1.Diagnosis and treatment of tubular carcinoma of the breast:report of 11 cases

Objective To explore the clinical characteristics,pathology,prognosis and proper treatment of tubular carcinoma of the breast. Methods The clinical data of 11 patients with tubular carcinoma of the breast treated in Peking Union Medical College Hospital were retrospectively analyzed.Results The incidence of tubular carcinoma of the breast account for 0.4% of the total breast cancer cases.Eight out of 11 cases had palpable painless lumps in the breast.Six cases received modified radical mastectomy,among which one patient received modified radical mastectomy of both sides.Four patients underwent breast conservation therapy.Sentinel lymph nodes biopsy was conducted in 2 patients.By immunohistochemistry ER was positive in 6 cases,PR was positive in 7 cases,2 cases were axiilary lymph node positive.Three patients received chemotherapy.Two patients received endocrine therapy(tamoxifen or aromatase inhibitor).Other patients received combined therapy including radiation,endocrine therapy and chemotherapy.All the patients have been followed up from 1 month to 7 years and within the period there is no recurrence,metastasis and death. Conclusion Tubulax carcinoma of the breast is a kind of low malignant tumor.Proper surgery and adjuvant therapy is important to improve survival and the quality of life.
2.Thermal Desorption-Gas Chromatography or Gas Chromatograph-Mass Spectrometry for Analysis of Semi-Volatile Compounds on Atmospheric Particulate Matters

A thermal desorption ( TD) device was developed and coupled to gas chromatography ( GC) or gas chromatography-mass spectrometry ( GC-MS ) for the qualitative and quantitative analysis of semi-volatile organic compounds on atmospheric particulate matters ( PM ) . The TD was operated by direct heating and placed on the GC injector, leading to high heating rate and easy transfer of analytes to GC without focusing of analytes by cold trap. For establishing the TD-GC method, the materials used for supporting PM samples, temperature and time of thermal desorption, and types of sample injection were investigated for detection of sixteen polycyclic aromatic hydrocarbons ( PAHs) and nine n-alkanes. The limits of detection of the proposed TD-GC method were in the range of 0. 014-0. 093 ng for PAHs, and 0. 016-0. 026 ng for n-alkanes, respectively, with the correlation coefficients of correlation above 0. 9975. The TD-GC method was applied to the determination of trace PAHs and n-alkanes on PM10 samples from three cities. The recoveries were in the range of 95%-135% ( PAHs) and 95%-115% ( n-alkanes) , respectively. Finally, the TD was coupled to GC-MS for comparison of the contents of PAHs and n-alkanes on PMx with different particulate size ( x=10 , 5, 2, 1, 0. 5, 0. 25, 0. 1).
3.Molecular subtype and its association with prognosis in axillary lymph node-negative invasive breast cancer patients

Objective:To analyze the distribution, clinico-pathologic features, and survival status of different subtypes in axillary lymph node-negative invasive breast cancer patients. Methods:In this study, data of 183 patients were included and retrospectively ana-lyzed in terms of age distribution, clinico-pathologic features, disease-free survival (DFS), and overall survival based on different sub-types (luminal, basal-like, and HER-2 over-expression). Results:No significant differences in age, tumor size, and TNM stage was ob-served among different subtypes. The relapse rates of luminal, basal-like, and HER-2 over-expression subtypes were 3.9% (4/102), 20.4% (10/49), and 6.3% (2/32), respectively (P=0.002). The death rates of luminal, basal-like, and HER-2 over-expression subtypes were 2.0%(2/102), 6.1%(3/49), and 3.1%(1/32), respectively (P>0.05). Kaplan-Meier analysis showed that the DFS of basal-like sub-type was much lower compared with that of the luminal and HER-2 over-expression subtypes (P=0.002). Cox analysis showed that the subtype was an independent prognostic indicator (P=0.001). Conclusion:In node-negative invasive breast cancer, no significant differ-ences in age distribution, tumor size, and TNM stage was observed among different subtypes. The basal-like subtype has the worst prog-nosis. Therefore, subtype is an important independent prognostic indicator.
4.ERβ expression in breast cancer at different stages and molecular subtypes

Objective To explore the association of estrogen receptor β expression with different stages and molecular subtypes of invasive breast cancer.Methods The clinicopathologic data of 446 invasive breast cancer cases was retrospectively analyzed.ERβ expression was evaluated by types and stages.Results Of all 446 invasive breast cancer cases,328 (73.5%) were ERβ positive.The ERβ positive rate was 77.9% (240/308) and 63.8% (88/138) in ERα + group and ERα-group,respectively.The ERβ expression in breast cancer was positively correlated with ERα (P < 0.01) while it had no correlation with PR,histological grade,HER-2 and Ki-67 (P > 0.05).ERβ expression was not significantly different among different age,tumor size and axillary lymph node groups(all P > 0.05).A total of 418 invasive breast cancer cases were recruited for pathologic stage and NPI analysis,including 168 cases at stage Ⅰ,152 cases at stage Ⅱ and 98 cases at stage Ⅲ.ERβ expression was not significantly different among different stages of breast cancer(P =0.743).Analyzed in these 418 cases,NPI was < 3.4 in 126 cases,3.4-5.4 in 207 cases and > 5.4 in 85 cases.ERβ expression was not significantly different among different NPI group (P =0.644).The ERβ positive rate in Luminal A subtype,Luminal B1 subtype,Luminal B2 subtype,HER-2 subtype and TN subtype was 75.6% (88/118),75.9% (110/145),85.2% (46/54),68.4% (39/57) and 62.5% (45/72) respectively.ERβ expression was significantly different between Luminal subtype and non-Luminal subtype (P =0.007).Conclusions ERβ was not differentially expressed among different breast cancer stages and NPI groups.ERβ was differentially expressed in different breast cancer molecular subtypes.
5.Imaging Features Multi-slice Spiral CT and PET/CT in Lymphoma with Pulmonary Lesions as the Initial Presentation

PurposeTo discuss the imaging features of multi-slice spiral CT (MSCT) and PET/CT in lymphoma with pulmonary lesions as the initial presentation.Materials and Methods Eighteen cases of lymphoma with pulmonary lesions as initial presentation confirmed by operation and pathology were retrospectively analyzed including Hodgkin lymphoma (HL) in 3 cases and non-Hodgkin lymphoma (NHL) in 15 cases. All 18 patients underwent MSCT, of which 11 also underwent PET/CT. The MSCT manifestations included number of lesions, morphology, density and bronchial change. PET/CT evaluated lesions in other body parts and SUVmax of lung lesions.Results MSCT showed single lung lesion in 6 cases, multiple lung lesions in 12 cases. Masses were seen in 9 cases, non-masses in 7 cases, and mixed type in 2 cases. There was single consolidation in 3 cases and mixed consolidation in 15 cases. Air bronchogram was identified in 15 cases. CT predictive diagnosis was lymphoma in 3 cases, lung cancer in 5 cases, pneumonia in 7 cases, granuloma in 1 patient, and indeterminate in 2 cases. PET/CT showed hypermetabolic lung lesions in 11 patients, mediastinal lymph node in 3 cases, and unilateral supraclavicular lymphadenopathy in 1 case. One patient showed pancreatic tail masses, gastric wall thickening and haunch masses. All 11 patients were diagnosed as malignant tumor, of which 8 cases as lymphoma.Conclusion MSCT manifestations of lymphoma with pulmonary lesions as initial presentation include multiple lesions, polymorphic type and heterogeneity with air bronchogram. PET/CT has better accuracy than MSCT.
6.The clinical value of fine needle biopsy in breast cancer

Background and purpose: With the development of the means of preoperative diagnosis and treatment of breast cancer, ifne needle puncture no longer seems to be popular with the clinical use. The safety and effectiveness of fine needle aspiration as preoperative diagnosis of breast cancer are controversial. This study was aimed to investigate the application of fine needle aspiration (FNA) of breast cancer and association between FNA and recurrence and metastasis for 30 years in our hospital. Methods:A total number of 1 260 patients with breast cancer treated in Peking Union Medical College Hospital from 1975 to 2006 were reviewed in two groups for clinical characters, recurrence, and survival rate. The two groups were FNA and non-FNA. Results:The positive rate of FNA was 61.4%and the highest rate was inⅣstage (100%). StageⅡandⅢwere the major ones in FNA group (45.2%). The local recurrence (7.0%vs 6.4%), recurrence and metastasis (13.2%vs 14.0%), 5-year (82.0%vs 81.0%) and 10-year disease free survival rates (51.0%vs 64.0%) have no signiifcant difference between FNA and non-FNA groups. Conclusion:FNA is still one of the most important methods for preoperative investigation of breast cancer. 8. Measurement of local tissue water in the diagnosis of breast cancer related lymphedema

Objective:
To investigate the value of local tissue water (LTW) in the diagnosis and evaluation for breast cancer related lymphedema(BCRL).
Methods:
Sixty one patients with BCRL in Peking Union Medical College Hospital were recruited. The tissue dielectric constant (TDC) method was used to measure the LTW of the ventral and dorsal midpoint of the bilateral upper arm and forearm of the patients. Meanwhile, the circumference of the bilateral upper arm and forearm was measured with a tape. The LTW ratio and the circumference ratio of the affected and the unaffected arms were calculated respectively.
Results:
The percentage of the patients who had two centimeter or above difference in the same reference point between their affected and unaffected arms or had 1.2 or above LTW ratio in their ventral upper arm and forearm were 29.5%, 39.3% and 52.5% respectively. The LTW ratio of the ventral reference point between the affected arms and the unaffected arms was significantly higher than that of the dorsal reference point (
9.Testing methods for seed quality of Amomum villosum.

OBJECTIVEThe aim of this study was to optimize the testing methods for seed quality, and to provide a basis for establishing seed testing criterion and quality standard of Amomum villosum.
METHODReferring to the International Seed Testing Rules made by ISTA and Rules for agricultural seed testing, the seed quality of A. villosum from different collection areas was measured.
RESULTThe samples weight of A. villosum for purity analysis were at least 500 g and for test were at least 50 g. Verification of genuineness was assayed by seed appearance comparing and weight of per hundred seeds was determined, the moisture content test was carried out by high temperature drying method (3 hours). The seeds were stored in wet sand for 20 days and then dipping in the 100 mg x L(-1) GA3 for 30 days before germination, seeds on filter papers germinated at 30/20 degrees C. The first germination-counting time was the 15th day of the test and the final time was the 50th day. Seed viability was tested by TTC method.
CONCLUSIONThe seed testing methods for quality items of A. villosum, including sampling, purity analysis, verification of genuineness, weight, moisture content, percentage germination and seed viability of A. villosum had been initially established.

10.Genetic relationship on several medicinal plants in Marsdenia from Yunnan in ISSR marker.

OBJECTIVETo analyze the genetic relationship of 9 Marsdenia species from Yunnan, especially the traditional Dai medicine "Dai Bai Jie" (M. auricularis).
METHODApplying the inter-simple sequence repeats (ISSR) markers technology.
RESULTTwenty-five primers were screened out of 60 ISSR random primers and produced 391 bands totally, every primer produce 8-21 bands and the mean number was 15.6. The range of the GS (genetic similarity) value was 0.6675-0.8210. In 9 Marsdenia species, M. auricularis is a relative of M. tenacissima. M. balansae and M. officinalis have the closest genetic relationship.
CONCLUSIONIt is supported by ISSR that the M. auricularis which is sib species of M. tenacissima, and the folk medicine of Marsdenia are worthy deep investigation and study.
