3.The effect of cardiopulmonary bypass on monocyte HLA-DR expression

Objective The purpose of this study was to investigate the effects of cardiopulmonary bypass(CPB) on monocyte HLA DR expression and immune function Methods Sixteen ASA Ⅱ Ⅳ patients [mean age (38 5?5 1)yr,mean body weight (42 9?10 2kg)] of both sexes (male 7,female 9) with rheumatic value disease undergoing mitral value replacement (11 patients) and mitral value and aortic value replacement (5 patients) with moderate hypothermic CPB were enrolled in this study Another ten patients undergoing lung lobectomy were used as control group Patients suffering from infection, immuno deficiency or receiving corticosteroid or immunoregulatory drugs were excluded Premedication included intramusallar midazolam 0 1mg?kg -1 and morphine 0 1mg?kg -1 Anesthesia was induced with intravenous midazolam 0 15mg?kg -1 , fentanyl 8?g/kg and vecuronium 0 1mg?kg -1 and maintained with isoflurane inhalation (0 8%~1 2%) and intermittent boluses of fentanyl and vecuronium The total dose of fentanyl was (18 3?3 2)?g/kg in CPB group and (9 6?1 3)?g/kg in control group CPB time was (74 2?22 5) min and aortic clamping time (57 5?19 2) min Blood samples were taken from CVP line before and 5 min after induction of anesthesia, before and after CPB and on the 1st, 3rd and 5th postoperative day for determination monocyte count and percentage of HLA DR + monocytes Results The number of monocytes decreased at the end of surgery but greatly increased on the postoperative days The percentage of HLA DR + monocytes also decreased at the end of surgery and decreased further on the 1st postoperative day but started increasing on the 3rd postoperative day In two patients who developed infection the percentage of HLA DR + monoeytes was lower than the average percentage of HLA DR + monocytes of the other 14 CPB patients In control group the percentage of HLA DR + monocytes also decreased significantly after operation but there was no significant change in the number of monocytes However there was a significant difference in monoeyte HLA DR expression between the two groups Conclusions The changes in monocyte HLA DR expression and monocyte count may be induced by impaired immune response after CPB and dynamic monitoring of monocyte HLA DR expression may help to predict complication of infection after CPB
4.Pharmacoeconomic Evaluation of Three Therapeutic Schemes for Preventing Post-appendectomy Infections in Children

OBJECTIVE:A pharmacoeconomical analysis was performed to evaluate three therapeutic schemes for preventing post-appendectomy infections in children METHODS:The study included 119 children undergoing appendectomy According to the pathological results of appendix,they were divided into group Ⅰ and group Ⅱ,and treated with three different schemes respectively The schemes consisted of A:cefazolin+benzylpenicillin+metronidazole,B:cefotaxime+benzylpenicillin+metronidazole and C:cefazolin+benzylpenicillin+metronidazole+claforan These three schemes were assessed with pharmacoeconomic evaluation RESULTS:As for group Ⅰ,scheme A and B were better than scheme C As for group Ⅱ,scheme A was the best CONCLUSION:Application of pharmacoeconomics will optimize therapeutic scheme and guide rational use of drugs
5.Clinical application and evaluation of 4 kinds of siphilis serological detection method

Objective To compare the value of four kinds of commonly used serological detection method for detecting Trepone-ma Pallidum,i.e.,Treponema Pallidum enzyme-linked immunosorbent assay(TP-ELISA),Treponema pallidum particle agglutina-tion test(TPPA),Treponema Pallidum rapid plasma reagin test(RPR)and Treponema Pallidum antibody detection(TP-AD,emul-sion method).Methods 5 870 specimens from outpatients and inpatients were screened by TP-ELISA.121 cases of detected posi-tive specimen were simultaneously detected by TP-AD,TPPA and RPR.Then the detection results were performed the comparative analysis.Results Among 5 870 specimens,121 cases were positive by ELISA,the detection rate was 2.06%.Among 121 positive cases,119 cases were positive by TPPA,the conformity degree was 98.34%,49 cases were positive by RPR,the conformity degree was 40.41%,113 cases were positive by TP-AD,the conformity degree was 93.38%.With the TPPA results as the standard,there was no statistically significant difference between TPPA and TP-AD(P >0.05),but there was statistically significant difference be-tween TPPA and RPR(P <0.01).Conclusion The four kinds of method have their applicability.ELISA d has good specificity and high sensitivity,and is simple to operate and suitable for the physical examination of large amount of pregnant women,parturients and normal people.TPPA has good specificity and high accuracy,is suitable for definite diagnosis.RPR is suitable for the monito-ring and the curative effect observation in the patients with the active stage of siphilis.Compared with ELISA,TP-AD has slightly less sensitivity,but good specificity and can be used for screening without specific instrument.
6.Emergency repair of the facial soft tissue injuries with plastic surgery

Objective To investigate the effect of cosmetic surgery on the facial soft tissue injuries.Methods The clinical data of 52 patients with facial soft tissue injuries from June 2013 to June 2014 were analyzed retrospectively.And plastic surgical techniques were applied to the early treatment of facial soft tissue injuries in patients,according to the size of defect and the degree of deformity of the patients,different plastic surgery treatment was chosen,such as skin flap or skin graft to repair wound surfaces.In this process,one must follow sterile noninvasive principle strictly with emphasis on the technique of plastic surgery such as entire debridement,wound healing application of skin flap and so on.Results Total 49 cases of facial soft tissue injuries were healed by first intention without significant complications,while 4 cases of them had mild scars.Absorbable suture exposition appeared in 3 patients after primary healing.Those sutures were romoved after disinfected and no sequelaes were happened.All patients were recovered with good appearances and functions to the most exent.Conclusions The treatment of facial soft tissue injuries,by applying plastic and aesthetic surgery,could reduce scar and postoperative deformity,and secondary corrective procedures can be avoided in most patients.

Talents constitute core resource in the development of healthcare sector. Based on the situation of the talent team in the Affiliated Hospital of Qingdao University, the authors systematically constructed the hierarchical and classified system for talent cultivation, centering on three key areas of talent cultivation, evaluation and assessment. The authors analyzed the achievements since the implementation in 2016, namely the innovation of talent evaluation, the construction of discipline echelon, and the communication and inter-departmental cooperation. At the same time, the authors further put forward targeted suggestions to promote the development of talent teams in terms of transforming the human resource management model, increasing policy support and funding, implementing refined management, and improving talent evaluation indicators by the levels and types.

The therapeutic effects and mechanisms of traditional Chinese kidney-tonifying drugs in treating osteoporosis have become the focus under study. Pharmacological studies have shown that traditional Chinese kidney-tonifying drugs are promoters for the proliferation of osteoblasts, inhibitors for the activity of osteoclasts, regulators for the estrogen level and its receptor, plays important roles in promoting osteogenesis and suppressing adipogenesis of marrow mesenchymal stem cells (MSCs), modulating the function of OPG/RANK/RANKL system and the metabolism of calcium and phosphorus, as well as antioxidation. The anti-osteoporotic active ingredients and pharmacological action mechanism of traditional Chinese kidney-tonifying drugs are summarized from the perspective of molecular and cell biology in this paper, so as to provide references for the study of their mechanism of anti-osteoporosis and for the development of traditional Chinese kidney-tonifying and bone-strengthening drugs.
