1.Synthesis and in vivo evaluation of 68Ga-DOTATATE in mice

Objective To synthesize 68 Ga-1,4,7,10-tetraazacyclododecane-N,N',N,N()-tetraacetic acid-D-Phe1-Tyr3-Thr8-octreotide (68Ga-DOTATATE) manually and automatically,validate its qualities in vitro,and evaluate its biodistribution in ICR mice and the microPET imaging in nude mice bearing pancreatic AR42J tumor.Methods 68Ga-DOTATATE was synthesized by automatic method using commercial metal isotope multifunctional module with strong cation exchange (SCX) column and by manual method.Both the products were measured for quality control.For the biodistribution study 5 groups of ICR mice were injected with 68Ga-DOTATATE(1.11 MBq) and executed at 10,30,60,120 and 240 min postinjection,respectively.The organs were weighted,and % ID/g was calculated.Nude mice bearing pancreatic AR42J tumor were intravenously injected with 3.7 MBq 68Ga-DOTATATE,and then microPET imaging was acquired at 10,30,60,120,18 and 240 min.Results 68Ga-DOTATATE could be successfully synthesized by the automatic and manual methods.Both the product injections were colorless and clear.The pH value was 6.5±0.1.For the products obtained from the two methods,the radiochemical purities were over 99%,and the products were stable for 4 h at room temperature.For the automatic method,68Ga-DOTATATE was synthesized within 30 min and with the radiochemical yield of (51.8±3.2)% (time decay corrected).For the manual method,the time used for the synthesis was 20 min,and the labeling yield was over 99%.Three batches of the products were aseptic and pyrogen-free.In ICR mice,68Ga-DOTATATE was excreted by the kidney,and showed relatively high accumulation in the liver,spleen,pancreas and adrenal glands,while lower in the bone and soft tissue.The clearance from blood was fast with (4.41±0.81) %ID/g at 10 min postinjection and (0.78 ± 0.32) % ID/g at 1 h.MicroPET imaging showed increased uptake of 68GaDOTATATE in the tumor tissues,and T/NT were 2.01±0.29(10 min),6.74±2.90(30 min) and 4.46±2.05 (60 min),respectively.Conclusions 68 Ga-DOTATATE could be successfully synthesized manually and automatically.The products reach to the specification of radioactive drugs and could be used as an attractive positron emitting radiotracer for detection of the somatostatin receptor-positive tumors.

Egypt’s medical insurance is mainly covered by government and commercial insurance. The low coverage of commercial insurance and the quality of medical services needs more improvement in Egypt. Recently, the incidence rate of diabetes, chronic kidney disease, hypertensive heart disease, COPD and liver cancer is rising. Traditional Egyptian medicine is similar to Traditional Chinese Medicine (TCM), and its modern traditional medicine is mainly Arabian medicine. Acupuncture, as the main form of TCM, was introduced into Egypt in the 1970s, but it has not been covered by the medical insurance system. The development of TCM in Egypt needs improvement. It is suggested that further explorated fields should be focused on the acupuncture therapy, TCM education and TCM treatment of keeping health in Egypt.
3.The clinical characteristics and risk factors of cerebral venous sinus thrombosis complicated by cerebral hemorrhage

Objective To analyze the characteristics of clinical manifestations, risk factors, therapies and acute outcomes in patients with cerebral venous sinus thrombosis complicated by cerebral hemorrhage. Methods Seventy-five patients with cerebral venous sinus thrombosis were included in the study. According to the radiological findings on the brain image, patients were divided into two subgroups:cerebral hemorrhage group and non-hemorrhage group. The demo?graphic data, potential risk factors, clinical manifestations, radiological features, therapeutic strategies and acute out?comes were compared between two subgroups, and high risk factors were also analyzed. Results There were seventy-five patients with cerebral venous sinus thrombosis in the present study. Twenty-eight patients of them (37.2%) had cerebral hemorrhage whereas the remaining forty-seven patients (62.7%) did not have cerebral hemorrhage. Pregnancy/puerperi?um were significantly higher in patients with cerebral hemorrhage (with vs without;28.6%vs. 6.4%, P=0.015), while in?fection was markedly higher in patients without cerebral hemorrhage (with vs without;7.1% vs. 29.8%, P=0.021). Head?ache (92.9% vs. 70.2%, P=0.021), unconsciousness (25.0% vs. 6.4%,P=0.034), seizures (53.6% vs. 19.1%, P=0.002) and motor deficits (35.7% vs. 12.8%, P=0.019) were more common in patients with cerebral hemorrhage. Moreover, mul?tiple sinus involvement (1.4% vs. 44.7%, P=0.024) was significantly higher and the acute outcomes(mRS≥3: 46.4%vs.17.0%, P=0.006)were poorer in patients with cerebral hemorrhage. Binary Logistic analysis showed that pregnancy/pu?erperium (P=0.004) and multiple sinus involvement were positively, whereas infection was negatively correlated with cere?bral venous sinus thrombosis and hemorrhage ( P=0.007;P=0.03). Conclusions Pregnancy/puerperium, headache, uncon?sciousness, seizures, motor deficits and multiple sinus involvement are more frequently in patients with cerebral venous sinus thrombosis and hemorrhage, and the acute outcomes are poorer in patients with cerebral venous sinus thrombosis complicated by cerebral hemorrhage.
4.Diagnostic value of transcranial Doppler ultrasound in early stage cerebral arteriosclerosis.

OBJECTIVE:
To investigate the diagnostic value of transcranial Doppler ultrasound in early stage cerebral arteriosclerosis.
METHODS:
We selected 50 patients with early cerebral arteriosclerosis as the disease group. At the same time we selected another 50 patients as a control group with no significant symptoms in the nervous system. By 2 MHz pulse Doppler probe through double-temporal windows and pillow windows Basilar artery (BA), the bilateral middle cerebral artery (MCA) was detected. In the TCD spectrum, we selected the spectrum of a single-family cardiac cycle, identified the starting point (ts), pulse-incisure point (ti), end point (te), and the peak velocity (tp), measured the time of the spectrum starting point to the peak velocity (Tp) and calculated the time required for the peak velocity in the share of ventricular systolic (Tp/Ti), the time required for the peak velocity in the share of the whole cardiac cycle (Tp/T). Tp, Tp/Ti and Tp/T were respectively named as time to peak velocity (TPV), peak-time index-1 (PTI-1) and peak-time index-2 (PTI-2). All data were analyzed by SPSS13.0.
RESULTS:
There was no significant difference in blood vessel velocity, PI, RI and S/D of BA and RMCA (P>0.05) between the control group and the disease group. Compared with the control group, TPV of the BA, LMCA and RMCA significantly extended, PTI-1 and PTI-2 of BA, LMCA and RMCA increased significantly in the disease group (P<0.01). In the disease group, there was no significant correlation between peak time index and PI, S/D (P>0.05).
CONCLUSION
TPV, PTI-1 and PTI-2 are sensitive indicators of early stage cerebral arteriosclerosis.

5.Evaluation on Key Production Process of Jiangshi Granules Based on Fingerprint and Quantity-Value Transfer Relationship

Objective To evaluate the key production process of Jiangshi Granules based on the new mode of combining fingerprint and quantity-value transfer relationship.Methods The fingerprints of Jiangshi Granules extract,extractum and granules were established by HPLC.The common peak transfer number of the fingerprint,the calculation of multi-component content transfer rate,and the paste rate were set as indicators to analyze the quantity and mass transfer law in the production process.The rationality of the preparation process design of Jiangshi Granules was evaluated.Results The fingerprints of 10 batches of Jiangshi Granules extract,extractum and granules were established.17 common peaks were calibrated and 8 peaks were identified.They were respectively tangshenoside I,liquiritin,lobetyolin,isoliquiritin apioside,isoliquiritin,liquiritigenin,glycyrrhizic acid,atractylenolide Ⅲ;the average transfer rates of 8 components from decoction pieces to extract were 29.42%,51.26%,23.81%,34.45%,28.29%,30.22%,42.67%,26.10%;the average transfer rates of extract to extractum were 50.05%,60.04%,51.04%,50.27%,47.60%,52.44%,53.44%,44.97%;the average transfer rates of extractum to granules were 64.83%,78.74%,70.16%,66.56%,70.62%,69.59%,76.97%,66.43%.Conclusion The established fingerprint of Jiangshi Granules extract,extractum and granules is stable and reliable,which emphasizes the integrity of the research process of TCM preparations and provides a basis for the quality control of Jiangshi Granules in the production process.
6.Research progress on pharmacological action and clinical application of Sijunzi decoction and its modified formula in preventing and treating diarrhea

Diarrhea is the most common clinical symptom of the digestive system,its pathogenesis may be related to intestinal infection,intestinal microecological instability,intestinal barrier dysfunction and so on.Sijunzi decoction has the effect of invigorating spleen and nourishing stomach,and supplementing qi and blood.In modern clinical practice,modified Sijunzi decoction combining with other prescriptions,Chinese medicine massage,acupuncture and western medicine attenuates synergies,improves the patients'quality of life,and is widely used in the treatment of diarrhea,but its mechanism has not yet been systematically clarified.Therefore,this article reviews the research progress of the mechanism and clinical application of Sijunzi decoction and its modified formula in the treatment of diarrhea,and provides reference for the mechanism research and prevention of Sijunzi decoction and its modified formula in the treatment of diarrhea.

The medical insurance is provided by the Unified Health System (SUS) covering most of Brazilians and alsocombined with private insurance, and every Brazilan could get access to basic health services. The major diseaseas leading to death in Brazil are ischemic heart disease, stroke and lower respiratory infection. The incidence of AD, chronic kidney disease and diabetes are increasing in recent years. Traditional Chinese Medicine (TCM), which was represented by acupuncture, was introduced to Brazil in the early 19th. At present, acupuncture has been widely accepted and used, which was included in SUS. A number of universities have acupuncture courses. However, the specialities and advantages of TCM needs to be strengthened, the local practioners lack the understanding of TCM theory, the appropriate use of herbal medicine, and there lacks TCM education standards, all these limit the development of TCM. Thus, in order to promote the development and dissemination of TCM in Brazil, it is suggested to give full play of the characteristics of TCM in preventing disease, increase the research and development input of herbal medicine, standardize the education stystem of TCM, etc.
8.Application of MRI for clinical staging and classification of Japanese encephalitis in adult patients

Objective:To investigate the application of magnetic resonance imaging (MRI) in clinical staging and classification of adult Japanese encephalitis.Methods:The clinical data and craniocerebral MRI findings of 35 adult patients with Japanese encephalitis admitted in General Hospital of Ningxia Medical University from August to September 2018 were analyzed retrospectively. The MRI imaging characteristics were compared among patients with different stages and types, the apparent diffusion coefficient (ADC value) of thalamic lesions in patients of different stages was analyzed. SPSS 23.0 software was used to analyze the data.Results:Six moderate cases all had lesions involving the thalamus, and the number of intracranial lesions was <2. In 11 severe cases, 8 had lesions involving thalamus and 5 had lesions involving hippocampus; the number of intracranial lesions was <2 in 3 cases, the number of intracranial lesions was 2-4 in 3 cases, and the number of intracranial lesions was >4 in 5 cases. In 18 cases critical cases, the lesions involved thalamic in 14 cases, hippocampus in 14 cases, cerebral cortex in 14 cases, cerebral feet in 9 cases, basal ganglia area in 6 cases, and brain stem in 2 cases, respectively; 2 cases had the intracranial lesions <2, 6 cases had intracranial lesions 2-4, 10 cases had intracranial lesions >4. In 11 preliminary stage patients, 9 cases had DWI high signal and 2 had FLAIR slightly high signal; in 19 extreme stage patients, 16 cases had DWI high signal, 11 cases had FLAIR slightly high signal, 3 cases had T1WI high signal and 6 cases had T2WI high signal. In 5 recovery stage patients, 1 case had DWI slightly high signal, 5 cases had FLAIR high signal, and 1 case had T2WI high signal. The ADC values of thalamic lesions in recovery and extreme patients were higher than those in the preliminary stage ( q=3.931 and 4.012, P<0.05). The ADC value of thalamic lesions in the recovery period was higher than that in the extreme period ( q=3.372, P<0.05). Conclusions:The number of lesions and the range of involvement are associated with disease severity in adult Japanese encephalitis. The DWI sequence is easy to detect at early stage, and the FLAIR sequence shows a long time span of lesions; and the DWI and FLAIR sequences are of great significance for the early clinical staging and classification of adult encephalitis patients. At the same time, the ADC value shows a trend of disease progresses, which can be used as a supplement for the clinical staging in adult encephalitis patients.
9.Role of orphan nuclear receptor Nur77 in tunicamycin-induced injury to hippocampal neurons of mice: relationship with endoplasmic reticulum stress

Objective:To evaluate the role of orphan nuclear receptor Nur77 in tunicamycin(TM)-induced injury to hippocampal neurons and the relationship with endoplasmic reticulum stress in mice.Methods:Mouse HT22 cells were divided into 4 groups ( n=10 each) using a random number table method: control group (C group), Nur77 specific agonist Csn-B group (Csn-B group), endoplasmic reticulum stress inducer TM group (TM group), and TM+ Csn-B group. Cells in C group were cultured for 24 h under normal condition. In Csn-B group, Csn-B at a final concentration of 10 μg/ml was added to the culture medium, and the cells were incubated for 24 h. In TM group, TM at a final concentration of 200 ng/ml was added to the culture medium and the cells were incubated for 24 h to induce cell endoplasmic reticulum stress injury. Cells in TM+ Csn-B group were pretreated with Csn-B at a final concentration of 10 μg/ml for 15 min, then TM at a final concentration of 200 ng/ml was added, and the cells were co-incubated for 24 h. The cell viability was examined by CCK-8 assay kit after treatment in each group. The expression of endoplasmic reticulum stress-related protein CCAAT/enhancer-binding protein homologous protein (CHOP), glucose regulated protein 78 (GRP78)and apoptosis-associated protein Bcl-2, Bax, caspase-3 and cleaved-caspase-3 was detected by Western blot. Results:Compared with C group, the cell viability was significantly decreased, and the expression of CHOP, GRP78, Bax and cleaved-caspase-3 was up-regulated, and the expression of Bcl-2 and caspase-3 was down-regulated in TM group ( P<0.05 or 0.01). Compared with TM group, the cell viability was significantly increased, the expression of CHOP, GRP78, Bax and cleaved-caspase-3 was down-regulated, and the expression of Bcl-2 and caspase-3 was up-regulated in TM+ Csn-B group ( P<0.05 or 0.01). Conclusions:Orphan nuclear receptor Nur77 is involved in TM-induced injury to hippocampal neurons, which is related to activation of the endoplasmic reticulum stress in mice.
10.Relationship between acute stress response and depression among college students: the mediating role of rumination and the moderated role of perceived social support

BackgroundDepression has become a public health concern that affects the physical and mental health of college students. acute stress response is a risk factor of depression. Exploring the relationship and mechanism between acute stress response and depression is of great significance for preventing and intervening depression in college students. ObjectiveTo examine the relationship between acute stress response and depression among college students, and to analyze the mediating role of rumination and the moderated effect of perceived social support, so as to provide references for the prevention and intervention of depression in college student . MethodsFrom March to April 2020, a cluster sampling method was employed to select 1 355 college students from three universities in Hubei, Jiangxi and Guangxi Zhuang Autonomous Region. Participants were assessed with Acute Stress Disorder Scale (ASDS), Ruminative Responses Scale (RRS), Brief form of Perceived Social Support Questionnaire (F-SozU) and Patients' Health Questionnaire Depression Scale-9 item (PHQ-9). Pearson correlation analysis was adopted to examine the correlation between the scores of each scale. The mediating role of rumination between acute stress response and depression and the moderated role of perceived social support were examined respectively by using Model 4 and Model 14 in Macro Program Process 3.3. ResultsA total of 1 303 valid questionnaires were collected, yielding a valid response rate of 96.16%. The results of Pearson correlation analysis showed that ASDS score was positively correlated with RRS score and PHQ-9 score (r=0.649, 0.528, P<0.01) among college students. The mediation analysis results demonstrated that rumination played a partial mediating role between acute stress response and depression, with the mediating effect value of 0.273 (95% CI:0.222~0.328), accounting for 68.59% of the total effect. Perceived social support played a moderated role in the latter path of the mediation model (rumination → depression) (β=-0.004, 95% CI: -0.017~-0.004, P<0.01). ConclusionRumination played a partial mediating role between acute stress response and depression in college students, and perceptive social support played a moderated role between rumination and depression.
