1.Application of high resolution melting curve analysis technique in detection ofβ-thalassemia gene mutations

Objective To investigate the feasibility of high resolution melting(HRM)curve analysis technique for detecting β-thalassemia gene mutations.Methods The HRM method was used to detect five kinds of common β-thalassemia mutations (-28, CD17,CD41-42,CD71-72 and IVS-2-654)in Guangdong province.Sixty specimens of the patients with suspectedβ-thalassemia were performed the HRM analysis.The results of HRM analysis were confirmed by the direct DNA sequencing.Results Among 60 specimens of the patients with suspectedβ-thalassemia,12 cases of heterozygosis mutant gene were detected,including 3 cases of-28,2 cases of CD17,5 cases of CD41-42,2 cases of CD71-72 and 2 cases of IVS-2-654 gene mutation;2 cases of homozygosis mu-tant gene were detected,including 1 case of-28 and 1 case of CD41-42 homozygous mutation (both were amniotic fluid specimens). The results of HRM analysis were consistent with the DNA sequencing results.Conclusion The HRM method can accurately de-tect five kinds of common β-thalassemia mutations in Guangdong province,has the advantages of simpleness,rapidness and high sensitivity and is expected to be a new method for screening β-thalassemia mutation in clinic.

Objective To evaluate the clinical effect of light cure Dyractflow (LCDF) in pit and fissure enamel-plasty sealant technique (EST). Methods A total of 400 first permanent molars in 200 patients aged from 6 to 10 years were treated with LCDF in occlusal pit and fissure enamel-plasty sealant technique. The opposite permanent teeth treated with LCDF in occlusal pit and fissure conventional sealant technique (CST) were employed as the controls. The patients were followed up for 3 years. Objective To explore the distributive changes of PIDD protein in HT29 cells stimulated by 5-fluorouracil(5-FU),and the influence of the PIDD protein interfered by small interference RNA(siRNA) on the drug resistance of HT29 cells in vitro.Methods siRNA was used to interfere the PIDD expression,and HT-29 cells were treated with 5-FU.Western blotting was employed to detect the PIDD expression before and after interference.The distribution of PIDD in nucleus and cytoplasm after 5-FU treatment was also detected.Changes in drug sensitivity of HT-29 cells to 5-FU were determined with MTT assay and IC50 values were evaluated.Results PIDD protein was expressed mainly in the cytoplasm before 5-FU stimulation,and it migraded into the nucleus after stimulation.Western blotting showed that the total PIDD expression and the expression both in cytoplasm and nuclei decreased significantly after RNA interference,and no increase of the total PIDD and that migrated into nucleus was found even after 5-FU stimulation.It was found that in the transfection group(cells were incubated 12h after being transfected with PIDD-360),normal group(cells without treatment) and control group(cells were incubated 12h after being transfected with negative transfection reagent),the IC50 values of cells treated with 5-FU were 0.23?0.06?g/ml,2.71?0.70?g/ml and 2.78?1.03?g/ml,respectively.The IC50 value declined significantly in transfection group compared with that of both the normal group and control group(P

6.Efficacy and safety of oral ibuprofen for closure of patent ductus arteriosus in preterm infants: a meta-analysis

Objective To evaluate the efficacy and safety of oral ibuprofen for closure of patent ductus arteriosus (PDA) in preterm infants.Methods The related literatures till December 31st,2011 in Cochrane Library,PubMed,EMBASE,Ovid,Springer,China Academic Journal Full-text Database,Wanfang Database,VIP Database and China Biological Medical Literature Database were searched.The inclusion criteria were:(1) the subject of the research was preterm infants with birth weight less than 2500 g and/or gestational age less than 37 weeks; (2) randomized or semirandomized controlled trial; (3) the intervention group received oral ibuprofen,while the control group received oral placebos / intravenous indomethacin or ibuprofen; (4) the main outcome was the failure rate of PDA closure; (5) hemodynamic changes with PDA were detected by ultrasonography.Meta-analysis was performed by Review Manager 4.22 software.Results Eleven randomized controlled trials were included,among which,three were high quality reports.Meta-analysis showed lower failure rate of PDA closure in subjects received oral ibuprofen than in those received placebos orally (RR =0.22,95 % CI:0.14-0.35),while the number of infants required operative closure of PDA decreased significantly (RR =0.16,95% CI:0.03-0.86).Further analysis showed the effect of oral ibuprofen was similar to intravenous indomethacin (RR =0.93,95 % CI:0.57-1.53),but better than intravenous ibuprofen (RR=0.42,95%CI:0.26-0.67).However,oral ibuprofen did not reduced the ratio of patients required operation compared with intravenous indomethacin or ibuprofen (RR=0.58,95%CI:0.24-1.41).The incidence of gastrointestinal hemorrhage was higher in oral ibuprofen group than that in placebos(RR=1.99,95%CI:1.13-3.50).The serum level of creatine was lower in oral ibuprofen group than in intravenous indomethacin or ibuprofen group (weighted average=-19.10,95% CI:-25.12-12.31).Compared with intravenous indomethcin group,less necrotizing enterocolitis cases were identified in oral ibuprofen group,but no statistical significance was found (RR=0.57,95% CI:0.30-1.09).No long-term outcome was reported in any selected literature.Conclusions Oral ibuprofen for PDA closure in preterm infants appears to be as effective as intravenous or indomethacin,and more effective than intraveous ibuproten.Oral ibuproten represents better safety.
7.Evaluation of double vital staining with iodine and methylene blue in diagnosing early esophageal car-cinoma

Objective To evaluate the value of double vital staining with both iodine and methylene blue in diagnosing early esophageal carcinoma. Methods Lugol' s iodine was sprayed on the esophageal lesions first, then methylene blue. The results before or after staining with mythylene blue were compared. The tumor infiltrative depth in the esophagus was assessed by gastric endoscopy and endoscopic ultrasonography (EUS). The tumors were resected surgically or under endoscopy. Results After Lugol's iodine spray it stained the normal esophageal squamous epithelium brown-black, whereas the lesions did not. In 10 cases with 11 lesions, 9 cases (81. 8% ) showed the margin of lesions fairly clear. After spray of methylene blue it stained all the lesions in blue color with clear margin. The mucosal resection was done in one case under endoscopy and surgical resection in 8 cases. All of the 9 cases were diagnosed pathologically squamus carcinoma including mucosal carcinoma, 6 lesions and submucosal carcinoma, 4 lesions. The pathological diagnostic accuracy in differentiating mucosal cancer from submucosal cancer by endoscopy or EUS was 80% ; by both endoscopy and EUS in conjunction was 90%. Conclusion Double vital staining with both iodine and methylene blue is very useful in revealing the area and infiltrative depth of early esophageal cancer.

The new cataloguing rules, RDA, have been implemented in NLM and more RDA bibliographic data can thus be accessed in domestic medical libraries when the Western documents are catalogued by offloading their bibliographic data.The differences in RDA and AACR2 were compared, the common modified fields and descrip-tive contents were described when the RDA bibliographic data were changed into AACR2 bibliographic data in order to provide certain reference for cataloguing of Western documents using the RDA bibliographic data.
9.Therapeutic effect observation of lymphoplasmapheresis in treatment of Guillain-Barre syndrome

Objective To observe the therapeutic effect of lymphoplasmapheresis (LPE) in treatment of Guillain-Barre syndrome (GBS). Methods The initial recovery time of muscle strength, muscle strength score difference, clinical therapeutic effect and the security after treatment with LPE were observed in 34 GBS patients. Meanwhile, 17 GBS patients applied with therapeutic plasma exchange (TPE) named TPE group were served as control group. Results LPE group was treated with LPE 51 times in all, and the mean times was 1.5. TPE group received TPE 33 times in all, and the mean times were 1.9. In LPE group, the average initial recovery time [(12.74?7.18)d] was significantly shorter than that in TPE group [(24.35?14.22)d] (P0.05).Conclusions The therapeutic effect of LPE is distinguished for treatment with GBS and the side effect is rare. It is worth to apply in clinic.
10.Role of μ-opioid receptor in attenuation of bone cancer pain by anti-nerve growth factor in rats

Objective To evaluate the role of μ-opioid receptor (MOR) in attenuation of bone cancer pain by anti-nerve growth factor (anti-NGF) in rats. Methods Part Ⅰ Sixty female SD rats weighing 200-220 g were randomly divided into 4 groups (n = 15 each): sham operation group (group S), sham operation + anti-NGF group (group SN), bone cancer pain group (group P) and bone cancer pain+ anti-NGF group (group PN) . Bonecancer was induced by intra-tibial inoculation of 1 × 105 Walker 256 breast cancer cells in group P and PN. Group S and SN received injection of PBS 10 μl. APE 10 catheter was inserted at L2,3 interspace into the epidural space 13 days after cancer cell inoculation. Three days after the catheter was successfully placed, group SN and PN received intrachecal (IT) injection of anti-NGF 10 μg (in normal saline (NS) 10 μl) and group S and P IT injection of NS 10 μl twice a day for 5 consecutive days. The number of spontaneous flinches (NSF), paw withdrawal latency (PWL) and paw withdrawal threshold (PWT) were measured before and 13, 16, 18, 21 day after cancer cell inoculation. The animals were sacrificed at 21 day after cancer cell inoculation and the spinal cord dorsal horn and dorsal root ganglion were removed for determination of MOR and MOR mRNA expression. Part Ⅱ Thirty female SD rats weighing 200-220 g were randomly divided into 2 groups (n = 15 each): bone cancer pain + anti-NGF group (group PN) and bone cancer pain + naloxone + anti-NGF group (group PNN). Bone cancer was induced by intratibial inoculation of 1 × 105 Walker 256 breast cancer cells. APE 10 catheter was inserted at L2-3 interspace into the epidural space 13 days after cancer cell inoculation. Three days after the catheter was successfully placed,group PN received IT injection of anti-NGF 10 μg (in NS 10 μl) and group PNN IT injection of naloxone 10μg (in NS 25 μl) and 0.5 h later IT injection of anti-NGF 10 μg (in NS 25μl) twice a day for 5 consecutive days. NSF,PWL and PWT were measured before and 13, 16, 18, 21 days after cancer cell inoculation. Results Part ⅠCompared with group S, no significant change was found in NSF, PWL and PWT in group SN, and in MOR and MOR mRNA expression in group SN and PN (P > 0.05), NSF was significantly increased, PWL shortened, PWT decreased at 13-21 days after inoculation in group P and PN, and MOR and MOR mRNA expression was down-regulated in group P (P < 0.05 or 0.01). Compared with group P, NSF was significantly decreased, PWL prolonged, PWT increased, MOR and MOR mRNA expression was up-regulated in group PN at 18-21 days after inoculation (P < 0.05 or 0.01). Part Ⅱ Compared with group PN, NSF was significantly increased, PWL shortened, PWT decreased at 18-21 days after inoculation in group PNN (P < 0.05 or 0. 01). Conclusion The mechanism by which anti-NGF attenuates bone cancer pain in rats is related to the activation of MOR.
