1.Diagnostic value of procalcitonin, C-reactive protein in children suffering from malignant solid tumor with concurrent infection and in progression of tumor

Objective To discuss the diagnostic value of serum procalcitonin (PCT) and C-reactive protein (CRP) in children suffering from malignant solid tumor with concurrent infection and in tumor progressing stage.Methods A total of 94 cases of malignant solid tumor at Department of Pediatrics,Beijing Tongren Hospital,Capital Medical University from March to August 2016,were collected and divided into 2 groups:infection group (n =17) and non-infection group (control group,n =77).Meanwhile,the control group were further divided into stable tumor group (n =48) and the advanced tumor group (n =29).All of the serum specimens were detected for PCT and CRP levels.PCT and CRP levels were compared and statistically analyzed among different groups.The median(four digit spacing) was used to describe the data.Results PCT level significantly increased in the infection group [0.67 (1.35) μg/L] compared with that of the control group [0.09 (0.08) μg/L],and the difference was statistically significant (Z =-6.371,P =0.000).CRP level significantly increased in the infection group[95.00(93.42) mg/L] compared with that of the control group[1.99 (12.49) mg/L],and the difference was statistically significant (Z =-5.555,P =0.000).During diagnosing infection,the area under the receiver operating characteristic curve (ROC curve) of PCT was 0.995(95% CI:0.987-1.000),and the area under the ROC curve of CRP was 0.932(95% CI:0.876-0.988);when the cutoff value of PCT was 0.468 μg/L,the sensitivity was 76.5%,and the specificity was 100.0%;when the cutoff value of CRP was 10.17 mg/L,the sensitivity was 88.2%,and the specificity was 72.7%.Compared to stable tumor group,PCT level of advanced tumor group significantly increased[0.12 (0.08) μg/L vs.0.07 (0.06) μg/L],and the difference was statistically significant(Z =-2.886,P =0.004);CRP level of the advanced tumor group sig nificantly increased [10.34 (27.84) mg/L vs.1.02 (2.12) mg/L],and the difference was statistically significant (Z =-4.862,P =0.000).During diagnosing tumor progression,the area under the ROC curve of PCT was 0.697 (95 % CI:0.572-0.823),and the area under the ROC curve of CRP was 0.832(95% CI:0.738-0.926);when the cutoff value of PCT was 0.094 μg/L,the sensitivity was 72.4%,and the specificity was 66.7%;when the cutoff value of CRP was 2.39 mg/L,the sensitivity was 72.4%,the specificity was 72.9%.Correlation analysis showed that the PCT and CRP levels were positively associated with tumor progression,and the correlation of CRP (r =0.558,P =0.000) was much higher than that of PCT (r =0.331,P =0.003).Conclusions Both serum PCT and CRP are effective indicators in diagnosing tumor with concurrent infection.PCT is better in diagnosing infection than CRP,and is less influenced by tumor.CRP is a better way to identify whether tumor was in progress than PCT.Joint detection of 2 indexes in identification of children suffering from malignant solid tumor with concurrent infection and in progress has practical value for the diagnosis and treatment.
2.Anti-SARS-CoV-2 activity of small molecule inhibitors of cathepsin L

The coronavirus disease 2019 (COVID-19) is an acute infectious disease caused by the new severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, which has led to serious worldwide economic burden. Due to the continuous emergence of variants, vaccines and monoclonal antibodies are only partial effective against infections caused by distinct strains of SARS-CoV-2. Therefore, it is still of great importance to call for the development of broad-spectrum and effective small molecule drugs to combat both current and future outbreaks triggered by SARS-CoV-2. Cathepsin L (CatL) cleaves the spike glycoprotein (S) of SARS-CoV-2, playing an indispensable role in enhancing virus entry into host cells. Therefore CatL is one of the ideal targets for the development of pan-coronavirus inhibitor-based drugs. In this study, a CatL enzyme inhibitor screening model was established based on fluorescein labeled substrate. Two CatL inhibitors IMB 6290 and IMB 8014 with low cytotoxicity were obtained through high-throughput screening, the half inhibition concentrations (IC50) of which were 11.53 ± 0.68 and 1.56 ± 1.10 μmol·L-1, respectively. SDS-PAGE and cell-cell fusion experiments confirmed that the compounds inhibited the hydrolysis of S protein by CatL in a concentration-dependent manner. Surface plasmon resonance (SPR) detection showed that both compounds exhibited moderate binding affinity with CatL. Molecular docking revealed the binding mode between the compound and the CatL active pocket. The pseudovirus experiment further confirmed the inhibitory effects of IMB 8014 on the S protein mediated entry process.
3.Emergency closed reduction and percutaneous Kirschner wire fixation for treatment of Gartland type II-III supracondylar fractures of the humerus in children.

OBJECTIVETo analyze the clinical effect and related risk factors of Gartland type II-III supracondylar fractures of humerus in children in the emergency closed reduction and percutaneous Kirschner wire fixation.
METHODSFrom January 2008 to June 2013,112 children of Gartland type II to III supracondylar humeral fractures were treated in children in emergency closed reduction and percutaneous K-wire fixation, including 72 males and 40 females with an average age of 6.2 years old ranging from 2 to 11 years old. Among them,74 cases were in Gartland type II fractures,38 cases were in type III; The duration from injury to surgery time was 2.5 to 8 hours (averaged 4.6 hours). Elbow cast was applied after operation with the elbow extended of 100 degrees for 4 to 6 weeks, then the gypsum and Kirschner wires were removed.
RESULTSAll patients were follow-up from 6 to 60 months (averaged 12 months). All fractures reached clinical healing. The final follow-up was assessed by Flynn criteria, the result was excellent in 86 cases, good in 23 cases, general in 3 cases, excellent and good rate was 97.3%. Three patients had mild cubitus varus deformity without orthopedic treatment. No pin tract infections, iatrogenic ulnar nerve injury, compartment syndrome, and complications such as Volkmann ischemic contracture occurred.
CONCLUSIONClosed reduction and percutaneous Kirschner wire fixation had advantages of exact reduction, firm fixation, fewer complications ,less pain in children undergoing emergency surgery, and.high success rate, so it is a safe and efficient treatment for humeral supracondylar fracture in children.

4.The profile of Th17/Treg balance in the peripheral blood of patients with systemic lupus erythematosus

ObjectiveTo investigate the profile of Th17/Treg balance in the peripheral blood of patients with systemic lupus erythematosus (SLE).MethodsThirty-two SLE patients in active disease were selected and 30 SLE patients in remission were included in this study.The control group was consisted of 25healthy individuals.The expressions of IL-17 and FoxP3 on CD4+ T cells in the peripheral blood were assessed by flow cytometry and the mRNA levels of these two cytokines were examined by quantitative PCR respectively.ANOVA was used for statistical analysis.ResultsThe percentage of CD4+IL-17+ T cells and IL-17mRNA expression of the active group were significantly higher than those of the remission group and control group[(l.0l±0.22)%,(2.04±0.63)vs (0.48±0.16)%,(1.12±0.34) vs (0.41±0.12)%,1; P<0.01].There was no difference between the remission group and control group(P>0.05).However,the percentage of CI4+FoxP3 + T cells and FoxP3 mRNA expression of the active group were significantly lower than those of the remission group and control group [(2.36±t0.54)%,(0.42±0.16) vs (4.34±0.95)%,(0.87±t0.28) vs (5.09±11.17)%,1; P<0.01 ],and there was also significant differencesbetween the remission group and control group(P<0.05).ConclusionThl7/Treg balance shift may exist in the peripheral blood of patients with SLE and the degree of imbalance may be related to disease activity of SLE.
5.Identification and Simulation Mutation of a High-productive Strain of Poly(?-glutamic acid) Independent of Glutamic Acid

A high productive poly ?-glutamic acid(?-PGA) strain PGA-N in a culture medium containing no L-glutamine was isolated from fermentation products.With the following identifications of colony mor-phology,physiological and biochemistry experiments,and genetics,the strain PGA-N was classified as a Bacillus licheniformis.According to the product environment,the base culture medium having no L-glutamine was simulated.In order to enhance the production of the strain PGA-N,the fermentation condi-tions,such as carbon source,nitrogen source,were optimized and the ?-glutamic acid production reached 5.16 g/L after getting the optimum formulation of this culture medium.PGA-N was mutagenized with com-bination of NTG and UV.A mutant PGA-N-C10 was screened which PGA production was increased from 5.16 g/L to 8.82 g/L.The study also investigated the effects of agitation speed on the cell biomass,?-PGA production and the ?-PGA molecular weight.The ?-PGA yield of PGA-N-C10 was as high as 11.00 g/L when the agitation speed was 400 r/min.
6.Research Progress on the Polyphasic Taxonomy of Acetic Acid Bacteria

Acetic acid bacteria are Gram-negative,obligate aerobic bacteria that have the ability to incompletely oxidize alcohols or sugars to organic acids as end products. The taxonomy of acetic acid bacteria has undergone many changes in the last 30 years. The early classification systems for these bacteria were based on morphological and biochemical characteristics. Today,the acetic acid bacteria are classified as the consensus result of a polyphasic analysis,combining phenotypic,chemotaxonomic and genotypic data. This paper reviewed the polyphasic taxonomy of acetic acid bacteria,mainly introduced the current classification of acetic acid bacteria,then discussed the application of phenotypic,chemotaxonomic and genotypic method in the taxonomy of acetic acid bacteria.
7.Bile cystadenocarcinoma:MRI findings with pathologic correlation

Objective To describe the MRI features and pathologic findings of biliary cystadenocarcinoma(BCAC)and to assess the diagnostic value of MRI in those tumors.Methods Five cases of BCAC were collected.All cases were proved by pathology.Non-enhanced and multiphase-enhanced MRI were performed in all cases.MRCP were performed in two cases.The MRI features of the five cases were reviewed retrospectively and correlated with pathologic findings.Results Histological evidence demonstrated five cases of BCAC.Four cases were solitary,whereas the other case was multifocal.All cases were solid and cystic lesions.Two cases were unilocular,whereas the other three cases were multilocular. Multiple mural nodules and irregular thickening cystic walls were presented in all cases.The cystic parts of the lesions were homogeneous in signal intensity and showed no enhancement after contrast administration in the five BCAC.Septa were present in three BCAC with multilocular cyst.On MRCP the bile duct dilatation was found in two BCAC.Conclusion MRI can reveal the characteristic findings of BCAC and accurate preoperative diagnosis can be made.
8.Analysis of new effector T cells named TH17 cells in peripheral blood of patients with rhenmatoid arthritis

Objective To investigate the distribution of TH17 cells in peripheral blood of patients with rheumatoid arthritis(RA). Methods Intracelluar flow cytometirc detection of TH17 cells in peripheral blood was establised using PHA or PMA + Ion as stimulators in vitro. Thirty-five active and 30 stable RA pa-tients and sex and age paired healthy controls were enrolled in this study. Results The stimulating effect of PMA + Ion was better than that of PHA alone. Under PMA + Ion stimulation, the percentage of TH17 cells in peripheral blood from RA patients and healthy controls was increased significantly(P<0.05). With or with-out PMA + Ion stimulation, such percentage in active group was higher than that of stable group and both of them were higher than that of heathy controls(P<0.05). The distribution profile of TH1 cells was similar to that of TH17 cells. Conclusion There is a distribution abnormality of TH17 cells in peripheral blood of RA patients, which could be used as a new marker for the assessment of immunological condition in RA.
9.Effect of hydrogel bandage applicator together with wound care bandage in the prevention of indwelling intravenous catheter phlebitis

Objective To investigate the prevention of indwelling intravenous catheter phlebitis through nursing care.Methods 300 cases of postoperative patients admitted to the gynecology department,from January 2010 to October 2010,were randomly assigned into 3 groups.The punctured regions were treated by cold magnesium sulfate gauze together with wound care bandage ( experimental group 1 ),hydrogel bandage applicator together with wound care bandage (experimental group 2 ),and wound care bandage (control group ),respectively.The intravenous catheter retention time and incidence of phlebitis were observed.Results The 3-dgy retention rate of indwelling intravenous catheter of the three groups was 86%,97%,71%,respectively.There were significant differences between experimental group 1,experimental group 2 and the control group ( Hc =6.80,26.15 ;P < 0.05 or P < 0.01 ).There was also significant difference between experimental group land experimental group 2 ( Hc =7.80,P < 0.05 ). The incidence of phlebitis was 23%,6%,44%,respectively.There were significant differences between experimental group 1,experimental group 2 and the control group ( Hc =9.94,38.64 ; P < 0.05 or P < 0.01 ). There was also significant difference between experimental group 1and experimental group 2 (Hc =11.81,P < 0.01 ). The effective rate of phlebitis prevention was 90%,97%,78%,respectively.Conclusions The use of magnesium sulfate or hydrogel bandage can effectively extend the indwelling intravenous catheter retention time and prevent catheter phlebitis,and the latter may be superior in preventing phlebitis.
10.Establishment and application of a cell-based high-throughput screening model for TMPRSS2 inhibitors

Transmembrane serine protease 2 (TMPRSS2) is a cell surface protease widely present in the human body. It is involved in the infection of various viruses such as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), and in the cell invasion, tumor growth and metastasis processes of prostate cancer. This study used Boc-Gln-Ala-Arg-AMC as the fluorescent substrate to determine the cleavage activity of TMPRSS2 towards SARS-CoV-2 S protein. Then cell-based screening model for TMPRSS2 inhibitors was established in Vero E6 cells overexpressing TMPRSS2 (Vero E6/TMPRSS2). Seven compounds exhibiting TMPRSS2 inhibitory activities with low toxicity were obtained through high-throughput screening (HTS) from natural and synthetic compound pure product library of National Center for Screening Novel Microbial Drugs. Surface plasmon resonance (SPR) has shown that the obtained inhibitors could bind to TMPRSS2 with moderate affinity in a dose dependent manner. Cell-cell fusion experiments have shown that the obtained inhibitors can inhibit the occurrence of S protein mediated cell-cell fusion by inhibiting TMPRSS2 cleavage of SARS-CoV-2 S protein in a concentration dependent manner. Preliminary pseudovirus experiment showed that the inhibitors may reduce the pseudovirus infection into Opti-HEK-293T-ACE2 cells to varying degrees. In a word, this study successfully established a cell-based HTS model for TMPRSS2 inhibitor and preliminarily confirmed that the seven screened inhibitors possessed
