OBJECTIVE:To compare the cost-effective ratios between Granisetron and Ondansetron in the prevention of Cisplatin-induced nausea and vomiting.METHODS:In a randomized auto-controlled design,36patients receiving chemotherapy with Cisplatin were divided into2groups to compare the antiemetic effects and toxicity between Granisetron and Ondansetron and to carry out cost-effectiveness analysis.RESULTS:Granisetron could effectively prevent Cisplatin-induced nausea and vomiting and its antiemetic effect was comparable to that of Ondansetron. The average effective rates in preventing nausea and vomiting of Granisetron and Ondansetron were75.9%,77.8%and79.7%,75.0%respectively,and there were no significant differences between two drugs. The toxic reactions of Granisetron were slight. In cost-effectiveness analysis,Granisetron was better than Ondansetron.CONCLUSION:Granisetron is a safe, effective, economical antiemotic in chemotherapy of cancers. It is worth popularizing the application.

ObjectiveTo observe the efficacy of L-carnitine in prevention and treatment of toxicity of LFP chemotherapy with gastrointestinal cancer. Methods60 cases of gastrointestinal cancer were divided into 2 groups according to the admission date. The treatment groups received LFP chemotherapy and L-carnitine, while the control group received LFP chemotherapy alone. Both groups received 3 cycles chemotherapy. The gastrointestinal toxicity, neurotoxicity, hematologic toxicity and physical state were compared.ResultsThere were 12 cases of peripheral neurotoxicity in treatment group, the incidence rate was 40.0 %;but in control group there were 21 patients, and the incidence rate was 70.0 %. There was a significant difference between the two groups(x2=5.4545,P =0.0195). Anemia in the treatment group was 56.7 % (17/30); in the control group the rate was 86.7 % (26/30).There was a significant difference between the two groups (x2 =6.698,P =0.0351).After chemotherapy,in the treatment group, there were 4 cases with increasing Karnofsky score ≥ 10,19 patients whose Karnofsky score did not change, and 7 cases with reduced Karnofsky score ≥ 10; in the control group, there was 1 case with increasing Karnofsky score ≥ 10,13 cases with Karnofsky score no changing, and 16 cases with reduced Karnofsky score ≥ 10.The changes in physical state of two groups had a significant difference(x2 =5.711,P =0.0169).The gastrointestinal toxicity, thrombocytopenia, and neutropenia of two groups had no significant difference.ConclusionL-carnitine can reduce peripheral neurotoxicity and hematologic toxicity, improve the patient's physical state in patients received LFP chemotherapy.

Objective To select the components for quality control of Fructus Lycii based on the absorption of its extract. Methods To investigate metabolism of components of Fructus Lycii, everted rat gut sacs was carried out as well as the blood was taken from abdominal aorta,.and all samples were analysised by HPLC. Results There are twelve constituents absorbed between ileum and jejunum of rat , and four constituents were detected in the blood. Compound 2, 3, 4, 5, 6, 9, 10, 11 were absorbed in prototype forms in the intestine directly, and compound 1, 7, 8, 12 were new ones. On the other hand, four compositions(3, 7, 10, 13)could be absorbed into blood through analysis serum samples obtaining from aorta abdominalis of rats. Two of them (3, 10)could be absorbed directly by intestine, while(7)was absorbed into blood in new form . Conclusion Based on the intestinal absorption experiment and analysion of compsition in blood, components (3, 10, 13) can be the quality control ingredients of Fructus Lycii.

Objective To explore the clinical effect of traditional Chinese and Western medicine combined nursing intervention on pain control in patients after hip replacement .Methods Sixty-eight patients with hip replacement from January 2011 to April 2013 were divided into the experimental group and control group according to the random number table , each with 34 cases. The control group received the routine pain nursing intervention, and the experimental group received the auricular point and wrist ankle needle intervention on the basis of the control group. The degree of pain, adverse reactions, the days and costs of hospitalization, patients’ satisfaction and hip joint range of motion two weeks after operation .Results The scores of degree of pain in experimental group each time period were significantly lower than those in the control group , and the differences were statistically significant (P<0.05).The rate of adverse reactions and the days and costs of hospitalization were respectively 11.7%, (12.32 ±3.42)d, (38 561.46 ±3 247.02) yuan in the experimental group, and were significantly lower than 26.5%, (16.78 ±4.76) d, (42 512.87 ±3 853.23) yuan in the control group, and the differences were statistically significant (t=-3.452, 3.227, respectively;P<0.05). The patients’ satisfaction was 94.3% in the experimental group, and was significantly higher than 82.7% in the control group, and the difference was statistically significant (t=4.232, P<0.05).The hip joint range of active and passive motion two weeks after operation were respectively (97.86 ±6.56)°, (113.76 ±7.76)°in the experimental group, and were better than (90.14 ±6.23)°, (101.31 ±7.12)°in the control group, and the differences were statistically significant (t =-2.227, -3.216, respectively; P<0.05).Conclusions The traditional Chinese and Western medicine combined nursing intervention can effectively relieve the postoperative pain in patients after hip replacement , and is in favor of the rehabilitation of joint function so as to improve the quality of clinical nursing .

Objective:To construct the nutritional intervention regimen for elderly patients with hip fractures during the perioperative period and rehabilitation period. Methods:Based on Stetler evidence-based practice model, 151 elderly patients with hip fractures surgery who were admitted to Jiading District Central Hospital Affiliated to Shanghai University of Medicine and Health Sciences from January to May 2019 were investigated, and nutritional problems were found in the perioperative period and 3 months after surgery. Through systematic literature search, the best evidence was extracted to construct a nutritional intervention regimen for elderly hip fractures patients with oral nutrition supplements (ONS) . The expert meeting method was adopted to demonstrate the scheme and determine the final intervention scheme and process. Results:A total of 7 guidelines were included, and 25 recommendations were formed based on evidence, including 24 strong recommendations and 1 weak recommendation. Nutritional intervention scheme and process for elderly patients with hip fractures, mainly ONS, were determined. The expert authority coefficient was 0.89.Conclusions:In this study, the nutritional intervention regimen for elderly patients with hip fractures, mainly ONS, was constructed with a high degree of expert authority and reliable results, providing a theoretical basis for clinical nutrition intervention for elderly patients with hip fractures in the perioperative period.
