1.Application research of TBL based on evidence-based medicine PICOS model in practice teaching of Neurology

Objective To explore the effect of TBL based on evidence-based medicine PICOS model in practice teaching of neurology. Methods Totally 47 medical undergraduates in our department were chosen and randomly divided into 2 groups during March 2016 to April 2016. 24 students in the trial group were taught with TBL based on PICOS, while other 23 students in the control group were only taught with conventional TBL method. After three times clinical practices, the test referring to disease pathogenesis, clinical manifestations, auxiliary examinations, diagnosis and treatments of related diseases was performed, and meanwhile, questionnaires were distributed to students in order to survey their satisfaction degree of teaching methods. All the evaluation results and scores of two groups were compared. Statistical data were analyzed by using t-test or Chi-square test with SPSS 17.0 software. Results The score of the theoretical test of trial group was significantly superior to the control group [(89.08±3.60) vs. (79.09±7.75), t=5.707, P=0.03 ]. Survey showed that in the experimental group , the number of students in the trial group who thought teaching method could help understand clinical thinking of neurology and could help integrate the theory into clinical practicewas significantly higher than that in the control group (P<0.05). Conclusion In the clinical teaching of neurology, the TBL based on PICOS model is more effective than conventional TBL method for medical undergraduates.

3.Effect of Botulinum Toxin-A Injection on Central Facial Palsy Post Stroke

Objective To explore the effect of Botulinum toxin-A (BTX-A) injected into the contralateral facial muscle on central facial palsy post stroke. Methods 30 stroke patients with moderate to severe central facial palsy were recruited (course of 3~10 months). They were divided into control group (n=15) and treatment group (n=15), who accepted facial training and BTX-A injection in addition, respectively.The bilateral deference of distance from angulus oris to the midline of the teeth(D1) and from the paropia to the angulus oris (D2) were measured before and 4 weeks after injection. Results The D1 and D2 both at resting and movement all decreased after injection in the treatment group, and decreased more than those in the control group. Conclusion BTX-A injection can further correct central facial palsy post stroke.
4.Expressions of Twist and VEGF in epithelial ovarian cancer

Objective To investigate the role of Twist and vascular endothelial growth factor (VEGF)in the development of epithelial ovarian cancer and their relationship with clinical pathological features.Methods The expressions of Twist and VEGF were detected by immunohistochemical staining (SP)method in 80 cases of epithelial ovarian cancer and 10 paired cases of normal ovarian tissue.Results In the 80 cases of ovarian cancer tissues,the positive expression rate of Twist and VEGF in epithelial ovarian cancer was 85% and 86.25%,respectively,which were remarkably higher than those in normal ovarian tissues (P <0.01).The expressions of Twist and VEGF were closely associated with clinical stages,pathological grades and lymph node metastasis,but not correlated with age or pathological patterns (P >0.05).There was a significant correlation between the expressions of Twist and VEGF in epithelial ovarian cancer (r =0.646,P <0.01).Conclusion The high expressions of Twist and VEGF may play a potential role in the occurrence,development and invasion process of epithelial ovarian cancer.
5.Association between distributions of component genotype of three sites of SCN1B gene and epilepsy

Objective To study the single nucleotide polymorphisms (SNP) in 3 sites allele (T189M, R85H, C121W) of SCN1B and the association between gene distribution and epilepsy. Methods All 330 blood samples of refractory (80 cases), non-refractory (100 cases) epilepsy patients and healthy people (150 cases) were collected. Genomic DNA of leucocyte was extracted. SNPs of three sites allele of SCN1B were tested by allele-specific primer-polymerase chain reaction (ASP-PCR).Data were analyzed by SAS 8.1 statistical software. Results Epilepsy group and healthy group had significantly statistical difference in composition of 3 sites allele on single site genotype (x~2=11.19, 11.14 and 6.50, all P < 0.05).There was no statistical significance between refractory and non-refractory epilepsy group. On gene combination, in 27 different combinations of polymorphism, mutation frequency in 3 sites (CT + AG + CG) was highest in epilepsy group (18.40%).The next was one site in CT + GG + CC (16.80%).In healthy group, frequency of non-variant in CC + GG + CC was highest (16.67%), and the next was 2 sites in CT+ AG+CC (13.73%).Thirty-five cases in epilepsy group (28.80%) had 3 sites mutation compared with 10 cases in healthy group (9.71%), and their difference had statistical significance (x~2=12.54, P<0.05).Eighteen cases in refractory epilepsy group (30.51%) had 3 sites mutation compared with 21 cases in non-refractory epilepsy group (28.77%), and the difference had no statistical significance. Fifty cases in epilepsy group (40.00%) had 2 sites mutation compared with 41 cases in healthy group (40.20%), and there was no statistical significance between them; 25 cases in refractory epilepsy group (42.37%) had 2 sites mutation compared with 21 cases in non-refractory epilepsy group (28.71%), and their difference had no statistical significance. Conclusions Mutation, especially multisite mutation of SCN1B is relatively likely to cause epilepsy in human. Gene distribution and combination of three sites allele of SCN1B in refractory epilepsy is close to that in non-refractory epilepsy.
6.Preparation of novel gypenosides long-circulating liposomes consisted by sphingomyelin and beta-sitosterol modified by PEG.

OBJECTIVETo explore the feasibility of preparing novel gypenosides long-circulating liposomes with PEG grafted on beta-sitosterol (PEG-Sito).
METHODSuccinicanhydride was adopted to connect beta-sitosterol and PEG 2000. Sphingomyelin and PEG-Sito was used as material to prepare gypenosides long-circulating liposomes by using ethanol injection method. Encapsulation efficiency was determined by using protamine precipitation method. H-NMR was used to verify the synthesis of PEG-Sito, the novel gypenosides long-circulating liposomes were characterized by particle size, zeta potential and atomic force microscope.
RESULTThe synthesis of PEG-Sito was verified by 1H-NMR. Encapsulation efficiency of long-circulating liposomes prepared by ethanol injection method was 74.3%, particle size was 288.1 nm, zeta potential was -20.25 mV, the morphology were round observed by AFM.
CONCLUSIONThe novel gypenosides long-circulating liposomes prepared with PEG-Sito was feasible, it had a high encapsulation efficiency and good morphology.

7.Effects of atorvastatin on experimental autoimmune encephalomyelitis

Objective To investigate the effects of atorvastatin on the experimental autoimmune encephalomyelitis(EAE)and the underlying mechanism of immunoregulation.Method The Wistar rats were used to establish EAE model.After oral administration of 2, 8 mg? kg~(1)?d~(1)of atorvastatin, the rats were examined for the development of neurological signs, changes of histopathology and the expression of IL-4 and MMP-9.Result Though high dose treatment with atorvastatin, the frequency of EAE attacks degreased from 76.67% to 33.33%(P=0.008);the extent of inflammation degreased from 3.2?1.1 to 1.3?0.4(P=0.01);and the number of MMP-9 positive cells degreased from 37?7 to 26?5(P= 0.001), the expression of IL-4 could be increased from(0.35?0.12)ng/ml to(0.68?0.23)ng/ml (P=0.05).Conclusion Atorvastatin can reduce the inflammation and produce the recover of the neurological harm because of the changes of MMP-9 and IL-4.

10.Visual acuity outcome of cataract surgery in patients with wet age-related macular degeneration

AIM: To evaluate visual acuity outcomes after phacoemulsification and intraocular lens implantation in patients with wet age-related macular degeneration (wAMD).METHODS: We reviewed the medical documents of the patients who underwent phacoemulsification and intraocular lens implantation surgery during June 2013 and January 2016.Totally 61 eyes of 48 patients with wAMD in the stable stage were recruited.The pre-and post-operative vision of selected cases were recorded and compared.RESULTS: After phacoemulsification and intralocular lens implantation, visual acuity changes were as follows: 49 eyes improved, 11 eyes retained, and 1 eye deteriorated.Visual acuity improvement after cataract surger were statistically significant (P<0.001).Visual acuity improvement was not related to age.CONCLUSION: Visual acuity improved in patients with wet AMD after phacoemulcification and intraocular lens implantation.
