1.Relationship between PD-1 expression in liver tissues with TCM syndromes and liver pathology in patients with chronic hepatitis B

Objective To discuss the relationship between the expression of programmed death-1 (PD-1) in liver with traditional Chinese medicine (TCM) syndromes and pathological diagnosis in chronic hepatitis B.Methods 156 CHB patients treated in our hospital of infectious diseases department were recruited as an observation group. Based on the principle of informed voluntary and approved by the ethics committee, liver biopsy was adopted to make clear liver tissue pathology. According to TCM classification criteria, CHB patients were divided into five groups: a blood stasis group, a damp heat resistance group, a liver and spleen deficiency group, a liver and kidney yin deficiency group, and a spleen and kidney yang deficiency group. In the same period, 12 healthy persons were recruited as a control group. The PD-1 expression was detected with immunohistochemical SP method, and the correlation between expression of PD-1 in liver tissue and TCM syndrome type and liver pathology was analyzed.Results Different degrees of positive cell expression were found in the liver tissue. With the liver inflammation and fibrosis severity, the number of PD-1 positive cells also increased. The PD-1 expression levels varied with mild, moderate, and severe CHB patients (0.24 ± 0.03, 0.36 ± 0.05 vs. 0.43 ± 0.05) , which were statistically significant (P<0.05) . PD-1 expression levels also varied among different TCM type CHB patients, of which, PD-1 expression of blood stasis type was the highest (0.35 ± 0.04), while the liver and spleen deficiency type was the lowest (0.23 ± 0.03).Conclusion The expression levels of PD-1 has a certain correlation with the patients illness, chronic mechanism, and TCM syndromes. CHB patients can be treated by controlling the expression of PD-1.

Objective To study the clinical and pathologic features, diagnosis, treatment and prognosis of splenic marginal zone lymphoma (SMZL). Methods We reported the detailed clinical and pathologic presentation of a patient with SMZL, and monitored fellow-up to 16 years. Results Patients present with splenomegaly. SMZL usually involve bone marrow, peripheral blood and hilar splenic lympha nodes. Peripheral lymph nodes and nonhematolymphoid organs can be involved as well, but to a lesser extent.Microscopically, tumor cells are small to medium in size, with dispersed chromation. Tumor cells are slgM(+),CD+20, CD+79a,, bcl-2(+), CD-3, CD-5, CD-10, CD-23 and CyclinDl(-). Conclusion SMZL is a rare low-grade B-cell lymphoma with an indolent clinical course. Spleneetomy and chemotherapy are effective treatment but SMZL cannot be completely cured and may transform to large B-cell lymphoma at last.

5.Jinlong capsule combined with chemoradiotherapy for NSCLC: a Meta-analysis.

The purpose of this study was to evaluate the effect and safety of Jinlong capsule combined with chemotherapy or radio-therapy for non-small cell lung cancer (NSCLS) using Meta-analysis. PubMed, Embase, CNKI and Wanfang databases were all searched without language restriction, and searching time was from January 1990 to July 2015. All eligible published studies were included in this study for quality assessment and data extraction. All the data were analyzed using Revman 5.3. A total of ten studies including 736 subjects (370 in Jinlong capsule plus chemoradiotherapy and 366 in chemoradiotherapy only) were finally included in this Meta-analysis. The result of Meta analysis showed that compared with pure chemoradiotherapy group, Jinlong capsule combined with chemoradiotherapy for NSCLC could improve the patients' curative effect (OR = 1.77, 95% CI: 1.29-2.43, P < 0.05), clinical benefit rate (OR = 1.89, 95% CI: 1.22-2.91, P < 0.05), life quality improvement rate (OR = 2. 56, 95% CI: 1.61-4.05, P < 0.05), and decrease leucopenia incidence rate (OR = 0.35, 95% CI: 0. 22-0.56, P < 0.05) and gastrointestinal reaction rate (OR = 0.67, 95% CI: 0.40-1.11, P < 0.05). The pooled results showed that Jinlong capsule combined with chemoradiotherapy for NSCLC could improve the curative effect and life quality, and decrease the adverse reaction of patients.

7.Visual acuity outcome of cataract surgery in patients with wet age-related macular degeneration

AIM: To evaluate visual acuity outcomes after phacoemulsification and intraocular lens implantation in patients with wet age-related macular degeneration (wAMD).METHODS: We reviewed the medical documents of the patients who underwent phacoemulsification and intraocular lens implantation surgery during June 2013 and January 2016.Totally 61 eyes of 48 patients with wAMD in the stable stage were recruited.The pre-and post-operative vision of selected cases were recorded and compared.RESULTS: After phacoemulsification and intralocular lens implantation, visual acuity changes were as follows: 49 eyes improved, 11 eyes retained, and 1 eye deteriorated.Visual acuity improvement after cataract surger were statistically significant (P<0.001).Visual acuity improvement was not related to age.CONCLUSION: Visual acuity improved in patients with wet AMD after phacoemulcification and intraocular lens implantation.
8.Evaluation of clinical effectiveness of implantable contact lens for high myopia

Background Posterior chamber phakic intraocular lens implantation is a safe and effective method for correction of high myopia.Petacam-assisted observation of the intraocular lens position in the eye after surgery,provides a good way for the evaluation of operation safety.Objective This study was to evaluate the security and effectiveness of posterior chamber (phakic) implantable contact lens (ICL) implantation for high myopia.Methods Twenty-five eyes of eighteen high myopia patients who received ICL implantation were retrospectively analyzed from March 2009 to September 2012.The mean preoperative spherical equivalent (SE) of the patients was (-17.52 ± 1.73) D and the cylinder was (-0.75 ± 0.28) D.The clinical effective indexes included preoperative and postoperative uncorrect visual acuity (UCVA) and best correct visual acuity (BCVA),SE,intraocular pressure (IOP),corneal endothelial cell number and percentage of corneal endothelial hexagon cells.In addition,Pentacam analysis system was used to compare the anterior chamber depth,anterior chamber angle and space between crystal lens and ICL between before and 6 months after operation.Written informed consent was obtained from each patient before any medical procedure.Results ICL was successfully implanted in all the 25 eyes.UCVA was 1.47 ±0.26 and 0.26±0.15 respectively before and after operation,showing a significant difference between them (t =21.71,P =0.00).SE,corneal endothelial cell number and percentage of corneal endothelial hexagon cells were significantly lower than those before operation (SE:t =-48.60,P =0.00 ; corneal endothelial cell number:t =13.07,P =0.00 ;percentage of corneal endothelial hexagon cells:t =10.79,P<0.05).Anterior chamber angle was (25.02± 4.77)° 6 months after operation and was smaller than (38.43±4.04)° before operation (t=15.32,P=0.00).No significant differences were found in IOP and anterior chamber depth between before and after operation (IOP:t =-1.57,P =0.13 ; anterior chamber depth:t =1.46,P =0.16).The spaces between crystal lens and IOL was (363.33± 44.37) μm 6 months after operation,with a significant decline in comparison with (542.17±39.46) μm 1 week after operation (t =20.86,P =0.00).Glare sense appeared in 2 eyes after operation.Conclusions The results indicated that ICL implantation can retain the natural accommodation of lens.ICL implantation appears to be an effective and safe approach to high myopic eyes.However,the changes of corneal endothelial cells after operation should be payed more attention.

Objective To investigate the effects of intense pulse light(IPL)on the content of colla- gens and mRNA expression of procollagen in BALB/c mouse skin.Methods The BALB/c mice dorsal skin was irradiated by IPL.Skin specimens were taken at different time points after irradiation.Histopatho- logical changes were observed by hematoxylin-eosin-staining,quantitative assessment of collagenⅠand collagenⅢwas performed with immunohistochemical staining,and mRNA expression levels of procollagen were de- tected by RT-PCR.Results After the irradiation,no obvious change was observed for the staining intensity of collegen at 1 week;however,the thickening of dermis began at 2 week,and continued until 8 week.The staining of collagenⅠandⅢwas also stronger in IPL-irradiated regions than in sham-irradiated areas at 2 week(P
10.Anti-thrombopoietin antibodies in systemic lupus erythematosus with thrombocytopenia

Objective To investigate the existence and significance of circulating autoantibodies to thrombopoietin (TPO) in sera from patients with systemic lupus erythematosus (SLE).Methods Fifty-six consecutive patients with SLE,twenty patients with idiopathic thrombocytopenic purpura (ITP),and twenty normal individuals were involved in this study.The characteristics of all patients with SLE were analyzed.Antibodies to TPO were detected using an enzyme-linked immunosorbent assay (ELISA).For normal distribution count data,x2 test or Fisher exact test was used,t test was used for measurement data,and Wilcoxon's rank test for non-normally distributed data which was represented by M(Q).Results A higher frequency of antibodies to TPO were observed in SLE patients than those in healthy controls (39.3% vs 0,x2=11.058,P=0.001).Moreover,anti-TPO antibodies were detected in 15 (57.7%) of 26 SLE patients with thrombocytopenia,compared with that in 7 (23.3%,x2=6.894,P=0.009) of 30 patients without thrombocytopenia.Furthermore,the patients with antibodies to TPO exhibited more severe thrombocytopenia (t=3.010,P=0.004).Finally,anti-TPO antibodies seemed more likely to occur in patients with arthritis (x2=5.959,P=0.015),anti-dsDNA antibodies (x2=5.959,P=0.015).Conclusion The high incidence of antibodies to TPO in SLE patients with thrombocytopenia suggests that anti-TPO antibodies might play a vital role in SLE patients developing thrombocytopenia.Thus,there might be a clinical value by detecting anti-TPO antibodies in SLE patients with thrombocytopenia.
