Objective To explore the dispassed reason of Anhui provincial health senior titles evaluation.Methods The senior professional titles of the year 2009-2011 assessment results which were announced by the office of personnel titles of Anhui Provincial Health Department and the recorded results of the jury assessment were analyzed.Results In Anhui Province from 2009 to 2011 for three years,a total of 4 523 people participated in the senior title review by staff 3 039 people,by the proportion of 67.2%.2009,2010,2011 pass rate was 69.14%,66.85%,65.89%,respectively.1 484 people did not pass,and the rate was 32.8%.The main reason was the lack of reporting by professional and technical proficiency,non-standard format of application materials,assessment reporting material falsification.Conclusion Increasing the professional title appraisal policy propaganda,assessment focus which should be focused,the RAE quantitative score standards of health senior professional title,the establishment of a senior title of the series of health reporting assessment information system can improve this conditions.

Objective:To investigate the relationship between psychological factors and the serum level of estrogen in menopausal women with glossdynia.Methods:87 menopausal women with glossdynia and a randomly selected control group (n=82) were comprehensively investigated with the method of case-control study.The serum level of estradiol(E2) and follicle stimulating hormone(FSH) were measured.All cases with glossdynia and control were evaluated by the Self-Rating Annxiety Scale(SAS) and Self-Rating Depression Scale(SDS). 4.Risk factors of postoperatively prolonged mechanical ventilation after liver transplantation

Objective To analyze the risk factors of postoperatively prolonged mechanical ventilation (PMV) after liver transplantation.Method The clinical data of 117 patients who received liver transplantation were retrospectively reviewed.According to the duration of postoperative mechanical ventilation (<24 h or ≥24 h),the patients were divided into two groups.Commonly-used clinical and lab indexes before,during and after operations were analyzed by using single variance logistic regression analysis,and the screened indexes were analyzed by stepwise multiple variance logistic regression analysis.Result Forty-two patients (35.9%) were diagnosed with PMV after liver transplantation.The intensive care unit stay in the control group was (1.60 ± 1.17) days,shorter than in PMV proup (9.35 ± 10.61days).Sixty indexes were analyzed by univarite logistic regression,and 49 indexes showed statistically significant differences (P < 0.2).Multiple variance logistic regression analysis revealed that the levels of preoperative blood glucose,model for end-stage liver disease score (MELD),the blood loss volume during the operation and the levels of glutamic oxalacetic transaminase within 24 h after the operation showed significant difference.Conclusion Preoperative hyperglycemia,high MELD score,excessive intraoperative blood loss volume and the high level of glutamic oxalacetic transaminase after the operation are independent risk factors of PMV.
5.The expression and correlation between VEGF and TSP in oral submucous fibrosis

Objective:To study the expressions of vascular endothelial growth factor(VEGF), and thrombospondin (TSP) in oral submucous fibrosis(OSF), and to determine the relationship between VEGF and TSP, so as to investigate their roles in the pathogenesis of OSF. Methods:30 patients with OSF, including early (10 cases), middle (10 cases), late stage (10 cases) were studied. 5 healthy volunteers were chosen as control group. Buccal mucosa biopsies were taken in all samples. Expressions of VEGF and TSP were detected by immunohistochemistry. Results:The expression of VEGF increased in early stage compared with normal oral mucous, and decreased in middle and late stage. The expression of VEGF in early stage had statistical difference compared with control group, middle stage and late stage respectively(P<0.05).The expression of TSP upregulated in early and middle, and downregulated in late stage. There were no statistical significant differences among groups(P>0.05). The expressions of VEGF and TSP had negative correlation(r=-0.620,P<0.05). Conclusions: The abnormal expressions of VEGF and TSP may be a pathogenesis factor of OSF.

7.Effect of combining acupuncture and auricular point sticking on heart rate variability in patients with post-stroke depression

Objective: To investigate the effect of combining acupuncture and auricular point sticking on heart rate variability (HRV) in patients with post-stroke depression (PSD). Methods: A total of 80 cases with PSD were randomized into a treatment group and a control group. The control group was intervened by oral administration of paroxetine hydrochloride, whereas the treatment group received acupuncture plus auricular point sticking base on the same oral administration. The Hamilton depression rating scale (HAMD) and HRV were measured before and after treatment in both groups. Results: The individual and global scores of HAMD significantly dropped after 8 weeks of treatment in both groups (all P<0.05). In the treatment group, anxiety/somatization factor, sleep disturbance, hopelessness factor, cognition factor and global score were significantly different from those in the control group (all P<0.05). The 24 h standard deviation of all normal-to-normal R-R interval (SDNN), standard deviation of 5-minute average of normal R-R intervals (SDANN), root mean square of successive differences (RMSSD), percent of differences between adjacent normal R-R intervals >50 ms (PNN50) and high frequency (HF) were increased while low frequency (LF) and LF/HF decreased significantly after 8 weeks of treatment in both groups (P<0.05). All items in the treatment group were significantly different from those in the control group (all P<0.05). Conclusion: Combining acupuncture and auricular point sticking can enhance the conventional medical treatment for HRV in patients with PSD.
8.Alterations of Sirt1 and mTOR activity in gastrocnemius muscle of mice with sarcopenia

Objective To observe the alterations of Sirt1 and mTOR activity in gastrocnemius muscles of mice with sarcopenia. Methods Aged (26 months old) and young (6 months old) male C57BL/6J mice (n=16,respective-ly) were randomly arranged into 2 groups respectively, i. e. middle-aged control ( MC) , middle-aged suspension ( MS) , aged control( AC) and aged suspension( AS) . Aged mice were hindlimb suspended for two weeks to estab-lish a sarcopenic model. Kits and Western blot were used to detecte related factors and protein expression of Sirt1 and mTOR pathway. Results 1) the content of NAD+and the ratio of NAD+/NADH: AC group was significantly lower than MC group ( P<0.01 or P<0.05) , HLS group was significantly less than the control group ( P<0.01 or P<0.05) . 2) Sirt1 expression: AC group was significantly higher than MC group ( P<0.05) , and HLS group was significantly higher than that of the control group (P<0.05).3)Sirt1 downstream substrates (PGC-1α, FOXO1 and P53), NAMPT protein expression and Sirt1 activity: AC group was significantly lower than MC group ( P<0.05 or P<0.05 or P<0.01) , HLS group was significantly less than the control group ( P<0.05 or P<0.05 or P<0.05) . 4) phosphorylation of mTOR, p70S6K and 4E-BP1: AC group was significantly higher than MC group ( P<0.05) , HLS group was significantly less than the control group ( P<0.05) . Conclusions Sirt1 and mTOR are involved in the pathogenesis of mice with sarcopenia.

10.Diagnosis and treatment of metanephric adenoma

Objective To explore the clinical and histopathological features of metanephric adenoma (MA). MethodsClinical and pathological data of 10 cases of MA were analyzed retrospectively.There were 4 males and 6 females,aged from 33 to 65 years,with an average of 45 years.2 patients had flank pain,4 patients had gross hematuria,and 4 patients were found by physical examination.The average diameter of tumor was 4.5 cm (2.5 - 8.0 cm).All patients were diagnosed as renal tumor by CT scan.9 patients underwent radical nephrectomy and 1 patient underwent partial nephrectomy. Results Pathological examination found that the tumors are composed of densely packed small uniform cells with regular nuclei that formed a tubular or adenoid pattern.Mitotic figures were absent or rare.4 patients were diagnosed as MA,2 cases were diagnosed as low-grade malignant MA,and 4 cases were diagnosed as MA with malignant component (2 cases of adenocarcinoma,1 case of chromophobe cell carcinoma,and 1 case of well differentiated papillary adenocarcinoma),7 cases were followed up for 22 months ( 10 to 34 months) without recurrence or metastasis. Conclusions MA is very rare benign renal tumor originating from epithelium,and a few are malignant,and some may contain malignant ingredients.Nephron-sparing surgery and radical nephrectomy are eligible for the treatment of MA.Considering the uncertainty of the biological behavior and cellular origin of MA,a long-term follow-up is necessary.
