Objective To observe the effects of dexmedetomidine on inflammatory responses and insulin resistance during cardiac operations with cardiopulmonary bypass(CPB).Methods Fifty patients scheduled for elective cardiac valve replacement with CPB were equally and randomly divided into two groups using a random number table:observation group and control group(n =25).A loading dose of dexmedetomidine 1.0 μg/kg was injected intravenously over 1 5 min after induction,followed by continuous infusion at 0.4 μg·kg-1 ·h-1 until the end of CPB in doservation group.While the e-qual volume of normal saline was given in control group.After induction(T1 ),at 30 min after the be-ginning of CPB(T2 ),at the end of CPB(T3 ),and at 2 h after the end of CPB(T4 ),the jugular venous blood samples were taken for determination of serum concentrations of TNF-a,IL-6,insulin and blood glucose.The insulin sensitivity index (ISI ) = 1/(glucose × insulin )was obtained. Results Compared with control group,the serum TNF-a and IL-6 concentrations at T2-T4 were sig-nificantly decreased in observation group(P <0.05),the serum insulin and blood glucose concentra-tions at T2 ,T3 were significantly decreased in observation group(P <0.05 ),the ISI at T2 ,T3 were significantly increased in observation group(P <0.05).Conclusion Dexmedetomidine can reduce in-flammatory responses,thus reducing insulin resistance and blood glucose during cardiac operations with CPB.

Objective To investigate the clinical efficacy of Hiru doid,Sufu and Musk Hemorrhoids Ointment on preventing peripheral chemothera?peutic phlebitis due to continuous infusion of 5?fluorouracil. Methods Totally 90 patients with esophageal squamous cell carcinoma treated with continuous infusion of 5?fluorouracil were recruited and randomly divided into Hirudoid group,Sufu group and Musk Hemorrhoids Ointment group, with 30 cases in each group. Three groups were applied with drugs respectively along the vein punctured while connecting the pump of 5?fluorouracil 3 times a day(each time interval of 6 hours)for 5 consecutive days. The occurrence of chemotherapy phlebitis was observed and recorded. Results Musk Hemorrhoids Ointment and Sufu were more effective than Hirudoid in reducing the incidence of phlebitis and relieving the degree of venous in?jury(P<0.05);however,there were no differences between Musk Hemorrhoids Ointment and Sufu(P>0.05). Musk Hemorrhoids Ointment was the most effective among the three in relieving the pain caused by phlebitis(P<0.05). There were no difference between Sufu and Hirudoid(P>0.05). Conclusion Musk Hemorrhoids Ointment can effectively prevent peripheral chemotherapeutic phlebitis caused by continuous infusion of 5?fluorouracil with affordable expense which is worthy of promotion in clinic use.

Objective To evaluate the clinical significance of plasma ionized calcium (ICa) and total calcium (TCa) measurement in diagnosis of tumor-associated hypercalcemia. Methods The heparin lithium anticoagulated plasma were collected and measured with ion selective electrode (ISE) and orthocresolphthalein complexone (OCPC) method for ICa and TCa in 77 patients with malignant tumor (tumor group) and 44 healthy controls (control group). Meanwhile,plasma albumin and pH value were measured to correct TCa (cTCa) and ICa(nICa). The results were analyzed. Results The levels of cTCa,ICa and nICa in tumor group were significantly higher than those in control group(P＜ 0.01 ) ,but there was no significant difference of TCa between two groups (P ＞ 0.05 ). cTCa ＞ 2.70 mmol/L, nICa ＞ 1.35 mmol/L as the hypercalcemia eritical value, the positive rate of nICa and cTCa were 18.18% (14/77) and 6.49% (5/77)respectively,the difference was significant (P = 0.023). Conclusion Determination of ICa may be more clinical meaningful than TCa for tumor-associated hypercalcemia.

Objective To investigate the change of plasma marinobufagenin (MBG) level and the expression of its receptor Na+-K+-ATPase (NKA) in renal biopsy specimens of chronic glomerulonephritis (CGN) patients. Methods Twenty-eight CGN patients and 14 healthy people were enrolled in the study. The plasma MBG concentration was measured by competitive inhibition ELISA system. Immunofluorescence and immunohistochemistry staining were applied to detect the expression of NKA in renal biopsy specimens of 28 CGN patients and analyzed by semi quantitively. Results Compared with healthy controls, CGN patients had significant lower plasma MBG concentration [(0.579±0.214) nmol/L vs (0.715±0.154) nmol/L, P＜0.05], without further significant difference between CGN patients with hypertension and with normal blood pressure [(0.595±0.231) nmol/L vs (0.557±0.197) nmol/L, P＞0.05]. Meanwhile, proximal tubular staining of NKA was decreased compared with normal controls. The NKA positive staining area of the CGN group was lower than that of normal controls [2.1% (0.5%-6.2%) vs 5.6% (3.5%-10.8%), P＜0.01] and correlated with 24-hour urinary sodium excretion (r=0.551, P＜0.01).Conclusion Decreased plasma MBG level and its receptor expression on proximal tubules may play a role in the regulation of sodium in CGN.

objective To assess relationship between microvascular changes of hemodynamics of the fingertip and evolution of hypertension in patients with essential hypertension(EH)by enhanced power Doppler imaging(E-Flow).Methods The right ring fingertip'S blood flow peak systolic velocity(PSV),end-diastolic velocity(EDV),resistance index(RI)of finger ventral arteriole and nail bed arteriole in thirty-nine patients with EH were separately observed and were made contrast with 35 age-matched healthy subjects by E-Flow imaging.Results Compared with traditional colour Doppler flow imaging and power Doppler imaging,E-Flow imaging was clearer to demonstrate the distribution and movement of the fingertip'S microvascular.It was shown that the PSV and EDV in the ring finger ventral arteriole and nail bed arteriole of EH were decreased(P＜0.05),but the RI increased(P＜0.05),along with the evolution of EH.Conclusions Fingertip'S microvascular changes of hemodynamics could be a criterion to detect obstruction alternations of essential hypertension.E-Flow imaging iS superior to traditional color Doppler flow imaging and power Doppler imaging in differentiating microvascular and could provide a new method in assessing microvacular changes of patients with EH for clinic.

[Objective]To investigate the feasibility of screening for velamentous umbilical cord insertion in early pregnancy by ultrasound.[Methods]A total of 2,796 cases of pregnant women,2 998 cases of fetuses that underwent prenatal ultrasound screening in the early and middle stage pregnancy and delivered in Dalian Women and Children Healthcare Center were included. The results of ultrasound screening of umbilical cord insertion during early pregnancy(11~13+6 w)were divided into three groups. GroupⅠ:it is clear that the placenta of the umbilical cord is located in the fetal membranes ,and then along with the fetal membranes for a long distance,and then enter the placenta. GroupⅡ:The placenta of the umbilical cord is located at the edge of the placenta,and then insert point is not distinguished between the placenta and fetal membranes. Group Ⅲ:It is clearly shows that the umbilical cord inserted into the placenta.[Results]1. The display rate of the umbilical cord in early and mid pregnancy ultrasound examinationearly pregnancyan examination showed that the rate of 99.23%(2 975/2 998). The single child display rate of 99.77%(2 590/2 596),twindisplay rate of 95.96%(380/396),three fetal the rate of 83.33%(5/6).In the second trimester of pregnancy(22~26 w)and examination showed that the rate of 86.99%(2 608/2 998). The single child display rate of 90.99%(2 362/2 596),twin display rate of 61.61%(244/396),three fetal the rate of 33.33%(2/6). 2. The results of early pregnancy,pregnancy,the examination of after birth Regard?ing to early pregnancy,28 cases were included in GroupⅠ,42 cases in GroupⅡand 2928 cases in GroupⅢ. Regarding to second pregnancy,37 cases were evaluated as velamentous umbilical cord insertion,30 cases were evaluated as battledore placenta and 2931 cases were evaluated as normal placenta. The results of postnatal examination were as follows:36 cases were identified as velamentous umbilical cord insertion,30 cases were evaluated as battledore placenta and 2932 cases were evaluated as normal placenta. 3. Comparison of 3 groups,early pregnancy examination results and pregnancy and after the birth. In early pregnancy,28 cases of GroupⅠwere diagnosed as velamentous umbilical cord insertion in the examination during the second trimester of pregnancy and after delivery. In the 42 cases of early pregnancy in Group Ⅱ,7 cases were diagnosed as velamentous umbilical cord insertion ,22 cases were diagnosed as battledore placenta and 13 cases were normal placenta. After birth ,6 cases were diagnosed as diagnosed as velamentous umbilical cord insertion ,23 cases were battledore placenta and 13 cases were normal placenta. In the 2928 cases of early pregnancy of Group Ⅲ,2 cases were diagnosed as diagnosed as velamentous umbilical cord insertion ,8 cases were battledore placenta and 2918 cases were normal placenta. After birth ,2 cases were diagnosed as diagnosed as velamentous umbilical cord insertion,7 cases were battledore placenta and 2919 cases were normal placenta.The diagnostic accuracy of early pregnancy was 98.29%.[Conclusion]Application of HD-Flow blood flow pattern in early pregnancy can make a diagnosis of some of the sail-shaped placenta.

Objective To investigate the relationship between trace elements and rachitis in children.Methods Three hundred and twelve patients with rachitis and 297 healthy children were selected for this study.Blood zinc(Zn),iron(Fe),plasma copper(Cu),calcium(Ca),magnesium(Mg),lead(Pb) and cadmium(Cd) were assayed by atomic absorption spectrophotometer.Results The levels of Zn,Fe,Cu of rachitis in blood were significantly lower than those of healthy children,while the levels of Mg,Pb were higher.There were significant differences between 2 groups(P

To investigate the influence of the difference enhancers on the transport mechanism of chlorogenic acid (CGA) across Caco-2 cells model, a RP-HPLC method was adopted to detect the concentrations of CGA. At the concentrations of 20 to 80 microg x mL(-1), the difference of absorption rate constants (K(a)) was not statistically significant. At the concentrations of 40 and 20 microg x mL(-1), the ratios of apparent permeability coefficients (P(app)) of the apical to basolateral and the basolateral to apical were 1.14 and 1.18, respectively. With the effect of enhancers K(a) and P(app) increased, the absorption half-life (T1/2) decreased. CGA passed through the Caco-2 cell membrane mainly by passive transport. It showed that monocarboxylic acid transporter (MCT) could be involved in the across membrane transport process of CGA. Borneol had no effect on the cell membrane transport processes. The order of increasing absorption of CGA caused by the enhancers was sodium lauryl sulphate > sodium taurocholate > carbomer.

Objective To compare the perioperative outcomes of patients with primary hepatic carcinoma treated with laparoscopic hepatectomy (LH) with those treated with open hepatectomy (OH).
Methods From January 2010 to August 2014, 100 patients with primary hepatic carcinoma were randomly divided into the LH group and OH group respectively, 50 patients in each group. And the incision length, blood loss, operative time, postoperative liver function, anus exhaust time, complications, length of postoperative hospital stay, and cost measures were compared.
Results LH could achieve shorter incision length, less blood loss, more rapid recovery in liver function and gastrointestinal function, and shorter postoperative hospital stay length compared with OH for primary hepatic carcinoma patients (all P<0.05). However, LH could not significantly shorten operative time, and reduce postoperative complications and hospitalization cost (all P>0.05).
Conclusion Compared with OH, LH could improve perioperative outcomes of primary hepatic carcinoma patients.
