2.The relationship between different serum HBeAg levels and pathological stages of patients with chronic hepatitis B

Objective To investigate the relationship between serum HBeAg level and inflammation grade (G)/fibrosis stage (S) in the liver tissues of chronic hepatitis B (CHB) patients in the immune clearance phase (IC). Methods Both liver biopsy samples and serum samples were consecutively collected from CHB patients in Liver Center,First Affiliated Hospital,Fujian Medical University during March 2007 to June 2010.Electro-chemiluminescence and fluorogenic quantitative polymerase chain reaction (PCR) methods were used to determine HBeAg titer and hepatitis B virus (HBV) DNA level,respectively.The relationships between HBeAg titer and liver pathological stages were analyzed using Spearman rank correlation analysis.Receive operating characteristic (ROC) curve was used to evaluate the diagnostic value of HBeAg for liver pathological stages.Results Totally 249 patients with CHB were enrolled into this study.The serum HBeAg absorbances in patients with liver inflammation G1 to G4 were (2.93±2.85),(2.96±2.74),(2.69±2.67) and (2.30±2.41) lg s/co,respectively,while those in patients with liver fibrosis S1 to S4 were (2.99±2.74),(2.89±2.73),(2.58±2.55) and (2.32±2.44) lg s/co,respectively,which indicated that serum HBeAg titers were significant different in patients with different grading and staging of liver tissues (x2 =47.13,P<0.01; x2 =74.12,P<0.01).Spearman rank correlation analysis showed that serum HBeAg titer was negatively correlated with inflammation grades and fibrosis stages of liver tissues (r=-0.418 and-0.532,respectively; both P<0.01).ROC curve analysis revealed that the areas under the curve (AUC) were 0.74 (G≥≥3) and 0.73 (G≥4),and the HBeAg (s/co) cut-off values were 2.95 and 2.64 lg s/co,respectively.Similarly,ROC curve analysis revealed that the AUC were 0.80 (S≥3) and 0.77 S≥4),and the HBeAg cut-off values were 2.99 and 2.82 lg s/co,respectively.Conclusions The serum HBeAg titer is negatively correlated with the inflammation grades and fibrosis stages m liver tissues of CHB patients in IC phase.The level of HBeAg may be used as an adjunctive noninvasive marker to reflect the inflammation and fibrosis status in the liver.
3.The preliminary study of the relationship between ceruloplasmin levels and liver pathological stages in patients with chronic hepatitis B

Objective To investigate the relationship between levels of ceruloplasmin (CP) and inflammation grade,fibrosis stages in liver of patients with chronic hepatitis B (CHB),and to establish liver fibrosis non-invasive model and evaluate its diagnostic value for liver pathological stages.Methods Both liver biopsy samples and sera were collected from 148 consecutive CHB patients in Liver Center,First Affiliated Hospital,Fujian Medical University during January 2009 to June 2011.The relationships between CP and liver pathological stages were analyzed using Spearman rank correlation analysis.Receiver operator characteristic (ROC) curve was used to evaluate the diagnostic value of CP for liver pathological stages.The diagnostic values of relevant indicators were analyzed by Logistic regression.The liver pathology-predicting model was built and the diagnostic value of the model was analyzed by ROC curve.Results The mean values of CP in 148 CHB patients with liver inflammation grades of G1 to G4 were (212.5 ± 34.9),(205.5± 32.0),(201.4 ± 37.7) and (172.8 ± 20.4) mg/L,respectively,which were significantly different by ANOVA test (F=6.309,P<0.01).Similarly,the mean values of CP in patients with liver fibrosis stages of S1 to S4 were (217.4±32.3),(206.0±37.7),(194.2±29.8) and (179.7±30.4) mg/L,respectively,which were significantly different by ANOVA test (F =8.608,P < 0.01).Spearman rank correlation analysis showed that CP was negatively correlated with liver inflammation grades (r=-0.316,P<0.01) and fibrosis stages (r=-0.404,P<0.01).ROC curve analysis revealed that the area under the curves (AUC) were 0.71 (S≥2),0.70 (S≥3) and 0.72 (S=4).Multiple Logistic regression analysis showed that CP,α-fetoprotein,cholesterol,platelet and age were independent predictors for liver fibrosis.ROC curve analysis revealed that AUC were 0.84 in model-1 (S≥2),0.83 in model-2 (S≥3) and 0.87 in model-3 (S=4).The accuracy rates were 71.8%,80.3% and 79.2%,respectively.Conclusions The CP levels are negatively correlated with inflammation grades and fibrosis stages in the liver of CHB patients.CP could be an important non-invasive indicator for liver fibrosis and the model including CP can be used to predict liver fibrosis in CHB.
4.Hemorrhagic complications following abdominal paracentesis in patients with acute-on-chronic liver failure

Objective The aim of this prospective observational study was to analyze the prevalence and the predictive factors of hemorrhagic events after abdominal paracentesis in patients with acute-on-chronic liver failure (ACLF).Methods ACLF patients who received at least one episode of abdominal paracentesis were prospectively enrolled between January 2010 to December 2013. Prevalences of intraperitoneal and abdomen hemorrhage complications were examined. t test was performed for continuous variables and chi-square test was performed for categorical variables.Binary Logistic regression was used to analyze the risk factors of hemorrhage.Results A total of 525 abdominal paracenteses were carried out on 185 ACLF patients within a 4-year period,with 289 (55 .0%)for diagnostic purpose and 236 (45 .0%)for therapeutic purpose.A total of 16 (3.0%)hemorrhagic complications were identified, with 4 cases of abdominal wall hematomas and 12 cases of intraperitoneal hemorrhage.Patients were divided into hemorrhage group and non-hemorrhage group according to this complication.Age,gender, Child-Pugh score,volume of ascitic fluid removed,underlying cirrhosis,platelet count and thrombin time were not significantly different between two groups (all P > 0.05 ).Patients with bleeding events had lower fibrinogen levels and higher prothrombin time,international normalized ratio,activated partial thromboplastin time and model for end-stage liver disease score (all P <0.05).After adjustment of other factors,multivariate regression analysis indicated that low fibrinogen level was the only independent predictor of hemorrhagic complication (OR=0.105,95%CI :0.018-0.608,P =0.012).Conclusion Low fibrinogen level is the independent predictor of severe hemorrhagic complications following paracenteses in patients with ACLF.

The excipient processing is an essential part of traditional Chinese medicine processing, and understanding its scientific connotations is a critical scientific issue that urgently needs resolution. Building upon a foundation where the composition of traditional Chinese medicine substances is fundamentally clear, this paper applies the techniques and methods of chemoinformatics to the study of the excipient processing mechanism. Relevant information on traditional Chinese medicines processed with four kinds of excipients (wine, vinegar, salt and honey) was collected, including properties, taste, meridian tropism, chemical components, etc. Molecular descritors and skeletons corresponding to each chemical component were calculated using chemoinformatics to characterize the properties and structural features of the components. Objective To find out the association between the indicators(pulse concussion lung function test index) of bronchial hyperresponsiveness (BHR) with fractional concentration of exhaled nitric oxide (FeNO) at different control periods among preschool asthmatic children.Methods Totally 74 asthmatic children in the pediatric department of our hospital from April 2015 to February 2017 were enrolled in this study,and 25 children undergoing the lung function and FeNO examination served as the controls,aged 3-5 years old.The cases were divided into three groups according to the standard in 2016 version of the Prevention and Treament Guide of Children Bronchial Asthma:asthma control group(n =26),asthma non-control;group(n =48) and control group (n=25).All data of FeNO,resistance of the respiratory system at 5 Hz(R5),resistance of the respiratory system at 5 Hz (R20),difference of R5 and R20(R5-20),reactance area (AX),reactance of the respiratory system at 5 Hz (X5) and resonant frequency of reactance (Fres) were collected.The FeNO,pulse concussion lung function test value and their association were analyzed.Results (1) The FeNO value of asthma the non-control group was significantly higher than that of the asthma control group and the control group,which were 34.00 ± 18.17,20.23± 11.07 and 28.00± 17.30 respectively.The AX detection value of the asthma non-control group was significantly higher than that of the control group(37.29 ± 15.27 vs.30.17 ± 9.50,P<0.05).(2)R20 had weak correlation with FeNO in the control group(P<0.05),while R20 had no correlation with FeNO in the non-control group and control group (P>0.05).FeNO had no obvious correlation with R5,R520,AX,X5 and Fres in the asthma non-control group,asthma control group and control group(P>0.05).Conclusion In preschool children with asthma,FeNO can reflect the airway eosinophilic inflammation control,and does not reflect the airway hyperresponsiveness.Thereforeit ie needed to combined with FeNO and IOS indicators (airway hyperresponsiveness index AX,etc.),which can more precisely judge whether asthma being controlled.

8.Effect of Phenylephrin and Angiotensin Ⅱ on Reperfusion Arrhythmias in Isolated Rat Heart and Its Mechanism

Objective: Our aim was to investigate the effects of phenylephrine (PH) and angiotensin Ⅱ (AT) on reperfusion arrhythmias in the isolated rat heart and its mechanism. Methods: Isolated rat hearts were subjected to dual coronary perfusion. After 15 minutes of aerobic perfusion of both coronary beds, flow to the left coronary bed infused selectively into that bed. Ventricular tachycardia (VT) and fibrillation (VF) were analyzed 5 minutes after reperfusion. Results: The reperfusion arrhythmias was significantly exacerbated by PH; however, AT alone did not have the same effect. The selective Na+/H+ exchanger (NHE) inhibitor, HOE642, which was infused along with PH, reversed the proarrhythmic effects of PH. The specific protein kinase C (PKC) inhibitor GF109203X (GF) infused along with PH can only partially reverse the proarrhythmic effect of PH. The AT2 receptor inhibitor PD123319 (PD) alone increased the incidence of reperfusion-induced arrhythmias. However, no statistical significance showed. Combination of PD and AT significantly increased the incidence of reperfusion-induced arrhythmias. Conclusion: Activation of α1-adrenoceptor can exacerbate reperfusion-induced arrhythmias. The AT1 receptor activated by AT can exacerbate reperfusion-induced arrhythmias. The activation of AT2 receptor may reduce reperfusion-induced arrhythmias.
9.Effect of Botulinum Toxin Type A on Spastic Cerebral Palsy:Two Years Follow-up

Objective To investigate the long-term effect of Botulinum toxin type A (BTX-A) block on spasticity in cerebral palsy. Methods From June, 2005 to December, 2013, 51 children with spastic cerebral palsy accepted routine rehabilitation with (trial group, n=24) or without (control group, n=27) BTX-A block. They were assessed with modified Ashworth Scale (MAS), Physician Rating Scale (PRS) and Gross Motor Function Measure (GMFM) before treatment, and with PRS and GMFM two years after treatment. Results There was no significant difference between two groups in ages, body mass, and scores of MAS, PRS and GMFM before treatment (t<1.207,χ2=0.076, P>0.05). The scores of PRS and GMFM improved in both groups two years after treatment (t>2.217, P<0.05), and improved more in the trial group than in the control group (t>2.095, P<0.05). Conclusion The long-term effect of BTX-A block is positive. The rehabilitation training after block can promote their motor function recovery.
10.Effect of blood vessels at different locations and angle between blood vessel and sonic axis on HIFU peripheral ablation

Objective To investigate the effect of blood vessels at different locations and angles between blood vessel and sonic axis within the target area on HIFU peripheral ablation.Methods Totally 56 phantoms containing rabbit thoracic a orta were divided into group A (different vessel locations) and B (different vessel angles).According to the distance (d)between the blood vessel center and the deepest (top) layer of the peripheral scanning,group A was divided into A1 (d=0),A2 (d=10 mm),A3 (d=20 mm) subgroups.According to the angle (α) between the vessel long axis and the acoustic axis,group B was divided into B1 (α=0-5°),B2(α=60° 65°),B3 (α=90°-95°),B4 (α=115°-120°) subgroups.A4,B5 subgroups were set as the control groups.The vessel diameter and vessel wall thickness were measured.The lesion morphology was observed and the energy efficiency factor (EEF) was calculated.Results There was no significant difference in vessel diameter and vessel wall thickness in both group A and B (all P>0.05).There was no residual area in the target area of A1,B1 subgroup,while there was residual area in the target area of the other subgroups.EEF of group A in ascending order were A4,A1,A2 subgroup,and statistically significant differences in pairwise comparisons (all P<0.05) except for A1 and A3 subgroup (P>0.05).EEF of group B in ascending order were B5,B2,B3 subgroup,respectively,and statistically significant differences in pairwise comparisons (all P<0.05) except for B2 and B4,B5 and B1 subgroup (both P>0.05).Conclusion It is 10 mm that the distance between the blood vessel center and the deepest (top) layer of the pe ripheral scanning,and 90°-95° that the angle between the vessel long axis and the acoustic axis has the greatest influence on the ablation of the target area.
