1.Clinical study of phacoemulsification and goniosynechialysis for primary angle -closure glaucoma

AlM:To discuss the clinical effect and safety of the combined surgeries of phacoemulsification and goniosynechialysis for patients with primary angle -closure glaucoma ( PACG) .METHODS:Eighty-three eyes of 83 patients with PACG and cataract were ranomized into two groups. Group A:39 patients with 39 eyes of angle-closure <180.;Group B:44 patients with 44 eyes angle - closure ≥180.. The changes of visual acuity, anterior chamber depth ( ACD) , anterior chamber angle and lOP after operation were measured. The follow-up period was for 1a.RESULTS: The visual acuity was improved significantly in both group after operation. No significantly difference in ACD, anterior chamber angle after operation in two groups (P>0. 05). The lOP in group A was lower than that in group B 3mo after operation, there was statistical significance (P<0. 05).CONCLUSlON:Phacoemulsification and goniosynechialysis can deepen ACD significantly. This method make the anterior chamber angle open, can reduce lOP and improve the patients’ visual acuity, especially in patients with PACG. lt is a safe and effective approach to patients with PACG and cataract.
2.Chemical constituents in butanol extraction from stems and leaves of Arachis hypogaea

Objective To investigate the chemical constituents in the stems and leaves of Arachis hypogaea.Methods The isolated compounds Ⅰ—Ⅳ were obtained by the combination of silica gel,ODS-18,and Sephadex LH-20 column chromatographies.Structural elucidation was conducted by the moedrn spectral method.Results Seventeen compounds were isolated and identified from the water extraction,and there were four norsesquiterpene compounds from butanol extraction.They are arachiside A(Ⅰ),cucubalactone(Ⅱ),roseoside(Ⅲ),and citroside A(Ⅳ).Conclusion Compound Ⅰ is a new compound named arachiside A and compounds Ⅱ—Ⅳ are isolated from the plants of Arachis Linn.for the first time.
3.Chemical constituents in leaves of Piper laetispicum

Objective To study the chemical constituents in the leaves of Piper laetispicum.Methods Compounds were separated and purified by silica gel column and macroporous resin.Their structures were identified on the basis of spectral methods.Results Sixteen compounds were isolated and identified from the leaves of P.laetispicum:(2E,4E)-N-isobutyl-11-phenylundecadienamide(Ⅰ),(2E,4E)-N-isobutyl-15-phenylpentadecadienamide(Ⅱ),laetispicine(Ⅲ),(2E,4E)-N-isobutyl-7-(3',4'-methylene-dioxyphenyl)heptadienamide(Ⅳ),brachystamide A(Ⅴ),dihydropipercide(Ⅵ),pipermacramide A [(2E,4E)-N-isobutyleicosadienamide,Ⅶ],(+)-spathulenol(Ⅷ),d-sesamin(Ⅸ),piperitol(Ⅹ),vitexin(Ⅺ),vitexin-2″-O-?-D-glucoside(ⅩⅡ),N-p-coumaroyltyramine(ⅩⅢ),?-sitosterol(ⅩⅣ),stigmasterol(ⅩⅤ),and 1-dotriacontanol(ⅩⅥ).Conclusion Compounds Ⅰ and Ⅱ are isolated for the first time as natural substance,compounds Ⅴ,Ⅵ,and Ⅷ are first isolated from P.laetispicum,compounds Ⅺ,ⅩⅡ,and ⅩⅥ are obtained from the plants of Piper L.for the first time.
4.Determination of six catechins in Xinnaojian Capsules (Tablets) by QAMS

AIM To establish a quantitative analysis of multi-components by single marker(QAMS) method for the content determination of six catechins in Xinnaojian Capsules (Tablets) (tea extract).METHODS The analysis of 50% methanol extract of this drug was performed on a 35 ℃ thermostatic Shimadzu Wonda Cract ODS-2 column (4.6 mm ×250 mm,5 μm),with the mobile phase comprising of 0.5% acetic acid (A)-acetonitrile (B) flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelength was set at 280 nm.With epigallocatechin gallate as an internal standard,the relative correction factors of epigallocatechin,catechin,epicatechin,gallocatechin gallate and epicatechin gallate were calculated,from which the content determination was made.RESULTS Six constituents showed good linear relationships within their own ranges (r ≥ 0.999 8),whose average recoveries were 96.00%-98.47% with the RSDs of 2.09%-2.91%.The results obtained by QAMS method approximated those obtained by external standard method.CONCLUSION This simple and reliable method can be used for the quality control of Xinnaojian Capsules (Tablets).
5.Thyroid lymphoma progressed with Hashimoto's thyroiditis diagnosed by core-needle biopsy : case report and literature review

Objective To explore the significance of core-needle biopsy ( CNB) in the diagnosis of thyroid lymphoma. Methods A case of Hashimoto's thyroiditis for several decades, showed a thickening of the neck for several months, and no abnormalities were found by the method of fine-needle aspiration( FNA), then we performed CNB and flow cytology. Results Thyroid lymphoma was finally diagnosed through CNB and flow cytology. Conclusion Thyroid lymphoma should be considered when the neck became thickened in short time in patients with chronic Hashimoto's thyroiditis. Core-needle biopsy accompanied with flow cytometry and immunohistochemistry analysis should be suggested as routine diagnostic method especilly when fine-needle aspiration was negative.
6.Safety and efficacy of Tirofiban in patients with acute coronary syndrome

A total of 159 patients with acute coronary syndrome(ACS)were enrolled from December 2006 to June 2008 and divided into the percutaneous coronary intervention(PCI)group and the internal medicine treatment group.The participants in the two groups were further assigned to the Tirofiban or the placebo control group.The change in electrocardiograph within 48 hours,major adverse cardiac events (MACE)during hospital stay and 30 days' follow-up,and bleeding were compared between the sub-groups.As a result,in comparison with the placebo control groups,the Tirofiban sub-groups showed significant improvement in electrocardiography(P<0.01).In the internal medicine treatment group,the rate of MACE during 30 days' follow-up was significantly decreased in patients treated with Tirofiban(P<0.05),although no significant difference in bleeding rate was found.Our data suggest that Tirofiban may be safe and effective in the treatment of ACS.
7.Echocardiographic diagnosis of Berry syndrome

Objective To explore the value of transthoracic echocardiography in diagnosis of Berry syndrome and analyze the sonographic typing.Methods The echocardiographic results of 6 Berry patients were reviewed retrospectively.Their sonographic typing were also analyzed connected with the reports in the literature.Results The ultrasound diagnosis of 5cases were confirmed by surgery(3)or angiography(3) and the remain one infant was died 14 days after the echocardiographic procedure.The malformations of Berry syndrome consisted of the distal aorto-pulmonary septal defect(APSD),aortic origin of the right pulmonary artery(RPA),interruption of the aortic arch(5 cases,type A with patent ductus arteriosus)or coarctation of the aortic isthmus(1 case)with intact ventricular septum.In 6 cases,there were 2 sonographic types:(1)Type A(3 cases),the aorto-pulmonary septum was almost absent and the left and right pulmonary arteries remained widely separated,the blood of the RPA came almost from the aorta,the aortic arch was interrupted.(2)Type B(3 cases),the left and right pulmonary arteries remained adjacent and the bifurcation straddled the distal APSD,the blood of the RPA came from the aorta and the pulmonary trunk separately,the aortic arch was coarctated or interrupted. Conclusions The transthoracic echocardiography can be used to evaluate all containing malformations of Berry syndrome and it may be the first diagnostic choice.
8.Initial exploration of occlusive devices of ventricular septal defect by stereo 3-dimensional echocardiography

Objective To accurately investigate the characteristic and effective of occlusive devices in ventricular septal of defect (VSD) by using stereo 3D echocardiography. Methods Six patients with VSD were included who were undergone the defect closing operation with Amplatzer occluder device. Stereo 3D echocardiography was performed to show 3DE imaging appearance after the operative procedure. Results Stereo 3D echocardiography were performed after the procedure of implanting occluder device. It was clear to show the location of the occluder and the space relationship of each parts. Conclusions The appearance,location and space relationship of occluder could be seen clearly by stereo 3D echocardiography after implanting occluder device. Compare with 2D, real-time 3D echocardiography, stereo 3D echocardiography could provide more veracious information about heart structures and the therapeutic devices for clinical use.
9.Diagnosis of the criss-cross heart by echocardiography

Objective To explore the value of echocardiography in the diagnosis of the criss-cross heart. Methods The echocardiographic results of 6 patients with the criss-cross heart were reviewed retrospectively. The echocardiographic characteristics were analyzed and compared with the surgery results in 4 operated cases. Results The 6 cases were interpreted as representing a criss-cross heart with solitus atria,D-loop ventricles and concordant atrioventricular connections. The ventriculo-arterial alignments of 5 cases were abnormal including double outlet right ventricle with anterior aorta in 2 and transposition of the great arteries in 3 while 1 patient had concordant connection. The following principal characteristics were tilting the transducer from posterior to anterior could demonstrate the connection of the left-sided left atrium and the right-sided left ventricle through mitral valve at first. The more anterior angulation of the transducer then showed the right-sided right atrium was connected to the left-sided right ventricle through tricuspid imaging displayed the two atrial outflows crossed each other without mixing at atrioventricular valve level.right ventricle often occured. The echocardiographic diagnosis of the criss-cross heart and its associated cardiac abnormalities were confirmed by surgery in 4 cases except 1 persistent left superior vena cava was missed. Conclusions The invisibility of a standard 4-chamber view in any cut was very characteristic in the echocardiographic diagnosis of the criss-cross heart. The definitive appearance was the separate display of the two ventricular inlets and the crossed atrioventricular connections with each atrium emptying into the contralateral ventricle by continuous subxiphoid or apical scanning. The transthoracic echocardiography can diagnose this rare heart disease and associated cardiac abnormalities accurately.
10.Primary study on resting myocardial perfusion in type 2 diabetes mellitus patients with real-time myocardial contrast echocardiography

Objective To explore the characteristics of resting myocardial perfusion of type 2 diabetes mellitus(T2DM) by real-time myocardial contrast echocardiography(RT-MCE). Methods Thirteen patients with T2DM and 12 in control were enrolled in the study. RT-MCE was performed using a continuous infusion of SonoVue with vena mediana in elbow. Images of left ventricle filled with contrast were acquired from apical 4-, 2-chamber and long axis views with real-time myocardial contrast mode of GE Vivid 7 Dimension system. All above images were captured in continuous 3 cardiac cycles before "flash" ,and then 15 cardiac cycles after "flash". All clips were stored for off-line analysis. Results In quiescent condition, there were significant decrease on indices of A,k and A × k in patients compared with control group (A:6.46 ±1.60 vs 6.81 ± 1.53, P <0.05;k:1.04 ± 0.39 vs 1.28 ± 0.31*, P <0.01;A× k:6.55 ± 2.72 vs 8.78 ±3.16, P < 0.01, respectively). Conclusions RT-MCE could evaluate the early change of myocardial perfusion in T2DM patients and offers important reference for clinical diagnosis and treatment.
