy, and the pathogenesis of PDPsy. Methods Sixteen patients with PDPsy, 24 PD patients without psychotic symptoms and 20 healthy persons with the corresponding age and sex (normal control group) were enrolled in this study. The patients with PDPsy and patients without psychotic symptoms have the matched gender, age and disease duration. All patients were underwent routine MR plain scan and DTI scan. The FA graph was reestablished, and the FA values of the bilateral substantia nigra-striatum fiber loop through the site (substantia nigra, red nucleus, nucleus, globus pallidus), frontal, occipital lobe and cingulate associated with schizophrenia were statistical analyzed. Results The nigral FA values in PDPsy and PD groups were significantly lower than that in control group. The frontal lobe and occipital lobe FA values in PDPsy group were significantly lower than those in control group. The frontal lobe, occipital lobe and cingulate FA values in PDPsy group were lower than those in PD group, but there were no significant differences between the two groups. The frontal lobe and occipital lobe FA values in PD group were lower than those in the control group, but there were no significant differences between the two groups. Conclusion DTI is a noninvasive and quantitative measurement for FA values of various brain regions in patients with PDPsy. It can reveal the PDPsy corresponding neural loop changes at the molecular imaging level, and can provide more information for the pathogenesis and clinical manifestations of PDPsy.
9.Feasibility of rapid prenatal diagnosis in advanced maternal age women

Objective To analyze the feasibility of rapid prenatal diagnosis in the advanced maternal age women with or without positive serologic screening results.Methods We conducted a retrospective study of the women who underwent a mid-trimester amniocentesis in Peking University First Hospital from January 1,2001 to December 31,2012.Maternal age,indication for invasive prenatal diagnosis,karyotyping and pregnancy outcome were documented.Using a young population with high risk in serologic screening (S) as the standard,chromosome abnormalities in the advanced maternal age (A) group and the advanced maternal age with high risk in serologic screening (A+S) group were compared with the S group.Chromosome abnormalities were divided into detectable (D) and undetectable (U) during rapid prenatal diagnosis.Results Of 9 606 cases,222 (2.3%,222/9 606) cases with chromosome abnormalities were detected,23.0% (51/222) of which were undetectable by rapid prenatal diagnosis.The detection rate of detectable chromosome abnormalities was 1.8% (57/3 177) in group A,1.4%(13/925) in group A+S,and 1.8%(57/3 250) in group S (x2=0.662,P>0.05).The rate of undetectable chromosome abnormalities was 0.5% (15/3 177) in group A,0.3% (3/925) in group A+S,and 0.5% (16/3 250) in group S (x2=0.452,P>0.05).The most common indications for undetectable chromosome abnormalities in the young population were abnormal history of pregnancy,abnormal family history and chromosome abnormality history (16.4%,9/55),and abnormal ultrasound in the advanced maternal age population (4.4%,3/68).Conclusions The performance of rapid prenatal diagnosis in the advanced maternal age population with or without high risk in screening without abnormal findings in ultrasound,was similar to the young population with high risk in screening.Fluorescent in situ hybridization may be integrated into the strategy of prenatal diagnosis for this group of women.
10.Quantitative Analysis of Amygdalin inKuxingren Formula Granules and Study on Characteristic Chromatogram

ObjectiveTo establish an HPLC analytic method for amygdalin inKuxingrenFormula Granules and HPLC characteristic chromatogram.MethodsRP-HPLC method was adopted. The determination was performed on Dionex C18 column (4.6 mm×250 mm, 5μm) with acetonitrile-0.1% phosphoric acid solution (8:92) at the flow rate of 0.6 mL/min. The detection wavelength was set at 207 nm and sample size was 10μL. The column temperature was 30℃. The peak of amygdalin was set as refernce, and 10 batches of samples were analyzed. TCM Chromatogram Fingerprint Similarity Evaluation System (2004 A) was adopted to evaluate similatity.Results The linear equation of amygdalin wasY=6.176×10-7X?3.058×10-3, with a good linear relationship in the range of 0.122 5–1.225μg (r= 0.999 8). The average recovery rate was 98.75% (RSD=1.30%). There were 6 common peaks in the characteristic chromatogram ofKuxingrenFormula Granules, and the similarity of 10 batches of samples was higher than 0.981. ConclusionThe HPLC method is simple, accurate and reproducible, which can be used for the quality control of Kuxingren Granules.
