1.Effect evaluation of online lifestyle intervention on women with infertility and central obesity

Objective:
To evaluate the effect of online lifestyle intervention based on cognitive behavior adjustment on the female patients with infertility and central obesity.
Methods:
From November 2019 to November 2020, the women with infertility and central obesity in the Reproductive Medicine Center of Zhejiang Provincial People's Hospital were recruited and divided into two groups. The intervention group received online lifestyle intervention based on cognitive behavior adjustment for twelve weeks, and the control group received routine health education. Body weight, waist circumference, fasting blood glucose, fasting insulin, insulin resistance index (HOMA-IR), body fat, the pregnancy rate and the abortion rate were compared between the two groups.
Results:
There were 78 cases each in the intervention group and the control group, aged (31.15±4.87) years and (31.17±4.81) years, respectively. There were no significant differences between the two groups in age, body weight, body mass index, waist circumference, waist-hip ratio, fasting blood glucose, fasting insulin, HOMA-IR and body fat percentage before intervention (all P>0.05). After twelve-week intervention, the results of mixed effect model showed that the decrease of waist circumference, fasting insulin and body fat percentage in the intervention group were greater than those in the control group (all P<0.05). After six-month follow-up, there was no significant differences in the pregnancy rate (33.33% vs. 23.08%) and the abortion rate (7.69% vs. 16.67%) between the two groups (both P>0.05).
Conclusion
Online lifestyle intervention based on cognitive behavior adjustment helps to control central obesity and hyperinsulinemia.
3.Clinical analysis of alleviative treatment in 42 patients of advanced gastrointestinal cancer

Objective To study the short-term clinical efficacy of treating patients with advanced gastrointestinal cancer with lentinan injection and javanica oil emulsion injection.Methods Clinical information of 42 patients with advanced gastrointestinal cancer were retrospectively collected.The 42 patients were divided into two groups according to treatments,with 21 case in the control group who were treated with javanica oil emulsion injection,as well as 21 case in the treatment group treated with lentinan injection and javanica oil emulsion injection.The efficacy,quality of life (QOL) and adverse effects were observed after treatment for 3 weeks.Results 81.0% (17/21)of patients in the treatment group improved in QOL,which was much higher than that in the control group 47.6% ( 10/21 ) ( x2 =5.081,P =0.024 ).The objective remission rate was 19.0% (4/21)and 14.3% (3/21)in the treatment group and the control group respectively,with no significant differece bwtween the two groups( x2 =0.171,P =0.679 ).the disease control rate was 85.7% (18/21)in the treatment group,which was significantly higher than that of 61.9% (12/21)in the control group( x2 =4.200,P =0.040 ).The incidence of adverse effect related to hematological toxicity,liver and kidney function,the digestive tract and itching of skin were similar between the two groups (Ps > 0.05 ).Phlebitis in the treatment group was not as frequent as that in the control group(P <0.05).Conclusion Treating patients with advanced gastrointestinal cancer with lentinan injection and javanica oil emulsion injection had high efficacy than treating only with javanica oil emulsion injection,and it improved QOL signifiantly with safety.
4.Clinical efficacy of treating pains suffered from metastatic bone cancer with composite kushen injection and pamidronate disodium injection

Objective To explore the clinical efficacy of treating pains suffered from metastatic bone cancer with composite kushen injection and pamidronate disodium injection.Methods The clinical information of 60 cases of metastatic bone cancer patients suffered with pains was collected retrospectively.Thirty patients were assigned to the treatment group and 30 to the control group according to the treatment they underwent.The control group were treated with pamidronate disodium injection for 3 cycle,the treatment group were additionaly treated with composite kushen injection.The differences of two groups cases were compared in respect of the relief of pains and the changes of performance status (PS) and the incidence of adverse effects after treatment for 3 cycles.Results The objective remission rate of bone pain was 60.0% (18/30) in the treatment group,which was significantly higher than that of 30.0% (9/30) in the control group was higher( x2 =5.455,P=0.020 ).The incidence of adverse effect was 40.0% (12/30) in the treatment group and 46.6% (14/30) in the control group,with no significant difference between the two groups( x2 =0.271,P =0.602).In the treatment group the performance status of patients was( 2.30 ± 0.70 ) after treatment,which was better than that of( 1.80 ± 0.80 )before treatment(t =15.000,P =0.042),wheras there was no significant difference on performance status in the control group.Conclusion Kushen injection has synergistic effect with pamidronate disodium injection in treating pains with matstatic bone cancer.It could improve the short term efficacy,and significantly relief the pain and improve the quality of life.
5.Advances of polypeptide fragment of rabiesvirus glycoprotein as a brain-targeting carrier

Polypeptide fragment of rabies virus glycoprotein(RVG)has become one of the most popular polypeptides in drug delivery field,because of its advantages of neurotropiam,penetration of blood-brain barrier and biosafety.Polypeptide fragment of RVG can directly deliver proteins and nucleic acids into brain.Additionally,when polypeptide fragment of RVG couples with drug-loading polymers,nanoparticles or liposomes,it can mediate the latters into brain.The application of the RVG polypeptide fragment provides a safe and effective approach for the treatment of brain diseases with biomolecules,such as chemicals,proteins.plasmids,siRNA,miRNA.
6.Effects of ulinastatin on intestinal mucosal barrier after occlusion of portal vein in rats

Objective:To study the effect of portal vein occlusion on intestinal mucosal barrier in rats and the protection of ulinastatin to the injury,to present the experimental data for the clinical surgery.Methods:70 Sprague-Dawley rats were randomly divided into controlled group (n=10),operation group (n=30) and operation+medication group (n=30).The portal vein were occlused 40 min in the operation groups and operation+medication groups.2ml blood from portal vein,lymph nodes around appendix,1cm small intestine wall were taken for endotoxin levels,bacterial translocation and pathiology examinations in the all rats 280 mins after operation.The mocusal barrier and microscopic structure of intestine were observed.Results:Compared between the control group and the operation group,endotoxin levels,bacterial translocation rates rise greatly and gut structure change obviously in the latter.Compared between the operation group and operation+medication group,the former changes is also obvious.Conclusion:The occlusion of portal vein can leads the decrease of intestine mocusal barrier and the increase of its permeability.Ulinastatin has a good protective effect on the damages above.
7.Determination of ammonium glycyrrhizinate in Shufei Mixture by RP-HPLC

Objective To establish a RP-HPLC method for determination of ammonium glycyrrhizinate in Shufei Mixture .Methods The sample was analyzed on an Agilent Zorbax SB-C18 column (4.6mm ×150 mm, 5μm), and acetonitrile-0.0025 mol/L sodium heptanesulfonate solution–0.05 mol/L potassium dihydrogen phosphate solution(20:45:45) ( adjusted pH value of 7.2 ±0.05 with 20% sodium hydroxide solution) was used as mobile phase.The flow rate was at 1.0 mL/min.The detective wavelength was at 250 nm.The column temperature was 30 ℃.Results With this chromatographic condition, the ammonium glycyrrhizinate peak in Shufei Mixture sample chromatogram could be separated with other ingredient peaks completely.The negative sample had no interference.The calibration curve was linear at a ranges of 23.6-118.1μg/mL for ammonium glycyrrhizinate, and equation of regression was Y=0.1133X–0.00110,r=0.999 8.The average recovery from sample was 97.8% and RSD was 0.88%(n=6).The content range of ammonium glycyrrhizinate in three batch Shufei Mixture sample was 0.2532-0.2865 mg/mL, and average content was 0.2721 mg/mL. Conclusion This method is simple, accurate, and useful for control method of this preparation.
8.The Cause Analysis and Countermeasures of Frequent Shocks in Patients with Implantable Cardioverter Defibrillator

Objective To investigate the cause and countermeasures of frequent shocks in patients with implantable cardioverter defibrillators (ICD). Methods Eighty ICD patients with heart failure and malignant ventricular arrhythmias were followed up, including sixty-two male and eighteen female patients. There were 35 patients with single-chamber ICD, 23 with dual-chamber ICD and 22 with three-chamber ICD. Patients in this study were followed up for 1-6 years to analyze the reasons for ICD discharge. According to the specific circumstances, patients were treated. Results Twenty-three pa-tients in 80 patients suffered from shock treatment. Ten patients (12.5%) experienced frequent shocks. The causes of fre-quent shock included repeated episodes of ventricular tachycardia, invalid shock due to increased defibrillation threshold (DF) and false identification of the frequent episodes of paroxysmal ventricular tachycardia or arrhythmias. The management included the identification process adjustment of ventricular tachycardia and supraventricular tachycardia, increased num-bers of beats of ventricular tachycardia judgment and increase the basic pacing rate. The anti-arrhythmic drugs should be combinedly used, especially metoprolol and amiodarone. The ICD shock was significantly reduced after parameter optimiza-tion and anti-arrhythmic therapy. Conclusion The ICD shocks were effectively reduced with rational use of anti-arrhyth-mic drugs and valid ICD programming.

Objective To study the association between paternal CD3, CD4 and CD8 antigenecity to their pregnant spouses and the development of pregnancy induced hypertension (PIH). Methods Maternal serum from 15 women with PIH in the third trimester and 82 in normal pregnancies (16 in the first, 32 in the second and 34 in the third trimester) were incubated with paternal T lymphocytes. Monoclonal CD3, CD4 and CD8 fluorescent conjugated antibodies were then added and the percentage of paternal T cell differentiation antigen CD3, CD4 and CD8 were measured by flow cytometry. Results During normal pregnancy, the levels of maternal serum blocking antibodies on paternal CD3, CD4 and CD8 were (4.14?1.02, 2.02 ?0.24, 2.37?1.05)% in first trimester, (-0.29?0.13, 1.03?0.27, 0.65?0.23)% in the second trimester and (-1.33?1.47,0.15?0.01, -1.04? 0.37)% in the third trimester.
