1.Characteristics of Aphasia after Left Ganglion Impairment and Intervention: 6 Case Report

Objective To explore the speech characteristics and intervention of aphasia after left basal ganglion impairment. Methods Chinese Standard Language Test of Aphasia is used to assess 6 patients with aphasia after left basal ganglion impairment. Schuell's stimulation approach and communication intervention of daily life were used. Results All the patients were difficult for verbal naming and active description. The ability of listening comprehension, speech, and reading improved after intervention. Writing ability improved in 2 patients. Conclusion Aphasia after left basal ganglion impairment appears difficulty of verbal naming and active description. Schuell's stimulation approach combined with communication training of daily life may be helpful to improve their speech.
2.Antagonistic Effects of Ganoderma Spore on Mutation Induced by Sodium Nitrite in Drosophila melanogaster

Objective To study the antagonistic effects of Ganoderma spore on mutation induced by NaNO2 in Drosophila melanogaster. Methods After 24 hours of starvation,male Drosophila melanogaster were transferred to the culture bottles with medium with NaNO2 of 0,0.001,0.01,0.05 and 0.1 g/L for 24 hours and then cultured with female Drosophila melanogaster for 7 days. Male Drosophila melanogaster were treated with NaNO2 at the doses of 0,0.01,0.001 g/L for 24 hours and then transferred to the medium with Ganoderma spores at the dose of 0.2%,1%,2%,cultured with female Drosophila melanogaster for 7 days. The distilled water was used as the negative control. The amount of F1 generation and the mutation of the F1 generation were calculated. Results The mutation Drosophila melanogaster were mainly vestigial,some had malformation with white body and strange eyes. Compared with the negative control group,the count of offspring decreased,the rate of mutation proved positively correlated to the concentration of NaNO2. High dose of sporoderm-unbroken spores could cause a decrease of number of offspring. 3.Diagnosis of Arteriosclerosis Obliterans of Lower Extremity in Early Stage

Objective To explore the methods of early diagnosis of arteriosclerosis obliterans of lower extremity (ASOLE).Methods The related literatures on ASOLE detection means adopted clinically were reviewed,and their advantages and disadvantages were compared.Results Asymptomatic ASOLE could be discovered by determination of ankle brachial index (ABI) and toe brachial index (TBI),which was a good index for arterial function assessment of lower extremity.Pulse wave velocity (PWV) was more vulnerable and less sensitive than ABI,and therefore more suitable for screening of a large sample.ASI was an index to assess arterial structure and function,and it had a good correlation with PWV.Flow-mediated dilation (FMD) was a measurement evaluating the function of endothelial cell;Pulse wave measurement was simple,sensitive,and its result was reliable.Color Doppler ultrasonography could localizate the lesion and determine the degree of stenosis at the same time.Multiple-slice CT angiography (MSCTA) was more accurate than color Doppler ultrasonography,but its inherent shortcomings,such as nephrotoxicity of contrast agent,was still need to be resolved.3D-contrast enhancement magnetic resonance angiography (CE-MRA) had little nephrotoxicity,but a combination of other imaging methods was necessary.Microcirculation detections required high consistency of the measurement environment,but they were simple,sensitive and noninvasive,and therefore could be used for screening of ASO.Conclusion Publicity and education of high-risk groups,and reasonable selection of all kinds of detection means,are helpful to improve the early diagnosis of ASOLE.
4.Observation of layering fibula feet cushion in prevention of deep venous thrombosis after total hip replacement

Objective To evaluate the curative effect of layering fibula feet cushion to prevent deep venous thrombosis(DVT) after total hip replacement(THA).Methods 100 patients who underwent THA were divided into two groups at random.The research group was applied layering fibula feet cushion while the control group was applied general feet cushion.VAS,the occurrence rate of DVT,disappearance time of feet swelling and Harris hip function score were used to evaluate the curative effect.Results In according to these four observation indexes,the score of the research group was better than the control group.The effective rate of the research group was 96%,while the control group was 82%.There was a significant difference between the two groups.Conclusions The layering fibula feet cushion can effectively prevent deep venous thrombosis after total hip replacement.It can be generalized and applied in clinical nursing.

6.Initial therapy indications of acute primary immune thrombocytopenia in children

Objective To explore the initial therapy indications of acute immune thrombocytopenia (ITP) in children based on the classification treatment.Methods Three hundred and eighty newly diagnosed ITP cases were enrolled in this study from Jan.1st 2012 to Apr.30th 2013 in Children's Hospital Affiliated to Shanghai Jiaotong University.In total 380 patients,there were 214 male cases (56.31%) and 166 female cases (43.68%).The cases were divided into observation group and therapy group according to the initial platelet count which was ≤ 30 × 109/L or the bleeding over moderate volume or with active bleeding.Platelet values were observed in the observation group weekly,adrenal cortical hormone and immunoglobulin treatment were adopted in the therapy group,cases were followed up to Aug.30th 2013,9 months on the average.Results Three hundred and five cases showed overall response (80.26%) and 75 cases showed no response(19.74%).One hundred and seventy-eight cases were divided into observation group (46.84%),in which 133 cases (74.72%) showed complete response or response.Two hundred and two cases were included in therapy group (53.16%),in which 167 cases (82.67%) were with complete response or response.There was no statistical difference between the 2 groups in curative effect (Z =-0.54,P =0.59).Forty-five cases in observation group were no response and accepted therapy,35 of them (77.78%) had response.There were equal efficiency in the initial therapy group and the subsequent therapy group (x2 =3.60,P =0.06).There was no difference between the age of onset,sex and season in 2 groups.Cases aged from 1 month to 1 year seemed to have a high incidence because of vaccination,and in the cases aged from 3 to 14 years the onset was related to infection.The children over 3 years old had higher risk factors in self-healing and the curative effect.There was no severe bleeding or adverse effect or dead cases in this study.Conclusions It is feasible to take platelet count ≤30 × 109/L as the threshold for initial therapy indications.Almost half of the cases could avoid overtreatment and pretherapy observation will not reduce the initial cure effect; no severe internal bleeding was observed in all the cases.
7.Effect of Chinese herb drug Tongxinluo on the expression of nestin and insulin-like growth factor 1 mRNA after rat cerebral ischemia-reperfusion injury

Objective To investigate the expression of nestin protein and insulin-like growth factor 1(IGF-1) mRNA in rat brain injured by ischemia-reperfusion(I/R),and observe the effects of Tongxinluo,one of Chinese herb drugs,on the expression.Methods The rat model of cerebral I/R injury,reproduced by middle cerebral artery occlusion(MCAO) and release,were divided into three groups: high-dose Tongxinluo group(given 1g/kg?d of Tongxinluo],low-dose Tongxinluo group(given 0.5g/kg?d of Tongxinluo) and control group(administration of saline).2ml of Tongxinluo or saline each time was administered to each rat and 2 times every day up to day 3,5,7,14,21 and 30 after the injury.The expression of nestin in the subventricular zone(SVZ) and hippocampal dentate gyrus zone(DG) was detected by immunohistochemistry,and the IGF-1 mRNA expression around ischemic area was detected by RT-PCR.Results Compared with the control group,the immunofluorescence magnitude of nestin-positive neurons in SVZ and DG was significantly higher in high-and low-dose Tongxinluo groups(P0.05).Conclusions Tongxinluo,a kind of Chinese herb drugs,may promote the proliferation of neural stem cells in SVZ and DG after cerebral I/R injury in rats,and the mechanism may be related to upregulation of IGF-1 mRNA induced by Tongxinluo.
8.Management of ipsilateral Pilon and calcaneal fractures

Objective To analyze, treatment of ipsilateral Pilon and calcaneal fractures (IPCF). Methods Injury causes included fallings in seven patients, traffic injury in two and impact injury in one, with ISS score of 5-22 points. There were seven patients with open injuries and 3 with close ones. Nine pa-tients had Ⅱ-Ⅲ degree of Pilon fractures according to the AO/OTA classification and six comminuted com-pression calcaneal fractures according to the Essex-Loprest classification. The Pilon fractures were fixed by cannulated nail, screw or Kirschner wire through open reduction in six patients including four fixed with ex-ternal fixators (the calcaneal fractures were fixed with percutaneous eannulated nail or Kirschner wire by poking reduction). The calcaneal fractures were fixed with only Kirschner wire through poking reduction in two patients and double fractures treated conservatively in two. Results The reduction of Pilon and cal-caneal fractures treated by operation were markedly improved in six patients, while there occurred angular deformity and joint surface introeession in the patients treated conservatively. The wound healing of soft tis-sue was sound in seven patients but unsatisfactory in the other three. The ankle-hind foot score was 82-94 points in four patients who were followed up. Conclusions IPCF is caused by the high energy force and characterized by severe injures of the soft tissues and the bone tissues at the distal tibia and calcaneua, for which limited internal fixation plus external fixation are suitable treatment choice.
9.ELECTROCHEMICAL BEHAVIOR OF OFLOXACIN AT Pt/GC ION IMPLANTATION MODIFIED ELECTRODE AND ITS APPLICATION

AIM To study the electrochemical behavior of ofloxacin at Pt/GC ion implantation modified electrode. METHODS With Pt/GC ion implantation modified electrode as working electrode, the behavior of ofloxacin was studied by voltammetry in 0.40 mol*L-1 KCl solution. RESULTS A sensitive reductive peak of ofloxacin was obtained by linear sweep voltammetry. The peak potential was -1.35 V (vs SCE). The peak current was proportional to the concentration of ofloxacin over the range of 1.0×10-6-3.0×10-5 mol*L-1 with the detection limit of 5.0×10-7 mol*L-1. The behavior of reduction wave was studied and applied to determination of ofloxacin in tablets. CONCLUSION The reduction process was irreversible. The element composition, atomicity form and depth of distribution at the surface of Pt/GC electrode were determined by Auger electron spectroscopy (AES), X-ray photoelectron spectroscopy (XPS) and scannig electron microscope (SEM). The catalysis behavior and reaction mechanism at Pt/GC modified electrode was also studied.
10.System Reading Therapy on Alexia after Stroke

ObjectiveTo explore the effect of system reading therapy on alexia after stroke.Methods104 stroke patients with alexia were divided into two groups: observation group(51) and control group(53). General speech training were used for the two groups, while the observation group received system reading training at the same time, all for 30 minutes, once a day for 1 month. Chineses Standard Aphasic Examination was applied to assess the reading ability before and 1 month after treatment.ResultsThere are three kinds of alexia in the patients: frontal alexia, alexia with agaphia, and subcortical aphasic alexia. Reading ability improved in the two groups (P<0-01), but the observation group was better than the control group(P<0-05).ConclusionSystem reading therapy is very helpful to improve reading ability of stroke patients with alexia.
