3.Aescine in treatment of lumbar intervertebral disc prolapse

AIM:To observe the therapeutic effect of aescine in the treatment of lumbar intervertebral disc prolapse(LIDP).METHODS:One hundred and seventy-six patients with LIDP were divided into 2 groups:aescine group of 80 patients[M56,F24;age(43±s17)a,course of disease(7.4±2.3) mo] received conventional therapy plus aescine 10-15 mg in 5 % glucose or 0.9 % sodium chloride injection 250 mL,iv,drip,bid,for 7-10 d.Control group of 96 patients[M57,F39;age(42±16)a,course of disease(7±3)mo]only received conventional therapy,including towing,massage,acupuncture,anodyne.Therapeutic effects of both groups were compared after 4 wk.RESULTS:The total effective rate of aescine and control group was 95 % and 81 %,respectively(P<0.05),while in those course of disease less than 0.5 a the effective rate was 97 % and 85 %,respectively(P<0.05).No obvious adverse reactions occoured. CONCLUSION:Aescine is safe and effective for the treatment of LIDP.The shorter the disease course is,the better the therapeutic effects are.
4.Psychological Training of Self Consistency and Congruence in Army Men

Objective:To explore the effect of psychological training on self consistency and congruence of army men. Methods:532 army men were tested by Self Consistency and Congruence Scale(SCCS)、EPQ、simplified coping style questionnaire(SCSQ)and SCL-90. Results:There were significant difference in score of SCCS between army men group and norm.There were significant correlation between factor scores of SCCS and scores of EPQ,SCSQ and SCL-90. After psychological training,the study group had higher score in flexibility than that of norm. The study group had higher score in flexibility and positive coping style than control group,and lower scores in P and N of EPQ,total score,obsessive-compulsive,interpersonal sensitivity,anxiety,hostility,paranoid ideation and psychoticism of SCL-90 than control. Conclusion:Self consistency and congruence level of army men were associated with personality, coping style and mental health. The psychological training has effect on balancing personality,facilitating self consistency,improving coping style and the psychological status of army men.
5.Explorations for the evaluation index system of prevention and control of hospital associated infection

In accordance with the General Hospital Accreditation Standards Ⅲ (2011) and other HAI-related hospital guidelines,a quantitative assessment index system for infection control of general hospitals has been established.Based on these,departmental performance of the hospital has been regularly checked in their infection control,and the outcomes of assessment included into the performance appraisal system of the hospital.Thanks to this practice,there have been no outbreaks of hospital associated infections in the hospital in recent two years,the incidence of hospital associated infection decreased from 1.25% in 2012 to 1.05% in 2013.Proper hand hygiene rates,compliance rates increased from 45.3% and 26.9 % in 2012 to 92.4 %and 65.5 % in 2013.The index system for evaluating prevention and control of hospital associated infection is scientific and effective.
6.Investigation and control on the epidemic of surgical site infection follow-ing clean wound operation in orthopedics patients

Objective To explore the investigation and control method for the epidemic of surgical site infection (SSI)in a hospital.Methods Data of orthopedics patients receiving clean wound orthopedics operation were ana-lyzed retrospectively,and effective measures were taken to control the epidemic of HAI.Results From June to August 2012,a total of 312 patients received clean wound orthopedics operation,7 (2.24%)SSI cases occurred.SSI rates during the same period from 2009 to 2012 were significantly different(P <0.05),and was the highest in 2012. HAI management staff reviewed patients’medical records,communicated with staff in orthopedics department,and put forward advice for SSI control;orthopedics department worked out and implemented detailed infection control measures.The subsequent surveillance showed that the prevalence of SSI in orthopedics ward decreased to 0.38%, infection outbreak was effectively controlled.Conclusion HAI management department played an important role in the finding and control of HAI outbreak.Scientific analysis on monitored data and proper measures adopted by HAI management department and clinical department can effectively control HAI outbreak.
7.Teaching research of improving hand hygiene skills among medical students

Objective On the background of the clinical skills competition for national medi-cal students in the higher medical colleges and universities, to investigate medical students' mastery of hand hygiene knowledge and skills, explore the effective methods of teaching, and improve the medical students' hand hygiene compliance and accuracy in clinical practice. Methods 90 medical students at a tertiary-level teaching hospital clinical practice stage were randomly selected for the study, and performed hand hygiene theoretical knowledge and practical skills training in special education. Special teaching investigation was made before and after training, to investigate the students' hand hygiene compliance rate in clinical practice. And the correct rate of hand hygiene were examined and medical students' hand hygiene compliance rate and correct rate were compared before and after the special teaching and training. Statistical analysis was made by using chi square test 2. Results After theory teaching and additional special skills training the students' hand hygiene compliance and accuracy were improved significantly (79.00%vs. 35.50%, P=0.000). The correct rate of hand hygiene was also significantly higher than before training (92.22% vs. 61.11%, P=0.000). Conclusion Hand hygiene knowledge and skills training can significantly improve the medical students' hand hygiene compliance and accuracy in the clinical practice, and is conducive to the prevention of the spread of infection in hospitals, and then reduce the incidence of hospital infection.
8.Closed-loop of workflow-based hospital infection control network

Routine supervision on hospital infection control was normally fed back on paper-based media,plagued by low efficiency and poor traceability.A feedback process developed with Workflow technology,can loop the entire process ranging from routine supervision to identifying loopholes,to hospital infection and joint development of correctional measures clinically,all the way to implementation of infection control practice and sustained improvement.The closed-loop PDCA cycle management features greater efficiency and traceability in hospital management of its infection control,and builds a quality improvement model featuring close communication between infection control department and clinical departments.
9.Study on acute mountain sickness and the psychological factors in the relief force during the initial phase of acute plateau exposure

Objective To investigate the acute mountain sickness of servicemen during4.14Yushu earhquake relief work,and analyze the related factors of the sickness at the initial phase of acute plateau exposure.Methods 162 servicemen were sampled randomly and clusteringly from Yushu relief force during.the inidal phase of acute plateau exposure.According to theDiagnosis and treatment principles of the acute mountain sickness,the prevalence of the acute mountain sickness was judged among the servicemen.The scales of general status invested score(GSIS),the self-rating symptom scale(SCL-90)and the life event scale(LES)were used to investigate the related factors of the acute mountain sickness.Results 65.4% of servicemen in earthquake relief work had acute mountain sickness,including mild 17.3%,moderate 31.5%and severe 16.7%.The occurrences of pant,palpitation,dizziness,lips cyanosis and headache,were higher than other symptoms,accounting for 83.6%,79.1%,73.1%,61.2%and 58.2%.The positive significant correlation was found among the symptoms score of acute mountain sickness and age,marriage,trauma history,negative events,and the most genes(somatization,obsession,depression,anxiety,hostility,fear,paranoia,appetite,sleep)and total score of SCL-90(r=0.188~0.619,P<0.01 or 0.05).There was negative significant correlation among the symptoms score and the personality and social support(r=-0.254,P<0.01;r=-0.285,P<0.01).Regression analysis showed that the somatization,personality,marriage,social support and age were more important than other factors.Conclusion Attention should be paid to the acute mountmn sickness in the relief force during the initial phase of acute plateau exposure,and the morbidity rate of acute mountain sickness were correlated with the somatization,personality,marriage,social support and age.
10.Waist-to-height ratio as a predictor of dyslipidemia for Chinese adults

Objective To explore the association between waist-to-height ratio (WHtR) and dyslipidemia in Chinese adults and to find out an optimal threshold of WHtR for predicting dyslipidemia.Methods A total of 221 270 adults from 4 health checkup centers nationwide were selected by using cluster random sampling method.Height,body weight,waist circumference (WC),total cholesterol (TC),triglyceride (TG),high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol(LDL-C) were measured.Body mass index (BMI) and WHtR were then calculated.The area under the receiver operating characteristic curve(AUC) was analyzed and optimal cutoffs were estimated by maximizing the sums of sensitivity and specificity.Results WHtR showed the largest AUCs in the participants with higher TC,TG and LDL-C,followed by increased WC and BMI.In male and female,the optimal thresholds of WHtR,WC and BMI for predicting dyslipidemia were 0.49-0.50 and 0.47-0.49,83-85and 73-76 cm,and 24-25 and 22-23 kg/m2,respectively.Conclusion WHtR may be superior to WC and BMI for predicting dyslipidemia,and an unisex cutoff of 0.5 should be recommended.
