1.Effects of CD46-siRNA silencing on proliferation and metastasis of MCF-7 human breast cancer cell

Objective To establish and observe the effects of CD46-siRNA on proliferation,in-vasion and metastasis of MCF-7 human breast cancer cell.Methods The pEGFP-C1-CD46 siRNA was established and transfected into the MCF-7 cell.Changes in CD46 mRNA and protein expression were test-ed and variation of invasion and migration capacity in MCF-7 /CD46-siRNA cell was also observed.Results Compared with the control group and scrambled group,the expression of CD46 mRNA was significantly decreased(P <0.05)and CD46 protein expression was also suppressed in the MCF-7 cells of the silence group.Proliferation activity of MCF-7 cell in the control group and scrambled group was gradually in-creased with the time,but this phenomenon was not obvious in the silence group.At the 3th and 4th day af-ter transcription,differences in proliferation activity were significant among the groups,and activity in the silent group was significantly lower than the other two groups(P <0.05).Counts in invasion and migration of MCF-7 cell in the control group and scrambled group were significantly higher than the silent group (P <0.05 ).Conclusion The expression vector of pEGFP-C1-CD46 siRNA can suppress the CD46 mRNA and protein expression in MCF-7 human breast cancer cell and reduce the capacity in proliferation activity,invasion and migration.
2.The research of the brucea javanica oil emulsion combined with the whole radiotherapy in non-small cell lung cancer patients with brain metastasis

Objective To compare the efficacy between the brucea javanica oil emulsion combined with the whole radiotherapy group (treatment group) and only radiotherapy group (control group) in non-small cell lung cancer patients with brain metastases.Methods The clinical data of 53 non-small cell lung cancer patients with brain metastasis were retrospectively reviewed,including 27 cases in treatment group and 26 cases in control group.Results The remission rates were 77.8 % (21/27) and 69.2 % (18/26) respectively in treatment group and control group.The improvement rates of Kamofsky in the treatment group and control group were 66.7 % (18/27) and 30.8 % (8/26),the stable rates were 22.2 % (6/27) and 30.8 % (8/26),the decrease rates were 11.1% (3/27) and 38.5 % (10/26).The remission rates of clinical sympotoms were 92.6 % (25/27) and 69.2 % (18/26) respectively in treatment group and control group.The difference of remission rate between two groups has no statistic significance (P > 0.05),while the difference of the quality of life and clinical sympotoms between two groups were significant respectively (P < 0.05).Conclusion The brucea javanica oil emulsion combined with the whole radiotherapy can relieve clinical sympotoms and improve the quality of life of patients,and may play a role on improving the effect of radiotherapy.

4.Recent effects of IMRT plus gemcitabine for locally advanced non-small-cell lung cancer

Objective To evaluate the recent effects of IMRT plus gemcitabine for locally advanced non-small-cell lung cancer.Methods The data of forty-five patients were retrospectively reviewed.The treatment group consisted of 23 cases,treated with sequential same chemotherapy regimens and IMRT,the control group consisted of 22 cases,receiving same chemotherapy regimens after two cycles of gemcitabine and cisplatin induction chemotherapy.Resuls The remission rates were 65.2 % (15/23) and 31.8 % (7/22) respectively in treatment and control group.The 1 year survival rates were 66.4 % and 45.0 % respectively in treatment and control group.The remission rates and 1 year survival rates were significant different between two groups (both P < 0.05).Conclusion The short-term effect of sequential chemotherapy and IMRT is better than chemotherapy and the adverse events can be better tolerated.
6.Clinical effect of sufentanil and levobupivacaine on postoperative analgesia in pediatric caudal block anesthesia

Objective To invetigate the effect and safety of sufentanil mixed levobupivacaine on postoperative analgesia in pediatric caudal block anesthesia.Method Sixty pediatric patients (2 ~ 6 years old) who were undergoing elective abdominal surgery,such as repair hernia of high ligation,were randomly divided into three groups with 20 cases each.after intravenous induction,0.25% levobupivacaine was injected in sacrum tube in group Ⅰ,0.5 μg/ml sufentanil mixed 0.25% levobupivacaine and 1.0 μg/ml sufentanil mixed 0.25% levobupivacaine were injected in sacrum tube in group Ⅱ and group Ⅲ respectively.The analgesia effect,the analgesia time,recover time and adverse reaction were observed and recorded 2,4,8,12,16,24 hours after the surgery.Results The analgesia effect in group Ⅱ、Ⅲ were significantly better than the group Ⅰ when 4、8、12 hours after the operation(P <0.05),and the analgesia effect in groupⅢ were significantly better than the group Ⅱ when 8 hours after the operation (P < 0.05).There were no significant differences in three groups when 2、16、24 hours after the operation(P >0.05),the analgesia time in group Ⅱ、Ⅲ were significantly longer than the group Ⅰ (P < 0.05),and the analgesia time in group Ⅲ were significantly longer than the group Ⅱ (P < 0.05).There were no differences in the recovery time of three groups (P > 0.05).There were no adverse reactions in three groups.Conclusions 0.5 μg/ml and 1.0 μg/ml sufentanil mixed 0.25% levobupivacaine may be used on postoperative analgesia in pediatric caudal block anesthesia safely and analgesia effect and time were more better and longer than 0.25% levobupivacaine singly.The analgesia effect in group with 1.0μg/ml sufentanil mixed 0.25% levobupivacaine was the best in three groups with the fewest side effects.

Objective To investigate the characteristic of intravascular catheter-related infections (CRI) in Peking University Third Hospital in 2009. Methods The clinical and microbiological features of CRI were analyzed retrospectively. Results Two hundred and nineteen catheters were examined. Twenty-nine CRI cases were confirmed with colonization, 7 were insertion site infection and 6 were catheter-related bloodstream infections (CRBSI) by quantitative catheter culture method, but no CRBSI was diagnosed by differential time to positivity (DTTP) method. The incidence rates of CRBSI in intensive care units (ICU) were 0. 99- 3.03 per 1000 catheter-days.Among the colonized strains, 51.72 % were Gram-positive cocci, 20.69 % were non-fermenting bacilli (NFB), 13. 79% were Enterobacteriaceae and 10. 34% were Candida. One neonatal CRBSI was caused by Leuconostoc mesenteroides. Most of bacterial isolates were drug resistant. Seventy-seven percent of adult patients were older than 60 years and most of them had chronic underlying diseases.All of the 9 neonate patients were preterm neonate and 7 of them were extremely low birth weight infants. Conclusion There is a certain amount of CRI occurred in Peking University Third Hospital in 2009 and the intravascular catheter management should be enhanced.
9.The impact of aerobic combined strength and balance exercise on cognitive function in patients with cognitive impairment no dementia

Objective To evaluate the effects of aerobic combined strength and balance exercise on cognitive function and satisfaction in patients with cognitive impairment no dementia. Methods Four hundred and twenty patients with cognitive impairment no dementia were divided into control group (200 cases) and experimental group (220 cases) by random digits table method. The control group received general health education and rehabilitation training. The experimental group received aerobic combined strength and balance exercise. The patients were assessed with Montreal Cognitive Assessment (MoCA) to evaluate their cognition before training, as well as after training. And the patients′satisfaction with the questionnaire was evaluated. Results The total scores of MoCA, scores of name, attention, language, memory and directionafter training were (23.47 ±2.38), (2.77 ±0.42), (5.09 ±0.86), (2.50 ±0.65), (3.42 ±0.68), (5.03 ± 0.10) points in the experimental group, and (21.20±2.55), (2.31±0.76), (4.71±1.10), (2.35±0.70), (2.23±0.81), (4.48±0.96) points in the control group, and there were significant differences, P<0.05. There were no significant differences in the space and abstract thinking between experimental group and control group:(3.61±0.91) points vs. (3.45±1.09) points, (1.83±0.76 ) points vs. (1.72±0.52) points, P>0.05. The scores of satisfaction in the experimental group was significantly higher than that in the control group:(49.33 ±1.57) points vs. ( 48.20 ±2.14) points, P<0.01. Conclusion The application of aerobic combined strength and balance exercise could improve the cognitive function and patients′satisfaction.
10.Pathological observation of degenerative aortic valve disease

Objective To investigate the normal aortic valve histological manifestation and histopathological changes of degenerative aortic valve.Methods In Tianjin Chest Hospital,there were patients due to aortic valve disease undergoing aortic valve replacement surgery in the Department of Cardiovascular Surgery,and in 22 cases,their pathological aorta tricuspid valves were successfully excised from May 2009 to May 2010.The aortic valve tissues were collected,and according to hematoxylin and eosin (HE) staining of the degenerative calcified aortic valves with stenosis,they were divided into pathological valvular calcification locus group and non calcification locus group.Six morphologically normal aortic valve tissues were taken as a control group.The situation of collagen fibers,elastic fibers and collagen content in normal and disorder valves were analyzed by using of IDA-2000 high definition color medical image analysis system.Results With the naked eye,the thickness of the valve leaves with stenosis was increased,and at the valve leaf close to the side of arota,calcified foci were produced.Under light microscope,inflammatory changes were found in the aortic valve endothelium,in the tubular wall,there were aggregations of newly formed capillary with relatively thin wall,cholesterol crystals,cells similar to foam cells and diffuse and nodular calcification commonly seen,Compared with the normal valve,the ratio between collagenous and elastic fibers no matter at calcified or non-calcified region was greater in pathological valve [(3.19-± 1.01)%,(2.62-± 0.34)% vs.(1.89-± 0.29)%,both P < 0.05],collagen content (ratio =collagen/valve area) was also higher [(65.17 ± 11.48)%,(39.25-± 11.15)% vs.(22.99 ± 4.78)%,P < 0.05] in pathological valve,and the changes in calcified region were more significant than those in non-calcified region (both P < 0.05).Conclusions Proliferation of collagen and degradation of elastic fibers are obvious in degenerative aortic valve,and the changes are more prominent at calcified region.
