1.Evaluation of the changes of osteopontin level after chemotherapy in patients with cervical cancer

Objective To detect osteopontin (OPN) expression level of cervical cancer patients before and after chemotherapy , and explore the clinical value in the evaluation of chemotherapy efficacy .Methods 90 cases of cervical squamous cell carcinoma were treated with TP regimen ,immunohistochemical method was used to detect OPN expression before and after chemotherapy ,and studied the significance of OPN evaluation on curative effect in cancer chemotherapy .Results Before chemotherapy the positive ex-pression rate of OPN of tissue was 85 .56% ,and that after chemotherapy was 62 .22% ,the positive rate of OPN expression has sig-nificant difference before and after chemotherapy conclusion(χ2 =14 .118 ,P<0 .05) .The positive expression rate of OPN of effec-tive chemotherapy group before chemotherapy was 78 .69% ,and that after chemotherapy was 47 .54% ,there were significant differ-ences(χ2 =20 .840 ,P<0 .05) .Before chemotherapy ,the positive expression rate of OPN in ineffective chemotherapy was 100% ,af-ter chemotherapy ,it was 96 .56% ,there were no significant difference(χ2 =3 .046 ,P>0 .05) .Patients with OPN negative expres-sion rate of CR was 100% (13/13) ,OPN patients with positive expression rate of CR was 45 .46% (35/77) ,compared two groups with statistical difference (P<0 .05) .Conclusion The expression of OPN in cervical cancer tissue in the tumor is related with chemotherapy sensitivity and effect of chemotherapy .
2.The Role of MD-2 in The Process of Endotoxin Recognition and Signal Transduction

Lipopolysaccharide(LPS) can induce cell inflammation through interacting with TLR4. Recent studies have revealed that MD-2 participate in the process of LPS induced signal transduction pathway by forming a complex with TLR4. After binding to the MD-2 of the TLR4/MD-2 complex, LPS can induce TLR4- oligomerization and activate the downstream signal pathway. After being synthesized, most MD-2 can bind to TLR4 at the endoplasmic reticulum /Golgi apparatus and expresse as TLR4/MD-2 complex at the cellular surface. Therefore MD-2 not only can regulate the distribution of TLR4 in the cytoplasm, but also help TLR4 to recognize LPS. Another part of MD-2 can be released into plasma as soluble MD-2(sMD-2). With the help of CD14, sMD-2 would interact with LPS in the plasma to constitute LPS-sMD-2 complex, helping cell who express only TLR4, to recognize LPS, however excessive expressed sMD-2 would repress the LPS signal transduction pathway. In conclusion, MD-2 plays a crucially modulating role in the process of TLR4 mediated endotoxin recognition and signal transduction.
3.Distribution and Antibiotic Resistance of Escherichia coli in Hospital

OBJECTIVE To understand the distribution and antibiotic resistance of E.coli isolated from our hospital and offer suggestions for clinic.METHODS The antibiotic susceptibility to 21 kinds of antibiotics were tested by K-B method and analyzed by Whonet software.RESULTS The antibiotic resistance of 301 E.coli strains to ampicillin,quinolones,piperacillin,gentamicin,cefazolin,cefuroxime,ceftriaxone,ceftazidime,aztreonam,cefepime, ampicillin/sulbactam were 83.39%,69.44%,62.79%,44.19%,42.86%,36.88%,35.22%,31.23%,29.90%,29.57% and 26.91%,respectively.CONCLUSIONS E.coli isolated from our hospital show high resistance to several kinds of antibiotics.The drugs should be choosed reasonably according to their antibiotic suscepitibity results.
4.Resistance of Pseudomonas aeruginosa of Respiratory System Infection

OBJECTIVE To study resistance and the change of Pseudomonas aeruginosa of respiratory system infection,and instruct the antibacterial medicine to use rationally.METHODS The respiratory system infection patients from Jun 2004 to Dec 2005 were investigated.To the results of MICs with 229 P.aeruginasa strains were analyzed.RESULTS From inspected 5411 phlegm samples derived from Jun 2004 to Dec 2005,bacteria from 2291 samples(42.34%) were examined out,from them 229 strains(10.00%) were P.aeruginosa. The sensitive rate to tobramycin,imipenem/cilastatin sodium,piperacillin sodium,amikacin sulfate,ceftazidime,cefoperazone/sulbactam(Sulperazon),et al was greater than 60%.CONCLUSIONS We should consider the mechanism of P.aeruginosa resistance in treatment of respiratory system infection patients,and guide the management of antibacterial using.
5.Dosimetric study of intensity modulated direct aperture optimization radiation therapy for testicular seminoma

Objective To compare the field-matching techniques and dosimetric characteristics in the target area of testicular seminoma including the abdominal and pelvic cavities between direct aperture optimization intensity modulated radiation therapy (DAO-IMRT) and 3D conformal radiation therapy (3D-CRT),and to analyse the advantages and disadvantages of DAO-IMRT.Methods DAO-IMRT and 3D-CRT plans were designed with Pinnacle treatment planning system for 7 testicular seminoma patients,and the characteristics of both the irradiation methods were analyzed and evaluated by means of the parameters like dose homogeneity indexes,dose volume histograms and etc.Results DAO-IMRT had the hot and cold volumes in the target areas both significantly smaller than those by 3D-CRT,while the conformity index and homogeneity index superior to those by 3D-CRT.The mean doses (Dmean) by DAO-IMRT of the organs at risk (OAR) except the left femur head were all lower than those by 3D-CRT,aud the V15 values of liver,double kidney,small intestine and rectum by DAO-IMRT were statistically lower than those by 3D-CRT (P<0.01).3D-CRT had the V5 values of OAR all lower than those by DAO-IMRT,in which the differences of double kidney,small intestine,bladder,left femur head and normal tissues were significant statistically (P<0.05).The monitor units by DAO-IMRT roughly doubled when compared with those by 3D-CRT,and the difference was also significant (P<0.01).Conclusion DAO-IMRT with easy operation and high reliability can avoid the connection of fields in the target area of testicular seminoma,and is superior to 3D-CRT in dosimetry.
6.Analysis of gene expression patterns in atherosclerosis induced by homocysteine using cDNA microarray

Objective To investigate gene expression patterns of atherosclerosis (As) induced by homocysteine (Hcy). Methods Total RNAs were isolated from peripheral blood lymphocytes of 3 inpatients who had no traditional risk factors of coronary heart disease (CHD). They were one with normal coronary angiography and plasma Hcy level , one CHD patient with normal plasma Hcy level, and one CHD patient with high plasma homocysteine level . The RNAs were reversely transcribed to the cDNAs to prepare the hybridization probes. The probes were hybridized to the gene chip with 1152 genes to study gene expression patterns. Results There were 177 differentially expressed genes between the CHD patient with high plasma homocysteine level and the patient with normal coronary angiography and plasma Hcy level (group 1) . There were 151 differentially expressed genes between the CHD patient with high plasma homocysteine level and the CHD patient with normal plasma Hcy level (group 3) . A parallel comparison between group 1 and group 3 gene chips showed that there were 48 differentially expressed genes in common , of which there were 25 down-regulated and 23 up-regulated genes.Conclusions Among the differentially expressed genes, some are involved in the functions of apoptosis, signal transduction, immunology, translation of protein and proto-oncogene, etc. They had potential values in genesis of As induced by Hcy. The study based on gene chip in researching the differentially expressed genes of As induced by Hcy will provide new clues for finding out the pathogenesis of As.
7.Correlations of estradiol and testosterone in hyperuricemia patients

Objective To evaluate the serum concentrations of estradiol and testosterone in hyperuricemia patients and possible correlations of the two factors and uric acid (UA) in hyperuricemia (HUA) patients.Methods This was a case control study.We involved 90 hyperuricemia patients,103 healthy controls.Estradiol,testosterone,UA,serum glucose,lipid profile,creatinine and body mass index (BMI) were estimated in two groups.The statistical analysis of the data were performed using SPSS version 19.0 software.The estradiol,testosterone,UA,serum glucose,lipid profile,serum creatinine and BMI levels between the two groups were compared using the Student's t-test.Pearson correlation test was used to assess the correlation between serum UA levels and these indexes.Results Serum levels of estradiol in healthy subjects and hyperuricemia patients were 82.2 (55.6-108.8) pmol/L,65.8 (36.6-95.0) pmol/L respectively ;the serum levels of UA were 300.8(207.2-394.4) μmol/L,426.9(370.1-483.7) μmol/L,respectively.The levels of estradiol were higher in the healthy control group than those in the hyperuricemia group.There were significant differences of estradiol levels between these two groups (P < 0.05).Estradiol was negatively correlated with UA (r =-0.319,P < 0.01),so was testosterone (r =-0.312,P < 0.01).Conclusion The findings of the present study suggest that in hyperuricemia patients,there are associations between estradiol and UA and the levels of serum estradiol might be used as biomarkers in hyperuricemia.
8.Hospital Infection among Tumor Patients:Analysis and Prevention

OBJECTIVE To understand the characteristic of hospital infection among tumor patients and give information to reduce the tumor patients with hospital infection.METHODS Using the methods of initiative monitoring and the system review to carry on the statistics,analysis,and evaluation for the total of 2679 tumor patients occurred in Department of Oncology in Shaoxing People′s Hospital in 2005.RESULTS Among them there were 120 cases occurred hospital infection.The rate of hospital infection was 4.47%.The patients who infected one time were 179 cases,two times were 31 cases,and over three times were 10 cases.The main infected sites were respiratory tract including upper respiratory tract(38 cases) and lower respiratory tract(36 cases),followed by alimentary tract(10 cases),blood(9 cases),surgical incision infection(8 cases),urinary tract(6 cases) and the other locations(13 cases).CONCLUSIONS The main prevention and control measures of hospital infection among tumor patients are to improve the body immunity and control endogenous infections.

10.Plasmakinetic enucleation of the prostate: the complication and outcome according to the prostate size

Objective To evaluate surgical complications and outcomes based on prostate size in patients with benign prostatic hyperplasia (BPH) treated with plasmakinetic enucleation of prostate (PKEP).Methods A retrospective review was conducted of PKEP performed in 326 patients with BPH.According to the prostate size on preoperative transrectal ultrasonography measurement,patients were divided into three groups:groupl:<40ml (n =92),group2:40~80ml (n =155),and group3:>80ml (n =79).Intraoperative and perioperative parameters were evaluated.Patient perioperative data and postoperative outcomes were compared.Patients were followed up at 1,3,6 and 12 months after surgery.Early and late complications were recorded.Results There were significant differences among three groups regarding the mean operative time (P < 0.01) and the mean resected tissue weight (P < 0.01).However,enucleation efficiency (P < 0.01) in gm tissue per minute increased significantly as prostate size increased.Mean hemoglobin decrease (P >0.05),mean serum sodium decrease (P >0.05),mean postoperative irrigation time (P > 0.05),mean catheter time (P > 0.05) and mean hospital stay (P >0.05) did not differ significantly among three groups.Three groups had a similar and significant postoperative improvement in international prostate symptom score (IPSS),quality of life (QOL),maximum urine rate (Qmax),and post-void residual urine volume(PVR) independent of prostate size (P < 0.01),but no significant difference was found among three groups during 1,3,6 and 12-month follow-up (P > 0.05).Perioperative and postoperative complications did not depend on prostate size (P > 0.05).Conchsions Although patients with a larger BPH required significantly longer operation time in PKEP,prostate size did not affect perioperative and postoperative complications or micturition improvement.
