2.Causes of job burnout in medical professionals and relevant intervention strategies

Job burnout has been a common problem in modem society, and medical professionals are vulnerable to it. The causes of burnout in medical professionals involve factors of social environment and system, institutional factors, occupational factors and personal factors. The intervention strategies at multiple levels are put forward, including to advocate concerning about the well-being of others and attaching importance to cultural value orientation of qualitative life-style, to construct harmonious doctor- patient relationship and good working environment, to enhance social public relations and establish good media image, to improve management system and operational mechanism at organizational level, and to enhance the coping ability at individual level by training.

Medical workers are the main designers and participants of clinical trials.In this consideration,popularization of good clinical practice (GCP)knowledge among them can improve their execution of trials,standardize basic clinical trial requirements,protect legitimate rights and health/safety of the subjects,and make the trial results more reliable.These objectives,if accomplished,will upgrade the clinical trials competency of medical institutions in China,and encourage healthy growth of the medical and pharmaceutical industry.The paper probed into the occupation qualification system of clinical trial workers and their supervision mechanism,inclusion of GCP into the continuing education system.
4.Research Progress of the Regulation Function of Epigenetics on Bone Marrow Mesenchymal Stem Cells.

The regulation of epigenetics on bone marrow mesenchymal stem cells (BMSCs) has been a research hot spot in medical area. This paper mainly summarizes the progress of the regulation of DNA methylation, histone acetylation, small interfering RNA (siRNA) induced gene silence and microRNA (miRNA) on BMSCs. Our analysis shows that the regulation of epigenetics on BMSCs plays a significant role in the repair of bone tissue, nervous tissue and cardiac muscle.

5.The clinical analysis of ultrasound biomicroscope scanning photography in anterior segment structure changes in blunt eye injury

Objective To clinical analyze the changes of ocular anterior segment structure using ultrasound biomicroscope(UBM) and their clinical values in blunt eye injury.Methods Sixty-nine eyes with no-opening wound in blunt eye injury were examined by UBM(Paradigm Model P40).Scanning photography of central and 3,6,9,12 clock hour were recorded and printed,other clock hours were scanned if needed.These records were studied with clinical findings.Results UNM findings showed that 27 eyes had hyphema,22 eyes had anterior chamber angle recession,16 eyes had iridodialysis,19 eyes had local luxation of lens,5 eyes had vitreous henia in anterior chamber,19 eyes had cyclodialysis,23 eyes anterior choroidal detachment and 9 eyes had anterior vitreous haemorrhage.Of the 69 eyes,62 eyes had combined structure changes showed above,which account(89.9)%.Conclusions UBM is a practical tool for diagnosis of anterior segment structure changes in blunt eye injury,especially when there are corea opacity,hyphema,cyclodialysis,ocular hypotension and others.It can provide exact informations for the diagnosis and treatment,which can not be showed by routine ocular examination and B-scanning ultrasonography.
6.Application of Comfortable Nursing for Body Position in Endoscopic Thyroidectomy

[Summary] A total of 108 cases of endoscopic thyroidectomy via thoracic approach were conducted in our department between January 2012 and January 2015.The comfortable nursing was applied during the operation.The patients’ body position was adjusted properly according to requirements of the operation at different periods of pre-operation, intra-operation and post operation.No complications due to nursing mistakes or improper position occurred in the 108 patients. Follow-up checkups on the second postoperative day found 2 patients with vomiting.According to the Wong-Banker Faces Pain Scale to evaluate the head and back pain, there were 46 cases of 0 point, 51 cases of 1 point, 11 cases of 2 points.We got the conclusion that the application of comfortable nursing for proper position of patient not only reflects the nursing principle of Patients First, but also improves the entire quality of nursing services.
8.Differential expression of ?-and ?-defensin in human peripheral blood

Objective To examine the expression of a- and ?-defensin genes in human peripheral leukocytes. Methods Fifty-one healthy blood donors were studied. Peripheral leukocytes were stimulated by lipopolysaccharide ( LPS) 100 ng?ml-1 ex vivo. The level of defensin mRNA expression in the leukocytes were assessed after being incubated with LPS for 0 (baseline), 1, 3, 6, 12, 24 h by semi-quantitative RT-PCR. Southern blot analysis and sequencing were used to confirm the identity of defensin gene transcripts. Results Expression of ?-defensin 1-3 mRNA was detected in all donors studied, but no ?-defensin mRNA expression was detected in peripheral leukocytes before LPS-stimulation. Expression of ?-defensin-1 gene was detected in the leukocytes after being incuaated with LPS for 3 h in 45 of 51 donors (88.2% ) and ?-defensin-2 mRNA expression was positive in 20 donors (39.2 % ), but no ?-defensin-3 mRNA expression was detected. The levels of ?-defensin-1 and-2 mRNA peaked at 6 h and started decreasing at 12 h. In contrast there was no significant change in ?-defensin 1-3 mRNA in peripheral leukocytes after LPS-stimulation. Conclusion In human peripheral leukocytes ?-defensin-1 and-2 genes are induced transiently by LPS-stimulation;whereas ?-defensin 1-3 genes are constitutive. The induced expression of ?-defensin-1 and-2 genes show inter-individual variability.
9.Clinical Analysis of 10 Cases of Juvenile Rheumatoid Arthritis Associated Iridocyclitis

Objective To study the clinic features and prognosis of juvenile rheumatoid arthritis (JRA)-associated iridocyclitis.Methods Ten cases of JRA -associated iridocyclitis choosed from 107 cases with JRA were reviewed and analysed.Results Polyarticular( n =9) and pauciarticular( n =1) were observed among JRA -associated iridocyclitis.Four were acute cases and all were recovered.Secondary glaucoma ( n =1) and ablepsia( n =1) were find in the chronic cases of 6,respectively.Conclusions Iridocyclitis is a common complication of JRA. Most of them are polyarticular group. The acute cases have better results compared to the chronic cases which often have more complications such as glaucoma,cataract, et al .
10.Evaluation of ultrasonography in diagnosis of benign PH and malignant thyroid nodules by binary LogisticPHam

Objective To evaluate the clinical value of ultrasonography in diagnosis of benignPHand malignant thyroid nodules by binary Logistic regression model.Methods A retrospective analysis of 240 cases of thyroid nodules confirmed by pathology after operation excision was conducted.173 nodules were benign as control group,67 nodules were malignant as observation group.The ultrasonic features of thyroid nodules were collected.The factor had statistics significance by x2 test between the two groups were analyzed by binary Logistic regression.A logistic regression model was created.The ROC curve was drawn and the area under the curve was calculated.Results There were statistically significant differences among boundary,aspect ratio,shape,internal echo,calcification,acoustic halo,cervical lymphadenectasis between the control group and observation group(x2=45.392,7.590,30.039,24.168,37.406,6.893,16.078,all P<0.01).And after Logistic regression analysis,six variables that entered the regression equation were boundary(P=0.000,OR=8.437),aspect ratio(P=0.000,OR=12.816),microcalcification of the nodules(P=0.000,OR=8.893),shape(P=0.000,OR=8.791),internal echoes(P=0.001,OR=8.313)and cervical lymphadenectasis(P=0.001,OR=6.891).The accuracy of the model was 90.7%(223/246)in predicting malignant nodules in thyroid.The area under the ROC curve was(0.904±0.031).Conclusion The binary Logistic regression can be used to differentiate malignant and benign thyroid nodules,and make a more accurate judgment for the nodules in thyroid.
