1.Comparision of rivaroxaban and warfarin on preventing post-operation deep vein thrombosis in elderly hip fracture patients with diabetes mellitus

Objective To compare the clinical effects between rivaroxaban and warfarin on preventing deep vein thrombosis (DVT) in elderly hip fracture patients with diabetes mellitus (DM) after operation. Methods One hundred and sixty-eight elderly hip fracture patients with DM were randomly divided into the observation group and the control group. The observation group (88 cases) were treated with rivaroxaban (10mg, po, qd) after surgery, and the control group were treated with warfarin and low molecular weight heparin calcium for 14 days. Respectively after 7, 14, 21 and 28 days, the color doppler ultrasonography was used to examination vein blood flow and the DVT of affected lamb. In addition, the levels of D-dimer and bleeding were observed during treatment. Results The incidence of DVT in observation group (2 cases, 2.23%) was significantly lower than that in control group (7 cases, 8.75%), and DVT of control group was happened on 7th day after surgery. In addition, the level of D-dimer on 7th day post-surgery in observation group significantly decreased, while it significantly decreased on 14th day post-surgery in control group (P<0.05). However, the incidence of bleeding events in observation was less than that in control group (P<0.05). Conclusion Rivaroxaban can be more effective compared with warfarin in preventing diabetes mellitus elderly hip fracture patients with postoperative DVT formation and significantly reduce the bleeding risk, with more safe and convenient.
2.LONG-TERM EFFECT OF RADIOTHERAPY IN COMBINATION WITH CHEMOTHERAPY FOR PRIMARY OSTEOSARCOMA

Thirty-seven patients with primary osteosarcoma treated with local radiation and arterial perfusion of cytotoxic agents followed by systemic chemotherapy from April 1986 to April 1992. Several patients themselves stopped the treatment. All patients were divided into 3 groups: Group A (18 cases) were treated by local radiation and arterial perfusion of DDP, followed by systemic chemotherapy; Group B (14 cases) were treated by local radiation and arterial perfusion with no or insufficient dose of systemic chemotherapy; Group C (5 cases) were treated by local radiation and arterial perfusion with insufficient half dose and no systemic chemotherapy. Except the patients in group C, all other patients(32 cases) were followed-up from 24 to 95 months. Local control rate of the tumors was 93.8%, hypokinesis rate of the limps decreased from 93.8% to 40.6%, and mean survival time with no evidence of cancer was 38 months after treatment . survival rates of 2,3,5 and 7 years in group A were 94.4%,92.3%,72.7% and 50%,respectively.The mean survival time from lung metastasis to death in 19 patients was 13.6 months. 3.The expression and significance of KLK9 in liver cancer

Objective To investigate the expression of kallikrein 9 (KLK9) in liver cancer and to determine its significance.Methods The expression of KLK9 in liver cancer was detected by immunohistochemistry and RT-PCR techniques.Results The rate of expression of KLK9 protein in liver cancer tissues was significantly higher than paracarcinoma tissues and normal liver tissues (P < 0.05).The expression of KLK9 mRNA in liver cancer cells was significantly higher than normal liver cells (P < 0.05).The expression of KLK9 was related to metastasis,size of tumors,degree of malignancy and clinical staging of the liver cancer (P ≤ 0.05),but there was no associated with age,HbsAg and sex (P > 0.05).Conclusions KLK9 may play an important role in the occurrence and development of liver cancer.It may be used as a tumor marker and a prognostic factor.It may also provide a theoretical basis for the diagnosis and biological targeted therapy of liver cancer.
4.Experimental study of anteversion of femoral prosthesis in hemiarthroplasty for treating femoral in-tertrochanteric fracture in the elderly

Objective To study the accuracy of the anteversion of femoral prosthesis in hemiarthro-plasty for treating femoral intertrochanteric fracture in the elderly.Methods Twenty femoral neck antever-sions of specimens were measured, which were control group(A).The femoral specimens were made into the model of femoral intertrochanteric fracture, which were installed femoral prosthesis according to minor trochanter(B),identifying the max diameter of cavum medulla(C),ocular estimating femoral condylar line ( D) ,combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line( E) respectively.All the anteversion of femoral prosthesis were measured.Results The anteversion were:A group(8.98 ±4.31)°,B group (9.12 ±4.87)°,C group (9.57 ±6.49)°,D group (13.18 ± 5.83)°,E group (10.37 ±4.56)°respectively.The difference among the five groups was statistically sig-nificant.There was difference between D and A, there were no differences between B and A, C and A, E and A respectively.Conclusions In hemiarthroplasty for treating femoral intertrochanteric fracture, the an-teversion of femoral prosthesis is according to minor trochanter possibly; if the minor trochanter can not be got reduction, it is a good option according to combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line to make sure the accuracy of the anteversion of femoral prosthe-sis.
5.Evaluation and prediction of medical complications after hip fracture surgery

Objective To analyse the risk factors of medical complications after hip fracture surgery.Methods Clinical data of 327 hip fracture patients undergoing surgical treatment in Beijing Haidian Hospital from January 2009 to December 2013 were retrospectively studied.There were 114 males and 213 females with a mean age of (75.1 ±15.6) years, 129 patients(39.4%) had femoral neck fractures, and 198 patients ( 60.6%) had intertrochanteric fractures.Fifty four patients had medical complications during hospitalization ( complication group) and 273 patients did not have complications ( non-complications group ) . The clinical features, presurgical comorbidities and operative parameters were documented and compared between two groups.The risk factors of medical complications were assessed by regression analysis.Results Univariate analysis showed that age ( t =2.85, P=0.007), lying in bed before fracture(χ2 =12.86,P =0.000), the history of chronic obstructive pulmonary diseases(COPD) (χ2 =9.45, P=0.000), coronary heart disease(χ2 =10.67,P=0.000), heart failure(χ2 =15.85,P=0.000), diabetes (χ2 =3.22,P=0.024), cerebrovascular disease(χ2 =4.21,P=0.008), anesthesia method(χ2 =8.67,P=0.018), American Society of Anesthesiologists(ASA) score ≥3 level(χ2 =2.01, P=0.042) and the delay time of operation (t=4.24, P=0.037) were significantly different between two groups.Multivariate regression analysis revealed that age(β=0.030, OR=1.030), lying in bed before fracture (β=2.303, OR=10.716), the history of COPD (β=0.831, OR =1.995), coronary heart disease(β=0.858, OR=2.392), heart failure(β=1.971, OR=7.333)and ASA score ≥3 level(β=0.809,OR=2.583) were the independent risk factors of medical complications after hip fracture surgery. Conclusion Age, lying in bed before fracture, the history of COPD, coronary heart disease, heart failure and ASA score≥3 level would increase the risk of postoperative medical complications for patients with hip fracture.
7.Effectiveness of AC custom-made Ortho-K lens

AIM: To observe the effectiveness of AC custom-made orthokeratology(Ortho-K)lens in overnight orthokeratology fitting.
METHODS: A prospective study was conducted for 84 patients(165 eyes)with myopia who wore overnight Ortho-K lens for the first time. All patients were randomly divided into two groups with 38 patients(76 eyes)in observation group wearing AC custom-made Ortho-K lens and 46 patients(89 eyes)in control group wearing standard Ortho-K lens. Three items including the uncorrected visual acuity(UCVA), corneal topography, and the complications of corneal epithelium were observed and quantized to be evaluated at 1mo after Ortho-K lens wearing.
RESULTS: The assessment of 156 eyes(94.5%, including 73 eyes in observation group and 83 eyes in control group)in Ortho-K lens fitting were fine or acceptable at 1mo. Only 9 eyes(5.5%, including 3 eyes in observation group and 6 eyes in control group)were not good or unacceptable at that time, so that their Ortho-K lens had to be followed-up ulteriorly or changed instead. In all examination of this study, the UCVA of 159 eyes(96.4%, including 74 eyes in observation group and 85 eyes in control group)could reach one line lower to the BVCA or better in standard logarithmic visual acuity chart. The assessment of 157 eyes(95.2%, including 73 eyes in observation group and 84 eyes in control group)in corneal topography were fine or acceptable and there were no patients that had any serious complications in corneal epithelium. The observation group wearing AC custom-made Ortho-K lens performed better than the control group wearing standard Ortho-K lens in the result of UCVA, corneal topography, and the complications in corneal epithelium(P<0.05).
CONCLUSION: The overnight orthokeratology was effective and satisfying. AC custom-made Ortho-K lens performed better than standard Ortho-K lens in the assessment after wearing for 1mo.
8.Neuroprotective effects of blonanserin on H2O2-induced injury in PC12 cells

Objective To explore neuroprotective effects of blonanserin on H2O2-induced injury in PC12 cells. Meth?ods PC12 cells were divided into four groups:control group (C group), H2O2-treated group (H group), blonanserin pretreat?ed group (B group) and positive control group (vitamin E- pretreated, E group). The effects of different concentrations of blonanserin (0, 5, 10, 20, 40, 80 and 160 μmol·L-1) on cell proliferation in PC 12 cells were observed. MTT assay was used to detect the cell activity of different groups. The apoptotic rates of different groups were measured by TUNEL assay. The mor?phological changes were observed using inverted microscope and Hoechst 33258 staining. The superoxide dismutase (SOD) vi?ability and malondialdehyde (MDA) levels were detecded by biochemical methods in four groups. Results The appropriate concentration of blonanserin (0-20 μmol·L-1) can promote the growth of PC12 cells. Comparing with the C group, the apoptot?ic rate and MDA level were increased in group H, while the cell viability and the SOD viability were decreased obviously ( P<0.05). Compared with H group, the cell viability, SOD viability were significantly increased, while the MDA level and apoptotic rate were decreased (P<0.05). Conclusion Blonanserin shows neuroprotective effect on H2O2-induced injury in PC12 cells.
9.Spliting second toe for repairing more complex tissue defects in a hand

Objective To investigate a method of repairing complex tissue defects one stage in one finger or several fingers for more fine recipient site repairing and less donor area traum. Methods From August 2007 to August 2010,eight cases of 20 fingers were reconstructed according to the state of complex tissue defects in a hand,second toes (right or left side) were split into two or three parts as complex tissue flaps that may including joints,tendons,skin,nail beds,et al.These flaps then were translated to hand to repair complex tissue defects by anastomosing vessels.Results Twenty fingers in 8 cases were successfully reconstructed.Follow-up ranged from 3 months to 36 months,external appearance were fine.According to the Trial Evaluation Standard of Reconstructed Finger Function of Handsurgery Society of China,sixteen fingers were excellent,three fingers were good,one finger was fine.And there was no effect on foot.Conclusion Spliting single second toe is a good method for repairing more complex tissue defects in a hand.
10.The diagnostic value of multi-slice three-dimensional CT angiography for vertebrobasilar dolichoectasia

Objective To evaluate the diagnostic value of multi-slice three-dimensional computed tomographic angiography(MS 3D-CTA)for vertebrobasilar dolichoectasia(VBD).Methods MS 3D-CTA of 10 patients with VBD were retrospectively analysed.Source images were got by GE Lightspeed pro scanner.Volume rendering(VR)and maximum intensity project (MIP) were adopted to reconstruct 3D images in all cases.Twenty patients were selected as the control group by suspected cerebra[vascular diseases and underwent MS 3D-CTA at the same period.Enumeration data between the patient group and the control group was assessed by Wilcoxon.test.Results There were 2 types of 10 cases with VBD,including simple type(n=4)and saddle type(n=6).Compared with the control group of the length of the basilar artery(B 1,25.60 mm),the deviant degree(Bc,1.20 mm),the height(Bh,1.90 am),the length of the vertebral artery (V1,17.55 mm),the deviant degree(Vc,2.05 mm),and the diameter of BA and/or VA (Bw/Vw,3.05 mm),there is significant difference in the B1 30.20 mm,Bc 7.10 mm,Bh 8.80 mm,V1 23.00 mm,Vc 5.95 mm,and Bw/Vw 5.05 mm(P<0.01,all).Conclusion The clinical performances of VBD is different,MS 3D-CTA is a very effective method for the diagnosis of VBD.
