1.Application of warming humidification oxygen therapy in patients with tracheotomy

Objective To investigate the application value of warming humidification oxygen therapy in patients after tracheotomy.Methods Forty-one patients after tracheotomy were collected consecutively for this study.All patients were randomly assigned into the treatment group (21 cases)and the control group (20 cases).The treatment group adopted warming humidification oxygen therapy,and the control group adopted common airway moistening method.Moist effect judgment,viscosity of sputum,phlegm scab formation,respiratory irritation symptoms,airway mucosal bleeding,air way resistance,lung infection cases,average days in the ICU were investigated.Results There were statistically significant differences in moist effect judgment,viscosity of sputum,phlegm scab formation,respiratory irritation symptoms,airway mucosal bleeding,air way resistance,lung infection cases,average days in the ICU between the two groups.Conclusions Warming humidification oxygen therapy is more suitable to patients with tracheotomy than common airway moistening method.

4.Variation of asthma patient's quality oflife after anti-inflammation treatment

Aim To evaluate the influence of asthma and the effect of anti-inflammation therapy on the quality of life (QOL) in allergic asthma patients. Methods The QOL of 45 patients with asthma were investigated by collecting questionnaire before and after three months anti-inflammation treatment , at the same time the serum eosinophil cationic protein(ECP) level was detected in 20 patients. The QOL questionnaire included 4 domains, they were respiratory symptoms, physical activity limitation, emotional function and environmental stimuli. Results The factors which influenced the QOL in asthma patients ranging from high to low were environmental stimuli, respiratory symptom and emotion function and physical activity limitation. 5.Intra-hospital transport nursing of patients with severe inhalation pulmonary injury treated with extracorporeal membrane oxygenation

Objective To explore the nursing experience of the safe intra-hospital transport of patients with the severe aspiration of the inhalation injury. Method The nursing measures for the intra-hospital transport of 2 cases of smoke pot inhalation injury caused by extracorporeal membrane oxygenation treatment were taken, including disease risk assessment, preparation for transport, organization of a transport team, effective vital signs monitoring during transport, extracorporeal membrane oxygenation (ECMO) pipeline monitoring and nursing and observation and nursing of complications. Result No emergency was found during the transport of patients and both of them were safely transported. Conclusion Such nursing measures as pre-transport assessment and preparation and bettering predictive nursing for the patients with severe inhalation pulmonary injury treated with extracorporeal membrane oxygenation are key to the safety during intra-hospital transport.
6.Research on the strategy of doctor-patient knowledge transfer based on patient preference

Doctor-Patient knowledge transfer plays an important role in clinical service.In an evidence-based investigation on their basic features and preference,patients investigated fall into six categories according four influence factors.These factors comprise,set as the criteria,information preference,decision-making preference,perceived severity of disease and influence of reference group.The six groups are the contradiction group,reactive group,ego group,decision preference group,total equality group,and positive group.Doctor-Patient knowledge transfer stategiea are suggested based on the preference features of the six groups.These strategies serve as references and paramedics for doctors to perform effective doctor-patient communication.
9.Supervision on Measurement of Quality and Safety on Medical Treatment Facility

To combine "the rule of military medical measurement" with the current situation of medical treatment quality control, the foundation, important component and necessary condition of medical treatment quality control was described. Some idiographic advices of intercoordination and communication in medical measurement institution, the direction and training of special technique, and how to develop compulsion coerciveness checking in base course were given. These are valuable for insuring medical treatment facility in hospital.
10.A case-control study on effects of irbesartan and amlodipino on insulin sensitivity in obese hypertensive pa-tients

Objective To observe the effects of irbesartan and amlodipine on insulin sensitivity in obese hypertensive patients. Methods 157 moderate hypertensive patients were divided into two greups,which received irbesartan or amlodipine for 16 weeks. The changes of blood pressure,heart rate and weight were observed. Insulin re-sistance was computed using the homeostasis model assessment (HOMA-IR). Results There was no difference in antihypertensive efficacy between the two groups(P>0.05). After 16 weeks of treatment ,the insulin resistance index decreased by 8.7±2.4 vs 6.1±2.6 (P<0.01) and body mass index decreased(28.1±3.8) kg/m2 vs (27.2± 3.2 ) kg/m2 (P<0.05 ) and mean heart rate raised by (73.1 ±10.0 ) kg/m2 vs ( 79.1±11.4) kg/m2 ( P < 0.01 ) in irbesartan group while no such changes were found in amlodipine treated group. Conclusions In obese hypertensive patients,irbsartan and amlodipine exert similar antihypertensive effect, but irbesartan may help to improve some obe-sity-related metabolize disorder,so the obese hypertensive patients may benefit more from irbsartan than from amlo-dipine.
