1.Clinical Significance of serum thyroxine level in AIDS Patients

Objective To determine the level of the serum thyroxine in AIDS patients and investigate its content changes and clinical significance. Methods The serum levels of T3 ,T4 ,FT3 ,FT4 and TSH in AIDS patients were determined by chemilumiescence assay. The results were analyzed. Results The serum levels of T3 and FTM3 in AIDS patients of the observation group were significantly lower than that of the control group, showed significantly stiatistical differences [(1.78 ± 0.99) vs (2.43 ± 0.52) nmol/L, (3.75 ± 3.65) vs (5.68 ± 1.83) pmol/L, all P <0. 05]. While the levels of T4, FT4 and TSH in those of the observation group was higher than that of the controls without significant difference (all P > 0.05). Conclusion The variations of levels of T3, T4, FT3, FT4 and TSH in AIDS patients had important clinical significance.

An analysis method for requirement of medical consumptive materials on Kalman filter is put forward.An analysis mathematical model is established at first,its solution process is carried out at same time,and then an example using this solution is given according to the working practice.At last,the shortcomings using classical Kalman filter were discussed and a new method using adaptive Kalman filter was proposed.

4.Clinical features of immunocompetent patients with Penicilliosis marneffei

Objective To discuss the clinical features of immunocompetent patients with Penicilliosis marneffei.Methods To analyze and summarize the clinical data of 4 immunocompetent patients with Penicilliosis marneffei.Results Clinical manifestation of patients centered on intermittent slight or mild fever,lymphadenectasis,skin eruptions,osteolytic destruction,anaemia,and leukocytosis.The cultural Penicillium marneffei in specimen such as blood,pus showed a higher positive rate.Amphotericin B and itraconazole was available in treatment.Conclusion Immunocompetent people can be infected by Penicillium marneffei,and they appear nonspecific in clinical manifestation after infection.A timely and effective antifungal therapy can improve the prognosis of patients.
5.Analysis of Therapeutic Effect of Asthma-relieving Manipulation for Children with Acute Asthmatic Exacerbation by Lung Sound Spectrum

Objective By using lung sound spectrum analysis, the clinical efficacy of asthma -relieving manipulation, which is a gentle massage therapy mainly on the acupoints, on acute exacerbation of children asthma was evaluated. Methods We collected the lung sound of 11 children with acute asthmatic attack before and after asthma-relieving manipulation, and then input to the computer for obtaining lung sound spectrum signal data. The sound intensity, average respiratory muscle strength ( RMS) power, total RMS power and the volume of every 25 Hz frequency spectrum were extracted for comparison. Results After asthma-relieving manipulation, the sound intensity, average RMS power, and total RMS power of the children with acute asthmatic attack were significantly lower than those before the manipulation (P<0.01). The results of lung sound spectrum analysis displayed that lung sound volume was significantly lower at frequency band 0~300 Hz after manipulation ( P<0.05 compared with that before the manipulation) . The difference of the lung sound volume was insignificant over frequency band 300 Hz before and after treatment ( P>0.05). Conclusion Lung sound spectrum analysis can be applied to monitor lung function of asthmatic children objectively and quantitatively, and it is a new sensitive method for clinical detection. Asthma-relieving manipulation shows certain effect on acute exacerbation of children asthma.
6.Evaluation of the efficacy of the layered method of sputum suction in the acute stage of the postoperative elderly patients with craniocerebral trauma

Objective To observe the efficacy and safety in the postoperative craniocerebral trauma elderly patients us-ing the layered method of sputum suction. Methods Senventy patients aged 60 and above with craniocerebral trauma were divided into the layered method group (group A) and the traditional method group (group B). The vital sign, mu-cosa damage frequency, the times of average daily sputum suction and the incidence of VAP were compared. Results Compared with group B, group A could reduce the vital signs change and the times of average daily sputum suction, with lower airway mucosa damage and the incidence of VAP, the difference was statistically significant(P<0.05). Con-clusion The layered method of sputum suction is safer and more effective than traditional method in treatment of the postoperative elderly patients with craniocerebral trauma.
7.Clinical study on correlative factors of surgery caused infections in pediatric surgery

Objective To analyze the factors related to surgical caused infections in children for reducing the infection of operative site.Methods A total of 120 case-times of operation in this hospital were selected.The dynamical air colony number and colony number of surgical instruments were performed the dynamic monitoring.The bacterial colony in air and surface of operative instruments were performed the bacterial species analysis.Results The dynamical air colony number in class 100,1 000,10 000 operating room all presented as "N" character pattern distribution;at 1 h after operation beginning,the variation of air colony number in class 100 operating room was significantly higher than that in class 1 000 and 10 000 operating rooms(P<0.05).The surface colony increase rate of sterile towel wrapped sterile operating instruments was significantly slower than that of the operating instruments directly exposing to air(P<0.05).The surface colony increase rate of operating instruments scrubbed by 1% povidone-iodine was significantly slower than that of operating instruments without scrubbing bloodstain and with scrubbing bloodstain by normal saline(P<0.05).The bacterial species distribution in operating room air was consistent with that in operating instrument surface.Conclusion The operation caused infection in children is significantly correlated with the operating room bacterial colony number and operation time.
8.Finite element analysis of stress distribution of proximal femoral growth plate in adolescents

Objective:To explore the effects of mechanical factors on the morphology of the growth plate and proximal femur development, in order to provide a mechanical basis for the mechanism of Cam lesions in femoral acetabular impingement (FAI).Methods:Using CT scan data of hip joint from adolescents, we constructed three-dimensional finite element models of normal and extended proximal femoral epiphyseal growth plates. Dynamic mechanics and position data from daily activities (walk, stand on one and two feet, go upstairs and downstairs, sit and stand up, squat and stand up) and basketball layup drills were collected from five healthy volunteers using the Vicon system. The mean values of femoral head mechanical loads and positions at peak values, movement onset, and termination were selected for finite element analysis to observe the distribution of equivalent stress, normal stress, and shear stress on the two growth plate models under different activity modes.Results:Successful construction of three-dimensional finite element models for normal and extended proximal femoral epiphyseal growth plates was achieved. Mechanical load values and spatial positions of the femoral head during daily activities and layup movements were obtained. In daily activities, peak equivalent stress values for normal and extended growth plates ranged from 1.6 to 11.0 MPa; compressive stress ranged from 1.7 to 12.0 MPa; tensile stress ranged from 0.5 to 10.0 MPa; and shear force ranged from 0.4 to 7.1 MPa. The compressive stress during walking, standing on one or both feet, going up and down stairs, sitting and standing, and squatting was concentrated in the central and lateral areas; tensile stress was concentrated in the central and medial areas; shear force was concentrated in the medial area and the anterior and posterior edges of the growth plate. During a layup, the compressive stress in the center, outer upper side, and medial edge of the growth plate of the normal model was notably concentrated, with peaks fluctuating between 5.5-19.0 MPa, 5.7-11.0 MPa, and 5.4-7.3 MPa respectively; tensile stress and shear force were concentrated at the inner and outer edges as well as in the central area, with peaks fluctuating between 3.0-24.0 MPa and 3.0-26.0 MPa respectively, these values were significantly different from those observed during daily activities. For the elongated growth plate, compressive stress was concentrated at the central and lateral edges with peaks fluctuating between 17.0-41.0 MPa and 17.0-38.0 MPa respectively; simulated shear stress and tensile stress showed significant concentration at peaks fluctuating between 4.9-34.0 MPa, also significantly different from those observed during daily activities.Conclusion:The difference in mechanical distribution between daily activities, basketball layup training, as well as between normal and extended growth plates may be the mechanical initiating factor in the development of extended growth plates and the formation of Cam lesions, it may be the mechanical initiating factor for the development of extended growth plates and the collision of Cam lesions in FAI.
9.Influence of perceived stress on health-promoting behaviors in patients with metabolic syndrome: the multiple mediating roles of adaptability and social support.

: To explore the potential associations between perceived stress and health-promoting behaviors based on the theoretical schema of the middle-range theory of adaptation to chronic illness. From January to May 2021, a convenience sampling method was used to recruit 230 young and middle-aged patients with metabolic syndrome who underwent physical examination in the inpatient center of Sir Run Run Shaw Hospital, Zhejiang University School of Medicine. The Health-Promoting Health Profile-Ⅱ, Chinese Perceived Stress Scale, Coping and Adaptation Processing Scale-Short Form, and Multi-dimensional Scale of Perceived Social Support were used in the cross-sectional study. The chain mediation effect procedure and bootstrap sampling test were used to examine the mediating role of adaptability and social support between perceived stress and health-promoting behaviors. The mean score of health-promoting behaviors was 100.0±14.6, the mean score of perceived stress was 22.0± 6.9, the mean score of adaptability was 47.0±6.1, and the mean score of social support was 63.8±10.8. Perceived stress had a negative impact on patients' health-promoting behaviors (=-0.309, <0.05). The adaptability (effect size= -0.112, 95%:-0.199~-0.038) and social support (effect size= -0.032, 95%:-0.083~played a mediating role and a chain mediating role in the process of perceived stress influencing patients' health-promoting behaviors (effect size= -0.045, 95%:-0.093~-0.020). Adaptability and social support play an intermediary role between perceived stress and health-promoting behaviors in young and middle-aged patients with metabolic syndrome. Healthcare professionals can motivate patients to develop healthy behaviors by developing intervention strategies on adaptability and social support.

10.Design and development of a Wechat applet for intelligent health management of metabolic syndrome.

: To design and develop a Wechat applet for intelligent health management of metabolic syndrome. Based on the needs and requirements of individuals undergoing health check-up, patients with metabolic syndrome and medical workers, a Wechat applet for metabolic syndrome management was designed and developed, which involving health data collection, health risk prediction, health management knowledge base fusion and intelligent recommendation, data privacy and security. The platform consists of three user ports: individuals undergoing health check and patients with metabolic syndrome, the medical workers and the system administrators. The main functions of the platform included metabolic syndrome risk prediction, intelligent recommendation of health management strategies, health behavior record and supervision, experts' consultation and health knowledge guide. The Wechat applet developed in this study can be used for metabolic syndrome risk prediction for general population, and health management for patients with metabolic syndrome, which helps them to enhance health management awareness and health behavior adherence.
