1.Pharmacokinetic study of 3 blood-absorbed components of Xiangshao sanjie oral liquid in rats with hyperplasia of mammary gland

OBJECTIVE To explore the pharmacokinetic characteristics of 3 blood-absorbed components of Xiangshao sanjie oral liquid in rats with hyperplasia of mammary gland (HMG). METHODS Female SD rats were divided into control group and HMG group according to body weight, with 6 rats in each group. The HMG group was given estrogen+progesterone to construct HMG model. After modeling, two groups were given 1.485 g/kg of Xiangshao sanjie oral liquid (calculated by crude drug) intragastrically, once a day, for 7 consecutive days. Blood samples were collected before the first administration (0 h), and at 5, 15, 30 minutes and 1, 2, 4, 8, 12, 24 hours after the last administration, respectively. Using chlorzoxazone as the internal standard, the plasma concentrations of ferulic acid, paeoniflorin and rosmarinic acid in rats were detected by UPLC-Q/TOF-MS. The pharmacokinetic parameters [area under the drug time curve (AUC0-24 h, AUC0-∞), mean residence time (MRT0-∞), half-life (t1/2), peak time (tmax), peak concentration (cmax)] were calculated by the non-atrioventricular model using Phoenix WinNonlin 8.1 software. RESULTS Compared with the control group, the AUC0-24 h, AUC0-∞ and cmax of ferulic acid in the HMG group were significantly increased (P<0.05); the AUC0-24 h, AUC0-∞ , MRT0-∞ , t1/2 and cmax of paeoniflorin increased, but there was no significant difference between 2 groups (P>0.05); the AUC0-24 h and MRT0-∞ of rosmarinic acid were significantly increased or prolonged (P<0.05). C ONCLUSIONS In HMG model rats, the exposure of ferulic acid, paeoniflorin and rosmarinic acid in Xiangshao sanjie oral liquid all increase, and the retention time of rosmarinic acid is significantly prolonged.
2.Comprehensive value of novel oral anticoagulant drugs after major orthopedic surgery based on multi-criteria decision analysis

OBJECTIVE To evaluate the comprehensive value of novel oral anticoagulant drugs (NOACs) after major orthopedic surgery. METHODS The evaluation evidence was collected through literature research; evidence and value:impact on decision-making (EVIDEM) framework was introduced to integrate the evaluation process; the multi-criteria decision analysis (MCDA) method was used to construct a multi-dimensional evaluation system; the weights assigned to each evaluation criterion were determined by the combination of Delphi method and analytic hierarchy process, and the rivaroxaban, dabigatran and apixaban were comprehensively evaluated. RESULTS The clinical comprehensive evaluation system of NOACs after major orthopedic surgery was successfully established, and the final clinical comprehensive evaluation weights of NOACs (rivaroxaban, dabigatran, apixaban) after major orthopedic surgery were calculated, with scores of 0.399 7 for rivaroxaban, 0.244 4 for apixaban, and 0.355 9 for dabigatran, indicating that rivaroxaban demonstrated the highest overall clinical value. Among them, rivaroxaban had the highest weight score in the evaluation of pharmaceutical characteristics, cost-effectiveness and other attributes in a single dimension. In terms of efficacy and safety evaluation, apixaban had the highest weighting score. CONCLUSIONS Among NOACs, rivaroxaban is more suitable for routine anticoagulation management after major orthopedic surgery, especially in terms of pharmacological properties, cost-effectiveness and other attributes.
3.Evaluation of safety and quality management effect of ventilators in intensive care unit based on data envelopment analysis model

Objective:To explore the application effect of data envelopment analysis(DEA)model in the safety and quality management of ventilators in intensive care units.Methods:Based on the Chanis-Cooper-Rhodes(CCR)dual model,by adding constraints,the input-oriented Bank-Carnes-Cooper(BCC)model was constructed to construct the DEA model.Among the 124 related items in the DEA model,20 high-level related items were identified to carry out ventilator quality control management related to clinical service efficiency.A total of 33 ventilators in clinical use in the intensive care unit(ICU)of Jiangsu Province Hospital from October 2021 to September 2023 were selected.Among them,the ventilators used from October 2021 to September 2022 were managed by traditional quality control management methods,and the ventilators used from October 2022 to September 2023 were managed by DEA model.The satisfaction scores,management quality scores and clinical efficiency scores of participants in ventilator use management were compared between the two management methods.Results:The satisfaction scores of medical staff,patients and equipment engineers for ventilator management using the DEA model management method were(96.68±2.25)points,(96.32±2.18)points and(95.66±2.54)points,respectively,which were higher than those of the traditional quality control management method,the difference was statistically significant(t=10.940,15.220,6.674,P<0.05).The average scores of ventilator operation performance,inspection operation safety and timeliness using DEA model management method were(4.44±0.31)points,(4.53±0.37)points and(4.61±0.36)points,respectively,which were higher than those of the traditional quality control management method,the difference was statistically significant(t=12.692,14.979,17.025,P<0.05).The average scores of ventilator work efficiency,ventilator response time efficiency and ventilator treatment efficiency using the DEA model management method were(4.49±0.32)points,(4.75±0.35)points and(4.68±0.32)points,respectively,which were all higher than those of the traditional quality control management method,with statistically significant differences(t=21.179,16.785,16.347,P<0.05).Conclusion:The application of the DEA model in the management of ventilators in ICU can improve the efficiency of ventilator clinical service,improve the satisfaction of ventilator management personnel and the quality of ventilator management.

5.Impact of dexmedetomidine on malignant biological behavior of gastric cancer cells through immune regulation mechanism mediated by cGAS-STING pathway

Objective:To investigate the effects of dexmedetomidine(DEX)on the malignant biological behavior of gastric cancer(GC)cells through the immune regulation mechanism mediated by cyclic GMP-AMP synthase-stimulator of interferon gene(cGAS-STING)pathway.Methods:GC cell line MGC-803 was randomly divided into Control group(blank medium treatment),DEX low concentration group(DEX-L group,1 ng/ml),DEX medium concentration group(DEX-M group,10 ng/ml),DEX high concentra-tion group(DEX-H group,100 ng/ml)and DEX high concentration+cGAS inhibitor RU.521 group(DEX-H+RU.521 group,100 ng/ml DEX+1.0 μmol/L RU.521).Cell proliferation was detected by CCK-8 method.Cell scratch test was used to detect the migration ability of cells in each group.Transwell test was used to detect the invasive ability of cells in each group.The apoptosis rate was detected by flow cytometry.The levels of IL-2,interferon gamma(IFN-γ)and tumor necrosis factor alpha(TNF-α)in cells were detected by ELISA.Real time-fluorescent quantitative PCR(RT-qPCR)was applied to detect the expression levels of cGAS,STING and interferon typeⅠ(IFN-Ⅰ)mRNA.Western blot was used to detect the expressions of cGAS,STING,Bax,CyclinD1,matrix metalloproteinase 9(MMP9),N-cadherin,Vimentin,E-cadherin,Caspase3,Caspase8 and their shear type and phosphorylation level of TANK-binding kinase 1(TBK1)and interferon regulatory factor 3(IRF3).Results:Compared with Control group,the cell migration rate,number of cell invasions,TNF-α level,CyclinD1,MMP9,N-cadherin,Vimentin protein expressions in MGC-803 cells in DEX-M and DEX-H groups were decreased obviously(P<0.05),the growth inhibition rate(48 h,72 h),apoptosis rate,IL-2,IFN-γ,Bax,E-cadherin,Cleaved Caspase3,Cleaved Caspase8 protein expression levels,cGAS,STING,IFN-Ⅰ mRNA levels and protein expression levels and phosphorylation levels of TBK1 and IRF3 were increased obviously(P<0.05).RU.521 weakened the inhibitory effects of DEX on the proliferation,migration and invasion of GC cells and the ability to induce apoptosis,and alleviated the improvement on immune function.Conclusion:DEX may inhibit the proliferation,migration and invasion of GC cells and induce apoptosis of GC cells by acti-vating cGAS-STING pathway mediated immune regulation.
6.Reducing noise of low dose CT images with Zero-Shot Noise2Noise based on Z-axis correlation

Objective To observe the value of Zero-Shot Noise2Noise(ZS-N2N)based on Z-axis correlation for reducing noise of low dose CT(LDCT)images.Methods CT data of the cancer imaging archive were enrolled,including normal dose CT(NDCT)images and LDCT images,with 3 sets of chest and 3 sets of abdominal images.Noise on LDCT images were reduced with ZS-N2N method based on Z-axis correlation,and the peak signal-to-noise ratio(PSNR),structural similarity(SSIM)and time-consuming of reducing noise were compared with those of Self2Self,simple ZS-N2N and traditional Block-matching and 3D filtering(BM3D).Results After reducing noise,noise on Self2Self denoised images remained significant,the structure edges on BM3D denoised images were blurry with some details lost,while simple ZS-N2N and ZS-N2N based on Z-axis correlation denoised images preserved more details and had better quality.PSNR and SSIM of Self2Self denoised images were poor and the time-consuming were longer.PSNR,SSIM and time-consuming of the other 3 methods were similar,among which PSNR of ZS-N2N based on Z-axis correlation were slightly higher than BM3D and simple ZS-N2N,but the time-consuming were also slightly longer.Conclusion ZS-N2N based on Z-axis correlation had high value for reducing noise of LDCT images.
7.Association of Ego-depletion level with glycemic control and quality of life among adolescents with type 1 diabetes

Objective:To investigate the ego-depletion level of adolescents with type 1 diabetes mellitus (T1DM) and to explore its association with glycemic control and quality of life.Methods:This was a cross-sectional survey study. A total of 195 adolescents with T1DM were recruited from the First Affiliated Hospital of Nanjing Medical University from March to September 2022 by convenient sampling method. The Self-Regulatory Fatigue Scale (SRF-S) and Short Form of the Chinese version Diabetes Quality of Life for Youth Scale (C-DQOLY-SF) and the general information questionnaire were collected and the glycated hemoglobin (HbA 1c) value was detected. Results:The total score of self-regulatory fatigue for 195 adolescents with T1DM was (42.23 ± 9.94) points, with a scoring rate of 52.79%, which was at a medium level. Pearson correlation analysis showed that the total score of self-regulatory fatigue was positively correlated with HbA1c ( r = 0.25, P<0.01), and negatively correlated with quality of life ( r = -0.61, P<0.01). The hierarchical linear regression results showed that after controlling for demographic sociolagy and disease variables, ego-depletion had a positive predictive effect on HbA1c ( t = 3.69, P<0.01), while ego-depletion had a negative predictive effect on Quality of life ( t = -8.48, P<0.01). Conclusions:Ego-depletion of adolescents with T1DM may affect their blood glucose control and quality of life, which should be noticed by medical workers.
8.Effect analysis of PDCA cycle on prophylactic use of antibiotics in laparoscopic cholecystectomy during perioperative period

OBJECTIVE To evaluate the effect of PDCA cycle on prophylactic use of antibiotics in laparoscopic cholecystectomy during perioperative period and to conduct pharmacoeconomic analysis. METHODS Using retrospective analysis method, 80 discharged patients of each group underwent laparoscopic cholecystectomy were randomly selected from Hefei Second People’s Hospital before PDCA cycle (from May to June 2019), after the first round of PDCA cycle (from May to June 2020), after the second round of PDCA cycle (from May to June 2021) according to real or basic reasons for irrational drug use. The rationality of prophylactic use of antibiotics for patients was evaluated. The general situation, antibiotic use, clinical efficacy and treatment cost of patients were compared before cycle and after the first and second rounds of PDCA cycle. Cost-effectiveness analysis method and sensitivity analysis method were adopted to evaluate pharmacoeconomic significance of PDCA cycle. RESULTS After two rounds of PDCA cycle, the irrational rate of antibiotics, cost ratio of antibiotics, the number of days of antibiotics use, DDDs, drug utilization index, the frequency of antibiotics use per capita, the total amount of antibiotics, the cost of antibiotics, the total amount of drugs, and the total cost of hospitalization all decreased significantly (P<0.05). The results of cost-effectiveness analysis indicated that the pharmacoeconomic effect was the best after two rounds of PDCA cycle; the results of sensitivity analysis were consistent with it, which confirmed the reliability of the research results. CONCLUSIONS PDCA CPA- cycle promotes the rational use of antibiotics of laparoscopic cholecystectomy during perioperative period, reduces the cost of antibiotics and relieves the economic burden of patients.
9.Determination of fluoxetine ,norfluoxetine and sertraline concentration in human placental perfusate and their placental permeability by UFLC-MS/MS

OBJECTIVE To establish t he method for determining the concentrations of fluoxetine ,norfluoxetine and sertraline in human placental perfusate method and their placental permeability. METHODS Using glyburide as internal standard ,the samples were pretreated by protein precipitation method and detected by ultra-fast liquid chromatograph-mass spectrometer/mass spectrometer (UFLC-MS/MS). The determination was performed on Synergi TM Hydro-RP 80A LC column with mobile phase consisted of water (containing 0.1% formic acid )-acetonitrile(containing 0.1% formic acid )at the flow rate of 0.70 mL/min,with a gradient elution. The column temperature was set at 40 ℃,and sample size was 5 μL. Detection was performed with electrospray ionization source in multipl e reaction monitoring mode . The ion pairs for quantitative analysis we re m/z 309.9→148.1(fluoxetine),m/z 296.0→134.4 (-167), (norfluoxetine),m/z 306.1→159.0 (sertraline),m/z 493.9→ No.2018FE001(-207),(internal standard ). The perfusion model of singal placenta under bidrectional cardiopulmonary bypass was established. Fluoxetine (160 ng/mL),norfluoxetine(160 ng/mL), sertraline(100 ng/mL)and antipyrine (positive control ,ng/mL)were added into the maternal perfusate. The concen- 65324888 trations of fluoxe tine, norfluoxetine and sertrali ne were measured by above UFLC-MS/MS at 0,10,20,30,45,60,90,120,150 and 180 min of circulation ,and the placental permeability was calculated. RESULTS The linear range of fluoxetine ,norfluoxetine and sertraline were 5.00-500 ng/mL(all r> 0.990),and the lower limits of quantification were all 5.00 ng/mL. The RSDs of intra-day and inter-day were all less than 14.0%, and relative error ranged -9.6% to 14.7%. The relative error of stability test was -4.0% to 11.0%;the residual effect ,extraction method and matrix effect did not affect the quantitative analysis of the substance to be tested. Totally 31 perfusion model of human placenta under cardiopulmonary bypass were successfully established ,including 15 fluoxetine and norfluoxetine perfusion ,10 sertraline perfusion and 6 antipyrine perfusion. After 3 hours of perfusion ,the average placental permeability of fluoxetine , norfluoxetine and sertraline were (8.74 ± 1.67)% ,(10.70 ± 4.81)% ,(5.90 ± 1.25)% ,respectively. CONCLUSIONS The established UPLC-MS/MS is simple ,sensitive and accurate. It can be used for determination of fluoxetine ,norfluoxetine and sertraline in human placental perfusate. Fluoxetine ,norfluoxetine and sertraline can pass through the placenta ,but sertraline has a lower placental permeability.
10.Status quo of psychological resilience level in children and adolescents with type 1 diabetes mellitus and its effect on blood glucose control

Objective:To investigate the psychological resilience level of Chinese children and adolescents with type 1 diabetes mellitus (T1DM) and to explore its influence on glucose sugar control.Methods:Using the convenient sampling method, a total of 200 children and adolescents with T1DM in the First Affiliated Hospital of Nanjing Medical University and Nanjing Children's Hospital were selected as research obejcts from January 2019 to January 2021. The general information questionnaire, Diabetes Strengths and Resilience Measure for Adolescents (DSTAR-Teen) and Diabetes Behavior Rating Scale (DBRS) were used to investigate the level of psychological resilience and self-management behavior. And the glycated hemoglobin (HbA1c) value was collected, and analyzed the effect of mental toughness level on it.Results:A total of 200 questionnaires were distributed in this survey, and 196 valid questionnaires were recovered. The total score of DSTAR-Teen for 196 children and adolescents with T1DM was (43.60±8.55) and the total score of DBRS was (0.64±0.16) , the HbA1c value was 7.2% (6.5%, 8.3%) . The results of multiple linear regression analysis showed that the level of mental toughness was an influencing factor of HbA1c level in children and adolescents with T1DM ( P<0.05) . Conclusions:In this study, children and adolescents with T1DM have a higher level of psychological resilience, which may affect their blood glucose control. In the future, interventions that increase the level of mental resilience are suggested to be implemented to promote the improvement of health outcomes of patients.
