1.The analysis on the risk factors of circulatory crisis of distal finger after replantation

Objective To determine the main predictors of circulatory crisis after replantation in patients with severed distal finger so as to establish the theoretic basis in reducing the morbidity of circulatory crisis. Methods 65 severed distal fingers underwent replantation were respectively reviewed. All the clinical parameters including age, sex, dominant hand or finger, preservation of the severed finger, platelet level, ischemia duration, cause of injury, manner of venous drainage, repair of artery, order of reparation and plane of division were investigated as predictive risk factors for circulatory crisis of finger after replantation. In order to find the risk factors of circulatory crisis, the data was dealt with t test, ?2 test, analysis for variance or multivariate Logistic regression analysis. Results 13 replanted finger suffered from impaired circulation in 65 fingers. Univariate analysis demonstrated that many factors were significantly correlated with the circulatory crisis of finger, such as the manner of venous drainage(?2=6.714,P=0.035), the cause of injury (?2=9.049,P=0.011), the preservation of severed finger(?2=6.452,P=0.040), the age(?2=14.838,P=0.001), the platelet level(uc= 2.961, P=0.003). The multivariate Logistic regression analyses showed that platelet level (OR=1.015, P=0.030), the age(OR=0.349, P=0.031) and the manner of venous drainage(OR=0.278,P=0.036) were the significant independent predictors for circulatory crisis. Aim To study the inhibitory effect of anti-ErbB2 chimeric antibody chA21 on proliferation of human malignant breast cancer cell lines SKBR3 and explore its possible mechanisms through studying its effects on ErbB2 distribution and PI3K/Akt,Erk1/2 signal transduction.Methods The inhibitory effect of chA21 on SKBR3 was assessed by MTT assay.The effect of chA21 on distribution of ErbB2 on cell plasma membrane was determined by FACS(Fluorescence-activated cell sorting)analysis;p-Akt and p-Erk1/2 were detected by Western blot.Results The inhibitory rate of chA21 on SKBR3 cells in vitro increased in a dose and time dependent manner.Furthermore,it could down-regulate the distribution of ErbB2 on plasma membrane and decrease the activation of p-Akt and p-Erk1/2.Conclusions ChA21 can inhibit the proliferation of SKBR3 cell in vitro.Down-regulation ErbB2 on cell plasma membrane resulting in decreasing the activation of p-Akt and p-Erk1/2 may be one of the mechanisms of the inhibitory effect.
3.Treating lupus nephritis by a drug pair of radix astragali and rehmanniae radix combined with glucocorticoid: a preliminary clinical study.

OBJECTIVETo observe the therapeutic effect of a drug pair of Radix Astragali and Rehmanniae Radix combined with glucocorticoid (GC) in treating lupus nephritis (LN) patients and its influence on some experimental indices.
METHODSTotally 52 LN patients were randomly assigned to the treatment group (treated by routine Western medicine and a drug pair of Radix Astragali and Rehmanniae Radix, 25 cases) and the control group (treated by Western medicine, mainly by GC and cyclophosphamide, 27 cases). All patients received 6-month therapy. The GC dosage, the withdrawal and reduction dosage of GC, clinical efficacy, systemic lupus erythematosus disease activity index (SLEDAI) score, adverse reactions, and laboratory indicators were recorded.
RESULTS(1) All patients got relieved to some degree with the dosage of GC reduced. The total withdrawal and reduction dosage of GC was slightly higher in the treatment group than in the control group [(50.23 +/- 12.43) mg vs (48.76 +/- 13.61) mg, P > 0.05]. Besides, the prednisone dosage in the treatment group was lower than that in the control group, but without statistical difference (P > 0.05). The ratio of patients in need of adding prednisone for aggravating disease was 24.0%, significantly lower than that in the control group (44.44%, P < 0.05). (2) There was no statistical difference in the SLEDAI score, inflammatory indicators, liver and renal functions, blood electrolytes, blood glucose, blood and urine routines between the two groups (P > 0.05). The 24-h urinary protein count was (1.06 +/- 0.22) g/L in the treatment group, obviously lower than that in the control group (1.43 +/- 0.55 g/L, P < 0.05). (3) There was no statistical difference in the incidence rate of infection, gastrointestinal hemorrhage, psychoneuroses, Cushing's syndrome, cardiovascular anomalies, and femoral head necrosis between the two groups (P > 0.05). But the incidence of adverse reactions such as insomnia, tidal fever, spontaneous sweat, and obesity was less in the treatment group than in the control group (P < 0.05).
CONCLUSIONSUsing a drug pair of Radix Astragali and Rehmanniae Radix combined with GC in treating LN could withdraw the dosage of GC and relieve symptoms it induced. It was advantageous in reducing the dosage of GC and stabilizing patients' conditions.

4.The hemostatic effect of emergent hemostasis patch

Objective To observe the hemostatic effect of emergent hemostasis patch (EHP). Methods The coagulation time was studied with tube method in vitro. Hemostasis was studied with femoral artery hemorrhage models of rats and dogs as well as rabbit hepatorrhexis model in vivo. Results The clotting time of EHP of high, medium and low concentrations was (4.9±1.3)min, (5.7±1.8)min and (5.8±1.3)min, respectively, which was significantly lower than that of control (14.4±1.6)min in vitro. In femoral hemorrhage models of rats and dogs, the duration and amount of bleeding in EHP of high, medium and low doses were significantly decreased compared with those in the PVF sponge group (P<0.01 or P<0.05). The results of rabbit hepatorrhexis experiment showed that EHP of three doses obviously decreased the hemostatic time and blood loss compared with PVF sponge group (P<0.01). Conclusion EHP can shorten coagulation time and bleeding time, and reduce bleeding amount.

6.The clinical analysis of 14 cases of systemic lupus erythematosus complicated by thrombotic thrombocytopenic purpura

Objective To analyze the clinical features,diagnosis and treatment of thrombotic thrombocytopenic purpura (TTP) in patients with systemic lupus erythematosus (SLE).Methods Clinical manifestations,laboratory findings,diagnosis,treatment and prognosis of 14 SLE patients with TTP were retrospectively analyzed.Results Of the 14 patients diagnosed with SLE and TTP,4 were men and 10 were women.The median age at diagnosis was 23 (17-69) years old.In five patients,the onset of SLE preceded TTP,and in nine patients SLE and TTP occurred simultaneouslv.All the 14 patients had thrombocytopenia and hemolytic anemia,12 had fever,11 had neurologic abnormalities,and 11 had renal dysfunction.Eight patients presented with the classic pentad of symptoms.Six patients were given steroids (alone or in combination with intravenous immunoglobulin and cyclophosphamide),and eight patients were treated with steroids in combination with plasmapheresis,with response rates of 2/6 and 6/8,respectively.Six patients died,with overall mortality rate of 6/14.No patients relapsed during the follow-up period.Conclusions SLE and TTP share some similar clinical symptoms.As a result,repeated examinations of peripheral blood smears are very important for early diagnosis.The renal damage in patients of co-existing diseases is more serious than those with TTP alone or SLE alone.Early diagnosis and prompt treatment with plasma exchange and steroids may improve the prognosis in SLE patients with TTP.
7.Insulin-like growth factor 1 promotes in vitro proliferation of chondrocytes in traumatic arthritis

BACKGROUND:The early damaged chondrocytes are susceptible to de-differentiate and exert unstable phenotype during the in vitro culture, thus needing some growth factors.
OBJECTIVE:To observe the promotion effect of insulin-like growth factor 1 on the in vitro proliferation of chondrocytes in adult rabbits with traumatic arthritis.
METHODS:Traumatic arthritis models of adult rabbits were established by using the modified Hulth method. After the models were successful y established, the distal femur and proximal tibia were harvested under sterile conditions, the chondrocytes were cultured. The cultured cells were divided into two groups:control group was cultured with Dulbecco’s modified Eagle’s medium containing 10%fetal bovine serum, while experimental group was cultured with Dulbecco's modified Eagle’s medium containing 100μg/L insulin-like growth factor 1. The effect of insulin-like growth factor 1 on the proliferation of chondrocytes in adult rabbits with traumatic arthritis was determined through the cytomorphology, cellcounting, and cellactivity.
RESULTS AND CONCLUSION:The chondrocytes in adult rabbits with traumatic arthritis were successful y cultured, the majority of cells were mini-cells, presenting smal fusiform, round or polygonal shape. Hematoxylin-eosin staining showed that the number of cells in experimental group was higher than that in control group. MTT assay found that the absorbance of cells in experimental group was greater than that in control group (P<0.01). Our findings indicate that, insulin-like growth factor 1 can promote the in vitro proliferation of chondrocytes in adult rabbits with traumatic arthritis.
8.How to relieve pre-operative parental anxiety before anesthesia: Chinese parental attitudes

Objective To assess Chinese parental requirement for relieving their preoperative anxiety,and their attitudes regarding the measures reducing their anxiety and to investigate the possible factors associated with increased anxiety.Methods One hundred and four pairs of pediatric patients (aged 2-12 yr,of ASA physical status Ⅰ or Ⅱ) undergoing elective surgery under general anesthesia and their parents were enrolled in this study.The general data of the pediatric patients and their parents and parental requirement for relieving their preoperative anxiety,and their attitudes regarding the measures reducing their anxiety were obtained using a questionnaire.Parental and children's anxiety? levels were assessed using the State-Trait Anxiety Inventory (STAI) and modified Yale Preoperative Anxiety Scale (m-YPAS),respectively.Results Parental anxiety levels were positively correlated with the children's anxiety levels (r =0.254).Parental desires to relieve their own anxiety under the personal direction of medical staff were significantly higher than the desires for the other measures; parental desires to relieve their own anxiety through parent-present induction of anesthesia (PPIA) were significantly lower than the desires for the other measures.The factors influencing parental attitudes included gender,educational level,preoperative anxiety level,and preoperative children' s anxiety level.Conclusion To relieve Chinese parental anxiety before surgery,the anesthesiologists should be aware that the majority of parents have a high desire to receive verbal instructions from medical staff before surgery was performed on their children and should provide enough information related to anesthesia and surgery; when PPIA is applied,parental desire for PPIA should be taken into full account; the anesthesiologists offer support and comfort for the parents properly.
