1.Effect analysis of continuous epidural anesthesia and psychological intervention in painless labor

Objective To analyze the clinical effects of continuous epidural anesthesia and psychological intervention in painless labor. Methods From June 2016 to December 2016110 cases of maternal treatment in Ningbo city town of dragon longsai hospital as the research object, randomly divided into control group and observation group, control group of women to provide continuous epidural anesthesia, the observation group in continuous epidural anesthesia combined with psychological intervention measures. Results The two groups of pregnant women at different time scales VAS score, duration and other indicators, the data into the SPSS software, given the corresponding analysis and draw conclusions. Results The two groups of pain VAS score had no significant difference, the observation group each time after analgesia VAS score decreased than the control group(P<0.05); the first stage of the observation group was shorter than the control group (P<0.05), there was no significant difference between the 2 groups of second, third labor time. Conclusion Conclusion Epidural anesthesia and psychological intervention combined with painless labor have good analgesic effect and shorten the first stage of labor. .
2.Application of enteral nutrients in bowel preparation of diabetes patients before electronic colonoscopy

Objective To explore the effect of enteral nutrition applied to bowel preparation of diabetes patients before electronic colonoscopy. Methods A total of 172 diabetes patients were randomly allocated to the experimental group and the control group with 86 patients in each group. The experimental groups executed no dietary restriction within 3 days before the examination, and received Ensure 1h before colonoscopy. However,the control group was given low residue diet within 3 days before the examination, and received no Ensure before colonoscopy. The enteral cleanness and incidence of hypoglycemic reaction, abdominal pain and horror was compared between the two groups. Results 72 cases(83.72%) of the experimental group and 70 cases (81.40%) of the control group had better enteral cleanness, and there was no statistical difference in intestinal cleanness between the two groups. In the experimental group the incidence of hypoglycemic reaction, abdominal pain, horror was significantly lower than those in the control group. Conclusions Bowel preparation with unrestricted diet and ingested ensure before colonoscopy can not only ensure intestinal cleanness but also enhance endurance of colonoscopy.
3.Ca2+/calmodulin-dependent protein kinase Ⅱ inhibitor K N-93 aggravates the calcium paradox-induced heart injury

Aim ToinvestigatetheeffectsofCa2+/calmodulin-dependent protein kinase Ⅱ inhibitor KN-93 on calcium overload-induced heart injury.Methods Thirty-twoisolatedratheartswererandomlydivided into the control group,KN-93 control group,calcium paradox group,and calcium paradox with KN-93 treat-ment group.Left ventricular pressure were recorded, and the heart function was evaluated by the left ventric-ular end-diastolic pressure (LVEDP ) and developed pressure (LVDP).Coronary flow (CF)were collect-ed,and lactate dehydrogenase (LDH)content was de-termined.Triphenyltetrazolium chloride staining was usedtomeasuretheinfarctsize.Results Compared with the control group,KN-93 at 2. 5 μmol·L-1 had no effects on coronary flow,cardiac performance and cell death at the end of perfusion in normal rats (P>0. 05 );The hearts of calcium paradox exhibited a de-crease in LVDP and CF,meanwhile an increase in LV-EDP,LDH,and infarct size of 18 ±7. 2% (P <0. 01).2. 5 μmol·L-1 KN-93 further increased the levels of LVEDP,LDH and infarct size (P<0. 01)in Ca2+paradoxical hearts,while it provoked the decline intheCFandLVDP(P<0.01).Conclusion The data demonstrates that KN-93 aggravates heart injury in calcium paradox,it also suggests that CaMKⅡ is in-volved in the Ca2+overload-induced heart injury.
4.Distribution and characteristics of advanced schistosomiasis in Hunan Province

Objective To investigate the distribution and characteristics of advanced schistosomi-asis in Hunan Province in order to provide some references for policy making of the control program. Methods The survey scheme of advanced schistosomiasis was made out, in which investigation variables and diagnostic criteria were standardized. The survey was carried out in the whole province. Results There were 5 405 cases of advanced schistosomiasis, reduced by 21. 64% compared with the registration number in the previous files. The prevalence rate was 8. 69/10 000, of which 16. 21% was new cases, 97. 06% were distributed in lake regions and 2. 94% in hilly regions. Of total cases, 78. 70% was located in transmission areas, 15.64% in transmission controlled areas, and 5. 66% in transmission interrupted areas. Cases with splenomegaly made up 59. 4%, ones with ascitis 37. 30% and colonic granulomatous and dwarfism type 3. 16%. Male cases took up 77.39% and female ones 22.61%. In the splenomegaly type 52.24% of patients had performed splenectomy. The youngest case was 14 years old and the oldest 90 years old. Cases of 50-, 60- age group made up 65. 18%, and 32.7% of cases came from 30-, 40-, 70- age group and 2. 09% from the remaining age groups. Farmer took up 87. 58% of all occupations and others occupied 12. 42%. Conclusion The advanced schistosomiasis cases in Hunan Province are showing the falling tendency, although some new cases occur. The main advanced cases come from the farmer in lake regions.
5.Rapid Detection and Identification of Pathogenic Fungi of Some Deep Fungal Infections by PCR in Combination with Genescan Analysis

Objective To rapidly detect and identify pathogenic fungi of some deep fungal infections by PCR.Methods The suspensions of22pathogenic fungi(23strains)were amplified by PCR with fungal universal primers ITS86and ITS4which were labeled by FAM.The precise length of amplified fragments was determined by ABI PRISM TM 377Sequencer and Genescan analysis software,then compared with that of am-plicons of corresponding fungal DNA which were previously extracted.Results(1)Amplification of17pathogenic fungi with ITS4,ITS86resulted in a unique fragment length(except for A.nidulans and A.niger,C.albicans and C.stellatoidea,F.pedrosoi and E.dermatitidis).(2)No significant difference of the length of am-plicons was found between the fungal suspension and control organisms,based on the results of Genescan analysis.(3)The whole process took only6h to complete the detection.Conclusion The combination of fun-gal suspension PCR with ITS fungal universal primers and Genescan analysis might provide an accurate,spe-cific,sensitive,and rapid approach to detect and identify22pathogenic fungi causing deep fungal infections,and hold promise to be applied for the diagnosis of deep fungal infection.
6.The identification of the 16SrDNA of the diatoms in rabbits' internal organs in the summer and winter in Yongjiang River of Ningbo

Objective To evaluate the application value of identification on drown by detection 16SrDNA of the diatoms in rabbits' internal organs in summer month of July and winter month of December in YongJiang River of Ningbo. Methods 60 Rabbits were randomly and medially divided into three groups in summer and winter: drowning group, postmortem immersion group and using only lethal aeroembolism as control group. Specimen including heart, liver, lung and kidney from each rabbit were tested with diatom 16SrDNA PCR method. Results Compared with postmortem immersion group, detection rate of diatom 16SrDNA of heart, liver, lung, renal tissue in drowning group was significantly higher than that in summer month of July (P<0.05), In December, the 16SrDNA of the drowning group was detected in heart and lung tissues, There was no significant difference compared with postmortem immersion group (P>0.05) In summer month of July, detection rate of 16SrDNA of heart, liver, lung, renal tissues in drowning group was significantly higher than that in winter month of December (P<0.05). Diatom 16SrDNA of heart, liver, lung, kidney tissues in air embolism group were not detected In summer month of July and winter month of December. Conclusion With the higher detection rate of diatom 16SrDNA in drowning rabbit in summer, the diatom 16SrDNA PCR method can be used for the diagnosis of drowning in Yongjiang River of Ningbo; while in winter , it should be carefully apllied with the lower detection rate of diatom 16SrDNA.
7.Impact of pixel shine algorithm based on deep machine learning on image quality of abdominal low-dose plain CT scanning in patients with high body mass index

Objective To investigate the impact of deep machine learning Pixel Shine (PS) algorithm on image quality of abdominal low-dose plain CT scanning in BMI≥25 kg/m2 patients.Methods A total of 59 patients (BMI≥25 kg/m2) who underwent abdominal CT scan were collected.The patients were divided into group A (100 kVp,n=30) and B (120 kVp,n=29) according to the tube voltage.According to different reconstruction algorithms and treatment methods,patients in group A were divided into A1 (FBP),A2 (FBP+PS),A3 (50%ASiR-V) and A4 (50%ASiR-V+PS) subgroups,while in group B were divided into B1 (FBP) and B2 (50%ASiR-V) subgroups.CT and SD values of right hepatic lobe and right erector spinae were measured,then SNR and CNR of liver and CT dose index of volume (CTDIvol) were calculated.The consistency of parameters measured by two observers was evaluated.Results The consistency of parameters measured by two observers was good (all ICC>0.80).There was no statistical difference of CT values of liver and erector among A1-A4 subgroups (all P>0.05),whereas statistical differences of SD values of liver and erector spinae,also of SNR and CNR of liver were found (all P<0.001).Among A1-A4 subgroups,SDA4 <SDA2 <SDA3 <SDA1,SNRA4 >SNRA2 >SNRA3 > SNRA1 (all P<0.001) was observed.There was no significant difference of CNR between A1 and A3 subgroup (P=0.078),while CNRA4> CNRA2> CNRA3 or CNRA1 (P<0.001) was noticed.SD values of the liver in subgroup A2 was lower than subgroup B1,and A4 was lower than B2 subgroup (all P<0.001),and SNR and CNR increased significantly in A2 and A4 subgroups (all P<0.001).CTDIvol of group A was lower than that of group B (P<0.001).Conclusion Deep machine learning PS algorithm can improve image quality of abdominal low-dose plain CT scanning in high-BMI patients.
8.Relationship between cerebrovascular variation and the occurrence and recurrence of cerebral infarction

Objective:To investigate the relationship between cerebrovascular variation and the occurrence and recurrence of cerebral infarction, and provide a theoretical basis for the precise prevention and treatment of cerebral infarction.Methods:Totally 13 939 patients who underwent magnetic resonance imaging(MRI) and magnetic resonance angiography(MRA) examination at the Second Affiliated Hospital of Shandong First Medical University from January 2020 to December 2021 were grouped according to clinical symptoms combined with the imaging report, including 4 412 cases in the cerebral infarction group and 9 527 cases in the control group.2 048 patients in the cerebral infarction group were eventually enrolled in the study according to the inclusion and exclusion criteria, including 1 479 cases of initial cerebral infarction and 569 cases of recurrent cerebral infarction.SPSS 25.0 statistical software was used for data analysis.The χ2 test was used to compare the incidence of cerebral infarction with different cerebrovascular variations.Univariate analysis of suspected risk factors for recurrent cerebral infarction was performed with χ2 test, nonparametric test and t test.The binary logistic regression was used to analyze independent risk factors of recurrent cerebral infarction. Results:The incidence of cerebral infarction in the dual-system cerebrovascular variant patients, the single-system cerebrovascular variant patients, and the non-cerebrovascular variant patients were 40.9%, 30.7% and 31.8% respectively.The incidence of cerebral infarction in the dual-system cerebrovascular variant patients was the highest compared with those in the single-system cerebrovascular variant patients and the non-cerebrovascular variant patients (both P<0.05). The incidence rates of embryonic posterior cerebral artery, vertebral artery dominance, and bilateral common origin anterior cerebral arteries were 14.09%, 10.76% and 5.32%, respectively.The incidence of bilateral common origin anterior cerebral arteries in the cerebral infarction group was significantly higher than that in the control group and the difference was statistically significant.Patients with cerebral infarction who were familial aggregation ( OR=2.207, 95% CI=1.591-3.062), hyperhomocysteinemia ( OR=1.262, 95% CI=1.014-1.570), hypertension ( OR=1.461, 95% CI=1.114-1.918), diabetes mellitus ( OR=1.348, 95% CI=1.072-1.694), coronary heart disease ( OR=1.491, 95% CI=1.196-1.858) were more likely to recurrent cerebral infarction ( P<0.05), and patients with cerebral infarction had a significantly increased risk of recurrent cerebral infarction with age ( OR=1.031, 95% CI=1.020-1.042, P<0.05). Conclusion:Dual-system cerebrovascular variation and bilateral common origin anterior cerebral arteries are risk factors for cerebral infarction.
9. Application of intelligent real-time monitoring in infusion safety

Objective:
To explore the application of intelligent real-time monitoring in infusion safety.
Methods:
Totally 1 200 patients who received intravenous infusion from November 2017 to June 2018 in the Department of gastroenterology were selected as the study subjects. With system sampling method, 600 patients were selected as observation group who using Intelligent infusion system to monitor the infusion process. Another 600 patients were selected as control group without using the system. The current use rate of infusion fluid, the severity of local exudation during intravenous infusion, the blocking rate of intravenous pathway, the number of nurses dealing with dripping empty infusion of two groups and the rate of erroneous running caused by patients′ wrong ringing or false alarm of intelligent infusion system were compared between the two groups.
Results:
The current use rate of infusion fluid in the observation group was 91.33% (548/600) higher than 80.00% (480/600) in the control group (
