1.Diagnosis and treatment of bacterial infection in children with febrile neutropenia

Infection rate was high in children with hematologic cancers and febrile neutropenia,especially the number of drug-resistant bacteria showed an increasing trend.Because of defects of the immune function,the typical symptoms,pathogenic bacteria and infection was not clear in these children,the fever may be the only sign of serious underlying infection,mortality related with infection was high,so it was important to evalute the clinical risk accuratly and select antibiotic properly.
2.Initial research of serum glycated albumin during pregnancy

Objective To explore the normal range of serum glycated albumin (GA) during the second trimester in non-gestational diabetes mellitus (GDM) population and the value of serum GA in the blood glucose monitoring during pregnancy.Methods The GA was measured in 101 healthy gravida during the second trimester and 80 gravida with GDM and diabetes mellitus who were in treatment at Peking University First Hospital between August 2011 and December 2011,in order to analyze the normal range of GA and the relationship between GA and the level of blood glucose.Results (1) The normal range of GA during the second trimester was 10.9%-15.3%,which was negatively correlated with body mass index (P<0.01).(2) Significant correlations were observed between GA and the level of hemoglobin A1c (HbA1c),preprandial,postprandial and mean plasma glucose in gravida with GDM and diabetes mellitus (r:0.361,0.252,0.338,0.310 ; all P < 0.05).(3) When the level of GA was 13.97%,the sensitivity and specificity index for glucose control were 78.0% and 74.4%.Conclusions GA could evaluate the severity of disease in gravida with GDM and diabetes mellitus.10.9%-15.3% could be suggested as normal range of GA for the gravida at the second trimester.
3.CULTIVATION OF RETINAL PROGENITOR CELLS OF DIFFERENT AGES

Objective To study the cultivation of retinal progenitor cells of different ages. Methods Retinal progenitor cells of E14 and E18 SD rats were isolated,cultivated in suspension in modified DMEM/F12 serum-free medium and then differentiation was induced in vitro.Cells were observed under phase-contrast microscopy daily and identified by immunocytochemistry,scanning electron microscopy and transmission electron microscopy. Results Cultivated in DMEM/F12 serum-free medium,retinal progenitor cells formed cell spheres.After being plated,isolated cells migrated outwards and differentiated.Scanning electron microscopy demonstrated the morphology of the cell spheres and differentiated cells.Transmission electron microscopy demonstrated that there were stem cell-like cells in cell spheres and neuron-and glia-like cells after the plating.Immunocytochemistry demonstrated that most cells in spheres expressed neural stem cell marker nestin and cell division marker BrdU.After the plating,retinal progenitor cells could be induced to differentiate into various retinal cells,including Thy1.1-positive retinal ganglion cells.The percentage of retinal ganglion cells was 16.91%?4.05% at E14 and 4.65%?1.88% at E18.The differences were statistically significant(t=15.04,P
4.Impact of lactose-free hydrolyzed protein formula on feeding intolerance and weight gain in very low birth weight infants

Objective To study whether a lactose-free,hydrolyzed protein formula would ameliorate feeding intolerance and weight gain in very low birth weight infants.Methods The study was conducted on very low birth weight infants ( gestational age ≤34 weeks, birth weight ≤1500 g) in our NICU from August 2010 to August 2013. According to the number of admission, the cases were assigned into lactose-free hydrolyzed protein formula group ( hydrolyzed protein group) and preterm formula group. The data of basic information ( gestational age, birth weight, way of labor, sex, asphyxia ), commencement of feeding time ( days) , enteral caloric intake, parenteral nutrition caloric intake, The time ( days) required to regain birth weight, weight gain index, days of parenteral nutrition, prealbumin level, feeding intolerance, the incidence of necrotizing enterocolitis ( NEC ) and extrauterine growth retardation(EUGR)werecollected.Results A total of 83 infants were recruited in the study. 40 infants were in hydrolyzed protein group and 43 infants were in preterm formula group. There were no differences between groups for basic information ( P>0. 05). The weight gain index of hydrolyzed protein group was higher than that of preterm formula group (13. 5 ± 2. 4 g/d vs. 11. 9 ± 3. 7 g/d, P=0. 036). There were less cases of feeding intolerance in hydrolyzed protein group than in preterm formula group & nbsp;(28/40 vs. 38/43, P=0. 038). The differences of the incidence of NEC (1/40 vs. 2/43) and EUGR (21/40vs.25/43)showed no significance statistically.Conclusions Lactose-free hydrolyzed protein formula can obviously improve feeding intolerance and early weight gain of premature infants.

6.Contrast-enhanced ultrasonic diagnosis of benign and malignant solid thyroid lesions

ObjectiveTo investigate the value of CEUS in differential diagnosis of benign and malignant thyroid solid lesions.MethodsTotally 45 patients with thyroid solid lesions underwent CEUS examination.The enhancement mode and blood perfusion characteristics of malignant and benign lesions were analyzed,and the results were compared with pathology.ResultsCEUS diagnosed 31 benign lesions and 14 malignant masses.The diagnostic accordance rate of CEUS was 93.75% (30/32) and 92.31% (12/13) for benign and malignant lesions respectively.In 31 benign lesions,27 (27/31,87.10 %) showed entire homogeneous enhancement,4 (4/31,12.90%) presented inhomogeneous enhancement.In 14 malignant masses,1 (1/14,7.14 %) showed homogeneous hypoechoic enhancement,12 (12/14,85.71%) presented inhomogeneous hyperechoic enhancement and 1 (1/14,7.14%) presented non-enhancement.ConclusionCombined with two-dimensional grey scale ultrasound and CDFI,CEUS enables dynamic observation of the microvascular perfusion of thyroid lesions,therefore helps to differentiate benign and malignant thyroid lesions.
7.Association of microalbuminuria and von Willebrand factor with metabolic syndrome in hypertensive patients

Objective:To compare the difference of microalbuminuria (MAU) and von Willebrand factor (vWF) between essential hypertensives and hypertensive patients with metabolic syndrome (MS), and to explore the association of these factors with MS. Methods: According to the new definition of MS by International Diabetes Federation (IDF) for 2005, 133 consecutive essential hypertensives were divi-ded into two groups: hypertensive patients with metabolic syndrome (EH-MS group, n=97), and essential hypertensives (EH group, n=36). Biochemistry assay, plasma vWF, 24 hours urine albumin excretion (UAE) and morning urine albumin/creatinine ratio (ACR) were measured. The association of MAU and vWF with MS was analysed. Results: Waist circumference, body mass index, triglyceride (TG), low density lipoprotein cholesterol (LDL-C), plasma vWF and UAE were increased(P0.05, respectively). 8.Glycosylated hemoglobin level in diagnosis and management of gestational diabetes mellitus

Objective To evaluate the value of glycosylated hemoglobin (HbAlc) level in glucose monitoring,insulin therapy and neonatal birth weight prognosis in gestational diabetes mellitus (GDM) patients.Methods Data of 1074 women who were diagnosed as GDM and delivered at Peking University First Hospital between January 1,2005 and August 31,2011 were collected and retrospective analysis was conducted.The relationship of HbAlc level with plasma glucose level of oral glucose tolerance test (OGTT),insulin administration and neonatal birth weight were investigated with Pearson correlation analysis,Logistic regression analysis or receiver operating characteristic (ROC) curve.Results OGTT was done on 1074 women with (31.8± 4.0) years old during the second and third trimester and diagnosed GDM at (27.6 ± 3.4) weeks of gestational age.Among them,glucose level of fasting and 1 and 2 hours after OGTT were (5.2 ± 0.7),(10.9± 1.4) and (9.4±1.5) mmol/L.The levels of HbAlcwere (5.57±0.48)% at the fist time of HbAlc testing at (31.8±4.3) weeks of gestational age.(1)The average value of HbAlc in 726 women determined at one month after OGTT was (5.54±0.47)%,and increased from (5.34±0.41)% in women with one abnormal item in OGTT to (5.47±0.41)% and (5.71±0.46)% in women with two or three abnormal items inOGTT (t=3.025 and 6.399,all P<0.01).(2) HbAlc level in women received insulin therapy was higher than those who did not [(5.78±0.58) % vs (5.42±0.37) %,t=9.431,P<0.01].The sensitivity and specificity were 57.8% and 75.5% respectively in predicting insulin requirement in women with GDM at the cut-off value of 5.67% for HbAlc (Logistics analysis:OR=6.847,95%CI:4.588-10.218,P<0.01).(3) Maternal HbAlc levelat the third trimester was higher in large for gestational age(LGA) group than that in non-LGA group [(5.75±0.52)% vs (5.54±0.42)%,t=6.845,P<0.01].The sensitivity and specificity were 44.8% and 73.8% respectively in predicting LGA at the cut-off value of 5.75% for HbAlc (Logistics analysis:OR=2.187,95%CI:2.097-3.783,P<0.01).(4) Maternal HbAlc level at the third trimester was higher in macrosomia group than that in non-macrosomia group [(5.88±0.53)% vs (5.57±0.45)%,t=5.990,P<0.01].The sensitivity and specificity were 50.4% and 79.8% respectively in predicting macrosomia at the cut-off value of 5.85% for HbAlc (Logistics analysis:OR=3.299,95%CI:2.237-4.865,P<0.01).Conclusions HbAlc level at the time of GDM diagnosis could imply the severity of disease and might predict insulin requirement.While maternal HbAlc level at the third trimester may predict the occurrence of LGA and macrosomia.
9.Influencing factors for fungal infection in hospitalized patients with ac-quired immunodeficiency syndrome

Objective To analyze the influencing factors for fungal infection in hospitalized patients with acquired immunodeficiency syndrome(AIDS),and provide basis for taking preventive and control measures.Methods Clini-cal data of 112 inpatients with AIDS in a tertiary first-class hospital from January 2010 to October 2015 were ana-lyzed retrospectively,related risk factors were analyzed in patients with fungal infection,univariate analysis was performed byχ2 or t test,and multivariate analysis was performed by logistic regression analysis.Results Among 112 AIDS inpatients,41 (36.61%)had fungal infection.Candida albicans was the main pathogen (n=52, 76.74%)and was mainly isolated from lower respiratory tract (n=29,42.65%).Univariate analysis showed that invasive procedures,duration of antimicrobial use,combined antimicrobial use,and decreased CD4+ T lymphocyte count in peripheral blood were risk factors for fungal infection in AIDS inpatients(all P<0.05),while oral cleaning care and systemic antiviral therapy were protective factors for fungal infection(all P<0.05);multivariate analysis showed that decreased CD4+ T lymphocyte count in peripheral blood(OR,1.017[95% CI,1.009-1.025])and combined antimicrobial use(OR,3.975[95%CI,1.093-14.448])were independent risk factors for fungal infection in AIDS inpatients,while systemic antiviral therapy was independent protective factor for fungal infection (OR, 0.288[95%CI,0.099-0.841]).Conclusion There are many factors influencing fungal infection in AIDS patients, rational use of antimicrobial agents,systemic antiviral therapy,and increasing serum albumin concentration are main methods to prevent fungal infection,avoid unnecessary invasive procedures and application of preventive antifungal therapy for high-risk patients are also effective to prevent fungal infection.
