1.Subcutaneous Phaeohyphomycosis Caused by Curvularia clavata: The First Case Report in China

Objective To report a case of subcutaneous phaeohyphomycosis caused by Curvularia clavata in China. Methods The skin specimen was collected, and examined by KOH preparation, fungus culture and histopathology. Results A 17-year-old male farmer had dull red, warty and well-defined plaques with some ulceration on his face and his left upper arm for nine years and a history of trauma of his face before the appearance of the lesions. General examination did not reveal abnormal findings except the skin lesions. KOH direct examination showed septate hyphae with irregular branches. On the culture medium the colonies were black in color. Under microscope the conidia were borne singly or in groups at the tip or at the sides of the conidiophores, they were light brown to brown, smooth, straight or slightly curved and clavate, measuring 15 ~ 30 ?m long and 6 ~ 12 ?m thick, with 3 ~ 4 septates. Histopathology showed a granulomatous response. PAS staining showed irregularly swollen or toruloid conidia with branched hyphae. The isolated strain was identified as Curvularia clavata Jain. Conclusion It is the first case report of subcutaneous phaeohyphomycosis caused by Curvularia clavata in China.

To review the study progresses in venom protein, peptide and amino acids in natural drug. The study progresses were reviewed on the basis of analying the collected articles. Concerning the distribution, chemical structure, property, pharmacology and toxicity of ricin, abrin, riscotoxin, snake venom, bee venom and buthotoxin. These compounds have certain toxicity and biological activity to animals, It's worth exploiting and utilizing them in conjunction with the achievement in modern chemistry and pharmacology.
3.Curative effect of arthroscopic treatment for lateral discord meniscus tear in children

Objective To investigate the curative effect of arthroscopic treatment for lateral dis-cord meniscus tear in children. Methods The study involved 229 child patients with lateral discord meniscal tear treated arthroscopically from March 1996 to March 2008. There were 133 males and 96 fe-males, at age of (9±3.5) years (range 4-14 years). The treatment methods included meniscus-plasty in 219 patients (including two children with postoperative recurrence), meniscus-plasty-suture in eight and partial meniscectomy in two. The function of the knees was assessed pre-operationally and post-operation-ally by using Lysholm score. Results Of all, 221 patients were followed up for a period of (65±48.9) months (range 5-144 months). The pain and swelling were alleviated, with obvious improvement in range of motion and knee function in all patients six months after operation. There were no complica-tions including intra-articular infection, iatrogenic injury, fibroarthritis or loss of range of motion. Ly-sholm score was (61.85±7.65) points (range 45-73) before operation, which was improved to (95.30±3.50) points (range 87-100 points) six months after operation (t= 26.778, P < 0.01). Conclusion Treatment of lateral discord meniscus tear under arthroscopic guidance in children has the advantages of minor trauma, satisfactory results, can maintain good shape of the meniscus and good func-tion of knee and reduce incidence rate of osteoarthritis.

This study was designed to assess the effect of implantation site and environment on early in vivo degradation behaviors of poly(L-lactide) (PLLA) and poly(L-lactide-co-glycolide) (PLGA) copolymer. The rods were implanted at two sites in each of 24 New Zealand White rabbits. The first site was within the suprapatellar bursa of the joint cavities (JC) and the second site was in the opposite condyles of femurs (CF). Three rabbits of each group underwent explantation of rods after 4, 8, 12, and 16 weeks. At each interval, measures were taken to evaluate the molecular weight, shear strength, weight loss and thermal properties of PLLA and PLGA. It was found that PLGA degraded slightly faster than PLLA. After 16 weeks, PLLA's initial inherent viscosity of 4.6 decreased to about 3.4 in both implantation sites while that of PLGA decreased from 4.6 to about 2.2. Both PLGA and PLLA showed enough shear strength retention in 16 weeks (> or = 53MPa) within 16 weeks. Autocatalysis mechanism was confirmed by the fact of accelerated weight loss of PLGA after 8 weeks and of PLLA after 12 weeks. The results revealed that PLGA could be a promising candidate material as a replacement of PLLA in internal fixation of bone fractures, and no significant difference of early in vivo degradation behaviors between PLLA and PLGA was observed in regard to different implantation sites in 16 weeks.

6.Moderate functioning mechanisms of self-efficacy and perceived social support between stress and maladjustment among intensive nurses

Objective To explore the relationship between functioning mechanisms of self-efficacy and perceived social support in stress and maladjustment among intensive nurses .Methods A total of 446 intensive nurses were assessed by general information questionnaire , General Self-efficacy Scale(GSES), Perceived Social Support Scale ( PSSS ) , General Maladjustment Scale ( Gm ) , and Chinese Nurse Stressor Scale ( CNSS ) . Results The scores of Gm were positively correlated with the scores of CNSS (r =0.466,P <0.05), and negatively correlated with the scores of GSES and PSSS ( r=-0.205,-0.333,respectively;P<0.05).Self-efficacy had an obvious effect on mediating between stress and maladjustment ( P <0.05).Perceived social support had an obvious effect on mediating and moderating between stress and maladjustment ( P <0.05). Conclusions Self-efficacy among intensive nurses can mediate the effects of stress on maladjustment .Perceived social support can mediate and adjust the effects of stress on maladjustment .
7.To investigate the efficacy of pentoxifylline sequential therapy combined with rasagiline and levodopa and benserazide in the treatment of Parkinson disease with fluctuations in elderly patients

Objective:To investigate the clinical efficacy of pentoxifylline sequential therapy combined with rasagiline and levodopa in the treatment of elderly patients with symptoms of Parkinson disease (PD), and the influence on hemorheology and serum Toll-like receptor 4 (TLR4) signaling pathway downstream related inflammatory factors.Methods:A prospective study method was used to select 90 elderly patients with PD with fluctuating symptoms who were admitted to the Eighth People′s Hospital of Hebei Province from June 2021 to October 2022 as research objects. The patients were divided into observation group and control group with 45 cases in each group according to random number table method. The observation group was treated with pentoxifylline sequential therapy combined with rasagiline and levodopa. The control group was treated with rasagiline combined with levodopa. The clinical efficacy of the two groups was compared. The unified Parkinson disease rating scale (UPDRS), Montreal cognitive assessment scale (MoCA), Berg balance scale (BBS) and 39-item Parkinson disease quality of life questionnaire (PDQ-39) were scored before and after treatment. Hemorheology indexes and serum levels of related inflammatory factors downstream of TLR4 signaling pathway, including whole blood high viscosity (HBV), whole blood low shear viscosity (LBV), plasma viscosity (PV), fibrinogen (FIB); TLR4, interleukin (IL)-1β, IL-6, tumor necrosis factor (TNF) -α, were detected before and after treatment in both groups. The adverse reactions of the two groups were compared.Results:The total effective rate in observation group was significantly higher than that in control group: 93.33% (42/45) vs. 77.78% (35/45), and there was statistical difference ( P<0.05). After treatment, the UPDRS mental activity and emotional disorders, daily living ability, motor function, motor complications scores and PDQ-39 score of the two groups were significantly lower than before treatment, the MoCA and BBS scores were significantly higher than before treatment, and the improvement was more significant in the observation group, there were statistical differences ( P<0.05). After treatment, HBV, LBV, PV and FIB in the observation group were significantly decreased compared with those before treatment: (6.52 ± 0.92) mPa·s vs. (7.25 ± 1.24) mPa·s, (11.45 ± 1.24) mPa·s vs. (14.13 ± 1.64) mPa·s, (1.55 ± 0.17) mPa·s vs. (1.88 ± 0.22) mPa·s, (3.25 ± 0.47) g/L vs. (3.82 ± 0.52) g/L, and there were statistical differences ( P<0.05). There were no significant differences in hemorheology indexes of control group before and after treatment ( P>0.05). After treatment, serum levels of TLR4, IL-1β, IL-6 and TNF-α in both groups were significantly decreased compared with those before treatment, and the indexes in observation group were significantly lower than those in control group: (2.07 ± 0.18) ng/L vs. (2.58 ± 0.21) ng/L, (1.42 ± 0.17) ng/L vs. (2.28 ± 0.25) ng/L, (1.56 ± 0.22) ng/L vs. (2.42 ± 0.28) ng/L, (46.31 ± 3.17) ng/L vs. (54.34 ± 3.65) ng/L, and the differences were statistically significant ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Pentoxifylline sequential therapy combined with rasagiline and levodopa can effectively improve the hemorheology of elderly patients with PD accompanied by symptom fluctuations, reduce the levels of related inflammatory factors downstream of serum TLR4 signaling pathway, and improve clinical efficacy.
8.Path relationship of working stress, self-esteem and the sense of achievement for acute critical care nurses

Objective To explore the path relationship of working stress , self-esteem and the sense of achievement for acute critical care nurses .Methods Using the method of convenient sampling , a questionnaire survey was conducted on 265 emergency nurses and ICU nurses . The questionnaire included general information , nurse burnout scale , self-esteem scale and nurse stress scale .Results Totals of 265 questionnaires were distributed, and 255 questionnaires were returned , with an effective rate of 96.2%.The scores of stress scale for nurses who worked for one year , two-five years and over five years were (3.03 ±0.40), (3.12 ± 0.54) and (3.06 ±0.39), respectively.There was a significant difference among groups (F=23.534,P=0.00).The scores of self-esteem for nurses who worked for one year , two-five years and over five years were (19.87 ±2.17), (20.19 ±1.98) and (18.93 ±2.87), with a statistically significant difference (F=21.391, P=0.00) .The scores of working achievement for nurses who worked for one year , two-five years and over five years were (4.18 ±1.64), (4.25 ±1.43) and (4.20 ±1.63), and the difference was significant (F =18.733, P=0.00).There was a negative correlation between the critical nurses'working achievement and working stress (r=-0.317, -0.271, -0.211, -0.231, -0.356, respectively;P <0.05), and the working achievement and the self -esteem was positively related (r=0.357,P<0.01).The self-esteem was not completely intermediary between working stress and working achievement , and the indexes of model fitting were χ2/df=1.244,P=0.305, RMSEA=0.019, NFI=0.977, RFI=0.984, IFI=0.987, TLI=0.960, CFI=0.937.Conclusions Self-esteem plays a significant intermediary role between working stress and working achievement for acute and critical care nurses .
