1.Treatment of Recurrent Oral Ulceration (Yin Deficiency Fire Excess Type) by Qianjin Kouchuang Jiawei Granule: a Clinical Observation.

OBJECTIVETo observe the curative effect of Qianjin Kouchuang Jiawei Granule (QKJG) on recurrent oral ulceration [yin deficiency fire excess type (YDFET)].
METHODSTotally 120 patients who suffered from recurrent oral ulceration (YDFET) were randomly assigned to two groups, the experiment group and the control group, 60 in each group. Patients in the experiment group took QKJG, 20 g each time, twice per day, while those in the control group took Kouyanqing Granule (KG) , 20 g each time, twice per day. Fourteen days consisted of one therapeutic course, two for all. Scores for patients' symptoms and signs (ulcer area, exudation, hyperaemia, edema, the number of ulceration, burning sensation, and pain degrees) were assessed before treatment, at day 3 and 7 after treatment. Short-term efficacy was assessed by visual analogue scale (VAS). The total paralysis time and the total number of ulceration at month 12 after treatment were taken as judgment for long-term efficacy. Results Compared with before treatment in the same group, symptoms and signs were obviously improved at day 3 and 7 after treatment in the two groups (P < 0.05). Compared with the control group at day 3 after treatment, the improvement of edema, exudation, pain degree, and burning sensation was more obvious in the experiment group (P < 0.05, P < 0.01). The improvement of edema, pain degree, and burning sensation at day 7 after treatment was more obvious in the experiment group than in the control group (P < 0.05). As for short-term efficacy, the total effective rate was 86.67% (52/60 cases) in the experiment group and 83.33% (50/60 cases) in the control group, with no statistical difference between the two groups (P > 0.05). As for long-term efficacy, the total effective rate was 90.00% (54/60 cases) in the experiment group, significantly higher than that of the control group with statistical difference [81.67% (49/60 cases), P < 0.05]. At month 12 after treatment, the total number of ulceration was reduced and the paralysis time of ulcer attack prolonged in the experiment group, with statistical difference when compared with the control group (P < 0.05).
CONCLUSIONQKJG showed better long-term efficacy than that of KG in treating recurrent oral ulceration (YDFET).

3.Evaluation of application effect of nursing intervention on improvement of psychological behavior and quality of life for patients with Alzheimer disease

ObjectiveTo observe the effect of nursing interventions on improvement of psychological actions and quality of life of patients with Alzheimer disease. Methods50 patients with senile dementia diagnosed in our hospital were chosen.They were divided into the observation group and the control group with 25 patients in each group.All patients were taken with AD routine nursing care after admission.The observation group was taken with rehabilitation nursing interventions based on AD routine nursing care.The mini-mental state examination (MMSE)and the daily life ability scale (ADL)were used to evaluate the quality of life after nursing intervention. ResultsThe patients' MMSE score in the observation group was significantly higher than the control group,the difference was statistically significant.The ADL score of the observation group was significantly lower than the control group,the difference was statistically significant. ConclusionsThe use of integrated care intervention can significantly improve the cognitive function in AD patients,and improve daily living ability and quality of life significantly.
4.Effect of Occupational Therapy on Activities of Daily Living of the Children with Cerebral Palsy: 62 Case Report

ObjectiveTo observe the effect of occupational therapy (OT) on activities of daily living (ADL) of the children with cerebral palsy (CP).MethodsSixty-two CP children were treated with OT. The ADL scores of all children were assessed within one week after admission and training goal and plan were made according to evaluated result. The therapeutic effect was assessed before discharge.ResultsAfter OT treatment, the ADL scores of the children increased significantly ( P<0.01) and disable grades decreased (χ2=10.48, P< 0.01). The total effect rate was 87.1%.ConclusionOT training can improve ADL of the children with CP significantly.
5.Determination of Rebamipide in Rats' Plasma and Colonic Tissue by HPLC Gradient Elution

OBJECTIVE: To establish an HPLC method for the determination of rebamipide in rats' plasma and colonic tissue.METHODS: The rats were assigned to receive 4 mg rebamipide chitosan capsule or its gelatin capsule or its CMC-Na solution peros.Then the plasma and colon tissue samples were taken at different time for a determination of the concentration of rebamipide by HPLC gradient elution with Cmax computed.RESULTS: The linear range for rebamipide was from 20~2 000 ng?mL-1.The average extracting recoveries of rebamipide from the plasma and the colon were 88.4% and 90.8%,respectively.After oral administration of 4 mg rebamipide chitosan capsule or its gelatin capsule or its CMC-Na solution,the Cmax of rebamipide in the colon tissue were 5 963.9,2 190.7 and 1 185.8 ng?g-1 respectively versus 252.7,949.0 and 1 023.3 ng?mL-1 respectively in plasma.CONCLUSIONS: The established HPLC gradient elution in the study can be used to determine quantitatively the rebamipide level in plasma or colon tissue for the development of its colon specific delivery preparations.

PEI Xue-yi has 60 years,experience in treating pediatric diseases with traditional Chinese medicine,especially obtains good effect in children's nephrapiathy.Based on the regulation and characteristics of children's zang-fu,yin-yang,blood-qi and children's nephrapiathy,he makes different therapeutic plans in different phases.In clinical treatment,he emphasizes the lung,the spleen and the kidney and combines syndrome differentiation with disease diagnosis.Eliminaing pathogens,excessive damp-heat is used in acute phase and strengthening healthy qi,securing the spleen and kidney is used in recovery phase.Children's nephrapiathy is treated by three phases as the edema,protein urine and rescovery stage.He adopts the methods of dispersing lung qi and invigorating spleen for diuresis,clearing heat-damp,nourishing spleen and kidney,nourishing yin for protecting lower jiao.
7.Effect of ginsenoside-Rh2 on the apoptosis in myeloid leukemia cell strain: Dose- and time-dependent manners

AIM: It is found that some active priciples of ginseng can enhance and activate body immune system, and have many bioactivities such as anti-tumor, anti-aging and anti-radiation. This study examined the effect of ginsenoside-Rh2 (GS-Rh2) on proliferation and apoptosis in human myeloid leukemia cell strain HL60, and analyzed the dose- and time-dependent manners of GS-Rh2. METHODS: Experiments were performed at the Department of Clinical Laboratory of Jilin Medical College from July to August in 2006. ①Rh2 was purchased from Hongjiu Biotech Co., Ltd. (batch number 050801), and prepared into 50 g/L stock solution by dissolving in pH 7.4 phosphate buffer saline. The HL60 cell strain was purchased from Shanghai Institute of Cell Biology of Chinese Academy of Sciences. ②HL60 cells in logarithmic growth phase were inoculated into 3?108 L-1 cell suspension. After the cells were cultured for 6 hours, 100 ?L GS-Rh2 at different concentrations (5,10,20,40,80 mg/L) was added respectively. After the cells were administrated for 48 hours, cell inhibition ratio (IR) was evaluated by MTT colorimetric assay. The 50% inhibitory concentration (IC50) was worked out. HL60 cell was acted with this concentration for different time (6, 12, 24, 48 and 72 hours), cell inhibition ratio (IR) at different time points was evaluated by MTT colorimetric assay, and compare it to the control. After the IC50 of GS-Rh2 acted for 48 hours, HL60 cells were observed with an inverted microscope. HL60 cell was stained by Giemsa, and the typical apoptosis cells were discovered. RESULTS: ①Dose-effect relationship: When the concentration of GS-Rh2 was 5,10,20,40 mg/L, the IR of GS-Rh2 to the growth of HL60 cells was increased gradually in obviously dose-dependent manner. The IR was similar between 80 mg/L and 40 mg/L. After the cells were administrated for 48 hours, the IC50 value was 13.0 mg/L. ②Time-effect relationship: After the concentration of IC50 of GS-Rh2 (13.0 mg/L) acting on HL60 line at different time points (6, 12, 24, 48 and 72 hours), cell IR was increased gradually (F=9.32,P
8.The correlation between uric acid and the incidence and prognosis of kidney diseases: a systematic review and meta-analysis of cohort studies

Objective To evaluate the association of serum uric acid and the incidence and prognosis of kidney diseases systematically, so as to provide reference for the treatment and prevention of kidney diseases. Methods Literatures related to the associations between serum uric acid and incidence and prognosis of kidney diseases were selected from the Chinese Biomedical Literature Database ( CBM) (January 1982 to March 2010), EMBASE (January 1966 to March 2010) and Medline (January 1950 to March 2010) for cohort studies. Two researchers independently screened the studies, assessed the risk of bias of included studies using the Newcastle-Ottawa Quality Assessment Scale for Cohort Studies and extracted data. Stata 10. 0 was used to calculate the pooled relative risk. Results Twenty-one eligible cohort studies were selected, of which 11 on incidence of kidney diseases (n =276 801 ) , and 10 on the prognosis of kidney diseases ( n = 3004). Meta analysis was performed based on data influencing incidence and prognosis factors of kidney diseases except for serum uric acid. The results showed, (1) uric acid and incidence of kidney diseases: hyperuricemia could increase the risk of kidney diseases ( RR = 1. 49, 95% CI 1. 27-1. 75) ; (2) uric acid and prognosis of patients with kidney diseases:hyperuricemia could deteriorate the kidney function (RR = 1. 35, 95%CI 1. 12-1.63) and increase the risk of mortality (RR = 1.67, 95% CI 1.29-2.16). Conclusion Uric acid is an independent risk factor for incidence of kidney diseases and poor prognosis of patients with kidney diseases. Further high quality clinical trials with long-term follow up should be conducted to determine whether lowering uric acid levels would be of clinical benefit in the prevention and treatment of kidney diseases, so as to provide direct evidence for clarifying correlation between uric acid and kidney diseases and the prevention and treatment for patients with high uric acid.
9.Sleeve gastrectomy with ileal interposition and duodenojejunal bypass for the treatment of nonobese Type 2 diabetes mellitus

Objective To evaluate sleeve gastrectomy with ileal interposition and duodenojejunal bypass for the treatment of nonobese Type 2 diabetes mellitus.Methods Forty one patients of nonobese Type 2 diabetes mellitus underwent sleeve gastrectomy with ileal interposition and duodenojejunal bypass.Fasting glucose ( FPG ),glycosylated hemoglobin ( HbAlc ),fasting insulin and C-peptide,triglycerides (TG),high density lipoprotein(HDL),low density lipeprotein(LDL) were measured preoperatively and on postoperative first,3rd,6th month.Results Mean postoperative follow-up was 9.6 months (range 6-21 months).95% patients achieved adequate glycemic control (HbAlc < 7% ) without antidiabetic medication.Fasting glycemia decreased from ( 9.7 ± 0.4 ) mmol/L to ( 6.2 ± 0.3 ) mmol/L ( P < 0.01 ).Glycosylated hemoglobin decreased from 8.1% ± 1.4% to 5.8% ± 0.6% ( P < 0.01 ).2-hour postprandial blood glucose decreased from ( 13.6 ± 0.7 ) mmol/L to ( 10.6 ± 0.2 ) mmol/L ( P < 0.01 ).Insulin resistance (Homa-R) decreased from 4.8 ± 1.3 to 1.2 ±0.4 (P <0.01 ).Fasting C-peptide increased from ( 3.3 ± 1.7 ) ng/ml to (4.9 ± 0.2 ) ng/ml ( P < 0.01 ).Fasting insulin increased from ( 10.2 + 1.4 ) mlu/L to (15.6±0.7) mlu/L(P<0.01 ).Triglycerides (TG) decreased from (3.1 ±0.5) mmol/L to (1.9 ±0.4) mmol/L ( P < 0.01 ).High density lipoprotein (HDL) increased from ( 1.2 ±± 0.2 ) mmol/L to ( 1.9 ±0.8 ) mmol/L( P < 0.01 ).Low density lipoprotein (LDL) decreased from (3.5 ± 0.3 ) mmol/L to (2.4 ±0.6) mmol/L (P <0.01 ).Hypertension was controlled in 3/7 cases.Microalbuminuria resolved in 78% patients.Retinopathy was improved in 53% cases.Conclusions Sleeve gastrectomy with ileal interposition duodenojejunal bypass is effective for treatment of nonobese type 2 diabetes mellitus as showed by 6 month's follow-up.
10.Mifepristone repairs alteration of learning and memory abilities in rat model of depression.

This study is to investigate the amelioration effect of glucocorticoid receptor (GR) antagonist mifepristone on the changes of learning and memory abilities in rat model of depression. In the present study, a 35-day rat chronic unpredictable stress (CUS) model was used to observe both depression-like behaviors with sucrose preference test and open-field test and learning and memory-associated behaviors with Morris water maze test. A total of 45 male adult Sprague-Dawley rats were randomly assigned to three groups of equal size: control group (CON); CUS group (CUS); CUS + mifepristone group (CM). Animals in CM group were first exposed to CUS for 14 days, and then were administered with 50 mg x kg(-1) x d(-1) of mifepristone with continued CUS procedure. Corticosterone EIA Kit was used to detect the concentration of plasma corticosterone (CORT). Nissl staining was used to observe the structure of hippocampus. The results demonstrated that CUS exposure induced both depressive-like and learning and memory-associated behaviors and these deficits were reversed by mifepristone. Compared to CON group, the concentration of plasma CORT increased significantly in CUS group. CUS exposure damaged the structure of hippocampus, whereas mifepristone had an amelioration effect. Together, the structural deficits of hippocampus resulting from long-term stress exposure, which could contribute to the impairment of learning and memory in depression, are reversed by the GR receptor antagonist mifepristone.
