The paper analyzes the orientation of health management function of government under the perspective of new public service theory, tries to reveal the unimplementation, dislocation, offside and absence of government function at four aspects including health service system, health organization operating mechanism, health management system and health service supervision system, and puts forward six approaches for transformation of government function of health management.

Objective To investigate the appropriate timing of B-lynch suture in the treatment of atonic postpartum hemorrhage during cesarean section.Methods A total of 90 patients with atonic postpartum hemorrhage during cesarean section from January 2012 to December 2014 in our hospital were included in the study.They were divided into two groups, early suture group (applying B-lynch suture when the amount of postpartum hemorrhage reaching 500 ml, n=66 cases) and late suture group (applying B-lynch suture when the amount of postpartum hemorrhage reaching 1000 ml, n=24 cases).The hemostatic effect, amount of bleeding, blood transfusion rate, incidence of DIC, duration of hospitalization, and the puerperal morbidity were compared between the two groups.Results All the 90 patients were successfully treated and were discharged after comprehensive treatment.The amount of postpartum hemorrhage of the early suture group was (656±118) ml, which was significantly less than that of the late suture group [(1550±432) ml, t=-10.003, P=0.001].The rate of blood transfusion of the early suture group was 3.0% (2/66), which was significantly lower than that of the late suture group [(50.0%, 12/24), x2=26.092, P=0.000].There was no case of DIC in the early suture group, and the incidence of DIC in the late suture group was 16.7% (4/24), with the difference statistically significant (Fisher''s exact test, P=0.004).The puerperal morbidity in the early suture group was 3.0% (2/66) and that of the late suture group was 25.0% (6/24), with statistically significant difference (x2=7.952, P=0.005).There was no statistically significant difference in the hospitalization stay between the two groups (P>0.05).Conclusions B-Lynch suture is an effective method in the treatment of atonic postpartum hemorrhage during cesarean section, and early use of it can get better curative effect, such as less bleeding amount and lower rates of blood transfusion, DIC and complications like puerperal morbidity.In case the bleeding volume reaches 500 ml and the general conservative hemostatic methods are ineffective, B-Lynch suture is recommended immediately.

Objective To investigate the reasons of complications after ultrasound-guided PTBD and explore measures for their management. Methods The clinical data of 753 patients with obstructive jaundice treated with ultrasound-guided PTBD in our hospital from January 2001 to June 2008were retrospectively analyzed. Results The 753 patients received a total of 791 treatments of PTBD.The occurring rates of severe, common, and long-term complications were 1.14 % (9/791), 11.88 %(94/791 ) and 4.3% (9/791), respectively. Only 1 patient died of manipulation-related complications.Conclusion Ultrasound-guided PTBD is an effective method for treatment of obstructive jaundice.

Stribild is a compound preparation composed of elvitegravir, cobicistat, emtricitabine and tenofovir. As for the first-treated AIDs patients or re-treated patients with creatinine clearance≥70ml·min-1 , it has definite clinical effect. Owing to once a day, stribild can enhance the acceptability of the patients, and as for the patients treated with other regimen complicated with diarrhea, pain in stomach and numbness in hands and feet, it also provides a good option. However, its adverse effects on kidney, liver and bone density in the long term use should be paid attention.

Objective To study the effect of bromocriptine(BRC) on the activation of T lymphocyte stimulated by phytohemagglutinin(PHA).Methods After CD4+ T cell line Jurkat E6-1 cells were stimulated by PHA,prolactin(PRL) and BRC,respectively,the expression of linker for activation of T cells(LAT) and zeta-chain T cell receptor associated protein kinase 70 000(ZAP-70) mRNA of T lymphocytes were checked by RT-PCR.The expression of PRL mRNA of T lymphocytes was detected by Real time PCR.The expression of CD25(cluster of differentiation) as a marker of early activation on the surface of T lymphocytes was detected by flow cytometry,and the activation of nuclear factor-?B(NF-?B) was detected by luciferase reporter system.Results 1.BRC inhibited the expression of ZAP-70 as the common signal molecules both in the T lymphocyte activation pathway and PRL-prolactin-prolactin receptor(PRLR) signal transduction pathway,and decreased the expression of PRL mRNA produced by activation T lymphocytes.2.BRC enhanced the expression of LAT mRNA as another important signal molecular on the T lymphocytes and CD25 on the surface of the T lymphocytes.3.The activation of NF-?B of T lymphocytes was decreased.Conclusions BRC might inhibit the activation of T lymphocytes by inhibiting the expression of ZAP-70,the common signal molecules between T lymphocytes activation and PRL-PRL pathway,and PRL mRNA,the like-T lymphocyte growth factor.

Objective To study the diagnostic and treatment value of wire guided localization of breast biopsy for non-palpable breast lesions.Methods The clinical data of 314 cases of non-palpable breast lesion resection operation in our hospital were retrospectively analyzed.The accuracy rate of resection and postoperative pathological examination results were analyzed.Results All lesions were positioned accurately and completely resected,48 cases in 314 cases were breast cancer(15.3 ％),in which 42 cases were early breast cancer which were confirmed by pathology (87.5％).Conclusion Wire guided localization of breast biopsy has high accuracy,low cost,which is useful for the diagnosis of early breast cancer,there is a certain value in clinical application.

Objective To study the clinical value of mammography and ultrasound needle localization biopsy technique in clinical nonpalpable breast disease.Methods 148 patients with negative clinical palpation were operated only because of the mammography or ultrasound findings of lesions.They were undergone localized excision biopsy and derived definite pathological diagnosis.By histopathology diagnosis,benign and malignant lesions,lesion size,complete resection rate were statistically analyzed.Results 21 cases in 148 cases were malignant lesions(14.2％),which were all early breast cancer.The other 127 cases were benign breast disease(85.8％).147 cases were successfully resected at the first time and 1 case was successfully resected at the second time.Conclusion Mammography and Ultrasound fine needle localized biopsy in nonpalpable breast disease diagnosis and treatment has great significance,which can raise early diagnosis rate of breast cancer without mass,and this method is simple and easy for promotion.

Objective To understand epidemic data and provide evidence for prevention and control of infectious diseases in Yongchuan area .Methods We collected the clinical materials of patients hospitalized during January 2008 to December 2012 ,and retrospectively analyzed the spectrum ,classification and epidemic tendency of infectious diseases in a hospital .Results A total of 4 632 patients were admitted to the department of infectious disease .The top two diseases were always hepatitis B virus infection related diseases and tuberculosis .Fatality mainly also appeared in end-stage liver diseases and tuberculosis .Morbidity of measles and mumps increased obviously in recent years .Hand-foot-mouth disease and influenza A(H1N1) were epidemic in this area .Conclusion The main infectious diseases in Yongchuan area are hepatitis B virus infection related diseases and tuberculosis ,which still are the important points for control and prevention .Also we should strengthen prevention of other infectious diseases such as hand-foot-mouth disease ,H1N1 and the mumps in order to decrease the occurrence of these infectious diseases .

Objective To provide evidence for control and prevention of hand-foot-mouth disease(HFMD) ,we analyzed the epi-demiological and viral characteristics of HFMD in Yongchuan district from 2009 to 2012 .Methods We collected the clinical dates of HFMD cases from Diseases Infectious Department of the Yongchuan Affiliated Hospital of Chongqing Medical University during 2009 to 2012 ,and analyzed the epidemiological characteristics .Results There were 509 HFMD cases in recruitment ,including 2 se-vere cases(one was dead) .The peak incidence of HFMD occurred from May to July annually .Between 2011 and 2012 ,there was an-other peak incidence ,which occurred from November to December .Most of the patients were under 5 years-old .The number of male cases was larger than that of the female .The number of scattered children cases was larger than that of the nursery children .More cases were reported in rural area than in urban area .Coxsackie virus A16(Cox A16) and other enteroviruses were the predominant strains of HFMD ,however enterovirus 71(EV71) infected cases increased sharply between 2011and 2012 .Enterovirus 71(EV71) inflection is the main cause of severe and death of HFMD .Conclusion The prevalence of HFMD in Yongchuan district had signifi-cant seasonal ,sex and age-specific ,regional differences and etiological characteristics .It was very important to make practical man-agements to prevent occurrence of HFMD according to these epidemiological characteristics .
