1.Clinical outcome of CR type mobile bearing knee prosthesis

[Objective]To evaluate the role of mobile bearing knee prosthesis in TKA and the clinical results.[Method]A total of 301 TKAs by using mobile bearing knee prosthesis were gotten average 48.7 months follow-up.HSS knee scores and X-ray films were to used to evaluate the results.[Result]The mean HSS score improved from 51.3 before operation to 91.3 after operation.The ROM improved from extension/flexion(-9.5?/64.3?)before operation to extension/flexion(0?/97.5?)after operation.The maximum varus/valgus(25?/15?)before operation were improved to the mean valgus 5.3?.Two cases of septic loosening and 2 cases of femoral fracture were found.[Conclusion]The CR type mobile bearing knee prosthesis not only had advantages in kinematics and wear experiments,but also had achived good clinical result.
2.Search colorectal cancer antigen genes by screening the colorectal tissue cDNA phage expression libraries with SEREX

Objective Identification of colorectal cancer specific antigens from cDNA phage expression library by recombinant expression cloning (SEREX). Methods The cDNA phage expression library derived from colorectal cancer tissue was constructed using the SMART (Switching Mechanism at 5'end of RNA Transcript) techniques. The cDNA phage expression library was screened with SEREX (serological identification of antigen by recombinant cDNA expression libraries), the positive clones encoding antigenic genes were obtained after immunoscreening, and the nucleotide sequences of cDNA inserts were determined and analyzed with BLST software in GenBank, and the antigenic gene function was analyzed by bioinformatics. Results The cDNA phage expression library derived from colorectal cancer tissues was successfully constructed. The primary library consisted of 2.39?10 6 recombinants, and the recombinant rate was more than 97.5%. The titer of the amplified cDNA phage expression library was 4.1?10 10pfu/ml, and the size of inserted cDNA varied from 0.5 to 4.0kb. Sixteen positive clones encoding antigenic genes were obtained after immunoscreening, and results showed that 16 reactive clones were derived from 12 different genes. Ten of 12 genes were highly homologous to the genes known in GenBank, such as IFITM1, CD24, Survivin, KLK6, et al. Two expressed sequence tags (ESTs) were not found in GenBank. The antigenic genes included structural gene, regulation gene and metabolizing gene. Conclusion The tumor-associated antigen genes and the two ESTs were obtained by SEREX were worthy of further study on their structures and functions.
3.Association between the polymorphism of A9570G in angiotensin I converting enzyme 2 gene and cardiac dysfunction and ventricular remodeling after myocardial infarction

Objective To determine the effects of polymorphism of A9570G in angiotensin I converting enzyme 2 gene ( ACE2) gene on cardiac dysfunction and ventricular remodeling after myocardial infarction. Methods 252 patients with old myccardial infarction were included in this study. They were classified according to their polymorphisms of ACE 2 gene analyzed by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP). Echocardiograms were used to determine left ventricular end diastolic diameters (LVEDd) , Left ventricular mass index (LVMI) , mitral flow pattern early diastolic and late diastolic peak flow ratio (E/A) and left ventricular ejection fraction ( LVEF). Results In male, LVEDd, LVMI, and LVEF had significant difference among ACE2 genotypes ( t = 2. 609,3.527and 2.063, P =0.010,0.001 and 0.041), and no significant differences in E/A( t =0.689,P =0.492). In female,LVEDd, LVMI, E/A and LVEF had no significant difference among ACE2 genotypes( F =0. 848, 0.077,0. 985 and 1.611, P = 0. 432,0. 926,0. 377 and 0. 205 ). Conclusion The polymorphism of A9570G in ACE2 gene may be associated with cardiac dysfunction and ventricular remodeling after myocardial infarction in male. ACE 2 gene polymorphism may be a genetic factor on cardiac dysfunction and ventricular remodeling after myocardial infarction.
4.Experimental study of autogenous vein graft connector repairing laryngeal nerve defect in dogs

Objective To investigate the feasibility of autogenous vein graft as a connector to repair recurrent laryngeal nerve(RLN) defect. Methods 20 healthy dogs were randomly devided into 4 groups: group 1(n=6), 2(n=6), 3(n=5) and control group(n=3). Left RLN of dogs in group 1,2,3 were resected for 1,2,3 centimetres respectively and followed by repairment with autogenous vein graft, while RLNS of dogs in control group were resected but not conducted by autogenous vein graft. Six months after operation, the vocal fold movement, phonation and induced action potensial as well as morphologic observation of the aryngeal muslces were studied. Results In group 1, phonation of dogs was all restored to normal. The vocal fold of RLN resected side showed no atrophy symmetrical to the normal side and adducted apparently when phonating. Furthermore thyroarythenoid muscle and posterior cricoarytenoid muscle both could generate induced action potentially. Regenerated nerves in verin graft and distal nerve were observed by light microscope and electron microscope. In group 2,3 and control group hoarseness improved six months after op eration, the vocal fold of RLN resected side was atrophied apparently and fixed paramedianly. Fissure existed in glottis when vocal fold closed, and compensative movement of opposite vocal fold across over the median line was found. Thyoarthenoid muscle and posterior cricoarytenoid muscle unable to generate an action was found. Regenerated nerves were not observed in vein graft or distal nerve by light microscope and electron microscope. Conclusions It is feasible to repair RLN defect in certain length by autogenous vein graft.

6.The selection of treatment modalities for primary liver cancer:a report of 265 cases

Objective To explore the methods and outcome of comprehensive therapeatic for primary liver cancer. Methods Various therapeutic methods and outcome of 265 patients with primary liver cancer admitted in the last five years, were analyzed retrospectively .The group consisted of 238 male, and 27 female patients. The average age 47.5 years. Operation was performed in 215 cases and non-operative treatment is 50 cases. Operative treatment included resection of hepatic cancer and/or adjuvant therapy. Non-operative therapy included transcatheter hepatic chemo-embolization(TACE),percutaneous ethanol injection (PEI)、drug delivery system(DDS)、cryosurgery、radiofrequency ablation(RFA). Results The resection rate, and mortality and morbidity rates were 57%(215/377), 74.9%(161/215), 0.5%(1/215), and 19% respectively. The overall 1-,2-,3-year survival rate were 78.9 %,59.3%,33.5%, respectively, in patients who received comprehensive treatment centered around surgical resection, and 62.3%,32.3%,22.8%, respectively, in 45 patients who received non-surgical resection treatment. Conclusions Liver resection combined with other nonoperative modalities are ideal ways to treat primary liver cancer at the present time, and for different stages of primary liver cancer different treatment modalities are used.
7.Effects of G. Pentaphyllumon on photoaging skin in nake mice

Objective To investigate G. Pentaphyllums influence on the activity of glutathione peroxidase (GSH-PX) and catalase (CAT) in the skin of hireless photoaging mice, as well as the content of hydroxyproline (HYP). At the same time, G. Pentaphyllum's effect on the hireless photoaging mice was studied. Methods Healthy KM hireless mice were randomly divided into five groups. A skin photoaging model was established by G. Pentaphyllum administration. The changes of the activity of GSH-PX, CAT and the contents of dermal hydroxyproline in the skin were then measured by biochemical methods in each group of the experiment. Results In the model group, the activity of GSHPX, CAT and the content of HYP of skin were all decreased significantly (P<0. 05). And there were different degrees of improvements in those of the herb given groups. Conclusion G. Pentaphyllumhas the protective effect on the skin of the photoageing in the hairless mice. The mechanism may be that G. Pentaphyllum could inhibit the oxidative damages of ultraviolet and skin photoaging by increasing the synthesis of collagen in skin.

Objective To investigate the relationship of postoperative complications of thyroid surgery with primary thyroid disease, operation procedures and operative manipulation. Methods The clinical data of 1805 cases of thyroid diseases treated by surgery in our department between 1990~2001 were analyzed retrospectively . The characteristics of onset time of cough variant asthma (CVA) match with the human hypnagogic and arousal time nodes, which are also in accordance with the time of defensive qi entering yin and exiting yang of human body. Reviewing the generation and physiological function of defensive qi, we hold that the onset of CVA is related to the lung, the spleen and the kidney. Deficiency of the three viscera leads to defi ciency of defensive qi defi ciency, and blockade of the circulation pathway of defensive qi on body surface, thus interaction of the two factors results in the special time nods of CVA’s onset, which is also the basic pathomechenism of CVA.
