OBJECTIVETo study the chemical constituents of Memorialis hirta.

METHODCompounds were isolated and purified by multiple methods, and their structures were identified based on physicochemical property and spectrum data.

RESULT12 compounds were isolated from ethyl acetate from 95% ethanol extracts of M. hirta, they were isorhamnetin (1), kaempferol (2), quercetin (3), isorhamnetin-3-O-alpha-L-rhamnopyranoside (4), kaempferol-3-O-alpha-L-rhamnopyranoside (5), isorhamnetin-3-O-beta-D-glucopyranoside (6), kaempferol-3-O-beta-D-glucopyranoside (7), quercetin-3-O-alpha-L-rhamnopyranoside (8), quercetin-3-O-beta-D-glucopyranoside (9), isorhamnetin-3-O-rutinoside (10), kaempferol-3-O-rutinoside (11) and quercetin-3-O-rutinoside (12), respectively.

CONCLUSIONAll compounds were obtained from the genus Memorialis for the first time.

OBJECTIVEThe Ministry of Public Health released the National Surveillance project on Shigellosis in August, 2005. This study was to reveal the antimicrobial resistance status of Shigella isolates through the National Shigellosis Surveillance System in 2005 in China, so as to provide evidence for the development of surveillance, prevention and cure of Shigellosis.

METHODSAll the lab assistants received training from Chinese Center for Disease Control and Prevention. The project prescribed the uniform experimentation, quality control method, reagent, etc. Disc diffusion test(K-B) was carried out, following the CLSI methods. Data were analyzed by WHONET 5.4 software.

RESULTS(1) 3 serotypes were identified and S. flexneri was common that accounted for 75.5% of all Shigella isolates followed by 24.4% of S. sonnei, but only 1 strain of S. dysenteriae was separated. (2) The resistant rates to tetracycline and ampicillin in Shigella spp were quite high, as over 90.0%. However, the resistant rate to Cefotaxime was the lowest, only 6.1%. The resistant rates were different between serotypes with the resistant rates of S. flexneri to ampicillin, ampicillin/clavulanate and ciprofloxacin were higher than those of S. sonnei (P < 0.001). (3) The multiple-antibiotic-resistance status in Shigella spp was quite serious and the resistant rate to five and more antimicrobials was 54.9%. The most common resistant patterns were seen on ampicillin, nalidixin, tetracycline and sulfamethoxazole. (4) There were some differences in subtypes and antimicrobial resistance among different provinces.

CONCLUSIONCefotaxime seemed the best in curing Shigellosis at the clinic level. Programs regarding monitoring subtypes and antimicrobial resistance of Shigella should be in a continuous manner so as to understand the pathogens timely and to control the disease pertinently.

OBJECTIVETo investigate the compliance of standard antenatal care (ANC) model with 12-13 visits currently used in Beijing region, and to assess the efficacy of this model in reducing adverse maternal and perinatal complication.

METHODSThe clinical data of 544 women who delivered at Peking Union Medical College Hospital (West Section) from January 1, 1999 to December 31, 2002 were retrospectively reviewed and analyzed. Three areas were addressed in this paper: compliance of pregnant women with standard ANC model; association of maternal and perinatal complication with different number of ANC visits; effectiveness of screening for risk factors at the first ANC visit.

RESULTSA median of 8 ANC visits was made in 544 cases, of whom 22 cases (4.0%) never had ANC visit before delivery. The women were divided into three groups according to the status of residence and education levels: temporary residents in Beijing city (group A), permanent residents with middle or low education level (group B), and permanent residents with high education level (group C). The average number of ANC visits in group A was 4.55 +/- 3.1, which was much lower than in group B (8.71 +/- 2.2) and in group C (9.56 +/- 2.1) (P < 0.001). The mean duration of gestation at the first ANC visit in group A was (25.44 +/- 8.8) weeks much longer than (15.58 +/- 5.8) weeks in group B and (14.24 +/- 3.2) weeks in group C (P < 0.001). Among 544 cases, 93 (17.1%) had ANC visit for 0-3 times, 299 (55.0%) for 4-9 times and 152 (27.9%) for > or = 10 times. There was no statistical difference among varied number of ANC visits when the results were pooled for pregnancy induced hypertension, gestational diabetes mellitus, vaginal bleeding at the second and third trimesters, postpartum hemorrhage, fetal macrosomia, premature rupture of membrane, and fetal distress (P > 0.05). An increase in the number of ANC visits was associated with the decreased rates of fetal growth restriction (P < 0.05) and premature delivery (P < 0.05), whereas it was paralleled with increased rates of anemia and cesarean section (P < 0.001). It was found that 35.6% of women who developed maternal and perinatal complications would be identified through screening for risk factors at the first ANC visit.

CONCLUSIONSStandard ANC model is currently not well complied. It has limited efficacy in reducing most maternal and perinatal complications. A more practical and effective ANC model for low educated women and temporary residents needs to be explored.

Objective: To identify the relationship between Dietary Diversity and Nutritional Status in Elderly Inpatient. Methods: 136 elderly inpatients were selected in a tertiary hospital in Yunnan Provience. Food frequency questionnaire was used to collect dietary information and DDS9 was calculated; We used Mini Nutritional Assessment (MNA) to evaluate nutritional status, then analyzed the effect of Dietary Diversity on nutritional status in elderly inpatients. Results:① Patients at risk of malnutrition and with confirmed malnutrition in participants were 46. 3％, 33. 8％,, respectively. ② The incidence of insufficient diversity, moderate diversity and adequate diversity were 19. 9％, 77. 2％ and 2. 9％, respectively; the total scores of dietary diversity were (4. 68 ± 1. 27). However, apart from vegetables, the rest 8 kinds of food all showed lower scores of intra-group variety; while the consumption of beans, nuts and seafood was seriously inadequate. ③ The incidence of malnutrition risk and confirmed malnutrition in insufficient-diversity group and moderate-diversity group were 5. 109 times and 1. 094 times of adequacy group (P＜ 0. 05). Conclusion: Dietary diversity is strongly associated with putritional status in elderly inpatient. Improvement of dietary diversity status is potentially beneficial in the prevention of malnutrition.

Objective:To analyze original medical humanities articles written by staff of the hospital and extract key elements of hospital culture for staff perception, for recommendations on homogenous development of multiple campuses of the hospital.Methods:Original medical humanities articles written by staff of the hospital were collected from the WeChat accounts, websites and printouts of a tertiary hospital in Beijing from January 2013 to December 2019, with text mining made by Python3.7; Bert and Tencent open-source 8 million Chinese word dataset were used as entity identification training dataset; K-Means cluster analysis was used to analyze and select cluster results of higher degree of fitting; TextRank was used to screen keywords of each clustered category. Significance of each category was summarized in the end based on keywords following the screening.Results:Among the 341 articles collected, high frequency words were work, hospital, patient, sick person, development, medicine, medical treatment, doctor, outpatient service and clinic. The words fell into ten categories: process recall, work attribute, work responsibility, diagnosis and treatment behavior, growth experience, team cooperation, professionalism, management innovation, doctor-patient communication and others.Conclusions:Research on hospital staff culture perception is conducive to identifying the cultural content of the hospital. In the homogenous development of multiple campuses of the hospital, it is imperative to nurture the staff′s sense of identify and belonging to the hospital, enhance their sense of gain and happiness, and strengthen the scientific management system centering on patients.

Objective To examine the association between pregnancy-induced hypertension(PIH) and small-for-gestational-age babies(SGA)in a Chinese population.Methods Subjects were women who delivered a singleton baby(gestational weeks:equal to or greater than 28,and less than 42)in four cities or counties in Jiangsu and Zhejiang provinces,China,during the period of 1995-2000.A total number of 93 743 women were included.Incidence of SGA was calculated and compared between women with or without PIH and between groups with different severities of PIH.Multiple logistic regression was used to address the relationship between PIH and SGA while controlling for maternal age,occupation,education,parity,BMI,anemia,premature rupture of membranes and fetal sex.The association between PIH and SGA was also examined according to preterm or term delivery.Results The incidence of SGA in women with PIH(6.0%)was higher than women without(4.5%),and the incidence increased with severities of PIH.The adjusted relative risk rates(95%CI)of SGA in women with mild,moderate and severe PIH were 1.17(1.01-1.34),1.69(1.33-2.14),and 3.50(2.57-4.77),respectively,when confounders were controlled for.The risk ratios of SGA in women with PIH among women who delivered a preterm baby wete higher than those among women who delivered a term baby.Conclusion There seemed a statistical association between PIH and SGA,and women with PIH having higher incidence of SGA than those withont PIH.

BACKGROUNDThin endometrium is associated with poor reproductive outcomes; estrogen treatment can increase endometrial thickness (EMT). The aim of this retrospective cohort study was to investigate the factors influencing the effectiveness of estrogen treatment and reproductive outcomes after the treatment in patients with thin endometrium.

METHODSRelevant clinical data of 101 patients with thin endometrium who had undergone estrogen treatment were collected. Possible factors influencing the effectiveness of treatment were analyzed retrospectively by logistic regression analysis. Eighty-seven infertile women without thin endometrium who had undergone assisted reproduction served as controls. The cases and controls were matched for age, assisted reproduction method, and number of embryos transferred. Reproductive outcomes of study and control groups were compared using Student's t-test and the Chi-square test.

RESULTSAt the end of estrogen treatment, EMT was ≥8 mm in 93/101 patients (92.1%). Effectiveness of treatment was significantly associated with maximal pretreatment EMT (P = 0.017) and treatment duration (P = 0.004). The outcomes of assisted reproduction were similar in patients whose treatment was successful in increasing EMT to ≥8 mm and the control group. The rate of clinical pregnancy in patients was associated with the number of good-quality embryos transferred in both fresh (P = 0.005) and frozen-thawed (P = 0.000) embryo transfer cycles.

CONCLUSIONSThinner EMT before estrogen treatment requires longer treatment duration and predicts poorer treatment outcomes. The effectiveness of treatment depends on the duration of estrogen administration. Assisted reproductive outcomes of patients whose treatment is successful (i.e., achieves an EMT ≥8 mm) are similar to those of controls. The quality of embryos transferred is an important predictor of assisted reproductive outcomes in patients treated successfully with exogenous estrogen.

Medicinal plants or Chinese materia medica (CMM) are now attracting worldwide attention as they have increasingly prominent advantages over chemical drugs in disease treatment and healthcare. Since the 1990s, World Health Organization (WHO) and International Organization for Standardization established the Technical Committee of Traditional Chinese Medicine (ISO/TC 249) have carried out the development of quality standards on medicinal plants or CMMs respectively, and a considerable number of monographs and international standards have been published. Since the two international organizations adhere to different principles, the standards they develop naturally have different emphasis. Driven by market demand and international trade, ISO mainly takes quality, efficacy and safety into consideration when developing standards, while WHO pays more attention to clinical practice, quality control and medication guidance. Up to now, there is a lack of comparative analysis on the records, background, principles, basic content, and main requirements of quality standards on medicinal plants or CMMs respectively published by WHO and ISO. Therefore, based on international standards of CMM developed by ISO/TC 249 platform and WHO Monographs on Selected Medicinal Plants, this paper systematically compares the purposes, selected principles, standard-developing process, basic content, and main quality requirements to summarize their similarities and differences, and find their merits, aiming to serve as a reference to the development of international standards for CMMs that helps them go global.
