1.Expression of MDR1 and BAALC genes and its relationship with prognosis in patients with newly diagnosed acute myeloid leukemia

Objective To investigate the expression of multidrug resistance protein 1 (MDR1) and brain and acute leukemia cytoplasm (BAALC) genes and its relationship with prognosis in patients with newly diagnosed acute myeloid leukemia (AML).Methods A total of 100 adults with newly diagnosed AML were enrolled in the study.All patients were treated with anthracyclines induced chemotherapy combined with cytarabine.The expression levels of MDR1 and BAALC genes were detected,and the relationships with the clinical characteristics of patients with AML,genetic type,therapeutic effect and prognosis were analyzed.Results There was no significant differences in white blood cells,hemoglobin and platelet between patients with high and low expression of MDR1 gene and BAALC gene (P>0.05).There were significant differences in the expression level of MDR1 gene between patients with different chromosome risk stratification (P<0.05) but there was no significant difference in the expression level of BAALC gene (P>0.05).After chemotherapy intervention,CR of patients with high expression of MDR1 gene was significantly lower than patients with low expression of MDR1 gene (60.78% vs.87.76%)(P<0.05),CR of patients with high expression of BAALC gene was slightly lower than patients with low expression of MDR1 gene (67.31% vs 81.25%)(P>0.05).Comparison of median overall survival (OS) and overall survival (OS) in patients with high and low expression of MDR1 gene.The difference was statistically significant (P<0.05).Comparison of median overall survival (OS) and overall survival (OS) in patients with high and low expression of BAALC gene.There was no significant difference(P>0.05).Conclusion Compared with the expression of BAALC gene,the value of expression of MDR1 gene is higher in evaluating the curative effect in patients with newly diagnosed AML.Combined detection of MDR1 and BAALC genes can improve the accuracy in predicting the prognosis of patients.
2.Possible mechanism and significance of rare p53 mutation in Epstein-Barr virus-associated gastric cancer

In 2014, The Cancer Genome Atlas firstly classified gastric cancer into four types according to genotype. Epstein-Barr virus (EBV) positive gastric cancer or EBV-associated gastric cancer (EBVaGC) is attracting attention because it is a possibly suitable group for immunotherapy. Among the mutations observed in tumors, such as gastric cancer, p53 mutations are the most frequent. In particular, it occurs more frequently in EBVaGC than in EBV-negative gastric cancer (EBVnGC). Meanwhile, EBV infection is considered as an early event of tumorigenesis. The interactions between wild-type p53 proteins and BZLF1 (Z) proteins are essential in maintaining the latent state of EBV infection and promoting early replication. In the latter stages of replication, wild-type p53 proteins are degraded through the ubiquitination of some viral molecules. These findings may indicate the importance of wild-type p53 genes in EBVaGC formation. Inflammatory responses induced by EBV infection, tumor with a large number of lymphocyte infiltration, genome high mutation, and PD-L1 amplification make it possible to become the appropriate group of immunotherapy, which also illustrate that the important role of immune microenvironment during tumor progression. In EBVnGC, extremely high levels of p53 mutation were observed because of several associated factors, and the p53 protein encoded by the mutant p53 gene lost its antitumor function after tumorigenesis. In this review, the possible mechanisms of rare p53 mutation in EBVaGC are summarized.
3.Traditional Chinese Medical Syndrome Study on Diabetic Nephropathy

Objective To further discuss the characteristic of diabetic nephropahty,analyze its cause and pathogenesis,and make clear the relation between symptoms and syndrome,and to provide proof for prevention and therapy of this disease.Methods Records of 200 diabetic nephropathy patients treated clinically were collected and SPSS(Statistical Package for Social Science)12.0 was applied for analyzing.According to different conditions of the renal function,We divide the cases into two groups to study the relations between symptoms and syndrome.Results 1.The common symptoms of diabetic nephropathy are heavy body and drowsiness(74%),tired and debilitation(70%),and deficient breath and unwilling to speak(64%).2.The main syndromes of diabetic nephropathy are syndrome of deficiency of Qi、syndrome of yin asthenia,syndrome of dampness-heat blocking in the middle,and syndrome of blood stasis.Conclusion According to the research,the highest frequent appearing syndromes of deficiency were syndrome of Qi deficiency and syndrome of Yin deficiency,while the syndrome of dampness-heat blocking in the middle ranked No.1 in sthema syndromes and then caine the syndrome of blood stasis.However,the syndrome of blood stasis was the most severe,and this syndrome existed from the beginning to the end of this disease.
4.Risk Factors and Pathogens Distribution of Urinary Tract Infection in Patients with Spinal Cord Injury

Objective To analyze the risk factors and distribution of pathogenic bacteria of urinary tract infection (UTI) patients with spi-nal cord injury (SCI). Methods A total of 452 patients with SCI collected from December, 2015 to June, 2016 were retrospectively analyzed. Results 104 cases (23.1%) were diagnosed as UTI. 119 strains were identified, including 96 strains of Gram-negative bacteria (80.7%) and 22 strains of Gram-positive bacteria (19.3%). Female, invasive operation, depressed lower limb muscle strength, neurogenic bladder and pre-vention of antibacterial agents use were considered to be the main risk infection factors. The invasive operation included indwelling urethral catheterization, intermittent catheterization and other transurethral operation. The incidence rate of UTI in patients with SCI who also accept-ed indwelling catheter was 100.0%. Conclusion Gram-negative bacteria is the dominated pathogenic bacteria in SCI patients with UTI. It should be noticed the characteristics of pathogen, which both may have the high drug resistance rate and high separation rate, to select exact-ly antimicrobial agents to control the spread and drug resistance of pathogenic bacteria. In addition, patients with underlying diseases, inva-sive operation and lower strength should be monitored targeted.

Objective To analyse change of neuroendocrine hormone and cardiac ultra ejection force in patients with heart failure before and after treatment with Huangqi Decoction.Methods 100 patients diagnosed with chronic heart failure were collected.According to the different treatment regimens divided into two groups ( control group: conventional western medicine; traditional Chinese medicine group: conventional western medicine treatment +Astragalus Decoction orally for 2 weeks) , before and after treatment,nerve endocrine hormone using echocardiography evaluation index and ejection force were detected in two groups, and compare the results of data.Results Compared with control group, Chinese medicine group with better anti heart failure treatment, performance: the plasma calcitonin gene related peptide (CGRP), atrial natriuretic peptide (ANP), C type natriuretic peptide ( CNP ) , neuropeptide Y ( NPY ) levels were significantly lower ( P<0.05 ) . Capacity index significantly improved cardiac function (P<0.05).Left atrial diameter, left atrial maximum, minimum volume decreased significantly (P<0.05).Ejection force of left atrium and left ventricular increased significantly ( P<0.05 ) .The results were statistically significant.Conclusion Huangqi decoction can inhibit the secretion of CGRP, ANP, CNP,NPY and other neuroendocrine hormones in patients with heart failure, reduce cardiac load, increase cardiac ejection force.

9.Antiarrhythmic effect of TJ0711.

To study the antiarrhythmic effect of the newly developed alpha/beta-blocker TJ0711, a variety of animal models of arrhythmia were induced by CaCl2, ouabain and ischemia/reperfusion. Glass microelectrode technique was used to observe action potentials of right ventricular papillary muscle of guinea pig. The onset time of arrhythmia induced by CaCl2 was significantly prolonged by TJ0711 at 0.75, 1.5 and 3 mg x kg(-1) doses. TJ0711 (1.5 and 3 mg x kg(-1)) can significantly shorten the ventricular tachycardia (VT) and ventricular fibrillation (VF) duration, the incidence of VF and mortality were significantly reduced. On ischemia-reperfusion-induced arrhythmic model, TJ0711 (0.25, 0.5, 1 and 2 mg x kg(-1)) can significantly reduce the ventricular premature contraction (PVC), VT, VF incidence, mortality, arrhythmia score with a dose-dependent manner. At the same time, rats serum lactate dehydrogenase (LDH) and creatine kinase (CK) activities decreased significantly by TJ0711 (1 and 2 mg x kg(-1)). Ouabain could cause arrhythmia in guinea pigs, when TJ0711 (0.375, 0.75, 1.5 and 3 mg x kg(-1)) was given, the doses of ouabain inducing a variety of arrhythmia PVC, VT, VF, cardiac arrest (CA) were significantly increased with a dose-dependent manner. In the TJ0711 0.1-30 micromol x L(-1) concentration range, guinea pig right ventricular papillary muscle action potential RP (rest potential), APA (action potential amplitude) and V(max) (maximum velocity of depolarization) were not significantly affected. APD20, APD50 and APD90 had a shortening trend but no statistical difference with the increase of TJ0711 concentration. TJ0711 has antiarrhythmic effect on the sympathetic nerve excitement and myocardial cell high calcium animal arrhythmia model. Myocardial action potential zero phase conduction velocity and resting membrane potential were not inhibited by TJ0711. APD20, APD50 and APD90 were shortened by TJ0711 at high concentration. Its antiarrhythmic action mechanism may be besides the action of blocking beta1 receptor, may also have a strong selective blocking action on alpha1 receptor and reducing intracellular calcium concentration.

10.Analysis of consistency between clinical and pathological TNM staging for gastric cancer

Objective To explore the consistency between preoperative clinical TNM stage and postoperative pathological TNM stage of gastric cancer and look for the underlying reason.Methods Sixty-four patients of gastric cancer with complete clinical data from December 2010 to December 2011 in Gansu Provincial Hospital were enrolled into this study.All cases were performed postoperative pathological TNM staging according to the TNM stage criteria of gastric cancer from JGCA(the 14th edition,2010),and the results were compared with preoperative TNM staging,then we studied the consistency of T staging,N staging and M staging using SPSS 18.0.Results The Kappa value from T staging of preoperative and postoperative was 0.729,showing a good consistency.The N stage' s consistency was lower,Kappa value being only 0.216.For M staging,Kappa value 0.743,having a good consistency.Conclusions According to the General Rules for Gastric Cancer Treatment,a accurate TNM staging and individual treatment plan can be made for gastric cancer patients in clinic.But the overestimate and underestimate still existed,especially in N staging.Therefore,making a accurate judgment for N staging according to all kinds of preoperative clinical data and images is the key to improvement of preoperative TNM staging accuracy and treatment.
