1.Research progress in ethics related issues of xenocorneal transplantation

Objective To discuss and study the ethics related issues of xenocorneal transplantation.Methods The ethics related issues of xenocorneal transplantation were discussed through analysis of the cornea characteristics and present situation of corneal transplantation.Results The ethical problems faced by corneal transplantation mainly consisted of technical barriers social problems:immune rejection after corneal transplantation; species chosen whether or not to meet the ethical requirements,whether to violate the ethics; animal rights issues; whether can cause characteristics change of xenocorneal transplantation person; whether xenocorneal transplantation will lead to the spread of deadly diseases,and endanger public safety.Conclusions Xenocorneal transplantation can help patients regain their sight,its success requires not only the innovation breakthrough of science and technology,but also the attention of people on ethical issues of xenocorneal transplantation.
2. Effects of whole- course MDT nutrition management on radiotherapy in elderly patients with esophageal cancer

Objective: To explore the effect of MDT nutrition management on the radiotherapy process of elderly patients with esophageal cancer. Methods: From January 2018 to September 2019, 60 elderly patients with esophageal squamous cell carcinoma were selected, and randomly divided into the control group (Group A, 30 cases) and the experimental group (Group B, 30 cases). All the groups received radiotherapy in The First Hospital of Shanxi Medical Univesity. The Group A was treated with routine nutrition intervention and the Group B with multidisciplinary team (MDT) nutrition intervention. Body mass index (BMI), nutritional status, the incidence of adverse reactions and treatment time were eraluated respectively. Results: The patient-generated subjective global assessment (PG-SGA) score and the incidence of moderate to severe malnutrition in Group A (86.3%) were significantly higher than those in Group B (70%); the PG-SGA score after radiotherapy was higher than that before radiotherapy in both groups, and it was evidently higher in Group A (P<0.05). Total protein (TP), albumin (ALB) and prealbumin (PA) decreased first and then increased in Group A, but they increased steadily or gradually in Group B. WBC and Hb decreased significantly in Group A compared with those in Group B (P< 0.05). In comparison with Group A, the levels of ALB, PA, TP and BMI in Group B were increased after radiotherapy (P<0.05), the incidence of acute mucosal reaction above grade II (13.3%) and myelosuppression (26.3%) was lower (P<0.05), the quality of life was better (P<0.05) and the total treatment time was shorter [(41.97±2.59) d] (P<0.05). Conclusions: The total course of MDT nutrition management significantly improved malnutrition, reduced the incidence of adverse reactions and total treatment time, and improved the quality of life.

4.Association study between phosphodiesfierase-4A SNPs and bipolar disorder in the Han Chinese population

Objective To explore wbether there is an association between the single nucleotide polymorphisms (SNPs) of phosphodiestierase-4A (PDE4A) and bipolar disorder in Han population.Methods A casecontrol association study was done in this study,432 bipolar disorder patients(patients group),and 569 age and gender-matched controls(control group)were recruited.Real-time fluorescent quantitative polymerase chain reaction (PCR) was used to detect genotypes and alleles distributive frequency of rs1051738 and rs7256672 of 432 bipolar disorder patients and 569 normal people.Results The PDE4A rs1051738 genotypes and alleles frequency distribution between patients group(AA,AC,CC genotypes:12%,8%,80%,A,C alleles:16%,84%) and control group (AA,AC,CC genotypes:2%,19%,79%,A,C alleles:12%,88%) showed statistically significance (P< 0.01).The frequency of PDE4A rs7256672 genotypes and alleles distribution between patients group(GG,GT,TT genotypes:18%,50%,32%,G,T alleles:43%,57%) and control group (GG,GT,TT genotypes:20%,48%,32%,G,T alleles:45%,55%)showed no significant difference(P=0.620,P=0.446).Conclusion The PDE4A rs1051738 polymorphism may be responsible for an increase in susceptibility to bipolar disorder,indicating that it maybe a functional site with marked significance to bipolar disorder.
5.Correlation of serotonin 2A receptor and adenylate cyclase single gene polymorphisms and bipolar disorder

Objective To explore whether there is association between the polymorphisms of serotonin 2A receptor(5-HT2A) rs6311,rs6305 and adenylate cyclase rs2230739 with episode of bipolar disorder.Methods A case-control association study was done in this study,152 bipolar disorder patients (patients group),and 187 ageand gender-matched controls (control group) were recruited.Ligase detection reaction(LDR) was used to detect the distributive frequency of rs6311,rs6305 and rs2230739 of 152 bipolar disorder patients and 187 normal people.Results The 5-HT2A receptor rs6311 genotypes and alleles frequency distribution between patients group(11,12,22 genotypes:21%,61%,18% ; 1,2 alleles:51%,49%) and control group(11,12,22 genotypes:44%,45%,11% ; 1,2 alleles:66%,34%) showed statistically significance (P<0.01).There were no differences of 5-HT2A receptor rs6305 genotypes and alleles between patients group and control group.The frequency of rs2230739 genotypes and alleles distribution between patients group (11,12,22 genotypes: 20%,43%,37% ; 1,2 alleles: 42%,58%) and control group(11,12,22 genotypes: 17%,48%,35% ; 1,2 alleles: 41%,59%) showed no significant difference(P=0.661,P=0.846).Conclusion The 5-HT2A receptor rs6311 polymorphism may be responsible for an increase in susceptibility to bipolar disorder,indicating that it maybe an marker to bipolar disorder.
6.CT-guided 125I seed implantation for the treatment of lumbar lymph node metastases: a preliminary discussion on the puncturing approaches

Objective To discuss the commonly used puncturing approaches in CT-guided 125I seed implantation for lumbar lymph node metastases in order to provide safe and reliable technical guidance for clinical practice.Methods Under CT guidance,125I seed implantation for lumbar lymph node metastases was performed.According to different locations of metastatic lymphadenopathy (left waist,right waist or middle waist),the corresponding puncture route and implantation method were adopted.Meanwhile,different puncturing approach was designed in order to avoid damage to vital organs.Results For the performance of 125I seed implantation for the lymphadenopathy located at the left waist,right waist and middle waist,the commonly used puncturing approaches were percutaneous transthoracic lumboiliac costal muscle method (i.e.back approach),trans-hepatic trans-duodenal method (i.e.lateral approach) or back approach method,and trans-mesenteric approach together with coaxial needle method (i.e.anterior approach) respectively.Conclusion It is clinically feasible to use different puncturing approaches in performing 125I seed implantation for lumbar lymph node metastases,the suitable puncturing approach can ensure a successful and safe operation.
7.Experimental Study of the Effect of Fire Needling on Pain Threshold, and IL-1 and TNF-α in Rheumatoid Arthritis Rats

Objective To compare the analgesic effects of fire needling, conventional acupuncture and acupoint catgut embedding in rats with rheumatoid arthritis.Methods Fifty healthy male SD rats were randomized into control, model, acupuncture, fire needling and catgut embedding groups, 10 rats each. A SD rat model of rheumatoid arthritis was made by intracutaneous injection of complete Freund’s adjuvant into the right hind foot pad. Right Kunlun and Zusanli were selected as therapeutic points in the three groups and given fire needling, acupuncture or catgut embedding. The right paw mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL) were used as observation indices and measured in every group of rats at one day before and four days after model making and at one, seven and fourteen days after the beginning of treatment. Rat paw inflammatory tissues were separated after the completion of treatment. The IL-1 and TNF-α contents of them were measured. Results There were statistically significant differences in MWT and TWL between the model, acupuncture, fire needling or catgut embedding group and the control group after model making, and one, seven and fourteen days of treatment (P<0.01). There were statistically significant differences in MWT and TWL between the fire needling group and the model, acupuncture or catgut embedding group after one, seven and fourteen days of treatment (P<0.01,P<0.05). There was a statistically significant difference in MWT between the acupuncture or catgut embedding group and the model group after one, seven and fourteen days of treatment (P<0.01). There was a statistically significant difference in TWL between the acupuncture or catgut embedding group and the model group after seven and fourteen days of treatment (P<0.01,P<0.05). There were statistically significant differences in IL-1 and TNF-α contents between the model, acupuncture, catgut embedding or fire needling group and the control group after model making (P<0.01). after treatment, there were statistically significant differences in IL-1 and TNF-α contents between the acupuncture, catgut embedding or fire needling group and the model group and between the acupuncture or catgut embedding group and the fire needling group (P<0.05).Conclusions Fire needling can raise pain threshold in rats with rheumatoid arthritis. It alleviates rheumatoid arthritis-induced chronic pain by reducing the inflammatory factor content of affected muscular tissues.
8.Study on the expression and signification of TLR4/NO pathway in cervical tumorigenesis with high risk HPV infection

Objective To explore the role of toll-like receptor (TLR)/nitric oxide (NO) pathway in cervical tumor with high risk human papillomavirus (hrHPV) infection.Methods (1)Study was based on 36 women with nonmalignantcervical tissue as control group and 36 women with squamous cell cervical cancer (SCC),all with hrHPV infection which were assessed by using 14 types hrHPV E6/E7 mRNA real-time PCR kit.The amount of NO was detected by Griess reaction,the expression of inducible nitric oxide synthase (iNOS) was detected by immunohistochemistry (IHC).The mRNA expression of TLR3,TLR4,TLR7,TLR8,TLR9,nuclear factor-κB (NF-κB) p65 and iNOS in control and SCC epithelium which was captured by laser capture microdissection (LCM) were determined.(2)The expressions of TLR4 in CaSki,HeLa and C33a were detected by cell immunofluorescence method.The mRNA and protein expression of TLR/NO pathway transduction molecules including TLR4,NF-κBp65 and iNOS in CaSki,HeLa and C33a cell lines were detected by real-time PCR and western blot.Results (1)The level of NO was much higher in SCC group than that in control group [(42.92±0.36) μmol/L vs (15.49±0.24) μmol/L;P<0.05].iNOS was detected in 75% (27 cases) of patients with squamous cervical carcinoma,while only 6% (2 cases) of normal controls were confirmed with positve result (P<0.05).TLR/NO pathway maybe activated in SCC,for the mRNA levels of TLR3,TLR4,TLR7,TLR8,NF-κBp65 and iNOS increased significantly when compared to control group (all P<0.05),and the greatest change in the expression level of TLR in SCC was spotted on TLR4(7.41±0.39 vs 1.86±0.21).(2)The results of immunofluorescence showed that TLR4 was located at plasma membrance of hrHPV positive HeLa and CaSki cells,while the integral optical density of TLR4 in HeLa cells (3 599±427) or CaSki cells (2 080±456) were higher than that in C33a cells (730±96;P<0.05).The mRNA and protein level of TLR4,NF-κBp65 and iNOS in HeLa and CaSki cells were higher than those of C33a cells (P<0.05).Conclusion TLR4/NO pathway is highly expressed in cervical cancer with hrHPV infection,while the pathway may be involved in cervical tumorigenesis with hrHPV infection.
9.Investigation on the cognition and needs of integrating old-age care with medical service among different population groups

Objective To investigate the cognition and attitude toward integrating old-age care with medical services among the elderly,nurses from medical institutions and old-age care institution staffs,in order to provide references for developing implementation strategies of integrating old-age care with medical service.Methods The convenience sampling method was conducted,and 963 people.from 4 cities in Henan Province were enrolled in this survey from July to September 2016,including 599 elderly people,248 nurses from medical institutions and 166 old-age care institution staffs.All volunteers were investigated by using self-designed questionnaires.Results A total of 1 000 questionnaires were distributed,and 963 valid questionnaires were retrieved.The effective recovery rate was 96.3%.The cognition of integrating old-age care with medical service in all volunteers was low.There were differences in the needs of all nursing care services in the integrative old-age care and medical services,the needs of Chinese traditional medical care services were low.The highest demand of service capabilities was skills of coping urgent issues,while the lowest demand of service capabilities was education levels of care givers;the scores on evaluation of the two demands in the elderly were (4.803± 0.453) and (3.124± 1.246) points,those in the old-age care institution staffs were (4.819±0.468) and (2.698± 1.346) points,there was no statistically significant difference between the two groups (P>0.05).Conclusion In the progress of promoting the integration of old-age care and medical services,it is necessary pay more attention on the promotion among non-urban elderly people,and improving living facilities as well as the quality of care services.Integrated management and hierarchical training for nursing professionals and old-age care institutions staffs should be carried out gradually.
10. Effect of mandibular distraction osteogenesis on quality of life in children with Pierre-Robin sequence

Objective:
To explore the impact role of Mandibular Distraction Osteogenesis technique (MDO) on the health quality of life in children with Pierre-Robin Sequence (PRS).
Methods:
From January 2017 to August 2017, 44 cases of children with PRS admitted in children′s hospital of Nanjing Medical University were recruited. Glasgow Children′s benefit inventory(GCBI), Chinese version was used to study postoperative qualify of life in four aspects: physiology, emotions, learning, and vitality. Clinical data and the quality of life score were collected from the patients, and comparison analysis was performed.
Results:
GCBI total score of the 44 cases of PRS treated with MDO surgery was 46, indicating that the overall quality of life was improved; Physical dimension score was 72, which was the highest, showing the most positive changes. GCBI total scores for the children with different types of PRS were type Ⅰ62, type Ⅱ43, type Ⅲ18, respectively. Nonparametric test showed that quality of life in typeⅠpatients was improved compared to typeⅢ. The difference was statistically significant (
