1.The application of the external treatment of Glauber′s salt on cytokines of acute pancreatitis patients

Objective To discuss the application of glauber's salt external impact on cytokines in patients with celiac inflammation and clinical application value.Methods Clinical data of 180 patients with acute pancreatitis were retrospectively analyzed.According to the different treatment,they were divided into the observation group and control group,the control group were treated by routine western medicine,the observation group were treated with mirabilite topical therapy,records of two groups of patients with clinical therapeutic effect.Results The total effective rate of the observation group was 92.22%,which of the control group was 82.22%.The differences between the two groups was statistically significant (χ2 =4.037,P <0.05).The cytokines concentrations of the observation group after treatment were as follows:TNF alpha (40.02 ±5.19)ng/L,IL -6 (32.93 ±7.65)ng/L,IL -8 (23.41 ±3.86)ng/L, NF -κB (85.36 ±19.65)U /L;those in the control group after treatment:TNF alpha (63.38 ±8.97)ng/L,IL -6 (56.73 ±11.16)ng/L,IL -8 (40.85 ±7.43)ng/L,NF -κB (231.32 ±41.28)U /L,statistical analysis showed that contrast differences among groups with statistical significance (t =21.384,16.687,19.760,16.687,all P <0.05).The observation group (3.18 ±1.33)d abdominal distension relief time,gastrointestinal function recovery time (5.64 ±1.86)d,length of hospital stay (15.13 ±3.76)d;The control group (5.64 ±2.31)d abdominal distension relief time,gastrointestinal function recovery time (8.96 ±3.45)d,length of hospital stay (24.59 ±5.87)d;Statisti-cal analysis showed that contrast differences among groups with statistical significance(t =8.755,8.035,12.874,all P <0.05).Conclusion Using glauber's salt topical application in patients with celiac inflammation can effectively reduce inflammation in the body,improve the treatment effect,to speed up the patients with gastrointestinal functional recovery,which is worth popularization and application in clinic.
2.The clinical effect of inguinal hernia repair without tension on male sexual function and quality of life

Objective To discuss the effects on sexual function and quality of life by application of inguinal hernia without tension repair surgery in male patients,and the value of clinical application.Methods 1 10 cases of inguinal hernia patients were randomly divided into 2 groups,with each group of 55 cases.The control group was given conventional surgery,the observation group was conducted with inguinal hernia without tension repair surgery,clinical therapeutic effects of two groups were recorded.Results Testicular volume of the observation group 3 months after operation was (10.21 ±2.61)cm3,testicular artery biggest blood flow velocity was (3.38 ±1.21)cm/s.Testicular volume of the control group 3 months after operation was (10.14 ±2.63)cm3,testicular artery biggest blood flow velocity was (3.36 ±1.23)cm/s,the differences between the two groups had no statistical significance (t=0.1401, 0.0860,all P>0.05).The observation group had physiological function score of (76.64 ±6.29),physiological role score of (57.86 ±5.97),body pain score of (79.27 ±7.88)points,overall health score of (78.85 ±7.11mm), energy score of (64.35 ±5.63)points,social function score of (56.64 ±5.89)points,emotional functions score of (68.56 ±6.64)points and mental health score of (72.75 ±6.82)points.The control group had physiological function score of (70.03 ±3.78)points,physiological role score of (47.13 ±3.66),body pain score of (71.09 ± 5.14)points,overall health score of (69.93 ±5.05mm),energy score of (57.81 ±3.25)points,social function score of (50.33 ±4.14)points,emotional functions score of (60.76 ±5.15)points and mental health score of (65.22 ± 4.09)points,the differences between the two groups were statistically significant (t=6.680 1,11.363 7,6.448 1, 7.585 5,7.461 0,6.500 0,6.883 9,7.022 3,all P<0.05).Conclusion Application of inguinal hernia without tension repair surgery in male patients has mild effect on sexual function,it could improve patients quality of life,but the long-term effects remain to be further observed and summarized.
3.Research on performance tests and evaluation for mCT PET/CT

Objective:It is to discuss results and significance of PET/CT in the performance tests.Methods: According to NEMA NU 2-2007 Performance Measurements of Positron Emission Tomographs and GB 17589-2011 specifications for quality assurance test for computed tomography X-ray scanners standards, with phantoms of nuclear medicine and CT, performance testing was done.Results: Spatial Resolution (FWHM): transverse 1 cm 4.16 mm, axial: 1 cm 4.04 mm, transverse 10 cm 4.92 mm, axial: 10 cm 5.74 mm. Sensitivity: 9.6 s-1·kBq-1, scatter fraction: 35.4%, peak NEC rate: 1.69×105 s-1.Conclusion: According to manufacturers and national standards, the results of performances testing are up to standard. It is necessary to enhance the effect of in quality control(QC) of PET/CT and the fundamentality of the performance tests, so that the devices come to the demands for clinical practice.
4.SPECT system performance test and analysis of results

Objective To analyze the influence factors on, and the relationship between, the system performance parameters by testing SPECT equipment.Methods By reference to National Electrical Manufactures Association standards and manufacturer's specifications, the performances of a total of 31 SPECTs in 12 provinces were measured, for the first time, with regard to their system spatial resolution (SSR) , system planar sensitivity (SPS) and tomographic spatial resolution (TPR).Results The results were as follows: (7.90 ± 0.62) mm for SSR, with the highest 9.46 mm and the lowest 7.04 mm;(78.54± 13.17)s-1· MBq-1 for SPS, with the highest 123.80 s-1 · MBq-1 and the lowest 56.70 s-1·MBq-1;and (13.12 ± 2.59) mm for TSR, with the highest 18.13 mm and the lowest 8.45 mm.These values indicated a nearly consistent upward and downward trend and could meet the clinical requirements by comparison with the manufactuer' s specifications.Conclusions Mutual restrictions have been shown between SSR and SPS.Increased thickness of the system crystal plane can improve the SPS, but also has a negative impact on the SSR.For all the above reasons, it is the optimum solution to choose the right type of collimator and crystal thickness for different clinical applications.
5.A study on social help for the medical care of senior citizens with great financial difficulties(SCGFD) in Luwan District of Shanghai

Supported by various departments concerned of the district government, medical workers in Luwan District of Shanghai started in 1994 to set up in the whole district community based social help network for the medical care of SCGFD. In the last 4 years, the annual outpatient volume of SCGFD averaged 3.9 times and the annual visits received by SCGFD averaged 2.2 times. The conditions of 78.1% of SCGFD suffering from an illness turned for the better or became stable. The annual hospitalization utilization ratio averaged 5.4%. After hospitalization, 89.5% of SCGFD suffering from a serious illness fully recovered from their illness or were discharged after turning for the better. The average outpatient expenses per time, hospitalization expenses per day and per time of SCGFD respectively dropped 32.1%, 21.5% and 30.4%, as compared with the group of senior citizens enjoying free medical care. The authors hold that the social help network for the medical care of SCGFD in Luwan District is an easy to operate and effective community based medical security system for SCGFD. It has produced sound social repercussions after being put into practice. The controlled use of medical funds has ensured the normal operation of the network.
6.Dense embolization of intracranial aneurysms

Objective To investigate technique methods of dense emolization of intracranial aneurysms, factors resulting incomplete embolization. Methods 58 patients with intracranial aneurysms, using different embolization technique treated narrow and wide aneurysms with GDC. Dense emolization rate of aneurysms was elevated. Results dense emolization 50 aneurysms (86.2%), 95% in 3 aneurysms (5.2%), 90% in 3 aneurysms (5.2%), 80% in 2 aneurysms (3.4%). All embolizated aneurysms showed no enlarging recurrence or rebleeding.Conclusions Dense embolization of intracranial aneurysms should try to obtain.
7.Application of the Trufill detachable coil embolization in treatment of acute ruptured intracranial aneurysms

Objective To summarize the application of endovascular treatment of the acute ruptured intracranial aneurysms with the new Trufill detachable coil system.Methods Thirty five patients with the acute ruptured intracranial aneurysms were treated with DCS or DCS Orbit within 72 hours. Results All 35 cases were successful with compact embolization in 29 aneurysms,95% in 3 aneurysms,90% in 2 aneurysms,80% in 1 anearysm.One case hemorrhaged during operation and 1 case had thrombosis after operation,but with good prognosis of both.One case died of pneurmonia.Thirty cases achieved clinic follow-up 1 to 15 months after operation with only one rebleeing.Sixteen cases achieved DSA follow-up 3 to 12 months after the operation.Partial recurrence of the occluded aneurysm was shown in 1 case.Conclusions Trufill DCS or DCS Orbit is a safe,reliable and effective therapy for the endovascular treatment of intracraranial aneurysms.
8.Antithrombotic strategy for non-cardiac surgery after percutaneous coronary intervention

Objective To elucidate whether taking Ⅱ b/Ⅲ a receptor antagonist instead of oral antiplatelet drugs during perioperative in patients with drug-eluting stent implantation undergoing non-cardiac surgery would play a preventive role of stent thrombosis,without increasing surgical bleeding.Methods Six patients aged 60 -75 years old with drug-eluting stent implantation within 1 year taking dual antiplatelet drugs without any chest pain,and whose heart function classification for two (NYHA) were enrolled.They underwent surgical treatment due to ineffective conservative treatment of surgical disease,5 days before surgery intravenous infusion tirofiban 0.1 μg/( kg · min) micro pumps continuously instead of oral dual antiplatelet drugs,2 hours before surgery stop tirofiban and re-application of tirofiban 0.1 μg/( kg · rain) after surgery in the intensive care unit,and replacing tirofiban with oral dual antiplatelet as soon as possible according to the situation.Analyze cardiovascular events,especially stent thrombosis events and seriously bleeding,tirofiban adverse drug events during perioperative.Results Six patients have no perioperative malignant ischemic ventricular arrhythmia,angina,myocardial infarction,sudden cardiac death,no massive bleeding and adverse drug reactions.Conclusion Substitution of oral dual antiplatelet drugs for Ⅱ b/Ⅲ a receptor antagonists to prevent stent thrombosis treatment during perioperative in patients with drug-eluting stent implantation undergoing non=cardiac surgery may be feasible and safe,but needs to be further confirmed through large sample of randomly controlled trials.
9.Value of histogram analysis of susceptible signal intensity in differentiating papillary from chromophobe renal cell carcinoma

Objective To evaluate the feasibility of histogram analysis of susceptible signal intensity in differentiating papillary (pRCC) from chromophobe renal cell carcinoma(ChRCC). Methods Thirteen cases with pRCC and 9 cases with ChRCC, who underwent susceptible-weighted imaging(SWI), MR common scanning and enhancement, were assessed retrospectively. The histogram parameters of susceptible signal intensity were measured, including maximum, minimum, mean, median, skewness and kurtosis. The independent samples t test(normal distribution) and Mann-Whitney rank sum test(skewed distribution) were used to compare the differences in SWI parameters between pRCCs and ChRCCs. Receiver operating characteristic curve was used to evaluate the efficiency of the whole-tumor SWI parameters in differentiating pRCCs from ChRCCs. Results The significant differences of the minimum, mean, median and skewness between pRCCs and ChRCCs were present(P<0.05), and there was no significant differences in maximum and kurtosis between between pRCCs and ChRCCs(P>0.05). In all the histogram parameters, the area of ROC curve of mean value was largest(0.80). The sensitivity of minimum value (84.62%) was the highest and and the specificity of skewness(100.00%) was the highest. Conclusion Histogram analysis of susceptible signal intensity can help differentiate pRCCs from ChRCCs.
10.Study on coronary artery angiography and revascularization in day care ward of heart center

Objective To investigate the feasibility, safety and impact on performance indicators of coronary angio graphy, percutaneous coronary intervention and coronary artery bypass grafting (CABG) in day care ward of heart center. Methods 128 patients performing coronary angiography and receiving stent implantation or coronary artery bypass surgery when necessary from October 2014 to December 2016 were retrospectively analyzed. The success rate of procedure, intraoperative and postoperative adverse cardiac events (death, myocardial infarction and interventional complications: coronary artery spasm, dissection, perforation and occlusion, contrast agent allergy, upper extremity edema, patients with osteofascial compartment syndrome and radial artery occlusion), average hospitalization days and medical expenses were compared with patients at the same period in general ward. Results Seventy-six cases underwent coronary angiography, among whom 35 cases received stent implantation and 17 cases received coronary artery bypass grafting. The success rate was 100%. Adverse events were radial artery spasm in 5 cases, puncture site hematoma in 3 cases and contrast agent allergy in 1 case. Day care ward significantly shortened the average hospitalization days (P<0.05); there was a downward trend, although there was no statistical difference in hospitalization expenses (P>0.05). Conclusions Coronary angiography and revascularization at the day care ward model is safe and feasible in the heart center if diagnostic and therapeutic techniques are mature.
