1.Effect of age on pharmacodynamics of cisatracurium target-controlled infusion
The Journal of Clinical Anesthesiology 2010;26(2):115-117
Objective To evaluate the effect of age on pharmacodynamics of eisatracurium target-controlled infusion(TCI). Methods Forty patients under N_2O-O_2-propofol-fentanyl anesthesia were divided into groups of Ⅰ and Ⅱ with 20 cases each. Under TOF stimulation monitoring, eisatracurium 0. 15 mg/kg was injected and tracheal intubation was performed when TOFr being zero. and the intubation conditions were evaluated. When T1 restored to 50% cisatracurium TCI started to keep TOFr less than 10%. The recovery times of neuromuscular blockade were recorded after stopping infusion. The amount of cisatracurium consumption was calculated. Results Intubation conditions the onset time and recovery index were not significantly different between two groups. The nonresponse period, duration of action, and recovery time were longer in group Ⅰ than those in group B. Cisatracurium consumption was more in group Ⅰ than that in group Ⅱ (P<0.05). Conclusion By TCI,cisatracurium dosage needed for keeping TOFr less than 10% is less, but the recovery process is still longer in the elderly than those in the younger.

Chinese Journal of General Surgery 2010;25(6):433-435
Objective To identify risk factors for anastomotic leakage after low anterior resection (LAR) in rectal cancer patients and study its impact on patients'long-term prognosis. Methods Chnical data were analyzed from 720 patients who underwent low anterior resection(LAR) for rectal cancer between 1996 and 2006.Results Anastomotic leakage after anterior resection occurred in 44 cases(6.1%).The median time of anastomotic leakage after operation was 5.6 days.Muhivariate analysis showed male patients.history of preoperative chemoradiation therapy, diabetes, cancer distance from anus less than 7 cm and hand-sewed anastomosis were independent risk factors predisposing anastomosis leakage (P<0.05).Tumor local recurrence rate was 13.6% in patients suffering from leakage and 5.9% for those without leakage (χ2= 4.116,P<0.05).The distant metastasis rates were 25.0 and 14.1 percent for the leakage and noaleakage groups,respectively(χ2=3.943,P<0.05).The survival rates were 56.8 and 72.5 percent in the leakage and nonleakage groups,respectively(χ2=4.979,P<0.05).Conclusion Sex,preoperative chemoradiation therapy, diabetes, cancer distance from anus less than 7 cm and hand-sewed anastomosis were found to be independent risk factors for anastomotic leakage after rectal cancer surgery.Anastomotic leakage was associated with poor prognosis.

Converting two poorly water-soluble crystalline drugs to co-amorphous drug systems by ball milling, quench-cooling, or cryo-milling method can improve stability of the drug, enhance dissolution rates, and reduce adverse reactions of the single drug. Co-amorphous system has been used to solve problems of co-administration of medicines. Formation and intermolecular interactions of co-amorphous drug systems may be verified by differential scanning calorimetry (DSC), X-ray powder diffraction (XRPD), Raman spectroscopy (RS) and Fourier transform infrared spectroscopy (FT-IR). Stability of co-amorphous drug systems is influenced by their glass transition temperature (Tg) and intermolecular interactions. The theoretical Tg values and the interaction parameter x are calculated by Gordon-Taylor equation and the Flory-Huggins equation, respectively. Thus, co-amorphous drug systems are analyzed theoretically at molecular level. Co-amorphous drug systems provide a new sight for the co-administration of medicines.

Objective To study the cultivation of retinal progenitor cells of different ages. Methods Retinal progenitor cells of E14 and E18 SD rats were isolated, cultivated in suspension in modified DMEM/F12 serum-free medium and then differentiation was induced in vitro.Cells were observed under phase-contrast microscopy daily and identified by immunocytochemistry, scanning electron microscopy and transmission electron microscopy. Results Cultivated in DMEM/F12 serum-free medium, retinal progenitor cells formed cell spheres. After being plated, isolated cells migrated outwards and differentiated.Scanning electron microscopy demonstrated the morphology of the cell spheres and differentiated cells.Transmission electron microscopy demonstrated that there were stem cell-like cells in cell spheres and neuron-and glia-like cells after the plating.Immunocytochemistry demonstrated that most cells in spheres expressed neural stem cell marker nestin and cell division marker BrdU.After the plating, retinal progenitor cells could be induced to differentiate into various retinal cells, including Thy1.1-positive retinal ganglion cells. The percentage of retinal ganglion cells was 16.91%?4.05% at E14 and 4.65%?1.88% at E18.The differences were statistically significant(t=15.04,P

Objective To observe the effect of improved body turning approach on wound healing after operation of congenital craniovertebral junction abnormity. Methods 82 patients with congenital craniovertebral junction abnormity were performed occipital-cervical bony grafting posteriorly and they were cared with improved or routine body turning approach. Results The incidence of wound healing was 95.1% in routine group and was 100% in improved group. Conclusion The improved body turning approach could improve the wound healing after operation of congenital craniovertebral junction abnormity.

Journal of Peking University(Health Sciences) 2015;(3):499-503
Objective:To analyze the characteristics of hospital infection of hematological disease, so as to provide reference for clinical therapy. Methods: Bacterial strains and antimicrobial resistance of pa-tients with hospital infection in Department of Hematology, Peking University Third Hospital from Jan. 2011 to Dec. 2013 were identified and analyzed retrospectively. The specimens were from their blood, urine, sputum, throat swabs and etc. Results:Among the total of 168 isolates of bacteria, the majority of the bacteria strains were from sputum (42. 9%);114(67. 9%) bacteria strains were gram negative and 54(32. 1%) bacteria strains were gram positive;the pathogen testing showed that 20. 8% were Pseudo-monas aeruginosa,18. 5% Escherichia coli,17. 9% Staphylococcus aureus, 9. 5% Klebsiellar pneumonia, 5. 9% Staphylococcus epidermis and 27. 4% other bacteria ;The gram negative bacilli to cefepime, ami-kacin and carbapenems showed the lowest antimicrobial resistance rates, and S. aureus showed the lowest antimicrobial resistance rates to vancomycin and linezolid. Conclusion:Patients with hemopathy are the main population of hospital infections, the gram negative bacteria are the most common pathogens. It is very important to promptly know the change in distribution of the pathogens in order to rationally select antibiotics and reduce the incidence of bacterial infections.

Objective To investigate the variation of myocardial glycometabolism and the release of cardiac creatine kinase and cardiac troponin I on isolated rat heart after stimulated with hyperinsulinism. Methods Thirty Sprague-Dawley rats were random divided into five groups, and their hearts were taken out to be mounted onto a Langendorff perfusion apprartus to perfuse with krebs-Henseleit buffer contained diversity concentration of insulin as 0,10,20,30,50 U/L.The coronary effluent was collected on time, and the concentration of glum was determined to evaluate the glycometabolism of the rat heart, meanwhile the release of cardiac creutine kinase(CK)and cardiac troponin I were detected to evaluate the state of myocardial damage. Results Compared with control group, the rat hearts of the hyperinsulinism group had a significantly depression of glucose uptake and oxidation, and the level of CK and cTn-I in the coronary effluent was increased. Conclusion After the isolated rat hearts were stimulated with hyperinsulinism, the glucose uptake of cadiocyte were suppressed, myocardial insulin resistance appeared, and the rat myocardial got damaged.
