1.Effect of prone positioning on oxygenation and lung mechanics in neonates with mechanical ventilation

Objective To investigate the effects of prone positioning on oxygenation and lung mechanics in neonates with mechanical ventilation.Methods Twenty cases of neonates,selected from Shanghai Children′s Hospital(from May 2008 to Feb 2009),were randomized to supine-prone position group (n=10) and prone-supine position group (n=10) using a crossover design.During 6h,these neonates were placed in the supine position for 3h then in the prone position for 3h or in reversed order.The ventilator parameters,results of arterial blood gas and the lung mechanics were recorded.Results Without obvious changes of ventilator parameters,PaO2 improved significantly when the incubated neonates were placed in the prone position for 3h compared to supine [(74.50±15.37)mm Hg vs (67.60±13.66)mm Hg,1mm Hg=0.133kPa,P<0.05).Similarly,oxygenation index of prone positioning for 3h was (201.86±55.96)mm Hg,which was higher than that of (184.38±63.58)mm Hg,the value of supine positioning for 3 h.There were significant differences (P<0.05).Our results showed that 60% (12/20) of the neonates responded to prone positioning,with oxysenation index increasing as much as 17mm Hg on average.Improvement in minute volume,tidal volume and dynamic compliance of the lung were achieved after prone positioning for 3h (P=0.011,0.020,0.045).The airway resistance did not decrease significantly (P=0.422),but the average decreased in the prone position.Conclusion Oxygenation and the lung mechanics are improved significantly in the neonates with prone position compared with supine position.
2.Lentivirus-based RNAi and its application in the study of T lymphocytes

The technique of lentivirus-based RNAi ( RNA interference) is a combination of the effective infec-tion and integrating characteristics of lentivirus vector and the specificity of RNAi to silence a homologous gene expression. It caused the post-transcriptional gene silencing ( PTGS) by a specific target sequence of mRNA degradation.As a kind of commonly used target cells in gene therapy, T lymphocytes play an important role in graft-versus-host disease ( GVHD) and other diseases.It is of great significance for increasing the infection efficiency of the target gene to T lymphocytes on this kind of disease for gene therapy.In this paper, we summarize the application of lentivirus-based RNAi in T lymphocytes.
3.Advance in the diagnosis and treatment of hepatoblastoma in children

Hepatoblastoma (HB) is the most common malignant tumor of the liver in children,and it is most prevalent in children under 5 years old.The overall prognosis of childhood HB has been improved significantly due to multidisciplinary team approach consisting with surgical resection and combination chemotherapy.Nevertheless,the clinical outcomes of metastatic and relapsed HB remain to be poor.This paper makes a brief review on the pathogenesis,diagnosis and treatment of HB.
4.Study of fractional amplitude of low frequency fluctuation and regional homogeneity in idiopathic generalized epilepsy based on resting-state functional MRI

Objective To observe the changes of whole brain function in idiopathic generalized epilepsy(IGE).Methods Three-dimensional structure scan and resting-state functional scan were performed in 23 cases of IGE patients and 23 health controls at 3.0T super-conducting MRI scanner.Brain functional analysis by fALFF and ReHo methods were obtained and the changes of brain areas were compared.Correlation analysis between different brain regions of IGE patients with the course of disease were performed.Results Compared with normal controls,IGE group showed increasd fALFF in the bilateral precentral gyrus,left supplementary motor area, left cingulate gyrus,left paracentral lobule,left superior occipital gyrus,left calcarine cortex,left middle occipital gyrus and right precuneus;decreased fALFF in the bilateral inferior temporal gyrus,right parahippocampa gyrus,right insula,right precuneus and left inferior parietal lobule(P<0.005).IGE group showed increasd ReHo in the left calcarine cortex,left superior parietal lobule, left postcentral gyrus,right precentral gyrus;decreased ReHo in the right fusiform gyrus,left lentiform nucleus,right inferior frontal gyrus,right superior medial frontal gyrus,left middle occiptal gyrus,right insula and bilateral inferior parietal lobule(P<0.005). These different brain regions in fALFF and ReHo results for IGE patients were no correlated with the course of disease.Conclusion IGE patients having wide range of brain areas with abnormal changes in function is the basis of the IGE complex clinical manifestations of the nerve.Combined application of two analysis methods of RS-fMRI can evaluate the change of brain function more comprehensively, and provide functional neuroanatomical evidence for the researches on neuro pathogenesis mechanism of IGE.

6.Analysis of molecular classification and clinical characteristics in 36 patients with male breast cancer

Objective To analyze molecular classification and male breast cancer (MBC) triggers,clinical characteristics in patients with MBC and evaluate the prognosis.Methods 36 MBC patients enrolled in the past 5 years were retrospectively analyzed.100 patients with women breast cancer (WBC) in the same period were randomly selected as control group.The primary pathological type of MBC was the infiltrating ductal carcinoma.According to the expressions of estrogen receptor (ER),progesterone receptor (PR),and the epidermal growth factor receptor 2 (Her-2),the MBC can be divided into 4 kinds of molecular subtypes,Luminal type A,Luminal type B,Her-2 type,and Basal-like type.Results The ratio of MBC patients with grade Ⅲ was lower in the Luminal group A than that in the Luminal group B.There were significant differences (x2 =1.197,P < 0.05) between these two groups.Conclusion The incidence rate of MBC is low but the prognosis is poor.The primary pathological type is the infiltrating ductal carcinoma.The ratio of MBC patients with grade Ⅲ is lower in the common Luminal group A than that in the common Luminal group B.

9.The mediation effect of psychiatric nurses' communication ability on the relationship between personality characteristics and conflict management patterns

Objective:To understand the relationship between psychiatric nurses' communication ability, personality characteristics and conflict management patterns, and to provide a basis for training psychiatric nurses' conflict management ability courses.Methods:A convenient sampling method was used to investigate and investigate 208 nurses in the first-line clinical hospitals of a third-class psychiatric hospital, used the nurses′ clinical communication ability scale, the nurse′s interpersonal conflict management morphology scale, and the simplified version of the Chinese Big Five Personality Questionnaire.Results:Integration dimension (18.67±2.58) points, concession dimension (12.93±2.43) points, escape dimension (14.05±2.02) points, dominance dimension (14.49±3.06) points, and integration dimension and gender ( t value was -2.57, P=0.01)), education ( F value was 3.76, P=0.02), technical title ( F value was 3.03, P=0.02), position ( t value was 2.08, P=0.03), whether to participate in nurse-patient communication training ( t value was 2.54, P=0.02). Nurses with pleasant personality could better deal with interpersonal conflict ( r=0.361, P<0.01), and the nurse′s personality had a direct effect on the form of interpersonal conflict ( P=0.036, 95% CI 0.107-0.422), the nurse′s communication ability plays a part of indirect effect ( P=0.031, 95% CI 0.001-0.063). Conclusion:The form of interpersonal conflict management of psychiatric nurses was closely related to communication ability and personality characteristics. Nursing managers could carry out targeted training to improve the conflict management ability of nursing staff.
