2.Clinical pathological analysis of 201 young women with breast cancer

Objective To explore the clinicopathological characteristics in young women with breast cancer.Methods The clinicopathological data of 201 cases of breast cancer with age below 35 years from Apr.1997 to Apr.2010 in Anyang Tumor Hospital were retrospectively analyzed and compared with those of 251 cases of breast cancer patients selected randomly with age more than 60 years during the same period.All these cases had undergone surgical treatment and been confirmed by pathologist.Results In the young group,the percentage of patients with the course of disease less than six months was higher than that of the old-aged group [82.09 % (165/201),72.11% (181/251)] (x2 =6.19,P < 0.05).The incidence of the tumor size ≤ 2 cm in the young group was less than that of the old-aged group [19.90 % (40/201),30.28 % (76/ 251)] (x2 =6.302,P < 0.05).The incidence rate of metastasis of lymph nodes in the young group was less than that of the old-aged group [63.68 % (128/201),54.19 % (136/251)] (x2 =4.145,P < 0.05).The incidence rate of stage 0~ Ⅰ in the young group was more than that of the old-aged group [11.94 % (24/201),17.53 % (44/251)] (x2 =2.729,P > 0.05).Conclusion The tumor size in the young group is larger than that in the old-aged group,pTNM classification in the young group is later than that in the latter group and the incidence rate of metastasis of lymph nodes in the young group is higher than that in the latter group.In young patients with breast cancer the percentage of tumors with advanced tumor stage and poor prognosis are more than that in elder breast cancer patients.In addition,these young patients have lower awareness to mammary adenocarcinoma.Young ladies should examine breast cancer by themselves and go to hospital for regular inspections in order to discovere,diagnose and treat cancer earlier.
3.Correlation of quantitative examination of urinary sediment bacteria and urine culture

Objective To evaluate quantitative examination of urinary sediment bacteria as a basic feasibility of screening indicators for urinary tract infection .Methods 191 outpatients and inpatient specimens were gathered firstly to implement a urine culture ,and then the rest of the urine were used for sediment bacteria quantitative testing .Meanwhile ,bacterial culture was conducted as the standard .According to the results of bacterial culture ,receiver operating characteristic(ROC) was drawn ,the threshold values of leukocyte and bacteria counts for diagnosis of urinary tract infection were found out and its sensitivity ,specificity ,positive / negative predictive value ,false positive/false negative rate and accuracy were calculated .Results The positive rate of urine culture was 39 .7% ,and the most common pathogen was Escherichia coli .The threshold value of bacteria and leukocyte counts for diagnosis of urinary tract infection was 1 024 .5/μL and 135 .8/μL respectively .When combined leukocyte and bacteria counts for urinary tract infection ,the optimum sensitivity was 62 .5% ,specificity was 98 .1% ,positive predictive value was 95 .7% ,negative predictive value was 79 .6% ,false positive rate was 1 .9% ,false negative rate was 37 .5% ,and accuracy was 83 .8% .Conclusion With UF‐1000i urinary sediment analyzer ,the combined determination of leukocyte and bacteria counts can remove the great mass of negative results ,Especially the results of bacterial culture positive predictability is higher ,but still can not replace of quantitative bacterial culture .
4.Application of Spinal Ultrasound in Children with Congenital Anomalies of Spine

ObjectiveTo assess the value of spinal ultrasound in children with congenital anomalies of spine. MethodsUltrasound findings of 36 children with congenital anomalies of spine were analyzed and compared with the finding of corresponding MRI or operation. ResultsUltrasound finding was the same as MRI in the anatomy in 31/36 children, in addition, it also could show the pulsatile motion and blood flow signals of conus medullaris. ConclusionSpinal ultrasound can present more other informations for diagnosis for congenital anomalies of spine, especially the blood supply of conus medullaris.
5.Application of Color Doppler Ultrasonography for Superior Mesenteric Artery Diseases in the Aged

Objective To evaluate the value of color Doppler ultrasonography for diagnosis of superior mesenteric artery diseases. Methods78 patients with abdominal pain suspected to be superior mesenteric artery diseases underwent color Doppler ultrasonography. The results were compared with other imaging methods or findings in operation. Results4 patients with superior mesenteric artery embolization coincided with the founding in operation. 18 of the 22 patients suggested with superior mesenteric artery stenoses were confirmed by CTA or DSA. ConclusionColor Doppler ultrasonography can be used in diagnosis of superior mesenteric artery diseases.

In order to construct a novel multifunctional targeting nano-carrier based on pH-redox characteristics of tumor microenvironment,ketone bonds and disulfide bonds were bonded to the oligomeric hyaluronic acid (oHA) being sensitive to pH and the reduction environment.The chemical structure of oligomeric hyaluronic acid-8-mercaptomenthone 1,2-glycerolketal (oHMST) was characterized by 1H NMR,IR and ESI-MS.Curcuminloaded micelles were prepared by dialysis.The single factor investigation was carried out on the dosage form.Some properties,including particle size zeta potential,the morphology of micelles,and pH-sensitivity were studied.The materials were synthesized successfully.The micelles were spheric with a diameter of about 100 nm.The Zeta potential of the micelles was-(21.97 ± 1.08) mV.The in vitro test showed that oHMST carriers have good pH-sensitivity and redox-sensitivity.
7.Expression of fibroblast growth factor-2 and fibroblast growth factor receptor-4 in human papillary thyroid carcinoma and their significances

Objective To investigate the expression of fibroblast growth factor-2 (FGF-2) and fibroblast growth factor receptor-4 ( FGFR-4 ) in the papillary thyroid carcinomas ( PTC ) and clinical significance . Methods Immunohistochemistry and Western blotting for the expression of FGF-2 and FGFR-4 were performed in 89 cases of PTC and 30 cases of normal thyroid tissues ( NTT) adjacent to the tumors .Results Immunohistochemistry results showed that , FGF-2 and FGFR-4 expressions were high in thyroid carcinoma (P<0.01,P<0.01) in contrast to that in the normal thyroid tissues, and the difference was statistically significant;There was a positive linear correlation between expressions of FGF-2 and FGFR-4 and lymph node metastasis (χ2 =14.798,P<0.01;χ2 =7.27,P<0.01)and differentiation degree (χ2=13.824,P<0.01;χ2 =16.921, P<0.01) in papillary thyroid carcinoma ,while there was no difference in gender ,age and tumor size(P>0.05).Analyzed by Western blotting technique ,FGF-2 and FGFR-4 expressions in thyroid carcinoma were significantly higher than that in normal tissue ,with decrease of cancer degree of tissue differentiation and significantly up regulated expression (P<0.05).Expressions of FGF-2 and FGFR-4 were in a positive linear correlation in the disease (rs=0.434,P<0.01).Conclusion The expressions of FGF-2 and FGFR-4 are correlated with papillary thyroid cancer and they participated in the process of invasion and metastasis , both of which have a positive synergistic effect .The degree of malignancy and biological behavior are meaningful and comprehensive indicators ,which provide a theoretical basis for the subsequent experimental studies of cellular and molecular biology .
8.Three-dimensional finite-element analysis of maxillary protraction based on labiolingual appliance.

OBJECTIVEThis study aims to analyze the biomechanical effects of four sutures that are related to maxillary growth during a maxillary protraction treatment cycle based on labiolingual appliance.
METHODSA three-dimensional simulation of labiolingual appliance was performed based on a three-dimensional finite-element maxillofacial model combined with solid-modeled sutures.
RESULTSThe displacement and stress distributions were calculated. The labiolingual appliance was suitable for patients with underbites or deep overbites. Sutura frontomaxillaris and sutura zygomaticomaxillaris were vital to the forward displacement and counterclockwise rotation of the maxillary. The changes in sutura zygomaticotemporalis and sutura palatine were not significant.
CONCLUSIONThe influences of the labiolingual appliance on the sutures during the treatment cycle of maxillary protraction can be effectively simulated by the three-dimensional finite-element maxillofacial model.

Objective:To establish the high-level clinical research platform to promote scientific and technological innovation ability in hospitals.Methods:Summarize and share the practical experience of integrating and optimizing the construction of scientific research platform in our hospital, this paper aimed to illustrate the important role of scientific research platform in promoting the innovation ability of science and technology in hospital.Results:According to the construction practice of clinical research platform in our hospital, the planning of clinical research platform should be guided by clinical needs and application. At the same time, having better understandings of relevant national policies, as well as the overall national development strategy layout play important roles in establishing a high-starting point clinical research platform from the very beginning.Conclusions:Accordingly, the key factors to improve the hospital's scientific and technological innovation ability mainly include the construction of scientific research platform, the cultivation of dominant and characteristic disciplines, the cultivation of innovative talents, the cross-disciplinary cooperation, the transformation of innovative achievements and the innovative management mechanism, among which the construction of high-level scientific research platform is the most basic guarantee to enhance the scientific and technological innovation ability.
10.Exploration and reflection on the innovative incentive path of medical youth based on two-factor theory under high-quality development

Objective:According to the requirements of high-quality development of public hospitals, to explore the innovative incentive path for medical youth based on the two-factor theory, and provide a reference for promoting the high-quality development of public hospitals.Methods:Using the literature analysis method, the two-factor theory, hospital scientific research incentive mechanism, and scientific research incentive mechanism for young talents were investigated. Meanwhile, combining the two-factor theory and practical experience, the problems that existed in the innovation incentive policy of public hospitals for young medical talents were analyzed, and the corresponding countermeasures were proposed to build the innovation incentive path of young medical talents under the two-factor theory.Results:Based on analyzing the demand characteristics of young medical talents, managers should distinguish health care factors and incentive factors, and implement incentives from both aspects. Provide incentives through improving the personal sense of achievement, creating a personal growth environment, and promoting professional titles to stimulate young talents' innovation motivation; implement health care factors from aspects of working conditions, material benefits, salary levels, etc.Conclusions:As a new concept of development, high-quality development is not only reflected in scientific and technological innovation-driven, but also in the innovation of management mechanisms so that institutional innovation becomes the driving force for high-quality development.
