1.Research progress of Traditional Chinese Medicine treating IgA nephropathy from pharynx

Clinically, patients with IgA nephropathy often have symptoms of sore throat or discomfort in the pharynx, which needs to be paid attention to. Traditional Chinese Medicine has certain effect in the treatment of IgA nephropathy, starting from the pharynx.There are many commonly used herbal medicines that could relieve wind, clear heat, detoxify the body and relieve sore-thoat, such as platycodon grandiflorum, dandelion, burdock and other herbs, and pecking treatment could be added to increase the curative effect.
2.Research progress of microRNAs in colorectal cancer therapy

The molecular targeted therapy method using microRNAs(miRNAs)is gradually stepping into people′s vision. miRNAs affect colorectal cancer progress via abnormal expression in tumor cells or micro-environment. The high or low expressions of miRNAs in specific tissues probably have an impact on the expressions of oncogenes,tumor suppressor genes or other aspects including the surrounding environments,the metastases and the drug tolerance of tumors,thus contributing to curing the colorectal cancer. Nowadays, miRNA has entered the stage of animal experiments.
3.Subjective Evaluation of Mental Fatigue and Changes of Attentive Characteristics Under Condition of Driving Sitmulation

Objective:To explore the change of attentive characteristics and subjective evaluation of mental fatigue un-der the condition of driving simulation.Methods:16 healthy taxi drivers(all males) were involved in this research.The change of attentive characteristics and the self-rating scale for subjective fatigue symptoms were both recorded before and after six-hour driving simulation.Because of the own control design,the drivers' attentive characteristics and self-rating scales were measured again in their off days.Results:There was significant difference in the changes of attentive charac-teristics in the experimental group,but no significant difference in the control group.And there was no significant differ-ence of subjective evaluation either in the experimental group or control group.Conclusion:Under the condition of driv-ing simulation,the attentive characteristics of drivers were changed,say,they were impaired,even though they did not per-ceive the feelings of fatigue.
4.Laparoscopic Ovarian-conserving Surgery for Ovarian Cyst Pedicle Torsion:Report of 62 Cases

Objective To explore the value of laparoscopic ovarian-conserving restoration for ovarian cyst torsion . Methods A total of 62 cases of ovarian cyst torsion , 12 of which were combined with pregnancy , between January 2007 and June 2012 were retrospectively analysed .The surgical management involved laparoscopic high ligation of ovary blood vessels , restoration of the ovary, and resection of ovarian cysts . Results All the 62 cases of laparoscopic restoration were successfully performed with ovarian reservation.The cysts twisted for 180°~1080°, including 19 cases of <360°, 21 cases of 360°~720°, and 22 cases of >720°.The cysts were coloured purple black in 26 cases, purple or normal in 36 cases.The operation time was (57 ±23) min, the intraoperative blood loss was 5-130 ml (mean, 50.6 ml), and anal exsufflation time was (24 ±13) hours.No embolism, infection, and abortion occurred during post-operation.The level of estrogen recovered to normal in 1-3 months postoperatively .Ovulation at the procedure side was found in 43 cases in 6-24 months. Conclusions Laparoscopic ovarian-conserving surgery is a safe procedure for ovarian cyst pedicle torsion .High ligation of ovarian blood vessles and resection of ovarian cysts can not only avoid the possibility of thrombosis but also preserve the ovarian functions .The shortage of this operation is complicated to perform .
5.The relationship of serum vitamin D3 level and inflammatory factor in primary hypertensive patients

Objective To study the relationship between serum 1,25 -dihydroxy vitamin D3 level and inflammatory factor in elderly primary hypertension patients.Methods Sixty -eight primary hypertension patients were selected as hypertension group,and 60 healthy subjects were selected as control group in this study.The serum 1,25 -dihydroxy vitamin D3 level and plasma inflammatory factor were measured and compared.Results The serum 1,25 -dihydroxy vitamin D3 level in the hypertension group was significantly lower,while plasma inflammatory factor in the hypertension group was significantly higher than in the control group(t =6.445,7.326,8.174,8.556,all P <0.01).Pearson correlation analysis showed that the serum 1,25 -dihydroxy vitamin D3 level was negatively related with systolic blood pressure,plasma diastolic blood pressure and inflammatory factor (r =-0.425, -0.367, -0.254, -0.263, -0.296,all P <0.05).Logistic repression analysis revealed that the inflammatory factor was a risk factor for hypertension,while the 1,25 -dihydroxy vitamin D3 level was a protecting factor for primary hypertension. Conclusion The serum 1,25 -dihydroxy vitamin D3 level is significantly lower in elderly primary hypertension patients than in healthy subjects.The serum 1,25 -dihydroxy vitamin D3 level and the inflammatory factor are predic-tive factors for hypertension.
6.Application of bedside continuous blood purification in the treatment of neonatal multiple organ failure

Objective To explore the efficacy and safety of bedside continuous blood purification (CBP) in the treatment of neonatal multiple organ failure (MOF).Methods Totally 6 newborn infants of MOF were hospitalized in department of neonatology in our hospital from June 2011 to June 2013.These 6 cases of clinical data were retrospectively analyzed,6 neonates were treated with CBP combined with conventional treatment.The model for CBP was continuous veno-venous hemodialysis filtration (CVVHDF),blood flow velocity was 3 to 5 ml/(kg· min),replacement fluid dose was 20 to 30 ml/(kg· h),dialysis fluid dose was 15 to 25 rnl/(min· m2).The clinical outcome measures included,blood pressure,blood pH,K+,Na+,blood urea nitrogen,creatinine,urine volume,PaO2/FiO2 and epinephrine intravenous dose,respectively before CBP treatment,6 h,12 h,24 h,48 h after CBP treatment and the end of CBP treatment.The efficacy of CBP treatment was evaluated in neonatal MOF.Results Gestational age of 6 neonates with MOF was 33 to 41 weeks,2 to 19 days old,2.25 to 3.36 kg birth weight.Primary disease was 4 cases of neonatal septicemia(1 case with congenital hereditary metabolic disease),2 cases of severe neonatal asphyxia.All 6 cases of venous catheter were smoothly done.CBP treatment persisted for 49 to 106 hours.Compared with before CVVHDF treatment,blood K+,blood urea nitrogen,creatinine significantly decreased at 12 h after CVVHDF treatment [(5.32 ± 1.84) mmol/L vs.(9.81 ±3.61) mmol/L,(9.0 ±3.4) mmol/L vs.(12.8 ±6.1) mmol/L,(99 ± 16) μmol/L vs.(176 ±25) μmol/L,P <0.05],and reached the normal range at 24 h after treatment,urine volume significantly increased at 24 h after treatment (P < 0.05).PaO2/FiO2 reached 200 mmHg (1 mmHg =0.133 kPa) at 6 h after treatment and more than 300 mmHg at 24 h after treatment(P <0.05).Fifty percent of epinephrine intravenous dose were down-regulation at 12 h after treatment and stopped using epinephrine at 48 h after treatment.CBP treatment of 6 cases showed effective.Conclusion Application of bedside CBP treatment in neonatal MOF is safe,can effectively help neonates with MOF to skip over renal failure stage.
7.Intergrated analysis of CYP3A5 and ABCB1 gene polymorphisms in renal transplanted recipients treated by Tacrolimus

Objective To approach the effect of CYP3A5 6986A>G (rs776746) and ABCB1 3435C>T (rs104564) on plasma tacrolimus(FK506) dose adjusted blood trough concentrations(C0/D) in kidney transplanted patients.Method Totally 125 renal transplanted recipients were recruited,and gene sequencing method were used to detect the genotype of CYP3A5 6986A>G and ABCB1 3435C> T.FK506 trough concentration of 125 patients was detected by enzyme multiplies immunoassay technique (EMIT) after the surgery over 6 month.The relevance between the ratio of trough concentration/dosage and polymorphisms was analyzed.Result In 125 renal transplanted receipients,the allele frequency of CYP3A5 and ABCB1 genetic variants was 74% and 34.8%,respectively.After 6 months,the C0/D ration in the homozygous mutant CYP3A5 * 3/* 3 receiptients was significant higher than that in the homozygous wild type * 1/* 3 and heterozygous mutant * 1/* 1 receiptients (P < 0.05).And there was no significant difference of the Co/D ration between CYP3A5 * 1/* 1 and CYP3A5 * 1/* 3 receipients(P>0.05).The the C0/D ration in homozygous wild type ABCB1 T/T and homozygous mutant T/C receipients was significant higher than that in heterozygous mutant C/C receipients (P<0.05),and there was no significant difference of the C0/D ration between T/C and T/T receipients (P> 0.05).Conclusion The study shows that the the polymorphism of CYP3A5 and ABCB1 genes was significantly correlated with the trough C0/D value of the recipients after renal transplantation who were treated by Tacrolimus for more than half a year.The detection of the SNP of the CYP3A5 and ABCB1 will be useful for FK506 dosage adjustment.
8.Painful heating for evaluating trigeminal small nerve fiber function in patients with migraine

Objective To study any functional changes in the trigeminal nociceptive system in cases of migraine with aura (MWA) using painful heat-evoked potentials (CHEPs),and to explore the value of CHEP in evaluating the trigeminal small nerve fibers of patients with MWA.Methods Sixty patients with MWA were recruited as the MWA group,and 60 healthy persons were selected as a normal control group.Thermal stimulation at 51 ℃ was applied to the skin above the patients' right and left eyebrows.The N wave latencies and N-P amplitudes of the CHEPs were recorded at the vertex (Cz) of the head.The intensity of the stimulus was graded using a visual analogue scale (VAS).Results In the control group the simulation produced no significant differences in the N550 or P750 latencies or in the N-P amplitudes.In the MWA group no significant differences were observed when the headache side was stimulated,but the N550 and P750 latencies were both significantly shorter than those of the control group and the N-P amplitude was significantly greater than that of the control group.The average VAS score on both sides in the patients with migraine was significantly higher than those of the control group.However,there was no significant difference in average VAS scores between the headache and non-headache sides of the patients with migraine.Conclusions CHEP is a reliable method for detecting any pathological changes in the trigeminal small nerve sensory pathway.Increased cutaneous allodynia in acute migraine attacks is detectable using CHEPs.
9.Clinical Observation of Tiao Ren Tong Du Needling plus Bloodletting at Jing-well Points for Post-stroke Spastic State MA

Objective To observe the clinical efficacy of Tiao Ren Tong Du (regulating the Conception Vessel and unblocking the Governor Vessel) needling method plus bloodletting at the Jing-well points in treating post-stroke spastic state. Method Sixty-seven eligible patients were randomized into a group to receive Tiao Ren Tong Du needling plus bloodletting at the Jing-well points (treatment group) and a rehabilitation group (control group). For the two groups, the treatment was given once a day, 6 times as a treatment course, for 4 courses in total. The Modified Ashworth Scale was used to evaluate the spasticity intensity, and the Patient Reported Outcome (PRO) for post-stroke spastic hemiplegia was used to measure the patients from the following four aspects:subjective symptoms, objective symptoms, mental state, and social function. Result After intervention, the scores of spasticity, and physiological, mental, social, and environmental factors were significantly improved in both groups (P<0.01,P<0.05). Compared to rehabilitation, Tiao Ren Tong Du needling plus bloodletting at Jing-well points can more significantly release the muscular spasticity and reduce the PRO score (P<0.01). The recovery plus markedly-effective rate was 70.6% in the treatment group versus 42.4% in the control group. Conclusion Tiao Ren Tong Du needling plus bloodletting at Jing-well points can effectively improve post-stroke spastic state and the quality of life.
10.Serum microRNA profiles as novel biomarkers for the post-operative evaluation and survival of patients with glioblastoma multiform

Objective:To investigate the differentially expressed miRNAs in serum collected post operation and compared these miR-NAs with those collected pre-surgery among patients suffering from glioblastoma multiform (GBM) and undergoing regular clinical fol-low-up. These miRNAs may be potential biomarkers for the post-operative evaluation of patients with GBM. Methods:Forty-eight pa-tients with GBM and clinical pathological diagnosis were enrolled in this study. In the initial biomarker screening stage, total RNAs were extracted and subjected to Solexa sequencing to select miRNAs with significantly altered expression pre-and post-operation. Some of these differentially expressed miRNAs were chosen and verified through TaqMan probe-based qRT-PCR assay. A t-test was performed to determine the miRNAs that satisfied the two criteria, namely, fold change>2 and P<0.05. All of the patients were fol-lowed-up, and survival data were collected. The patients were then classified into two groups, namely, long-and short-survival groups, on the basis of the median of the miR-30e expression levels in the sera collected post-operation. Kaplan-Meier method and Log-rank test (SPSS version 19.0, IBM) were employed to determine the possible relationships between miR-30e expression levels in the sera collected post-operation and patients' overall survival. Results: Solexa revealed 63 differentially expressed miRNAs. Four miRNAs, namely, miR-26b, miR-30e, miR-129-3p, and miR-206, were selected on the basis of previous and present findings. These miRNAs were then verified in the RT-qPCR phase. Among these miRNAs, only miR-30e was significantly upregulated post-operation. The serum miR-30e expression level post-operation was not significantly associated with the overall survival of the patients. A low miR-30e expression level corresponded to prolonged survival. Conclusion:miR-30e was upregulated in the sera collected post-operation from patients with GBM. This miRNA may be negatively related to the tumor load of these patients. The miR-30e expression level in the serum col-lected post-surgery serum was not significantly associated with overall survival. Therefore, miR-30e may serve as a novel potential non-invasive biomarker for the post-operative evaluation of patients with GBM.
