2.Significance of AKR1C3 for esophageal cancer patients under radiotherapy

Objective To evaluate the significance of the expression level of aldo-keto reductase family 1 member C3(AKR1C3) for esophageal carcinoma patients treated with radiotherapy.Methods The clinical data of 28 patients with locally advanced esophageal carcinoma treated with radiotherapy alone was retrospectively analyzed, and the expression level of AKR1C3 was detected by immunohistochemistry.Statistical analysis was performed using SAS 8 statistical software, P<0.05 was considered statistically significant difference.Results The expression level of AKR1C3 was different in esophageal carcinoma patients at different levels of differentiation.The level of AKR1C3 expression was negatively correlated with efficacy of radiotherapy (P=0.031, 95%CI 0.151-0.914).Conclusion AKR1C3 can serve as a promising biomarker of the efficacy of radiotherapy for esophageal cancer.
3.Gene mutation in fetuses with short limbs deformity

Objective To identify the genetic mechanism of fetuses with short limbs deformity.Methods From Aug.2008 to Aug.2011,ten fetuses with obvious short limbs were found in ultrasound screening performed at 18-24 and (or) 30-32 gestational weeks and underwent artificial induced labor with the patient' consent.Amniotic fluid or cord blood of the fetuses was collected for karyotyping analysis and detection of mutation point of fibroblast growth factor receptor 3 (FGFR3)gene by polymerase chain reaction and gene sequencing.One fetus (case 3) who presented with achondrogenesis underwent sequencing of SLC26A2 and Trip11 gene meanwhile.Results Among the 10 fetuses with short limbs deformity,five cases were found during second trimester and five during third trimester.Nine cases were identified as normal karyotype and one was chimera (46,XY/45,XY,- 18).One fetus carried a rare FGFR3 mutation of c.1108G>T (G370C) and was diagnosed as thanatophoric dysplasia at 21+3 weeks.Three fetus carried c.1138G>A (G380R) mutation and were diagnosed as achondroplasia.These four families had low recurrent risk because no gene mutations were found in the parents.Three mothers of these four fetuses were pregnant again and had normal neonates now.No mutations were found in all gene sequencing in case 3.Conclusions Karyotyping analysis and sequencing of FGFR3 gene could find causative gene mutations and provide genetic counselling and prenatal diagnosis for some fetuses with short limbs deformity.In the third trimester,achondroplasia is the most possible diagnosis when short limbs fetus is found by ultrasound.
4.Optimal use of forehead flap pedicled with supratrochlear artery for aesthetic reconstruction of nose

Objective To explore the design of forehead flap pedicled with supratrochlear artery and shift way for aesthetic reconstruction of the nose without secondary operations for debulking.Methods Origin of blood supplies and the distributed situation of the vessel in the frontal region were studied through the internal carotid artery and the external carotid artery X-ray angiography.The frontal region existed polyphyletic blood supplies,the supratrochlear artery and the supraorbital artery set out many branches,respectively,in the level of frontal bone eyebrow plane.One constant,thicker horizontal branch from the superficial temporal artery obviously extended to the forehead median.The branches from three blood vessels mentioned above constituted the network of artery adequate in the muscle and the subcutaneous tissue level,assumed the three-dimensional distribution in the forchead region.The nasal reconstruction had been done for 9 patients with the oblique orientation designed forehead flap by using one side of supratrochlear artery as the pedicle and cutting skin paddle with the same or the opposite side of forehead flap.Results All flap survived completely with the restoration of nasal shape and functional satisfaction.Conclusion The forehead oblique orientation flap pedicled with one side supratrochlear artery is nourished by the rich blood supply.The flap can be split into two flaps:a myofascial flap and a skin flap in far part.Myofascial flap may be the good package of cartilage framework,and simultaneously split skin flap is favorable of the shape models.The flap may satisfy the demand of the nasal restoration.
5.The application of ultrasound-guided arterial catheterization in internships of anesthesiology

Objective To evaluate the effect of ultrasound-guided arterial catheterization in internships of anesthesiology.Methods Sixty students were randomly divided into ultrasound-guided teaching group(n=30,3 male) and traditional teaching group (n=30,4 male).In the two groups,the ultrasound-guided arterial catheterization technology and artery catheterization technique were adopted respectively for teaching.The success rate of arterial catheterization was compared between the two groups after 2 months internships and when finishing internships of anesthesiology.The questionnaire was investigated in ultrasoundguided teaching group after finished internships of anesthesiology.Results The success rate of arterial catheterization in ultrasound-guided teaching group was extremely higher than in traditional teaching group after 2 months internships of anesthesiology (52 ± 8)% vs.(38 ± 10)%,(x2=7.746,P=0.005).After finishing internships of anesthesiology,the success rate of arterial catheteri-zation was (67±7)% in ultrasound-guided teaching group and (59 ± 9)% in traditional teaching group (x2=5.531,P=0.036).And all students thought ultrasound-guided arterial catheters teaching was helpful to them and could improve their learning interest.Conclusion Ultrasound-guided arterial catheterization is beneficial to the internship teaching of undergraduate of anesthesiology,which can help them to master the arterial catheterization technique better,and improve the success rate of arterial catheterization and their learning interest.
6.Renal protection of Tangke Decoction on rats with diabetes and its effect on the expression of TGF-beta1/Smad4.

OBJECTIVETo observe the effect of Tangke Decoction (TD) on the expression of TGF-beta1/Smad4 of rats with early diabetes and to explore the effect and mechanism of TD against the renal injury induced by diabetes.
METHODSSD rats were randomly divided into the normal control group (n = 12), the model group (n = 10), the Chinese herbs prevented group (n =10), the Chinese herbs treated group (n = 10), and the Western medicine control group (n = 10). TD (18 mg/kg) was given by gastrogavage to rats in the Chinese herbs prevented group immediately after successful modeling for 12 weeks, once daily. At the 4th week of successful modeling, rats in the rest 4 groups were administered by gastrogavage. Equal volume of normal saline was given to rats in the model group and the normal control group. Benazepril suspension (1 mg/kg) was administered by gastrogavage to rats in the Western medicine control group for 8 weeks, once daily. TD (18 mg/kg) was given by gastrogavage to rats in the Chinese herbs treated group for 8 weeks, once daily. The body weight, kidney weight, index of kidney weight, fasting blood sugar, 24 h urinary albumin excretion rate were examined after experiment. The pathological changes of the renal tissue were observed by HE staining, Masson staining, and electron microscope. The expression of renal transforming growth factor-beta1, (TGF-beta1) and Smad4 were detected using immunohistochemical assay.
RESULTSCompared with the normal control group, the body weight of rats decreased significantly; the kidney weight, index of kidney weight, blood sugar, 24 h urinary protein excretion, the urinary albumin excretion rate,TGF-beta1 and Smad4 expression increased significantly in the model group (all P < 0.01). Compared with the model group, the aforesaid indices were improved in each treatment group with statistical difference (P < 0.05, P < 0.01). Compared with the Western medicine control group, the kidney weight, index of kidney weight, blood sugar, 24 h urinary protein excretion, and the urinary albumin excretion rate were obviously improved in the Chinese herbs prevented group (P < 0.01). The renal pathological changes were most obvious in the model group significantly, but they were improved in all treatment groups.
CONCLUSIONTD could obviously improve the symptoms of diabetes and down-regulate the expression of renal TGF-beta1 and Smad4 of early diabetic nephropathy rats, which suggested that TD had certain preventive effect on early diabetic nephropathy.

8.Quality analysis of outpatient prescriptions at both township and village levels in two counties of Hubei province

Objective To study the quality of outpatient prescriptions for patients of different age groups at both township and village level,for the purpose of evaluating the outcomes and problems of the ongoing healthcare reform at primary levels.Methods 100 outpatient prescriptions of April 2015 were mechanically sampled randomly from two township hospitals and 8 village clinics in counties A and B in Hubei province.These prescriptions were analyzed for the drug count per prescription,percentage of intravenous prescriptions,that of antibiotics,that of hormones and average cost per prescription.Results At the township hospitals,the drug count per prescription(2.02)of county A was less than that of county B(3.26),while the percentage of intravenous prescriptions(30.30%),that of antibiotics(47.98%)and hormones(6.57%)of county A were significantly less than those of county B(50.65%,69.08% and 15.13% respectively).At village clinic level,the percentage of intravenous prescriptions(31.05%)of county A was higher than that of county B(20.34%),the average prescription costs(¥29.28)of county A was less than county B(¥31.45);while in terms of children′s prescriptions,average drug count of county A(2.50)was higher than county B(2.09),and its proportion of antibiotics(65.91%)was higher than county B(45.56%).Conclusions General primary care reform is faced with challenges of poor control of intravenous injection and use of antibiotics,particularly at village clinics and pediatric drug use in terms of prescription quality control.Both township and village levels should strengthen the supervision over the drug suppliers,guide the demand side to rationally use drugs and focus on the reasonableness of the medication of village clinics and children.
9.Relationship between the expression of human leukocyte antigen-b in peripheral blood leukocytes and the differentiation and metastasis of gastric carcinoma.

OBJECTIVETo investigate the relationship between the expression of host human leukocyte antigen-B mRNA (HLA-B mRNA) and HLA-B antigen in peripheral blood leukocytes (PBLs) and the differentiation and metastasis of gastric carcinoma (GC).
METHODSTo design and screen specific primers of HLA-B gene independently, detect the expression of HLA-B mRNA from 30 GC patients by reverse transcription-PCR and compare with the HLA-B antigen expression measured by flow cytometry.
RESULTSThe expression rate of PBL HLA-B mRNA from GC patients (23. 3% ) was very significantly lower than that of normals (87. 5% ) (P <0. 01) , especially concerning the poorly differentiated GC patients with lymph node metastasis (16. 0% ). Measured by flow cytometry, the expression percentage of HLA-B antigen of well-differentiated GC patients without lymph node metastasis was 88. 2% , an obviously decreasing tendency was showed in comparison with that in the normal group (98. 8% ) , although the difference was not significant (P = 0. 056) , and the expression percentage in poorly differentiated GC patients with lymph node metastasis(73. 3% )was declined significantly (P <0. 05).
CONCLUSIONThe expression of PBL HLA-B mRNA and HLA-B antigen in GC patients is decreased or lost, and correlated with differentiation and metastasis of the cancer. The expression of PBL HLA-B mRNA may more directly reflect its relationship with the tumor differentiation and metastasis than that of HLA-B antigen.

10.Analysis on the prevalence of thyroid cancer in Jing′an District, Shanghai

Objective To explore and analyse the prevalence of thyroid cancer among population of Jing′an District, Shanghai, providing a scientific basis for prevention and intervention. Methods Analysis was made on the prevalence of thyroid cancer in Jing′an District in 2014 and its incidence and death trend from 2009 to 2014 based on the data of Shanghai Cancer Registration and Reporting System. Results From 2005 to 2014, the total number of thyroid cancer cases in Jing′an District were 2 430, ranking fourth of all malignant tumors.The crude incidence rate of thyroid cancer was 24.33/100 000, the standardized incidence rate was 16.67/100 000.The ratio of male to female was 0.34 : 1;the difference in incidence between male and female was significant(
