1.Clinical analysis of Takayasu's arteritis with cerebral infarction

Objective To study the clinical features and treatment of the patients with Takayasu's arteritis (TA) with cerebral infarction.Methods Retrospective analysis was done in 14 TA patients with cerebral infarction from 1998 to 2003.Results The patients with TA complicated with cerebral infarction accounted for 10.6%(14/132) of all the hospitalized patients with TA.The time from cerebral infarction to the symptom onset of TA was 1 month to 15 years.In all the cases,there were more than 3 branches stenosis or occlusion of aortic arch and hemiparalysis.Basal ganglion was the most frequently involved infarction area.Corticosteroid and cytotoxic drugs were effective therapy.Conclusion The cases of TA with cerebral infarction have extensive arterial lesions.We must pay attention to it to avoid misdiagnosis.The key point of treatment is to control the primary disease.
2.A Comparison of Bone Mineral Density between Female Soccer Players and Sedentary College Students

Objective To determine the effect of high-intensity physical activity on bone mineral density(BMD). Methods Thirty-six female soccer players (19.56±1.46 years) with more than two years training history and at least ten months soccer training annually were recruited as experiment group. Thirty-three female sedentary college students, whose physical activities were limited to the compulsory physical education curriculum,were assigned to the control group. BMD of lumbar spine and femoral neck were obtained before and after 24 months research,using dual energy X-ray absorptiometry. Results After 24 months research, increased BMD were found in both groups. Soccer players had 2.25% increase of lumbar spine BMD(P<0.05) and 4.90% increase of femoral neck BMD(P<0.05). Sedentary college students had 0.89% increase of lumbar spine BMD(P>0.05) and 2.03% increase of femoral neck BMD(P>0.05). Compared with sedentary college students,Lumbar spine and femoral neck BMD in soccer players were significantly higher(P<0.05) after 24 months research. Conclusion Long-term soccer training had the effect of greater acquisition of BMD.
3.Quantitative detection of Porphyromonas gingivalis by real-time PCR of orthodontic patients after removal of orthodontic appliances

Objective To detect Porphyromonas gingivalis and clinical index in subgingival specimens after removal of orthodontic appliances and to evaluate clinical index and P.gingivalis factors associated with orthodontic appliances during an episode of gingival inflammation and the impact of appliance removal on periodontal health.Methods Choose 20 patients prepare to removal of orthodontic appliances.Plaque index(PLI),sulcus bleeding index(SBI) and probing depth(PD) of observed teeth were examined and subgingival plaques were collected at prior to appliance removal(baseline) and 1,3 and 6 months after appliance removal.The numbers of P.gingivalis,total bacteria,the proportion of P.gingivalis and the prevalence of P.gingivalis in the samples were detected with the TaqMan real-time PCR.Results The prevalence of P.gingivalis decreased from baseline to the 6th month after fixed appliance removal in two groups,and significant change was found in 6th month(P < 0.05); The numbers of P.gingivalis significantly reduced from baseline to the 6th month(P < 0.05); The proportion of P.gingivalis in subgingival plaque decreased from baseline to the 6th month(P <0.05).PLI,SBI and PD significantly decreased from baseline to the 6th month after fixed appliance removal.Conclusion After fixed appliance removal,most of patients can recover periodontal tissue health in good oral hygiene.The percentage of P.gingivalis are closely associated with periodontal status.TaqMan real-time polymerase chain reaction has a high sensitivity and specificity.It was demonstrated the broad potential of application on detection of oral microbes.
4.The Efficacy and Safety of Low Molecular Weight Heparin and Unfarction Heparin were Compared in Patients with Coronary Heart Disease during Percutaneous Coronary Intervention

Objective:To evaluate the efficacy and safety of low molecular weight heparin and unfarction heparin in patients with coronary heart disease during percutaneous coronary intervention by investigating the MACE beteewn the percutaneous coronary intervention procedure and post percutaneous coronary intervention 72 hours.Methods:200 patients with coronary heart disease who accepted percutaneous coronary intervention were investigated in this study.According to the anticoagulants,these patients were divided into LMWH subgroup(117 cases) and UFH subgroup(83 cases).According to conventional method,the MACE what happened during percutaneous coronary intervention procedure and post percutaneous coronary intervention 72 hours come from each group of patients was investigated and these statistics were analysised so that evaluate the efficacy and safety of low molecular weight heparin and unfarction heparin.Results:(1) There were no statistical significance in baseline characteristics between the each group (P>0.05).(2) There were statistical significance in the incidence of TIMI flow slows between the each group (P<0.05),low molecular weight heparin is superior to unfarction heparin in terms of efficacy.(3)There were no statistical significance in death between the each group (P>0.05),but there were statistical significance in bleeding / hematoma complications,and other (pericardial tamponade,chest pain,cardiogenic shock,cardiac rapture,ventricular septal perforation,ventricular tachycardia,ventricular fibrillation,cardiac arrest,Aspen attack,stent thrombosis and so on) between the each group (P<0.05),low molecular weight heparin less adverse reactions,higher safety.Conclusion:Low molecular weight heparin in the percutaneous coronary intervention effect is more significant,and less than UFH adverse reactions and high safety,more suitable for percutaneous coronary intervention anticoagulant therapy.
5.Vitamin D status in healthy adults in Guiyang of China

Objective To evaluate vitamin D status in healthy adults living in Guiyang. Method 1 500 healthy volunteers aged 20-79 years ( mean 45.2 years ) were recruited from a community in Guiyang by cluster sampling method. Questionnaires for living habits and fasting blood samples were collected from November, 2009 to February, 2010. Serum 25 ( OH ) D concentrations were measured by radioimmunoassay, using the DiaSorin kit,USA. Results The average serum 25 ( OH ) D level was ( 20. 4±9.0 ) ng/ml. The percentages of vitamin D deficiency [25 ( OH ) D < 20 ng/ml], insufficiency [20 ng/ml ≤ 25 ( OH ) D < 30 ng/ml], and sufficiency [25 ( OH ) D ≥ 30ng/ml] were 52. 3% , 32. 3% , and 15.4% , respectively. The 25 (OH) D concentrations in the young, middle-aged,and elderly were ( 18. 2±9. 2), (22. 8±8. 7), and ( 19. 9±7.8) ng/ml, respectively. The percentages of vitamin D deficiency in these groups were 62. 8%, 40. 2%, and 55.4%, being 61.6% in higher educational group ( ≥ 13 years) and 64. 4% in the group with lower body mass index ( < 18.5 kg/m2 ). Conclusion Vitamin D deficiency is common in Guiyang including all age groups, especially among the youth and the elderly. Serum 25-hydroxyvitamin D level is also influenced by education, age, smoking, and other factors.
6.Vitamin D status and its effect on bone mineral density of the middle and old aged women in the urban area of Guiyang

Objective To evaluate vitamin D status of the middle and old aged women in the urban area of Guiyang,and to explore the impact of vitamin D status on the bone mineral density.Methods A total of 511 healthy women,aged 40 to 79 years old,living in one of the urban communities in Guiyang,were selected by means of stratified cluster sampling.The levels of serum 25-hydroxyvitamin D [25 (OH) D] and intact parathyroid hormone (iPTH) were detected by radioimmunoassay.In addition,the bone mineral density (BMD) of hip,lumbar vertebrae 1-4 (L1-4),and neck of left femur were detected by Dual energy X ray absorptiometry.Results The average serum level of 25(OH)D in all subjects was (21.3 ± 7.9) ng/ml and that of iPFH was (29.3 ± 16.5) pg/ml.Severe deficiency [25 (OH) D< 10 ng/ml],deficiency [10 ≤ 25 (OH) D<20 ng/ml],insufficiency [20 ≤ 25 (OH) D ≤ 30ng/ml],and sufficiency [25 (OH) D> 30 ng/ml] of vitamin D were ascertained in 5.3%,42.3%,37.9%,and 14.5% of the total cases respectively.The morbidity of secondary hyperparathyroidism was 4.7%.The level of 25 (OH) D was positively correlated with the BMD of L1-4 and neck of left femur (both P<0.01).The level of iPTH was negatively correlated with BMD of neck of left femur and L1-4 (both P<0.05).Conclusion The majority of middle and old aged women in the urban area of Guiyang city were found to be of vitamin D deficiency and insufficiency.Vitamin D status is positively correlated with BMD at lumbar spine and proximal femur.Serum 25 (OH) level below 20 ng/ml is associated with raised parathyroid hormone,which increases the risk of osteoporosis.
7.Expression of EV71-VP1, PSGL-1 and SCARB2 in Tissues of Infants with Brain Stem Encephalitis

Objective To understand the correlation of enterovirus 71 (EV71), P-selectin glycoprotein ligand-1 (PSG L-1), and scavenger receptor B2 (SCARB2) and to explore the possible pathway and mechanismof EV71 infection by observing the expression of EV71, PSG L-1 and SCARB2 in tissues of infants with brain stemencephalitis. Methods T he organs and tissues of infants with EV71-VP1 positivi-ty in their brain stems were chosen. Expression and distribution of EV71-VP1, PSG L-1, and SCARB2 were detected and compared by immunohistochemistry. Results Strong staining of EV71-VP1 was ob-served in the neuron, glial cells, the inflammatory cells of perivascular cuffing, parietal cells of the gas-tric fundus gland while alveolar macrophages, intestinal gland epitheliumcells, mucosa lymphoid nodule and lymphocyte of palatine tonsil showed moderate staining and weak staining were displayed in mesen-teric lymph nodes and lymphocyte of spleen. PSG L-1 expression was detected in parietal cells of the gastric fundus gland, tonsillar crypt squamous epithelium, alveolar macrophages and leukocytes in each tissue. SCARB2 expression was observed in all the above tissues except the intestines and spleen. Con-clusion T he distribution of EV71 correlates with SCARB2 expression. SCARB2 plays an important role in virus infection and replication. Stomach may be an important site for EV71 replication.
8.Dataset collection and visualization for first visible human female in China

Objective To build the dataset of Chinese visible human female. Methods After undergoing macroscopical, CT and MRI examinations to exclude organic lesions, a young female cadaver of medium height was selected as the subject. After morphological measurement and vascular perfusion, the cadaver was embedded with 5% gelatin and cryopreserved in a -30 ℃ icehouse for 1 week. A digital milling machine TK 6350 (milling accuracy of 0.001 mm) was used to shave off slices of the body layer by layer from head to foot in a laboratory at -25 ℃. The successive cross sections were photographed with a high definition digital camera, and the pictures were put into a computer to establish a dataset of human body. By utilizing the image dataset derived from the successive cross sections, 3D reconstruction and stereodisplay of human structure were finished with a SGI Workstation which was equipped with an independently self developed software package for 3D reconstruction. Results The selected specimen, a 22 year old female native of Chongqing, was 1 620 mm in height, 54 kg in weight and died of non organic disease. CT scans were made in every 1.0 mm for head and neck and every 2.0 mm for rest parts, and the thickness for MRI scans was 1.5 mm for head and 3.0 mm for rest parts. For serial cross sections, the thickness was 0.25 mm for head and 0.5 mm for rest parts. Thus, a total of 3640 slices were obtained, and the photo for every slice was saved as a 36 MB file in a resolution of 6 291 456 pixels (3 072?2 048). Finally, the complete data files reached to 131.04 GB. Conclusion ① This is the first formally reported case of Chinese visible human female, suggesting that China becomes the second country owning visible human female dataset of her population. We set up a website for the purpose of exchanging ideas and information on this subject. So, the results are issued simultaneously on the Internet (http://www.chinesevisiblehuman.com).② According to US Visible Human Project(VHP), the data of the 3 junctional parts of their female cadaver were absent because the body was cut into 4 segments. Taking the age of 59 year old into account, the visible human female's body was not exactly perfect. The sections of 0.33 mm in thickness were saved to pictures at a resolution of 2 490 368 pixels (2 048?1 216). While, the first Chinese visible human female reported here is a young female without organic disease or lesion. No sectional datum is lost for being acquired from successive sections of the whole body. The resolution of cross sectional image reaches to 6 291 456 pixels (3 072?2 048).
9.Application of chromosomal microarray analysis in prenatal diagnosis of pregnant women with advanced age.

OBJECTIVE:
To assess the value of chromosomal microarray analysis (CMA) for the detection of fetal anomalies among pregnant women with advanced age.
METHODS:
CMA results of 562 cases, in addition with the outcome of pregnancy and neonatal follow-up were reviewed.
RESULTS:
Among the 562 amniotic fluid samples, 73 cases (12.99%) of fetal chromosomal abnormalities were detected, which included 21 cases (3.73%) of chromosomal aneuploidies and 52 cases (9.25%) of copy number variations (CNVs). The latters included 27 cases of pathological CNVs (4.80%), 4 cases of possible pathogenic CNVs (0.71%) and 42 cases of variants with unknown clinical significance (7.47%). Compared with those under 35, the detection rate of fetal chromosomal aneuploidies for women with advanced age was higher under the indications of voluntary test, abnormal ultrasonic structures, abnormal ultrasonic soft index and risks indicated by non-invasive prenatal testing (NIPT). No significant difference was found in the detection rate of CNVs between those ≥35 and <35 and between those with age factor only and with additional indications (P> 0.05). 552 cases (98.22%) of pregnant women have completed the followed up. Among 31 women with pathological and possible pathogenic fetal CNVs detected by CMA, 25 had terminated the pregnancy, 6 (19.35%) have delivered without obvious abnormality. 41 pregnant women with fetal CNVs of unknown clinical significance have completed the follow up, among whom 3 had terminated the pregnancy, 1 newborn was found with malformation after birth, which yielded an abnormal pregnancy rate of 9.76%. 480 pregnant women with negative CMA results have completed the follow up, among whom 5 (1.04%) had abnormal pregnancy or delivered a child with birth defect.
CONCLUSION
There is a certain difference between the outcome of pregnancy predicted by CMA testing and the actual outcome. The pregnancies with fetal CNVs with unknown clinical significance detected by CMA have a high adverse rate, which should attract clinical attention. CMA testing should be recommended for pregnant women with advanced age regardless of whether they have other symptoms. CMA combined with other detection methods is the trend for prenatal diagnosis.

10.Exploration of Traditional Chinese and Western Medicine in Prevention and Treatment of DKD Based on Mitochondrial Autophagy Mediated by PINK1/Parkin Signaling Pathway: A Review

Diabetic kidney disease (DKD) is one of the more common chronic kidney diseases,and its causes are complex. DKD is very easy to progress to end-stage renal disease,and the current therapeutic effect still needs to be improved. As an important excretive organ of the human body, the kidney has physiological functions such as discharging metabolic waste, regulating fluid balance, and maintaining the stability of the body's internal environment. These highly complex biochemical processes all depend on the energy support provided by mitochondria. Mitochondrial dysfunction is a key factor causing kidney injury, and the imbalance of mitochondrial homeostasis is an important link leading to mitochondrial dysfunction. The occurrence and development of DKD are often accompanied by the imbalance of mitochondrial homeostasis in renal cells. Mitochondrial autophagy, as a means of regulating mitochondrial homeostasis, is very important for the prevention and treatment of DKD. The PTEN-induced putative kinase 1 (PINK1)/Parkin pathway is one of the most classical pathways to regulate mitochondrial autophagy. Recent studies have found that some drugs can regulate the PINK1/Parkin signaling pathway to target mitochondrial homeostasis and exert renoprotective effects. In particular, traditional Chinese medicine has a significant effect on early and middle stage DKD by regulating PINK1/Parkin pathway-mediated mitochondrial autophagy. This article discussed the mechanism of PINK1/Parkin pathway in mitochondrial autophagy and DKD and reviewed the effect of PINK1/Parkin pathway-mediated mitochondrial autophagy on DKD. At the same time, it explored the therapeutic effect of traditional Chinese and western medicine on DKD mediated by PINK1/Parkin-mediated mitochondrial autophagy, aiming to broaden the ideas of traditional Chinese and western medicine for the prevention and treatment of DKD from the perspective of PINK1/Parkin regulating mitochondrial autophagy.
