Obiective To discuss influence of psychological intervention on compliance improvement of patients with liver cirrhosis.Methods Patients with liver cirrhosis(100 cases)were divided in to the experimental group and the control group with 50 cases in each group.The control group received routine nursing.The experimental group and their family members were given systemic psychological in tervention by responsibility nudes.The treatment effect after nursing and intervention in the two groups was compared.Results The compliance of the experimental group was better than that of the control group,P＜0.05,which had statistical difference.Conclusions Implementation of psychological intervention shows significant effect on compliance improvement of patients with liver cirrhosis.

Objective To evaluate the value of volumetric brain MRI in multiple system atrophy.Methods Eleven patients diagnosed as multiple system atrophy were recruited,includin 5 parkinsonism dominant(MSA-P)and 6 cerebellar dominant(MSA-C).9 patients with parkinsonism of other types and 6 healthy persons were set as case control and healthy control,respectively.T1 weighted (T1W)sagittal and axial images and T2-weighted(T2W)axial images were obtained from all patients and controls at 3.0T scanner.Diameters of the brain structures were measured infratentorially(brainstem, middle cerebellar peduncles(MCP),dentate and red neelus)and supratentorially(globns pallidus and putamen).Results The transverse diameter of the pons was significantly smaller in MSA patients than in the case and healthy controls((27.6?2.0)mm and(30.5?0.6)mm and(29.9?1.1)mm).The significance could be seen when comparing MSA-C and MSA-P with healthy control.The anteroposterior diameter of the fourth ventricle was significantly dilated in MSA patients than in healthy control((11.9? 2.8)mm and(9.0?2.1)mm).The MRI of MSA-C showed narrower MCP((13.3?1.9)mm and (15.8?1.2)mm and larger fourth ventricle((17.3?2.1)mm and(12.6?2.7)mm)than that of MSA-P.The MRI of MSA-P showed smaller globus pallidus and red neclei.Conclusions The volumetric MRI is a useful means in evaluating the brain structure atrophy in multiple system atrophy.The transverse diameter of the pans,though objectively reflecting the atrophy of the pons,can' t be used to differentiate MSA-P from MSA-C.The atrophy of MCP and the dilated fourth ventricle are common in MSA-C,while the atrophy of red neclei is common in MSA-P.

Objective To investigate the resistance rates of the Candida albicans strains isolated from patients with vulvovaginal candidiasis to 5 antifungal agents and examine the mechanism of azole resistance in these strains.Methods A total of 1 646 C.albicans strains were collected in Obstetrics and Gynecology Hospital of Fudan University from January to December 2015.The resistance rates of these isolates to five antifungal agents were analyzed.Azole-resistant (n=30),dose dependent sensitive (S-DD) (n=13),and susceptible isolates (n=10) were randomly selected from the microbiology laboratories of three obstetrics and gynecology hospitals in Shanghai.The expression levels of drug efflux pump related gene CDR1,CDR2,MDR1 and drug target enzyme gene ERG11 were analyzed by real-time fluorescence quantitative polymerase chain reaction (PCR).At the same time,the ERG11 and ERG3 genes were amplified by PCR and sequenced,and analyzed for resistance-related mutations.Results Of the 1 646 C.albicans strains,5.2％,3.2％,2.5％ and 2.1％ were resistant to itraconazole,voriconazole,fluconazole and 5-fluorocytosine,respectively.All isolates were sensitive to amphotericin B.The expression of ERG11 gene was significantly higher in S-DD group and azole-resistant group than in azole-sensitive group (P＜0.05).The expression of CDR1,CDR2 and MDR1 did not show significant difference among the three groups.There were 13 missense mutations in the ERG11 gene,of which T123I,P98S and Y286D amino acid substitutions were newly discovered.Both T123I and Y132H were identified in 26 resistant isolates,of which 16 gene mutation was detected in two pan-azole-resistant isolates.Conclusions The C.albicans strains isolated from vulvovaginal candidiasis showed higher resistance rates to azole antifumgal agents than that to 5-fluorocytosine and amphotericin B.Mutation and over-expression ofERG11 gene may be one of the prevalent molecular mechanisms underlying azole resistance in C.albicans.were pan-azole-resistant.In addition,the ERG3 heterozygous

The content of gamma-aminobutyric acid in the aerial part of Panax notoginseng in different productive area was determined by using high performance liquid chromatography. HPLC analysis was made on a C18 (4.6 mm x 250 mm,5 microm) with acetonitrile and water containing 4.1 g x L(-1) sodium acetate as mobile phase. The detection wavelength was 254 nm. The HPLC method showed good linearity within the range of 0.01 - 1.03 g x L(-1). The average recovery of GABA in the stems and leaves of P. notoginseng and the flowers of P. notoginseng was 101.7% (RSD 1.1%, n = 3) and 97.3% (RSD 0.38%, n = 3), respectively. The contents of GABA in the samples of different productive areas were not significant different, and the average contents of GABA in the stems and leaves of P. notoginseng and the flowers of P. notoginseng were 0.49% and 0.53%. This method was simple and reliable, and it was suitable for the determination of gamma-aminobutyric acid in the aerial part of P. notoginseng.

Objective To observe the effects of iodine excess on thyroid morphology,the expression of thyroid peroxidase and sodium iodide symporter mRNA and to explore their mechanisms.Methods One-month SD rats were divided into three groups:control iodine(CI),high iodine Ⅰ(HI Ⅰ)and high iodineⅡ(HI Ⅱ)and were fed with water containing iodine in different concentrations by adding K103(5,5000,10 000μg/L)respectively.Rats were sacrificed after being fed for six months.The morphology of thyroid was investigated under light microscopy and electron microscopy,the serum thyroid hormones and ratio of TPO/β-actin and NIS/β-actin were measured by radio-immunoassay and RT-PCR method.Results The major changes were increased follicles with colloid accumulation in HI groups.The levels of serum thyroid hormones TT3 and TT4 were decreased gradually from CI[(75.68±13.99,1.45±0.49)nmol/L]to HI Ⅰ[(73.82±16.48,1.34±0.31)nmol/L]and HIⅡ groups[(70.65±11.43,1.15±0.39)nmol/L],but there were no significant differences among three groups(F=O.371,l.163,P＞0.05).The TPO and NIS mRNA expressions in HI Ⅰ(1.28±0.10,0.56±O.17)and HI Ⅱ(1.14±0.04,0.39±0.06)were significantly lower(F=30.863,62.62.675,P＜O.05)than those of control group(1.39±0.08,0.71±0.13).Conclusions Chronic iodine excess leads to definite histological changes in rat thyroid,and inhibits the expressions of TPO and NIS mRNA as well as thyroid hormone synthesis,which in turn acts as a protective mechanism against iodine excess.

The marine biological source of mineral drugs recorded in Chinese Pharmacopoeia (2010 version) mainly including pearl, nacre, clam shell, common oyster shell, ark shell, cuttle bone, and sea-ear shell are widely used in clinical. Calcium carbonate and a small amount of protein are the main components in this type of drugs. In this paper, a systematical and comparable study were carried out by determination of calcium carbonate by EDTA titration method, the crystal of calcium carbonate by X-Ray powder diffraction and the total amino acids (TAAs) of the hydrolyzed samples by ultraviolet spectrophotometry method. As a result, the crystal structure is calcite for common oyster shell, mixture of calcite and aragonite for nacre and sea-ear shell, aragonite for the other drugs. The content of calcium carbonate ranged from 86% to 96%. Cuttle bone has the highest amount of TAAs among the seven drugs which reached 1.7% while clam shell has the lowest content of 0.16% on average. In conclusion, an effective method was developed for the quality control of marine mineral drugs by comprehensive analysis of calcium carbonate and TAAs in the seven marine mineral drugs.

Objective To assess the changing trend of serum uric acid (SUA) levles in health check-up subjects of Tianjin municipality 2007-2015.Method Data were collected from 266 043 subjects receiving annual health check-up from 2007 to 2015 in Tianjin municipality.Changing trends of SUA levels and incidence of hyperuricemia were analyzed during the 9 year period.Results From 2007 to 2015 SUA levels in males were increased from (347 ± 73) μmol/L to (373-± 78) μmol/L,while in females were increased from(253 ± 59) μmol/L to (267-± 61) μmol/L.The trend test showed that both for men and woinen the incidence of hypemricemia was increased in each year (men:x2 =270.30,women:x2 =15.10,both P =0.00).The incidence of hyperuricemia by each year was increased more markedly in men than that in women (x2 level:194.80-1 877.00,all P =0.00).The incidence of hyperuricemia in young adults (20-39) was higher than that in middle-ages (40-59),and the latter was higher than that in elderly (＞ 60).For the 7-year (2009-2015) incidence of hyperuricemia in young and middle-aged men,the trend test showed statistical siganificance (x2 level:14.11-233.90,all P =0.00).By contrast,the incidence hyperuricemia in women was increased with aging,and for the 9-year incidence of hyperuricemia in young women,the trend test showed statistical siganificance (x2 level:115.30-480.20,all P =0.00).Conclusion The incidence rates of hyperuricemia are high in Tianjin health check-up subjects,especiallly in young males and old females.There is a time trend of increasing incidence of hyperuricemia in the young and middle-aged subjects.The results indicate that the prevention and control of hyperuricemia in Tianjin are of clinical and social significance.
