Objective To summarize the experience of prevention and treatment of sinus after laparoscopic ap-pendectomy. Methods Operation method was decided by the sinus condition: method A, fistula incision and inflam-matory granulation shaving;method B, removing he sinus completely. Then compare the efficacy and the risk of op-eration. Results Cure rate of method A was 66.7 %, cure rate of method B was 100.0 %. The total cure rate was 90.5 %. Sinus wall ruptured in 1 during the process of removing. Conclusions Suitable operation method should decided by the sinus condition, and make the premunition for fistula formation.

Objective To evaluate the effect of holidays on door-to-reperfusion time in patients with ST-segment elevation myocardial infarction(STEMI).Methods Using the data from the Registry of ST-segment elevation Myocardial Infarction in Beijing,we studied the data of 297 patients with STEMI who received acute reperfusion from November 2005 to July 2006.The association between holidays and door-to-reperfusion time in these patients was studied.Results Ninty four patients in this study were admitted in holidays(26 of them received fibrinolytic therapy and 68 received PCI)and the other 203 patients were admitted during routine weekdays(61 of them received fibrinolytic therapy and 142 received PCI).After adjusting for patient characteristics,admission during holidays was associated with a longer median door-to-balloon time[162.2 min(95% CI:160.8,165.3)vs 141.8 min(95% CI:137.8,144.1);P

Objective: The aim of this study was to develop and assess the computer-assisted fabricated individual titanium mesh technique applied for reconstruction of fractured orbital walls to correct post-traumatic enophthalmus and diplopia. Methods: Twenty patients (12 males and 8 females) were inclu-ded in this study. All of patients suffered from unilateral impure orbital fractures. Group A included 8 patients who were reconstructed for the fractured orbit with computer-assisted fabricated titanium mesh technique. Group B had 12 patients treated with traditional methods (titanium meshes in 2, autogenous bone grafts in 7 and Medpor implantation in 3). CT data of group A was input into a image software to rebuild a 3-D orbit and then mirror the unaffected side onto the affected side to replace the demolished orbit. A resin model of the reshaped orbit was then produced and was used to develop custom-fabricated titanium mesh for repair of the fractured orbital wall. The fabricated mesh was used in operation to reconstruct the orbital wall. Treatment was assessed in comparison with traditional way by means of clinical examinations and CT measurements. Results: The difference between pre-operative and post-operative enophthalmos: the mean value of group A was 2.6 mm, group B was 2.0 mm;the difference between the pre-operative and post-operative enlargement volume of traumatic orbit: the mean value of group A was 4.3 mL, group B was 3.7 mL; Index of Medial orbital wall of both groups was 100% and Index of inferior wall reconstruction of group A was 84.7%, group B was 53.3%; The mean depth of implants of group A was 31.2 mm, of group B was 28.3 mm; Improvement of diplopia of group A was 1.5 quadrants, that of group B was 1 quadrant. There were no infection, gaze restriction, or graft extrusion in all patients after operation. Conclusion: Computer-assisted fabricated titanium mesh technique could provide more accurate orbital reconstruction and improve the correction of enophthalmus and diplopia.

Objective To evaluate the diagnostic value and limitation of MRI for chromophobe renal cell carcinoma.Methods MRI features of 5 cases with pathology proved chromophobe renal cell carcinoma were analyzed retrospectively.Results All tumors showed homogenous isoin-tensity or slightly hypointensity on T1 weighted images and isointensity or slightly hyperintensity on T2 weighted images.Slightly hyperintensity were showed on DWI images,the mean ADC value of tumors was 1.42×10-3 mm2/s.On the contrast enhanced images,all the mass showed slight to moderate enhancement ,1 case had spoke-wheel-like enhancement,1 case showed flowed out blood vessels in the tumor.Conclusion The preoperative diagnosis is difficult for chromophobe renal cell carcinoma,MRI is a valuable method which could provide useful information for qualitative diagnosis.

Objective To examine whether ghrelin has beneficial effect on survival of pancreatic INS-1 beta cell.Methods Rat INS-1 cells were cultured separately in 5.6 mmol/L glucose (NG group),33.3 mmol/L glucose (HG group),33.3 mmol/L glucose plus 10 nmol/L acylated ghrelin (HG+AG group),and 33.3 mmol/L glucose plus 10 μmol/L unacylated ghrelin(HG+UG group).After being incubated for different hours,cell suvival rate was determined by MTT.Activity of caspase-3 was estimated by spectrophotometry,activity of GRP78,and cytochrome c was analyzed by confocal microscopy.Results Both acylated ghrelin and unacylated ghrelin inhibited the rise in activity of GRP78,caspase-3,and cytochrome c induced by sustained high glucose.Conclusions These findings indicate that ghrelin is able to inhibit endoplasmic reticulum stress and mitochondrial dysfunction of INS-1 β-cell caused by persistent high glucose,and the effect of ghrelin is not affected by acylation.

Objective:To compare 3 maxillofacial trauma scoring systems in mandible fracture grading.Methods:Maxillofacial Injury Severity Score(MISS),Maxillofacial Injury Severity Score(MFISS),Mandible Injury Severity Score(S5)were used in the analysis of mandible fracture severity in 313 cases with mandible fractures.The results were statistically analyzed.Results:S5 score showed higher correlation with operation time,operation charge and hospitalization expenses than MFISS and MISS,and it could distinguish single and multiple mandible fractures effectively.It was also a significant factor affecting the cost of hospitalization.Conclusion:The Mandible Injury Severity Score was more suitable for the scoring of mandible fracture.

It's necessary to monitor ECG and CO2, the two important physiological parameters. This paper designs a kind of realtime, wireless and concentrated monitoring system for ECG and CO2. With a high cost performance ratio, the system can be applied in the rooms for operation, monitoring, first aid and etc. At the same time, this paper does some researches on the antiinterference method for wireless communication.

Objective To evaluate the differences of the lower extremity atherosclerosis between patients with and without type 2 diabetes using dual-source CT angiography.Methods Dual-source CT angiography of lower extremity was performed in 87 patients with (n=30)or without (n= 57 )diabetes.Extent of luminal stenosis,and the type,distribution and range of the plaques were compared.Results 342 plaques in 540 segments (63.3%)in diabetic patients,and 500 plaques in 1 026 segments (48.7%)in non-diabetic ones were detected respectively.Compared with non-diabetic patients,the diabetic ones had a higher overall incidence of plaques (P <0.05).Calcified plaques were the most common in both kinds of patients,and the incidence of mixed plaques was high-er in diabetic patients than that in non-diabetic ones (35.6 % vs.28.4%,P <0.05).Light to moderate stenosis occurred in most diabetic patients,and fewer occlusion was found compared with non-diabetic ones (9.1% vs.1 7.0%,P <0.05).The most common sites of the plaques in diabetic patients were located at distal small arteries below the knee.However,those were located at proximal arteries above the knee for non-diabetic ones.The involvement of atherosclerosis in diabetic patients was more diffused,and the de-gree of Ⅳ (75%-100%)was higher than that in non-diabetic ones (P <0.05).Conclusion Atherosclerosis in lower extremity on dual-source CT angiography is very common in diabetic patients with multi-segmental,diffused,non-obstructive involvement of dis-tal small arteries below the knee.

Objective:To review and compare the clinical features and treatment outcomes of oral and maxillofacial space infection (OMSI)between diabetic and non-diabetic patients.Methods:Clinical data of 43 diabetic patients with OMSI(simultaneoustly treated by blood sugar control)and 84 of non-diabetic patients with OMSI were reviewed,the clinical features and treatment outcomes were compared.Statistical analyses were conducted by T test,the chi square test and variance analysis.Results:Diabetic patients with OM-SI were older(P =0.000),had more spaces involved concurrently(P =0.035 )and had higher blood sugar at presentation(P =0.000).There was no significant difference between groups about the use and change of antibiotics,the incision times,hospital stay durition and the incidence of the complications.Conclusion:The prognosis of the diabetic patients with OMSI under the strict control of the blood glucose concentration has no significant difference from the non-diabetic patients with OMSI.Diabetic patients with OMSI have older age and more spaces involved,and special attention should be paid.
