Chinese Acupuncture & Moxibustion 2015;35(4):377-383
OBJECTIVEBy using questionnaire method to understand the evaluation regarding cost-effectiveness of acupuncture for migraine in both doctors and patients.
METHODSBy using questionnaire method, questionnaires for doctors and patients were made respectively; by using network platform, two sets of questionnaires were publicly released, which were filled in online. The results were real-time background collected and then analyzed.
RESULTSTotally 696 effective questionnaires for patients and 114 questionnaires for doctors were collected. The results indicated that (1)the direct cost (including treatment fee and material fee) for preventive treatment of migraine was 109 yuan per time in regular acupuncture, 152 yuan per time in regular acupuncture + electroacupuncture (EA) and 238 yuan per time in acupoint catgut embedding. The travelling expense was 42 yuan per time within the city and 193 yuan per time between cities. The indirect cost included cost for loss of working time (208 yuan per time), cost for treatment time (acupoint catgut embedding: 10 to 30 min per time, once one to three weeks; the remaining two treatments: 20 to 50 min per time, once to 5 times every week), time cost for arriving and departing hospital (1 to 3 hours per time within the city and 4 hours per time between cities). From the angle of treatment course, acupoint catgut embedding reduced the treatment frequency, leading to a lower total cost. (2)A mong the reasons to select different acupuncture methods to treat migraine, the results of patient questionnaire indicated that the focus in the patients who selected acupoint catgut embedding was different from that in the patients who selected two other treatments. The main advantage of acupoint catgut embedding was superior and lasting efficacy with low cost. The results of doctor questionnaire indicated the main reason to select acupoint catgut embedding was "lasting efficacy after single treatment" (87. 5%) and "lower frequency of treatment" (75. 0%). (3) The main reason to obstruct the popularization of acupoint catgut embedding was this method was not widely known.
CONCLUSIONThe acupoint catgut embedding is one ideal preventive treatment for migraine, which has higher cost-effectiveness, however, currently it is not widely applied. In future advertisement and training program should be strengthened to perform targeted popularization of acupoint catgut embedding for migraine.

OBJECTIVETo investigate the significance of beta-adrenergic receptor 2 (beta2-AR) and vascular endothelial growth factor-2 (VEGFR-2) in the occurrence and development of infantile hemangioma through detecting the expression of beta2-AR and VEGFR-2 in the different stages of infantile hemangiomas.
METHODSAccording to the Mulliken's classification standard, we classified the specimens as proliferating group (32 cases), involuting group (17 cases) and involuted group (11 cases). Normal skin tissue surrounding the hemangioma from 7 cases were chosen as control group. The expression of beta2-AR and VEGFR-2 was detected by immunohistochemical technique in proliferating hemangioma, involuting hemangioma, involuted hemangioma. The mean optical density was measured by image analysis system (Image Pro Plus 6.0) and SPSS 16.0 software was applied for statistical analysis.
RESULTSThe expression of beta2-AR and VEGFR-2 was strongly positive in proliferating hemangioma, while positive in involuting hemangioma and weakly positive in the involuted stage. The mean optical density of each phase was 0.064 751 2 +/- 0.012 747, 0.031 6017 +/- 0.006 848,0.011 869 8 +/- 0.039 349 for beta2-AR, and 0.068 940 9 +/- 0.029 274, 0.028 445 5 +/- 0.006 396, 0.011 184 1 +/- 0.004 198 for VEGFR-2. The differences between different stages had a statistically significance (P < 0.05). Correlation analysis on the mean optical density between beta2-AR and VEGFR-2 had a statistically significance (P < 0. 05).
CONCLUSIONSBeta2-AR and VEGFR-2 may be involved in the occurrence and development of infantile hemangioma.

Objective: Pervious RT-PCR results revealed the possibility of effective regulation channel of Xiaoyao San in treating rats of syndrome of liver invading the spleen. This experiment was designed to explore CIS rats’ immunohistochemistry findings in hippocampal subregion CA1 and amygdaloid subregion BLA after suppressing both amygdaloid AMPA receptors by microinjection of CNQX. To compare Xiaoyao San group with CNQX group, and to analysis of these findings may prove the possibility of effective regulation channel of Xiaoyao san treating this syndrome. Methods: 75 male SD rats were randomly divided into 5 equal groups: normal group, CIS group, sham-operation group, CNQX group and Xiaoyao San group. The number of GluR2 positive cells in hippocampal subregion CA1 and amygdaloid subregion BLA in these 5 groups were examined by immunohistochemistry test. Results: Compared with control group, the number of GluR2 positive cells decreased in hippocampal subregion CA1. Statistical analysis showed that no difference existed in control, CNQX and Xiaoyao San groups in CA1 region. Except CNQX group, tendency of changes were contradictory in BLA and CA1 region. In addition, the least in BLA in CNQX group. This finding revealed that Xiaoyao San may act the same as CNQX, thus Xiaoyao San afforded effective protection against this depression syndrome through suppression of both amygdaloid excitability. Conclusion: The balance coordination between hippocampus and amygdala may be the key in transformation from adaption to damage in a state of stress, was one of the central mechanisms of liver governing smoothing qi flow (LGSQF) on regulating the stress. Xiaoyao San may effectively regulate the balance of the excitability of hippocampus and amygdala. This hypothesis may reveal the regulation channel on Xiaoyao San affording effective protection against liver invading the spleen syndrome.

Objective To retrospectively study the biodistribution of 68Ga-DOTA-TATE as a SSTR imaging agent in human subjects. Methods A total of 106 patients with suspected disease were enrolled in this study. All patients were histologically proven for having either a single tumor <2 cm or without evidence of tumor during follow-up.Patients underwent PET/CT whole-body scan 17-100 min after intravenous injection of 55.2-220.0 MBq 68Ga-DOTA-TATE.ROI was drawn for measuring SUV of tracer-avid pathologies.One-way analysis of variance and two-sample t test were used for statistical analysis. Results High 68Ga-DOTA-TATE avidity was found in pituitary, with SUVmax of 4.00± 1.21.Tracer was excreted mainly through urinary system resulting in highest uptake in the urinary tracts. The SUVmax of kidney cortex was 19.01 ± 5.45.Mediastinal blood pool and liver SUVmax were 0.93±0.33 and 7.69±2.26,respectively.Mild uptake of 68Ga-DOTA-TATE was found in the brain, cerebellum, lung and muscle, all lower than that of mediastinal blood pool.Moderate accumulation of 68 Ga-DOTA-TATE (close to or slightly higher than liver) was found in adrenal gland and spleen, with SUVmax 7.61 ± 3.42 and 8.63± 2.31,respectively.Other organs (pituitary, salivary gland, thyroid, pancreas, small intestine, colon, uterus, prostate and bone) showed tracer uptake in the range between those of mediastinal blood pool and liver.68Ga-DOTA-TATE distribution in pancreas was not uniform.Nine patients had focal accumulation in the uncinate process of pancreas with highest SUVm~ up to 8.48.However,the SUVmax and SUV in the rest of pancreas (head, neck, body and tail) showed insignificant difference (F values:0.703,0.563,both P>0.05).68Ga-DOTA-TATE uptake in each organ reached equilibrium quickly after injection but with slight increase over time. The changes in SUV,however, showed insignificant difference among organs, including different parts of pancreas (t values:from -0.09 to 1.75,from-1.70 to-0.42,respectively,all P>0.05).Conclusions The biodistribution of 68Ga-DOTA-TATE reaches equilibrium shortly after intravenous administration and is stably maintained. The biodistribution activities are organ-specific,and characteristic to that of SSTR concentration.
7.Study on the influence of challenging-barrier pressure on the professional benefit of emergency nurses
Chinese Journal of Practical Nursing 2018;34(28):2226-2230
Objective To investigate the status of emergency nurses'challenging-barrier pressure and professional benefits, and analyze the influence of challenging-barrier pressure on professional benefits. Methods A total of 262 emergency nurses in 13 general hospitals of Shenyang were investigated by the Challenging-Barrier Pressure Scale and the Nurses' Professional Benefit Scale. Results Emergency nurses challenging pressure score was (3.15±0.86) points, the barrier pressure score was (2.91±0.72) points , and professional benefit score was (48.52±8.55) points. The Pearson correlation analysis showed that the emergency nurses' professional benefits and barrier pressure was negatively correlated (r =-0.471, P < 0.01), the emergency nurses' job benefits and challenging pressure was positively correlated (r=0.550, P<0.01). Herarchical regression analysis showed that after the control of demographic data, challenging-barrier pressure emergency nurses could independently explain the professional benefit of 32.90% of the variance (△R2=0.329). Conclusions The challenging-barrier pressure is an important factor influencing the professional benefit of emergency nurses. The positive role of challenging pressure should be used to improve the professional benefit and enthusiasm of the nurses in the emergency department.

Chinese Journal of Biotechnology 2008;24(7):1306-1311
To investigate the effects of induction temperature on the expression product and the impact of urea concentration on the purification, HIV-1 Gag inclusion bodies from E. coli induced at 30 degrees C (IB30) and 37 degrees C (IB37) were dissolved with urea of different concentrations. The solubility and yield of refolding were compared. IB30 were dissolved with 2 mol/L and 8 mol/L urea, and then purified with chromatography. IB30 were found easier to be solubilized in low concentration of urea and easier to be refolded than IB37. Furthermore, compared to the IB30 dissolved in 8 mol/L urea, Gag protein solubilized in 2 mol/L urea was purified to higher purity with gel filtration (GF) and ion exchange (IEX) chromatography. Gag inclusion body induced at lower temperature may contain more protein with native-like or reversibly-denatured structures, and solubilization in the presence of low concentrations of urea can help to retain these structures. This study has provided new insights into the purification of proteins from inclusion bodies.

OBJECTIVETo evaluate the consistency between the clinical diagnostic criteria and the ascertained diagnostic criteria for diabetic peripheral neuropathy (DPN) in the Preventive and Treatment Guidelines of Diabetes in China (2013) and explore an economic, convenient, and accurate approach to DPN diagnosis.
METHODSThe patients with type 2 diabetes admitted in our department from April to June, 2014 were examined for nerve conduction velocity, 10 g nylon silk, vibration threshold value, sense of temperature and pain, and ankle reflex. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, and Kappa value were calculated to assess the diagnostic power of the two diagnostic criteria.
RESULTSOf the 151 patients enrolled, 106 (70.2%) had a diagnosis of DPN consistent with the ascertained diagnostic criteria, as compared to 86 (56.95%) who were diagnosed according to the clinical diagnostic criteria; the latter patients accounted for 81.13% of former cases. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index, and Kappa value of the clinical diagnostic criteria were 80.19%, 97.78%, 98.84%, 67.69%, 77.97%, and 0.69, respectively, which were highly consistent with those of the ascertained diagnostic criteria; the sensitivity to compression showed a poor consistency between the two diagnostic criteria. In the 5 screening tests, the combined test of temperature sensation, vibration perception, and ankle reflex showed the highest AUC value among their different combinations.
CONCLUSIONThe clinical diagnostic criteria for DPN show good consistency with the ascertained diagnostic criteria, and for patients with clinical symptoms or with only one positive sign, combination of the two diagnostic criteria can achieve the maximum diagnostic power.

Objective:To explore the application and treatment efficacy of less invasive surfactant administration (LISA) and nasal high-frequency oscillation ventilation (nHFOV) in very low and extremely low birth weight preterm infants with neonatal respiratory distress syndrome (NRDS).Methods:A total of 85 very low and extremely low birth weight preterm infants with gestational age ranging between 27-32 weeks who were diagnosed with NRDS in the Second Affiliated Hospital of Zhengzhou University from July 2018 to October 2019 were enrolled. After being stratified by gestational age of >27-29 weeks, >29-30 weeks, >30-31 weeks, >31-32 weeks, the neonates were divided into the LISA group (40 cases) and the intubation-surfactant-extubation (INSURE) group (45 cases) by the random envelope method. The LISA group was subdivided into the continuous positive airway pressure (CPAP) group (25 cases) and the nHFOV group (15 cases) by the same method. The patients in the INSURE group were infused with pulmonary surfactant (PS) through the endotracheal tube under positive airway pressure, and then treated with CPAP after extubation. The patients in the LISA group were first treated with CPAP and injected with PS through the gastric tube. After removing the gastric tube, the patients in the CPAP group were given CPAP-assisted ventilation, while the patients in the nHFOV group were given nHFOV-assisted ventilation or mechanical ventilation if nHFOV-assisted ventilation failed. The feasibility of LISA technology and nHFOV was observed, and the adverse reactions, mechanical ventilation, oxygen duration, hospital stay and the incidence of NRDS complications in different groups of the patients were compared.Results:(1) The mechanical ventilation rate (5.0% vs.22.2%), the incidence of broncho-pulmonary dysplasia (BPD) (20.0% vs.42.2%) and the incidence of periventricular leukomalacia (PVL) (12.5% vs.42.2%) in the LISA group were significantly lower than those in the INSURE group (all P<0.05). There were no statistically significant differences in total oxygen duration, hospitalization duration, intraventricular he-morrhage (IVH), retinopathy of prematurity (ROP), and necrotizing enterocolitis (NEC) between the LISA group and the INSURE group (all P>0.05). (2) There was no significant difference in adverse reactions between the LISA group and the INSURE group as well as between the CPAP group and the nHFOV group (all P>0.05). (3) The younger the gestational age at birth, the higher the incidence of NRDS complications.Patients in the LISA group had a lower incidence of NPDS complications than patients of the same gestational age in the INSURE group, but the diffe-rence was not statistically significant (all P>0.05). (4) There was no significant difference in the mechanical ventilation rate and the incidence of BPD, IVH, PVL, NEC and ROP complications between the CPAP group and the nHFOV group (all P>0.05). Conclusions:In the treatment of very low and extremely low birth weight preterm infants with NRDS at the gestational age of 27-32 weeks, LISA technology is a safe and effective PS delivery method, which can reduce the mechanical ventilation rate and the incidence of BPD and PVL.The nHFOV can be used as an initial model for respiratory support of NRDS preterm infants with very low and ultra-low birth weight.LISA combined with nHFOV is applicable to the treatment of preterm infants with NRDS.
