1.Correlation between CT features and clinical severity stratification in acute pulmonary embolism

Objective To analyze the correlation factors between CT imaging features of pulmonary embolism(PE)and clinical severity stratification,to explore the value of CT pulmonary angiography (CTPA)in acute PE severity stratification.Methods According to the clinical severity,48 patients with acute PE proved by CTPA were classified into two groups,including 21 critical and 27 non-critical patients. Embolism index,ratio of central pulmonary involvement,ratio of right ventricle maximum minor axis (RVMMA)to left ventricle maximum minor axis(LVMMA),namely RV:LV,dilation of main pulmonary and/or right pulmonary trunk,and dilation of bronchial arteries in both groups were analyzed comparatively. The correlation factors between CT imaging features and PE clinical severity stratification were explored.The correlation between RV:LV and embolism index of 48 patients was analyzed.Results Pulmonary embolism index(22.0%—85.0%,median 38.0%),ratio of central pulmonary involvement(42.5%),RV:LV (0.90—1.90,median 1.30),dilation of pulmonary artery(14 cases),and dilation of bronchial artery (8 cases)in critical group(21 cases)were higher than those corresponding factors(5%—48%,median 21.5%,31.25%,0.80—1.40,median 1.00,5 eases,and 3 eases)in non-critical group(27 cases) (Z=4.27,X~2=5.40,Z=2.58,X~2=11.45,X~2=4.87,P
2.Treatment of lower extremity diabetic atherosclerotic obliterans with shuxuetong injection.

OBJECTIVETo observe the effect and safety of Shuxuetong Injection (SXT) in treating patients with type 2 diabetes mellitus accompanying lower extremity atherosclerotic obliterans.
METHODSThe selected patients were assigned by randomized digital table to the treated group (180 cases treated with SXT) and the control group (80 cases treated with alprostadil). The clinical efficacy was assessed after 4 weeks of treatment, and the inner diameters and blood flow of the superficial femoral artery, the posterior tibial artery and the foot dorsal artery were measured and compared before and after treatment using Doppler's ultrasonography.
RESULTSThe total effective rate in the treated group was 94.4% (170/180), better than that in the control group, 85.0% (68/80, P < 0.01). Ultrasonic examination showed that the inner diameter and the blood flow of all the arteries measured in the treated groups increased after treatment (P < 0.01), with the increase superior to those in the control group (P < 0.05). No adverse reaction was found during the treatment.
CONCLUSIONSXT has the effect of activating blood circulation, dissolving stasis and dredging collaterals, it could reduce or eliminate the formation of thrombi, being effective and safe when applied in clinical practice for the treatment of lower extremity diabetic atherosclerotic obliterans.

3.Treatment of constipation by intensive acupoints thread embedding combined with local anal operation.

OBJECTIVETo search for the best therapy for slow transmission constipation (STC) or mixed constipation (MC) by intensive acupoints thread embedding (IATE).
METHODSA total of 310 patients with constipation were assigned to 5 groups using a stratification control method. The 62 patients of STC in the observation group I were treated by IATE at Dachangshu (BL 25), Tianshu (ST 25), Zhongji (CV 3) and Zusanli (ST 36) acupoints; the 62 of MC in the observation group II by IATE at the same acupoints and local anal operation; both the 62 of STC in the control group I and the 62 of MC in the control group 1I were treated by oral administration of Simo Decoction, while the 62 of MC in the control group III were treated by local anal operation alone. Conditions of defecation were recorded every day in the 6-month follow-up period. Gastrointestinal transit test (GITT) was carried out before and after treatment. One follow-up was conducted at the 1st, 3rd and 6th month.
RESULTSIn the 124 patients of the two observation groups, 52 (41.9%) were cured after treatment, they could normally discharge stool, 5 (4.0%) showed markedly effective with their constipation alleviated, but having occasional bowel slackened sensation. In the control group I and II, none was cured or effectively treated, while in the control group III, the efficacy in 8 patients were judged as marked effective, but none was cured.
CONCLUSIONIATE is a convenient and effective composite therapy. It shows favorable clinical efficacy for STC, and also for MC in case of combined with local anal operation.

4.Efficacy controlled observation on acupuncture and western medicine for gastrointestinal dysmotility in liver cirrhosis.

OBJECTIVETo observe the therapeutic effect of acupuncture on gastrointestinal dysmotility in liver cirrhosis.
METHODSForty cases of gastrointestinal dysmotility in liver cirrhosis were randomized into an acupuncture group and a motilium group, 20 cases in each one. In the acupuncture group, on the basis of the conventional treatment, electroacupuncture was applied at Zusanli (ST 36), Sanyinjiao (SP 6) and Taichong (LR 3) for 30 min, once a day. In the motilium group, on the basis of the conventional treatment, motilium was taken orally 30 min before meals, 10 mg each time, three times a day. The treatment was required for 2 weeks in both groups. The changes in the digestive tract symptom score and liver function Child-Pugh score were observed and the efficacy was assessed.
RESULTSThe total effective rate of digestive tract symptoms was 85.0% (17/20) in the acupuncture group and 70.0% (14/20) in the motilium group. The score improvements in abdominal distention, belching and vomiting in the acupuncture group were superior to those in the motilium group (all P < 0.05). In the acupuncture group, the liver function Child-Pugh total score was 8.40 +/- 0.22 before treatment and reduced to 5.36 +/- 0.17 after treatment, in which the scores for ascites, serum bilirubin and albumin were all reduced (all P < 0.05) and the reducing range was increased in tendency with the improvements in digestive tract symptoms. In the motilium group, Child-Pugh score was not changed obviously as compared with that before treatment. CONCLUSION Acupuncture effectively alleviates digestive tract symptoms and improves liver function for the patients of liver cirrhosis, its efficacy on gastrointestinal dysmotility in liver cirrhosis is superior to motilium.

5.Killing effect of sequential Herceptin and adriamycin treatment on breast cancer cell line in vitro.

OBJECTIVETo observe the killing effect of Herceptin and adriamycin sequentially applied on breast cancer cell line in vitro.
METHODSBT-474 human breast cancer cells in exponential growth phase were treated with Herceptin alone, adriamycin alone and their sequential administration (Herceptin before adriamycin and vice versa), respectively. Under optical microscope, the morphological changes of the cells were observed before and after drug administration. The expression rate and mean fluorescence intensity (MFI) of HER-2/neu and cell death rate were detected by flow cytometry.
RESULTSMicroscopically, the cells treated with different protocols all exhibited such changes as darkening and increase of cellular debris with irregular cell morphology. Flow cytometry revealed no significant difference in the expression rate of HER-2/neu in each group before and after treatment, but the MFI of HER-2/neu and death rate of the treated cells were significant different from those of the control group (P<0.05). The cell death rate of Herceptin-pretreated cells was significantly higher than that of adriamycin-pretreated ones (P<0.05).
CONCLUSIONHerceptin pretreatment enhances the killing effect of adriamycin on breast cancer cell line BT-474, which provides experimental evidence for designing clinical sequential biochemotherapy of breast cancer.

6.A clinical trial on the treatment of compression fracture of osteoporotic vertebral body by self-made spinal expander.

OBJECTIVETo observe the effects of self-made spinal expander in treatment of compression fracture of osteoporotic vertebral body.
METHODSNinty-six patients were randomly divided into two groups, treatment group included 42 cases with 56 centrums, 14 males and 28 females, aged from 50 to 69 years, and control group included 54 cases with 68 centrums, 19 males and 35 females, aged from 51 to 75 years. All cases were examined by X-ray and CT. MRI was taken in 27 patients to make sure the diagnosis which was compression fracture of vertebral body due to osteoporosis. The degree of osteoporosis was judged by X-ray of L3 bone trabecula. Fifty-six centrums in treatment group were treated with self-made spinal expander and bone cement. Sixty-eight centrums in control group were treated with PVP. Compression rate and recovery rate of vertebral height, and leakage rate of bone cement were measured according to the Lee standard.
RESULTS(1) VAS between treatment group and control group had significant statistically difference (P<0.05). (2) In treatment group, compression rate of anterior and middle vertebal height were statistical difference before and after operation (P<0.05). In control group, the compression rate of pre-and post-operative vertebal height had no significant difference (P>0.05). (3) The recovery rate of kyphosis between treatment group (71.8%) and control group (37.8%) was significant statistically difference (P<0.05). (4) There was no cement leakage in the treatment group, however the leakage rate in control group was 47%. (5) The curative effect was assessed based on the standard of WHO. In treatment group, 36 cases was completely relieved, 4 cases was partially relieved and the relief rate was 95.24%. In control group, 43 cases was completely relieved, 3 cases was partially relieved and the relief rate was 84.19%. There was no statistically significant difference between two groups (P<0.05).
CONCLUSIONThe self-made spinal expander is a minimal invasive technique, which can substitute the imported balloon. This technique can alleviate pain rapidly and recover the height of vertebral body.

7.Establishment of the Chinese facial emotion images database with intensity classification

Objective: To establish an intensity-classified Chinese facial emotion image database and provide materials for emotional researches in China. Methods: Firstly, under the guidance of professional director, 22 professional dramatic actors put up seven kinds of emotion, including anger, disgust, fear, sadness, surprise, happy and calm. The process of facial emotional performance of actors were filmed by a high-speed camera. Then we made screen captures according to the intensity of emotion performed by the actors. A total of 814 black-and-white photographs with same size and gray level were made after we removed the irrelevant parts to the facial features. Each kind of emotion contains six levels of intensity scoring from 1 to 6. Totally, 112 healthy volunteers were recruited to evaluate the emotional intensity, pleasure and arousal of all images via a visual scale ranged from 0 to 100. Results: A Chinese face emotional intensity hierarchical picture library was set up, which contained 2 sublibraries. The first library was consisted of 436 images with hierarchical emotional intensity images collected from different performers. The second one was built with 640 images, in which the hierarchical emotional intensity image was collected from same actor/actress. Both image libraries had corresponding scores for intensity, pleasure, and arousal. Furthermore, both exhibited a high degree of internal consistency in reliability analysis (with a Cronbach' s alpha value larger than 0. 9). Conclusion: The present study has established an intensity-classified Chinese facial emotion image database with good recognition degree and psychological measurement attribution, which could provide materials for emotional researches in China.
8.Effect and Underlying Mechanism of Bu-Shen-An-Tai Recipe on Ovarian Apoptosis in Mice with Controlled Ovarian Hyperstimulation Implantation Dysfunction

The effect and underlying mechanism of Bu-Shen-An-Tai recipe on ovarian apoptosis in mice with controlled ovarian hyperstimulation (COH) implantation dysfunction were studied.The COH implantation dysfunction model in mice was established by intraperitoneal injection of 7.5 IU pregnant mare's serum gonadotrophin (PMSG),followed by 7.5 IU human chorionic gonadotrophin (HCG) 48 h later.Then the female mice were mated with male at a ratio of 2:l in the same cage at 6:00 p.m.The female mice from normal group were injected intraperitoneally with normal saline and mated at the corresponding time.Day 1 of pregnancy was recorded by examining its vaginal smears at 8:00 a.m.of the next day.Fifty successfully pregnant mice were equally randomly divided into 5 groups:normal control pregnant group (NC),COH implantation dysfunction model group (COH),low dosage of Bu-Shen-An-Tai recipe group (LOW),middle dosage of Bu-Shen-An-Tai recipe group (MID) and high dosage of Bu-Shen-An-Tai recipe group (HIGH).Then from day 1,the mice in different groups were respectively intragastrically given corresponding treatments at 9:00 a.m.for 5 consecutive days.The concentrations of 17β-estradiol (E2) and progesterone (P4) were determined by radioimmunoassay (RIA).The ultrastructural changes of ovarian tissues were observed by transmission electron microscope (TEM).The histopathological changes of ovarian tissues were observed by HE staining.The number of atretic follicles and pregnant corpus luteum were also recorded.TUNEL was applied to measure apoptotic cells of ovarian tissues.Western blotting was used to detect the protein expression of apoptosis-related factors like Bax,Bcl-2 and cleaved-caspase-3 in ovarian tissue of mice.The results showed that ovarian weight,the concentrations of E2 and P4,the number of atretic follicles and pregnant corpus luteum,as well as the apoptosis of granulosa cells were significantly increased in the COH group.The ultrastructures of ovarian tissues in the COH group showed that chromatin in granulosa cells was increased,agglutinated,aggregated or crescent-shaped.The focal cavitation and the typical apoptotic bodies could be seen in granulosa cells in the late stage of apoptosis.After the treatment with different doses of Bu-Shen-An-Tai recipe,the ultrastructural changes of ovarian granulosa cells apoptosis were dramatically improved and even disappeared under TEM.Visible mitochondria and mitochondrial cristae were increased and vacuoles were significantly reduced.The lipid dropltes were shown in a circluar or oval shape.The protein expression levels of Bax and cleaved-caspase-3 were decreased,and the expression of Bcl-2 protein was increased after treatment.It was concluded that Bu-Shen-An-Tai recipe can inhibit the apoptosis of ovarian granulosa cells,probably by up-regulating the protein expression of Bcl-2 and down-regulating Bax and cleaved-caspase-3,which contributes to the formation and maintenance of ovarian corpus luteum.It's helpful to promote the embryonic implantation,to reduce embryo loss and ultimately to improve the success rate of pregnancy.
9.Clinical features and StAR gene mutations in children with congenital lipoid adrenal hyperplasia.

This article reported the clinical manifestations, steroid profiles and adrenal ultrasound findings in two unrelated Chinese girls with lipoid congenital adrenal hyperplasia (LCAH). Direct DNA sequencing and restriction fragment length polymorphism (RFLP) analysis were used to identify the mutations of steroidogenic acute regulatory protein (StAR) gene. The two patients with 46,XX karyotype, presented hyperpigmentation, growth retardation, and hyponatremia. Steroid profiles analysis revealed elevated plasma adrenocorticotrophic hormone levels, decreased or normal serum cortisol levels and low levels of androgens. Ultrasound examinations revealed that enlarged adrenals in patient 1 and normal adrenals in patient 2. Direct DNA sequencing of StAR gene showed a reported homozygous for c.772C>T(p.Q258X) in patient 1. Compound heterozygous for c.367G>A(p.E123K) and IVS4+2T>A (both novel mutations) were found in patient 2, inherited from her mother and father respectively. The amino acid of mutant position of the novel p.E123K was highly conserved in ten different species and was predicted to have impacts on the structure and function of StAR protein by the PolyPhen-2 prediction software. RFLP analysis revealed three bands (670, 423 and 247 bp) in patient 2 and her father and two bands (423 and 247 bp) in her mother and 50 controls. It is concluded that LCAH should be considered in girls with early onset of adrenal insufficiency and that steroid profiles, karyotype analysis, adrenal ultrasound and StAR gene analysis may be helpful for the definite diagnosis of LCAH.

10.WT1 gene expression and its isoform ratio in different cell subsets of normal human bone marrow.

The Wilms' tumor gene (WT1) is a transcription factor involved in tumorigenesis, especially in leukemogenesis. However, the role of WT1 expression in nonmalignant hematopoietic cells remains unclear. Furthermore, due to alternative splicing at two sites: 17 amino acid residues of exon 5 (+17AA) and 3 amino acid residues (+KTS) between exons 9 and 10, WT1 gene has four main isoforms (17AA+/KTS+, 17AA+/KTS-, 17AA-/KTS+, 17AA-/KTS-, abbreviation: +/+, +/-, -/+, -/-). The isoforms probably existed in hematopoietic cells, which make the research more complex. The aim of study was to elucidate the expression and its isoforms of WT1 gene in different cell subsets of healthy bone marrow donors. The fluorescence RT-PCR detection system was established to measure the expressions of full-length WT1, WT1 (+17AA) and WT1 (+KTS) in CD34(+)CD38(-) (stem cell), CD34(+)CD38(+) (progenitor cell), CD15(+)CD11b(+) (granulocyte), CD33(+)CD14(+) (monocyte), CD20(+)CD5(-) (B-lymphocyte) and CD20(-)CD5(+) (T-lymphocyte) subsets from 18 normal human bone marrow samples. The results showed that WT1 expressed in CD34(+)CD38(-), CD34(+)CD38(+), CD15(+)CD11b(+) and CD33(+)CD14(+), but not in CD20(+)CD5(-) and CD20(-)CD5(+) subsets. The highest expression was in CD34(+)CD38(-), but decreased gradually in CD15(+)CD11b(+) and CD33(+)CD14(+) subsets. WT1 (+17AA), WT1 (+KTS) and WT1 (+/+) isoforms were predominant in CD34(+)CD38(-) and CD34(+)CD38(+) primitive subsets, while in CD15(+)CD11b(+) and CD33(+)CD14(+) the dominant isoforms were WT1 (-17AA), WT1 (-KTS) and WT1 (-/-). It is concluded that the expression of WT1 in normal bone marrow decreases gradually with cell differentiation. Hematopoietic cells may adjust the ratios of WT1 isoforms to inhibit or promote cell differentiation.
