BACKGROUND:With the development and improvement of liver transplantation technology, patients waiting for or undergoing liver transplantation have continued to increase in number. Due to their fears and concerns about post-transplantation rehabilitation, patients are under physical and mental stress, most of whom are shown to have a variety of mental disorders that affect rehabilitation. At present, mental problems of liver transplant patients are more concentrated in post-transplantation research, while patients waiting for liver transplantation do not get enough mental assessment and intervention.
OBJECTIVE:To investigate the suitable mode of cognitive-behavioral therapy for liver transplant recipients waiting for donor liver and to evaluate the effect to improve patient’s physical and mental state.
METHODS:Twelve liver transplant patients who underwent pre-transplantation assessment were randomly divided into experimental group and control group, each group with six cases. The control group received conventional treatment. The experimental group received cognitive-behavioral therapy in addition to conventional treatment. Self-rating anxiety scale (SAS) and vital signs were measured in the two groups on admission, 1 week and 3 weeks after admission.
RESULTS AND CONCLUSION:The SAS score of the experimental group was lower than that of the control group at 1 and 3 weeks after admission, and SAS score decreased as the intervention extended. The systolic blood pressure of the experimental group was lower than that of the control group at 3 weeks after admission, but there was no significant difference in diastolic blood pressure between two groups. The heart rate of the experimental group was lower than that of the control group at 1 and 3 weeks after admission. The respiratory rate of the experimental group was lower than that of the control group at 3 weeks after admission. Cognitive-behavioral therapy can reduce the level of anxiety and keep vital signs stable with good feasibility and effectiveness in patients waiting for liver transplantation.

Gastrointestinal cancers are the common malignant tumors of the digestive system, and their morbidity and mortality are in the forefront of malignant tumors. Currently, cancer immunotherapy is the hottest topic in cancer research field. Although cancer immunotherapy has achieved some results in the fundamental research and clinical application of gastrointestinal tumors, there are still a series of problems that need to be resolved. In this article, we review the fundamental and clinical research progress of several common methods of cancer immunotherapy in the field of gastrointestinal tumors.

Objective To summarize the existing animal models of cholelithiasis,and to explore a pathological model that can better reflect the characteristics of clinical syndromes of traditional Chinese and western medicine and meet the needs of the development of clinical and basic research of traditional Chinese medicine. Methods The animal models for experimental research on cholelithiasis at home and abroad were collected and sorted out by searching CNKI,Wanfang,VIP,SinoMed,PubMed and other databases. The animal types,modeling methods,modeling cycles,detection indicators and positive drugs of the models were statistically analyzed. Results Among the 128 articles included,the animal types of cholelithiasis models were mainly guinea pigs,rabbits and C57BL/6J mice. The most used modeling method is high-fat diet,and the feeding cycle takes eight weeks to complete. High-frequency detection indicators were stone formation rate,total cholesterol,phospholipids,total bilirubin,total bile acid,etc. The commonly used intervention methods are traditional Chinese medicine compound,western medicine and single Chinese medicine or extract. Ursodeoxycholic acid (UDCA) is mainly used for western medicine intervention,whereas the intervention of traditional Chinese medicine mainly includes traditional Chinese medicine compound,acupuncture,exercise,diet and other methods. The positive control drug in the experiment was mostly UDCA. Conclusion With the continuous improvement and development of cholelithiasis animal model,there are many kinds of modeling methods to simulate the clinical manifestations of cholelithiasis in Chinese and western medicine,but there are also some limitations. This paper aims to provide reference for the selection,application and improvement of cholelithiasis model through data mining and characteristic analysis.

Objective To explore the expression of liver fatty acid binding protein (L-FABP) in tissues of hilar cholangiocarcinoma and the relationship between expression of L-FABP and clinicopathological factors and prognosis of the patients.Methods The retrospective case-control study was conducted.The clinicopathological data of 132 patients with hilar cholangiocarcinoma who were admitted to the Navy General Hospital between January 2003 and January 2013 were collected.The expression of L-FABP in tumor tissues and adjacent tissues of hilar cholangiocarcinoma and normal bile duct tissues were respectively detected by immunohistochemistry.Observation indicators:(1) expression of L-FABP by immunohistochemistry;(2) relationship between clinicopathological factors of patients and expression of L-FABP in tumor tissues;(3) follow-up and survival situations;(4) prognostic analysis of patients after radical resection of hilar cholangiocarcinoma.Follow-up using outpatient examination and telephone interview was performed to detect postoperative overall survival time up to June 2017.Count data were described as percentage and compared using the chi-square test.The survival time was calculated by the Kaplan-Meier method.Measurement data with skewed distribution were described as M (range).The univariate analysis and multivariate analysis were respectively done using the nonparametric test and COX regression model.Results (1) Expression of L-FABP by immunohistochemistry:the positive expressions of L-FABP were located in the cytoplasm.The low,moderate and high expression rates of L-FABP in tumor tissues were respectively 11.36％ (15/132),71.97％ (95/132) and 16.67％ (22/132),and positive-staining cells showed platy and / or diffuse distribution;the low,moderate and high expression rates of L-FABP in adjacent tissues of hilar cholangiocarcinoma were respectively 77.27％ (102/132),7.58％ (10/132) and 15.15％ (20/132),and positive-staining cells showed scattered or platy distribution,with a weaker staining intensity compared with tumor tissues;there was no positive expression in normal bile duct tissues.There was a statistically significant difference in expressions of L-FABP among tumor tissues and adjacent tissues of hilar cholangiocarcinoma and normal bile duct tissues (x2=5.423,P ＜ 0.05).(2) Relationship between clinicopathological factors of patients and expression of L-FABP in tumor tissues:cases with low,moderate and high expressions of L-FABP in tumor tissues were respectively 10,30,5 in 45 patients with tumor diameter ＜ 3 cm and 4,29,9 in 42 patients with 3 cm ≤ tumor diameter ≤ 5 cm and 1,36,8 in 45 patients with tumor diameter ＞ 5 cm,with a statistically significant difference (x2 =10.171,P＜ 0.05).(3) Follow-up and survival situations:132 patients were followed up for 5-90 months,with a median time of 33 months.During the followup,postoperative overall median survival time of 132 patients was 31 months.(4) Prognostic analysis of patients after radical resection of hilar cholangiocarcinoma:results of univariate analysis showed that tumor differentiation,lymph node metastasis and expressions of L-FABP in tumor tissues were related factors affecting prognosis of patients after radical resection of hilar cholangiocarcinoma (Z =1.845,3.156,1.243,P＜0.05).Results of multivariate analysis showed that tumor differentiation,lymph node metastasis and expressions of L-FABP in tumor tissues were independent factors affecting prognosis of patients after radical resection of hilar cholangiocarcinoma (odds ratio =0.431,1.806,3.692,95％ confidence interval:0.292-0.693,0.974-2.973,1.875-11.364,P＜0.05).Conclusions The high expression of L-FABP in tumor tissues is significantly correlated with the tumor diameter.Tumor differentiation,lymph node metastasis and expressions of L-FABP in tumor tissues are independent factors affecting prognosis of patients after radical resection of hilar cholangiocarcinoma.

Objective:To compare the clinical characteristics of congenital chylothorax in preterm and term infants.Method:From January 2011 to December 2019, the clinical data of infants with congenital chylothorax admitted to our hospital were retrospectively analyzed. The infants were assigned into preterm group (<37 weeks) and term group (≥37 weeks) according to their gestational age. The general information, clinical manifestations, laboratory results, treatment and prognosis of the two groups were compared.Result:A total of 34 infants with congenital chylothorax were included, including 11 premature infants and 23 term infants. No significant differences existed in gender, delivery mode, prenatal diagnosis of pleural effusion, congenital heart disease/chromosome abnormality, birth asphyxia, dyspnea, fetal edema, and location of effusion between the two groups ( P>0.05). Compared with term group, preterm group had significantly fewer leukocytes [3 245(1 007, 7 403)×10 6/L vs. 10 214(6 233,16 458)×10 6/L] and lower protein level [(28.1±7.6) g/L vs. (33.3±6.3) g/L] in the pleural fluid ( P<0.05). No significant differences existed in the proportion of pleural lymphocytes between the two groups ( P>0.05). The proportion of mechanical ventilation (MV) in the preterm group was statistically higher than that the term group [100%(11/11) vs. 65.2%(15/23)], and the duration of MV was statistically longer than the term group [(16(10,25) d) vs. (1(0,11) d)] ( P<0.05). No significant differences existed between the two groups in the application of other treatment options (thoracentesis/drainage, fasting, octreotide and erythromycin pleural injection), time needed for the disappearance of effusion, duration of hospital stay and cure/improvement rate ( P>0.05). Conclusion:Preterm infants may have lower leukocyte count and protein level in the pleural effusion than the term infants. Both preterm and term infants have higher proportion of lymphocytes in the pleural effusion fluid. Although most preterm infants need ventilator support after delivery, most of them achieve complete remission after conservative treatment and the overall prognosis is as good as term infants.

The liver is closely associated with inflammation and the redox response， and inflammation is the body's innate defense system for clearing away harmful stimuli and participating in the liver's wound-healing response.Oxidative stress is associated with the activation of inflammatory pathways， and sustained inflammation and the corresponding regenerative wound healing response can induce fibrosis， cirrhosis， progression to end-stage liver disease or hepatocellular carcinoma， and ultimately death.Some "medicine and food homology" traditional Chinese medicine has been used in clinical effect， showing the ability to protect the liver.This paper reviewed the relationship between liver and oxidative stress response and inflammation response， and sorted out 110 "medicine and food same origin" traditional Chinese medicines based on the Chinese Pharmacopoeia（2020 edition） and the Chinese materia medica.The results showed that common floweringqince fruit， licorice root， cassia seed， emblic，seabuckthorn fruit，Chinese date， honeysuckle， ginger， cape jasmine fruit， platycodon root， lotus leaf， dandelion， reed root， honey， mountain honeysuckle， milkvetch root， glossy ganoderma， Gastrodia gastrodia and eucommia leaf were recorded to have liver protection effects.The liver protection mechanism is mainly anti-inflammatory， antioxidant and lipid peroxidation inhibition. Some Chinese herbs can also play a liver protection role by inhibiting the growth of hepatitis virus and liver cancer cells and regulating bile acid metabolism.In addition， the biological mechanism of its liver protection effect through antioxidant and anti-inflammatory effects in animal experiments was analyzed， and it was found that it plays a role through multiple pathways and multiple targets， providing new ideas for the role of "medicine and food homology" traditional Chinese medicine in the treatment strategy of liver diseases.

Saponins are widely found in various Chinese medicines such as Ginseng Radix et Rhizoma， Notoginseng Radix et Rhizoma， and Bupleuri Radix. They possess multiple biological activities， including anti-inflammatory， antioxidant， antitumor， hepatoprotective， lipid-lowering， and hypoglycemic effects. They play an important role in the prevention and treatment of diseases such as acute lung injury， hyperlipidemia， and diabetic cardiovascular complications， and hold significant research potential and value. Metabolism-related fatty liver disease is a prevalent chronic liver disease characterized by excessive lipid accumulation in hepatocytes. According to the "multiple hit" theory， its occurrence is the result of systemic homeostasis disorder， influenced by abnormal lipid metabolism， oxidative stress， inflammation， insulin resistance， mitochondrial dysfunction， and intestinal microbiota. However， there is currently no effective treatment available in clinical practice. Recent studies have found that Chinese herbal saponins can alleviate metabolism-related fatty liver disease through various pathways， including regulating abnormal lipid metabolism， inhibiting inflammatory responses， alleviating oxidative stress， reducing insulin resistance， improving mitochondrial dysfunction， modulating intestinal flora， inhibiting hepatocyte programmed death， regulating liver autophagy， and correcting immune imbalances. Therefore， this review summarized the pharmacological effects and related mechanisms of Chinese herbal saponins in improving metabolism-related fatty liver disease in recent years， aiming to provide a theoretical reference for the clinical application and experimental research of Chinese herbal saponins in the prevention and treatment of metabolism-related fatty liver disease.

Objective To explore syndrome differentiation of traditional Chinese medicine(TCM)and medication rules of functional dyspepsia,and to provide reference for modern TCM clinical treatment.Methods The clinical research literature on the treatment of functional dyspepsia with TCM were searched from CNKI,Wanfang,VIP,PubMed and Web of Science databases,TCM compounds were collected and database was established.Excel 2019,Clementine 12.0 and SPSS 21.0 were used for frequency analysis of TCM syndrome types,drug frequency,efficacy classifications,properties,tastes,and meridian tropisms of functional dyspepsia,and association rule analysis,factor analysis and cluster analysis were performed on high-frequency drugs.Results A total of 428 articles were included,and 442 prescriptions were extracted,involving 225 Chinese medicines and 24 high-frequency drugs,among which Codonopsis Radix,Poria,Citri Reticulatae Pericarpium and Bupleuri Radix were most frequently used.Tonic deficiency drugs and regulating qi drugs were the most common efficacy categories.The property was mainly warm,the tastes were mainly bitter,pungent and sweet,most of which belonged to spleen,lung and stomach meridians.Twenty drug combinations were obtained by association rule analysis,nine common factors were extracted by factor analysis,and drugs could be divided into four categories by cluster analysis.Functional dyspepsia with the liver-stomach disharmony is the most common syndrome.Twelve drug combinations were obtained by association rule analysis,three common factors were extracted by factor analysis,and drugs could be divided into two categories by cluster analysis.Conclusion The clinical treatment of functional dyspepsia in TCM is mainly to replenish qi and invigorate spleen,dredge liver and promote qi,replenish deficiency with sweet and warm drugs,disperse pungent and descend bitter,which were compatible with drugs for resolving dampness,relieving external symptoms and clearing heat according to syndrome differentiation.

Background: Gastric cancer is one of the common gastrointestinal malignancies. Early diagnosis can reduce the mortality rate significantly. In the Chinese consensus published in 2017, the New Gastric Cancer Screening Scoring System was recommended to be used for risk stratification of gastric cancer. Aims: To preliminarily explore the value of the New Gastric Cancer Screening Scoring System in early gastric cancer screening in asymptomatic community population. Methods: At several communities in Suzhou City Xiangcheng District, a questionnaire survey was conducted in asymptomatic community residents willing to accept voluntary serum tests to collect information on high risk factors of gastric cancer. Serum pepsinogen (PG), PGⅡ, gastrin 17 (G-17) and Helicobacter pylori (Hp) IgG were tested simultaneously. Risk stratification of gastric cancer was carried out in accordance with the New Gastric Cancer Screening Scoring System. Gastroscopy was recommended for moderate to high risk individuals. Results: A total of 540 asymptomatic individuals completed the study, of which 11 were categorized as high risk (2.0%), 168 as moderate risk (31.1%), and 361 as low risk (66.9%). Sixty-four moderate to high risk individuals completed the gastroscopy with a response rate of 35.8%. Four precancerous lesions were detected (6.2%), including 3 gastric low-grade intraepithelial neoplasia and 1 duodenal adenoma. No gastric cancer was detected. Conclusions: The New Gastric Cancer Screening Scoring System is useful for risk stratification of gastric cancer in asymptomatic population and may provide a basis for further endoscopic examination. However, the value of this scoring system in low risk areas of gastric cancer needs to be further verified.
